Objective: To investigate the factors influencing the avascular necrosis of femoral head after internal fixation of femoral neck fracture. Methods: A total of 99 patients with femoral neck fracture, who were treated with internal fixation between May 1999 and April 2004, were followed up in outpatient Department and by telephone. The data was set up and analyzed by SPSS 14.0 software. Results: Until January 2006, avascular necrosis of the femoral head was found in 15 cases (15.2%). Age X internal fixation,gender X the time of reduction were found interactive in two factors interactive analysis. Age, the displacement of fractures,the quality of reduction,and internal fixation were found to be significant factors in single factor logistic regression analysis. Age, age X internal fixation, and the quality of reduction were found to be significant factors in multivariate logistic regression analysis. In fine analysis of interactive after multivariate logistic regression analysis,the removal of internal fixation was the most important risk factor for femoral head necrosis in 56-85yrs group, with the risk ratio being 26.00(95% CI = 3.076-219.747). Conclusion: Elder patients have less chance for avascular necrosis of femoral head. The extraction of cannulated screws might lead to more avascular necrosis of femoral head and collapse in old patients. Dissection reduction can help to reduce avascular necrosis.

Objective To investigate the effects of essential trace elements selenium, zinc, copper, iron,cobalt, chromium and molybdenum upon arsenic poisoning caused by coal-burning. Methods Stratified random sampling method was used to conduct epidemiological investigation on 139 arsenic exposed residents(including nonpatient, light, moderate and severe patients) in an area polluted by coal-burning arsenic in Xingren county of Guizhou province as exposure group. Control group included 34 residents who lived about 13 km away from the endemic area of arsenic contamination. Inductively coupled plasma-optical emission spectrometry (ICP-OES) was used to analyze arsenic, selenium, zinc, copper, iron, cobalt, chromium and molybdenum in coal, soil, rice,corn, chilli, hair, blood and urine. Results Arsenic content in coal, soil, corn and chilli of polluted area were 4.894,146.551,0.522,1.440 mg/kg, respectively. These arsenic content were significantly higher than those in control area which were 1.980,50.167,0.296,0.948 mg/kg, respectively(P ＜ 0.05 or ＜ 0.01) . The content of selenium in soil of the diseased area(5.038 mg/kg) was significantly lower than that in soil of control area(8.948 mg/kg, P ＜0.05). The content of copper, iron, chromium in soil and iron in corn were 44.114,5731.500,98.323,89.996 mg/kg, respectively. These elements content were significantly higher than those in control area which were 13.473,1298.430,36.839,57.391 mg/kg, respectively (all P ＜ 0.05) . Hair and urine arsenic levels were 1.985mg/kg and 149.593 μg/g Cr in exposed group, respectively. These arsenic levels were significantly higher than those in control group which were 0.670 mg/kg and 49.853 μg/g Cr, respectively(all P ＜ 0.01) . Hair selenium level in exposed group(1.706 mg/kg) was significantly lower than that in control group(2.405 mg/kg, P ＜ 0.01). Hair levels of iron and chromium, blood level of eopper and the ratio between copper and zinc in exposed group were 88.295,8.933 mg/kg, 1.053 mg/L and 0.074, respectively. These element levels and elements ratio were significantly higher than those in control group which were 47.970,4.099 mg/kg, 0.934 mg/L and 0.065, respectively(P ＜ 0.01 or ＜ 0.05). Hair selenium level was negatively correlated with the progression of arsenism(r = - 0.414, P ＜ 0.01) .Hair levels of iron and chromium, the ratio between copper and zinc in blood were positively correlated with the progression of arsenism(r = 0.271,0.261,0.250, all P ＜ 0.01) . Conclusions Low selenium, high copper, high iron and high chromium coexists in arsenic polluted area. In exposed group, hair selenium is low, hair iron and chromium, blood copper and ratio between copper and zinc are high. These element changes with environment trend.These element changes are associated with the occurrence and development of the disease caused by coal-burning.

