1.Evaluation on the application effect of comprehensive management model for pulmonary tuberculosis patients

Objective To evaluate and analyze the feasibility and application effect of “DOTS + WeChat” in the treatment and management of tuberculosis patients. Methods From 2018 to 2019, a total of 2 420 active pulmonary tuberculosis patients were registered in Zibo City, and 1 988 patients meeting the inclusion criteria were selected as the research subjects. 836 patients were randomly enrolled under the “DOTS + WeChat” integrated management mode, while the other 1152 cases were treated with single DOTS management mode. The regular medication status, treatment and outcome, and core knowledge awareness of the two groups were analyzed by SPSS16.0 software, χ2- test and t- test methods. Result The “DOTS + WeChat” comprehensive management group had higher a regular medication rate (98.80%), coincidence rate of sputum test times (95.81%), and success rate of treatment (98.68%) than the single DOTS management group (92.10%, 90.19%, and 96.53%) (P=0.000, 0.000, 0.003). The rate of medical staff participating in supervision and management in the comprehensive management group (100%) was higher than that in the single management group (75.87%) (P=0.000). The complete follow-up rate in the consolidation period (100%) and the complete whole course follow-up rate (99.76%) were both higher than those in the single management group (P=0.000, 0.001). The awareness level of core knowledge in the comprehensive management group (78.58±4.32) was higher than that in the single management group (70.70±8.02) (P=0.000). Conclusion The application of WeChat management mode has a positive effect on the treatment and management of tuberculosis patients in Zibo City. It is a feasible and effective supplement and improvement to the current tuberculosis control and management measures, which is worthy of further promotion and exploration.
2.Epidemic characteristics of pulmonary tuberculosis in Zibo in 2012 - 2020

Objective To analyze the epidemic characteristics of tuberculosis in Zibo City in recent years, and to provide a scientific basis for the formulation of tuberculosis prevention and control strategies and measures during the 14th Five-Year Plan period. Methods Descriptive epidemiological analysis was used to analyze the information of tuberculosis patients registered in “China Disease Control and Prevention Information System-Tuberculosis Information System” from 2012 to 2020 in Zibo City. Results From 2012 to 2020, a total of 12,706 pulmonary tuberculosis patients were registered in Zibo City, and the registered incidence rate decreased year by year (F=53.348, P<0.001), with an average annual rate of 30.45/10 million. There was no significant change in mortality. In regional distribution, the incidence was higher in northern region of Zibo than that in the southern region, with a cross distribution in the central region. In terms of time distribution, there were cases in each month over the years, and the overall trend increased first, then decreased and once again increased (F=8.856, P=0.006<0.01), with two low points occurring between January to February and also in October, and two peaks occurring between May to June and also in December. In terms of population distribution, most patients were local cases (82.38%). The incidence rate of males was about 2.09 times that of females. The incidence rate of children aged 0-14 was the lowest, while elderly people aged 65 years old and over had the highest incidence (61.02/10 million). Farmers were the main occupation among tuberculosis cases, accounting for 60.59% of the total. The main source of patients was from tracking, accounting for more than 50% since 2016. Conclusion The epidemic of tuberculosis in Zibo is stable and the incidence rate is decreasing year by year, showing seasonal variations. The majority of the patients are male. Special attention should be paid to farmers and the elderly, and further research on risk factors in high-risk areas and counties should be strengthened.

OBJECTIVETo establish a method for determination of chloropicrin in the air of workplace by gas chromatography with liquid absorption collection.
METHODSChloropicrin in sample air was collected by absolute ethyl alcohol in a porous glass plate absorption tube. Following direct sample loading, samples were separated by gas chromatography and detected with a micro-electron capture detector. The present method was compared with the national standard method.
RESULTSThe range of linearity was 0-3.0 µg/ml, and the regression equation was y = 3488.80x-57.84, with a correlation coefficient of 0.999 4. The detection limit was 0.003 µg/ml, with a minimum detectable concentration of 0.005 mg/m³ based on a 3.0 L sampling volume. The average recovery rate was 98.9%∼103.2%. The within-run precision was 0.96%-2.12%, and the between-run precision was 2.75%-4.59%. The sampling efficiency was 98.6%. Samples in porous glass plate absorption tube could be stored at 4 °C for at least 5 days. The result ratio of this method to the national standard method was 97.7%-108.0% .
CONCLUSIONThe present method meets the requirements of "Guide for establishing occupational health standards-Part 4: Determination methods of air chemicals in workplace" (GBZ/T 210.4-2008), and is feasible for determination of chloropicrin in the air of workplace.