Hepatocyte growth factor (HGF) is a pleiotropic cytokine, its roles in the physiology and pathology of immune system, have been investigated thoroughly, great deal of data have been documented on its immunoregulatory activity. In this review, according to advance of study on HGF in recent years, the role of HGF in the immune regulation, such as immunoregulatory effects of HGF on T lymphocytes, B lymphocytes and dendritic cell, modulation of HGF on specific humoral and cellular immune response, control of acute GVHD and acceleration of myeloid and immunologic reconstitution in allogenetic bone marrow transplantation models, promotion of tissue repair and regeneration, and alleviation of immune rejection in allogeneic organ transplantation including the heart, liver and kidney transplantation, prevention of grafts from injury as well as applicably useful of HGF in the therapy of autoimmune disorders were summarized.

OBJECTIVETo investigate the expressions of vascular endothelial growth factors (VEGF)-A, -C and -D and their prognostic significance and relation to angio- and lymphangiogenesis in gastric cancer.

METHODSThe expression of VEGF-A, -C and -D in 123 primary gastric cancers was detected by immunohistochemical staining. The lymphatic vessel density (LVD) and microvessel density (MVD) were assessed after immunohistochemical double-staining with D2-40 and CD34, respectively. The correlation between the expression of those VEGF factors and clinicopathological parameters were analyzed by univariate method. The overall survival was evaluated by Kaplan-Meier method and log-rank test. Multivariate analysis was carried out using Cox proportion hazard model.

RESULTSThe positive expression rate of VEGF-A, -C and -D in primary gastric cancer samples were 64.2%, 65.9% and 41.5%, respectively. High expression of VEGF-A, or -C or -D, or any two of them was correlated with high LVD (P < 0.05). High expression of both VEGF-A and -C was associated with high MVD, lymph node metastasis, LVI and MVI (P < 0.05). Both VEGF-C and -D high expression was correlated with LVI and lymph node metastasis (P < 0.05). The patients with high expression of these factors had a statistically shorter overall survival (P < 0.05). The patients with both VEGF-A and -C expression had the shortest survival (56 months). Multivariate analysis showed that VEGF-A high expression, MVD, lymph node metastasis and depth of tumor invasion were independent survival predictors (P = 0.033, 0.002, 0.019 and P < 0.001, respectively).

CONCLUSIONHigh expression of both VEGF-A and -C imply high potential of lymphangiogenesis, metastasis and poorer survival in gastric cancer patients. High expression of VEGF-C and -D may induce lymphangiogenesis and promote lymph node metastasis, but only VEGF-A is an independent predictor of survival.

This study was undertaken to investigate the antitumor activity of rhG-CSF mobilized blood mononuclear cells (G-PBMC) from normal donor activated by rhIL-2 alone or in combination with rhIL-12 in vitro. LDH release assay was used to determine the cytotoxicity activity of G-PBMC against K562 (NK-sensitive) and Raji cells (NK-resistant). The phenotype of the activated G-PBMC was assayed by flow cytometry. Results suggested that the cytotoxicity of the fresh G-PBMC was low before IL-2/IL-12 stimulation. G-PBMC developed marked cytotoxicity after 24 hours of IL-2 activation, with more significant increase for further 48 hours induction. When G-PBMCs were exposed to combination of IL-2 and IL-12, the cytotoxicity was significantly enhanced. The proportions of CD3(+) T cells and CD8(+) T cells were increased when G-PBMC incubated with IL-2 for 72 hours. However, CD56(+) cells were significantly elevated when G-PBMC exposed to IL-2 and IL-12 for 7 days. It is concluded that G-PBMC activated by IL-2 had evident antitumor activity, which was further increased when IL-2 in combination with IL-12. These results demonstrate that IL-2 and IL-12 treatment in vitro might promote the graft-versus-leukemia (GVL) response of G-PBMC.

BACKGROUNDThe anterior cruciate ligament (ACL) is one of the most commonly injured knee ligaments. Even following ACL reconstruction, significant articular cartilage degeneration can be observed and most patients suffer from premature osteoarthritis. Articular cartilage degeneration and osteoarthritis development after ACL injury are regarded as progressive process that are affected by cyclic loading during frequently performed low-intensity daily activities. The purpose of this study was to perform a meta analysis on studies assessing the effects of ACL reconstruction on kinematics, kinetics and proprioception of knee during level walking.

METHODSThis meta analysis was conducted according to the methodological guidelines outlined by the Cochrane Collaboration. An electronic search of the literature was performed and all trials published between January 1966 and July 2010 comparing gait and proprioception of a reconstructed-ACL group with an intact-ACL group were pooled for this review. Thirteen studies were included in the final meta analysis.

RESULTSThere was no significant difference in step length, walking speed, maximum knee flexion angle during loading response, joint position sense and threshold to detect passive motion between the reconstructed-ACL group and the intact-ACL group (P > 0.05). However, there was a significant difference in peak knee flexion angle, maximum angular knee flexion excursion during stance, peak knee flexion moment during walking and maximum external tibial rotation angle throughout the gait cycle between the reconstructed-ACL group and the intact-ACL group (P < 0.05).

CONCLUSIONSStep length, walking speed, maximum knee flexion angle during loading response, joint position sense and threshold to detect passive motion usually observed with ACL deficiency were restored after the ACL reconstruction and rehabilitation, but no significant improvements were observed for peak knee flexion angle, maximum angular knee flexion excursion during stance, peak knee flexion moment during walking and maximum external tibial rotation angle throughout the gait cycle.

OBJECTIVETo investigate cyclooxygenase-2 (COX-2) expression and its relationship with mismatch repair (MMR) protein expression and microsatellite instability (MSI) in hereditary nonpolyposis colorectal cancer (HNPCC).

METHODSA total of 28 cases of colorectal adenoma and 14 cases of colorectal carcinoma were collected between July 2003 and July 2007 from 33 HNPCC families. Sporadic colorectal adenoma (n=32) and carcinoma patients (n=24) served as controls. With samples of tumor tissues and normal colonic mucosa collected from the patients, the protein expressions of COX-2 and MMR (hMLH1, hMSH2, and hMSH6) were examined with immunohistochemical assay. Frequency of MSI in five standard MSI loci BAT25, BAT26, D2S123, D5S346, and D17S250 were analyzed by means of polymerase chain reaction.

RESULTSThe rate of COX-2 high-expression was 53.6% (15/28) and 42.9% (6/14) in HNPCC adenoma and carcinoma; 62.5% (20/32) and 91.7% (22/24) in sporadic adenoma and carcinoma, respectively. That rate was lower in HNPCC carcinoma than in sporadic carcinoma (Pü0.05). MMR-deletion rate and percentage of high-frequency MSI (MSI-H) in HNPCC carcinoma were higher than those in sporadic colorectal carcinoma [both 71.4% (10/14) vs. 12.5% (3/24), both Pü0.01]. Among the 10 MMR-deficient HNPCC carcinoma patients, COX-2 low-expression was observed in 8 cases (80.0%), while COX-2 high-expression was observed in all of the 4 MMR-positive HNPCC carcinoma cases (Pü0.05). In comparison to MMR positive HNPCC carcinoma, HNPCC adenoma, and sporadic carcinoma, COX-2 expression was significantly lower in corresponding MMR-deficient cases (all Pü0.05). The rates of COX-2 low-expression in HNPCC adenoma, HNPCC carcinoma, and sporadic carcinoma with MSI-H were significantly higher than those in the cases with microsatellite stability (all Pü0.05).

CONCLUSIONCOX-2 is expressed at a low level in HNPCC carcinoma, different from the high COX-2 expression in sporadic carcinoma.

OBJECTIVETo evaluate therapeutic effects for reconstruction of anterior cruciate ligament(ACL)with hamstring tendon autografts and bioabsorbable interference screws fixation under arthroscopy.