5.Clinical effects of pulmonary recruitment maneuvers combined with pressure regulation volume control in premature infants with severe respiratory distress syndrome

Objective To study the clinical effects of pulmonary recruitment maneuvers combined with pressure regulation volume control (PRVC) in the treatment of severe respiratory distress syndrome (RDS) in premature infants.Method From July 2015 to September 2016,preterm infants of grade Ⅲ-Ⅳ RDS who received PRVC treatment in neonatal department of Huai'an Maternal and Child Health Hospital were assigned into recruitment maneuver group and control group (without recruitment maneuver) using randon number table.The ventilator parameters were observed at 1,2,6,12,18 h and 24 h after ventilation.Recovery rate,duration of oxygen therapy and ventilation,duration of hospital stay,incidence of second dose of pulmonary surfactant and complications were compared between two groups.Result A total of 18 cases were included in recruitment maneuver group and 19 cases in control group.The recovery rate of recruitment maneuver group was higher than control group (16/18 vs.10/19).The duration of oxygen therapy [(6.6 ± 2.3) d vs.(11.8 ± 3.0) d],duration of ventilation [(4.1 ± 2.3) d vs.(6.4 ± 2.8) d],duration of hospital stay [(26.7 ± 7.0) d vs.(33.0 ± 8.4) d] in recruitment maneuver group were significantly shorter than control group (P < 0.05).The proportion of bronchopulmonary dysplasia (1/18 vs.8/19),retinopathy of premature (1/18 vs.7/19),patent ductus arteriosus that require medication closure (1/18 vs.7/19)and incidence of second dose of pulmonary surfactant (2/18 vs.9/19) in recruitment maneuver group were significantly lower than control group (P < 0.05).While the complication of air leak,necrotizing enteritis,Ⅲ-V grade intracranial hemorrhage showed no significant differences between the two groups (P > 0.05).Conclusion Recruitment maneuvers combined with PRVC in treatment of severe RDS premature infants can improve recovery rate and oxygenation.It can also shorten the duration of oxygen therapy,ventilation and hospital stay.It can reduce the incidence of bronchopulmonary dysplasia and retinopathy of premature.It is worth spreading in clinical practice.reduce the incidence of bronchopuhmonary dysplasia and retinopathy.It is worthy of promotion.
6.Predictive value of Downes score on respiratory failure in term neonates

Objective To explore the clinical significance of Downes score in predicting respiratory failure (RF) in term neonates.Methods From June 1, 2011 to December 31, 2013, 243 hospitalized term neonates with dyspnea, who were born at 37 to 42 gestational age and admitted to the hospital within 24 h after birth, were selected from Neonatal Medical Center of Huai'an Maternity and Child Healthcare.Downes scores were evaluated based on five indicators including respiratory rate, skin color, three depressions sign, moaning and lung breath sounds at the time of admission.RF was diagnosed based on clinical menifestations and radial arterial blood gas analysis.Receiver operating characteristic (ROC) curve analysis was performed to evaluate the predictive value of Downes score on RF.Results The average Downes score of the 243 cases was 6.57±0.86 and the RF rate was 69.6% (169/243).The RF rate was 93.4% (169/181) when Downes score ≥ 4.5, with the sensitivity, specificity, misdiagnostic rate and missed diagnositic rate of 100.0%, 83.8%, 16.2% and 0.0%, respectively.While, the RF rate was 80.7% (130/161) when Downes score ≥ 5.5, with the sensitivity, specificity, misdiagnostic rate and missed diagnositic rate of 76.9%, 58.1%, 41.9% and 23.1%, respectively.When Downes score ≥ 6.5, the incidence of RF was 97.6% (123/126), with the sensitivity, specificity, misdiagnositic rate and missed diagnositic rate of 72.8%, 95.9%, 4.1% and 27.2%, respectively.The cutoff value was 6.5 according to the Youden Index, which with lower rate of misdiagnosis and missed diagnosis.When Downes score ≥ 7.5, the incidence of RF was 100.0%(22/22), with the sensitivity, specificity, misdiagnositic rate and missed diagnosis rate of 13.1%, 100.0%, 0.0% and 87.0%, respectively.The area under the ROC curve was 0.906 (95%CI: 0.866-0.955).The higher the Downes score, the greater the possibility of RF (P=0.000).Conclusions Dynamic Downes scores in term neonates with dyspnea could predict the risk of RF.The higher the Downes score, the greater the possibility of RF.
7.Early differential diagnosis between COVID-19 and mycoplasma pneumonia with chest CT scan.

OBJECTIVE:
To early differentiate between coronavirus disease 2019 (COVID-19) and adult mycoplasma pneumonia with chest CT scan.
METHODS:
Twenty-six patients with COVID-19 and 21 patients with adult mycoplasma pneumonia confirmed with RT-PCR test were enrolled from Zibo First Hospital and Lanshan People's Hospital during December 1st 2019 and March 14th 2020. The early chest CT manifestations were analyzed and compared between the two groups.
RESULTS:
The interstitial changes with ground glass density shadow (GGO) were similar in two groups during first chest CT examination (>0.05). There were more lung lobes involved on the first chest CT in COVID-19 patients, which were mostly distributed in the dorsal outer zone (23/26, 88.5%), and nearly half of them (12/26, 46.2%) were accompanied by crazy-paving sign; while the lesions in adult mycoplasma pneumonia patients were mostly distributed along the bronchi, and the bronchial wall was thickened (19/21, 90.5%), accompanied with tree buds / fog signs (19/21, 90.5%). The above CT signs were significantly different between the two kinds of pneumonia (all <0.01). COVID-19 had a longer course compared with mycoplasma pneumonia, the disease peaks of COVID-19 patients was on day (10.5±3.8), while the disease on CT was almost absorbed on day (7.9±2.2) in adult mycoplasma pneumonia. The length of hospital stay in COVID-19 patients was significantly longer than that of mycoplasma pneumonia patients [(19.5±4.3) d vs (7.9±2.2) d, <0.01].
CONCLUSIONS
The lesions of adult mycoplasma pneumonia are mostly distributed along the bronchi with tree buds/fog signs, while the lesions of COVID-19 are mainly distributed in the dorsal outer zone accompanied by crazy-paving sign, which can early distinguish two diseases.

8.Preparation of high immunity yolk antibody against Vibrio parahemolyticus and comparison of effectiveness between different extraction methods

Objective:To compare the effectiveness between three methods for purifying the immunoglobulin of egg yolk(IgY) which are polyethylene glycol (PEG) method, chloroform extraction method and chloroform / PEG method, and to provide basis for obtaining the batch of IgY.Methods:The inactivated vaccine of Vibrio parahemolyticus (V. parahemolyticus) was prepared and the hens were immunized by multi-point intramuscular injection.The eggs were collected and the IgY was purified by PEG method, chloroform extraction method and chloroform/PEG method.The protein extraction rate, the IgY titer and the purity of the antibody which purified by different methods were detected.Furthermore, the operation process, cost and safety of the three methods were analyzed.Results:The protein contents of the extraction belonging three methods from high to low in turn were chloroform extraction method, chloroform/PEG method, and PEG method.There was no significant difference in the antibody titer between three methods, and the tiler of chloroform extraction method was slightly high.The purities of purified antibody from high to low in turn were PEG method, chloroform/PEG method and chloroform method.The PEG method had better security but relatively lower extraction efficiency and higher cost.The chloroform/PEG method had high extraction efficiency and good antibody purity.Conclusion:The PEG method is suitable for a small amount of extraction in the laboratory.The chloroform/PEG method is appropriate for extracting the high quality IgY in a batch as it has high extraction efficiency and good antibody purity.
9.Determination of iodine in workplace air by inductively coupled plasma mass spectrometry

Objective To establish a solvent desorption inductively coupled plasma-mass spectrometry (ICP-MS) method for determination of iodine in workplace air. Methods Iodine in workplace air was collected with alkaline activated carbon tube and desorbed with 10.0 mL pure water or 20 mmol/L sodium bicarbonate solution. Rhenium-185 was used as an internal standard for quantification. The sample was determined in standard mode and kinetic energy discrimination collision (KED) mode by ICP-MS. Results In standard mode, iodine showed a good linear range in the concentration of 9.0 to 1 100.0 μg/L, with a correlation coefficient of 0.999 3 and a detection limit of 2.7 μg/L. In KED mode, iodine showed a good linear range in the concentration of 24.3 to 800.0 μg/L, with a correlation coefficient of 0.999 1 and a detection limit of 7.3 μg/L. The average desorption efficiency using pure water ranged from 99.1% to 106.7%, with within-run relative standard deviation (RSD) of 3.1% to 8.0% and between-run RSD of 4.9% to 9.3%. The average desorption efficiency using sodium bicarbonate solution ranged from 96.5% to 105.3%, with within-run RSD of 4.9% to 8.6% and between-run RSD of 2.5% to 9.9%. There were no statistical significant differences in the main effects of desorption solution, ICP-MS detection mode, their interaction on average desorption efficiency and within-run RSD (all P>0.05). Samples could be stored at room temperature for at least 7 days. Conclusion This method is highly sensitive, accurate, and suitable for the determination of iodine in workplace air. The sample pretreatment is simple and rapid.
10.Synthesis and biosafety evaluation of four acryl acetyl glucosamine with cell membrane binding ability

Objective To synthesize and characterize four acryl acetyl glucosamine (DA-NAG), and to determine its biocompatibility and cell membrane binding properties, so as to provide basis for its application in medical self agglutination gels. Methods DA-NAG was synthesized by esterification reaction. The products were characterized by mass spectrometry and hydrogen spectrum. Cytotoxicity test and subcutaneous implantation test were performed on the synthesized DA-NAG. The binding properties of DA-NAG to mouse fibroblast L929 cell membrane were detected using high performance liquid chromatography (HPLC). Results The characterization of mass spectrum and hydrogen spectrum are consistent with the characteristics of DA-NAG. The product has no cytotoxicity, and the subcutaneous implantation shows that the DA-NAG can be degraded at 4 weeks without obvious stimulation to the surrounding tissues. The result of HPLC shows the binding effect between the DA-NAG and cell membrane. Conclusions DA-NAG is successfully synthesized, and it has good cytocompatibility and binding ability to cell membrane.