METHODSThirty-one patients with ACL rupture were verified through arthroscopy. There were 27 patients were male and 4 patients were female, ranging in age from 17 to 40 years,with an average of 25 years. Among the patients, 26 patients combined with meniscus injuries, 3 patients with injuries of articular cartilage and 16 patients with I to II degree degeneration of articular cartilage. All the patients were performed ACL reconstruction with hamstring tendon autografts under arthroscopy and the reconstructed ligaments were fixed with bioabsorbable interference screws.

RESULTSNo severe complications occurred at early stage after operation. Thirty patients were followed up and ranged from 9 to 39 months,with an average of (19 +/- 9.0) months. Lysholm score significantly increased from average of 54.6 +/- 16.6 preoperatively to average of 92.5 +/- 5.7 at the end of follow-up period (t = 11.84, P < 0.01). Twenty-six patients restored to normal activity.

CONCLUSIONACL reconstructed with hamstring tendon autografts under arthroscopy has advantages of minimal trauma and satisfactory outcomes.

Objective: To observe the effects of manganese( Ⅲ ) meso-tetrakis (N, N'-diethylimidazolium-2-yl) porphyrin (MnTDM) in treatment of early Parkinson's disease(PD) mouse model induced by subcutaneous injection of 1-methyl-4-phenyl1, 2, 3, 6-tetrahydropyridine(MPTP) and to discuss its possible mechanism. Methods:Forty male C57BL/6 mice were evenly randomized into 4 groups: MPTP model group(subcutaneous injection of 25 mg/kg MPTP for 3 days), MnTDM+ MPTP group (15 mg/kg MnTDM was subcutaneously injected 1 h before MPTP injection), MnTDM control group, and normal saline group. Performance of animals in the pole and swimming test was observed 3 days after the last injection. Levels of dopamine (DA) and its metabolites(3,4-dihydroxyphenylacetic acid [DOPAC] and homovanillic acid [HVA]) in the striatum of animals were measured by high-performance liquid chromatography with an electrochemical detector(HPLC-ECD). Thiobarbituric acid (TBA) method was used to examine the levels of malondialdehyde(MDA). Results: Acute injection of MPTP could be used for establishment of PD model. The striatal levels of DA, DOPAC and HVA in MPTP group were significantly lower(P＜0.01)and the striatal level of MDA was significantly higher(P＜0.05) than those of the control group. MPTP had no obvious effect on the behavioral performance of the animals in a short term. MnTDM could partly inhibit the above effects of MPTP. Compared with MPTP group, MnTDM+ MPTP group had significantly higher DA, DOPAC, and HVA levels and significantly lower MDA level(all P＜0.05). There was no significant difference in the behavioral indices of animals between the 4 groups. Conclusion:MnTDM can inhibit lipid peroxidation and promote DA production; it has preventive and therapeutic effects on MPTP induced PD.

Soxhlet extraction is a common method of sample preparation. However, there has been no discussion about the efficiency of Soxhlet extraction from different batches and the factors that cause content fluctuation. In this study, Panax ginseng was selected as a model sample. Soxhlet extraction by means of a water bath, which has always been neglected, was identified as a novel key factor in the poor repeat-ability in different batches of Soxhlet extraction, as it can affect the siphon times and reflux time, which have been positively correlated with the ginsenoside contents. By substituting round bottom flasks in the same column, the relative standard deviation of the most fluctuated compound, ginsenoside Rb1, was decreased from 24.6% to 5.02%. Scanning electron microscopy analysis confirmed that the breakdown of the surface of the ginseng powder in the Soxhlet extraction led to a better dissolution of ginsenosides, indicating that chloroform may promote the extraction of ginsenosides by disrupting the cell structure. Moreover, 70% methanol was regarded as the better solvent for extracting the ginsenosides. Overall, this work offers a practical and effective protocol for improving the accuracy and repeatability of Soxhlet extraction methodology for ginsenosides and other analytes.
