ObjectiveTo observe the clinical effect of unstable femoral intertrochanteric fracture treated with DHS. Methods Summarized 37 cases of unstable femoral intertrochanteric fracture treated by DHS,all cases were commented by fracture type,operation method and clinical effect. ResultsWe have followed up 37 cases from 0.8 to 4.7 years with average of 2.5 years.Conclusions DHS is one of the best implants due to its merits of strong fixation, little complication and early rehabilitation exercise.
2.Epidemiological characteristics of multi-drug resistant tuberculosis and pre-extensively drug resistant tuberculosis in Changsha, 2018-2021

Abstract: Objective To understand the situation of drug-resistant tuberculosis screening and epidemiological characteristics of multi-drug resistant tuberculosis (MDR-TB) and pre-extensively drug resistant tuberculosis (pre-XDR-TB) in Changsha, in order to provide a scientific basis for improving the quality of drug-resistant tuberculosis prevention and control in the city. Methods Demographic information and drug susceptibility date of etiologically positive pulmonary tuberculosis patients in Changsha from 2018 to 2021 were collected, the successful rate of resistance screening, incidence and tendency in MDR-TB and pre-XDR-TB in patients included in this study were statistically analyzed accordingly. Results From 2018 to 2021, the successful screening rates were 86.2%, 87.7%, 81.9% and 71.5% for MDR-TB and 82.2%, 84.8%, 76.9% and 68.2% for pre-XDR-TB, respectively. In each year, MDR-TB patients identified accounted for 7.6% (101/1 222), 6.5%(124/1 774), 6.6%(110/1 555) and 6.3%(99/1 478), and pre-XDR-TB patients identified accounted for 3.6%(46/1 219), 3.8%(69/1 766), 4.4%(69/1 495) and 4.6%(69/1 436), correspondingly. The incidence of MDR-TB showed a slowly downward trend, while the incidence of pre-MDR-TB showed a slowly upward trend, with neither decreasing nor increasing trends being statistically significant ((χ2=1.947,0.806,P>0.050). The incidence of MDR-TB in the retreatment failure population was 66.6% (2/3), and the others, failure initial treatment and recrudescence populations were 23.5% (19/81), 16.7% (2/12) and 15.2% (70/461), respectively. Similar to the incidence above, the incidence of pre-XDR-TB was 16.7% (2/12) among patients who failed in initial treatment, and 12.2% (9/74), 9.8% (43/439), and 4.5% (2/44) among the others, recrudescence and returned patients, respectively. The incidence rates of MDR-TB and pre-XDR-TB in different populations were significantly different (χ2=117.600,59.030,P<0.05). Conclusions There are still areas for improvement in tuberculosis drug resistance surveillance system in Changsha. On the premise of paying attention to patients in retreatment failure, other, initial treatment failure and relapse patients, high sensitivity molecular drug susceptibility testing, and scientifically efficient screening strategies must be explored.
3.Automatic synthesis of N-succinimidyl 4-18F-fluorobenzoate and its utility for 18F labeled C2A donain of synaptotagmin I

Objective To develop and optimize a module for the automatic production of N-succinimidyl-4-[18F] fluorobenzoate (18F-SFB) that is used for further 18F labelling C2A domain of Synaptotagmin Ⅰ . The conjugated compound was applied for detecting the tumor apoptosis in rabbit model after chemotherapy. Methods GE TRACERlab and TRACERlab FXF-N modules were modified and programmed to automatically produce 18 F-SFB which was further analyzed by high performance liquid chromatography (HPLC).C2A-glutathione S transferase (GST) was conjugated with 18F-SFB (18F-FB-C2A-GST) and subsequently purified by HPLC. Two rabbits grafted with VX2 lung cancer were first treated with chemotherapy and then,37 MBq of 18F-FB-C2A-GST was administered via the auricular vein. Serial PET/CT imagings were performed at 0.5, 1 and 2 h post-injection respectively. Tumor apoptosis was examined by pathological study. Results The TRACERlab FXFoG and TRACERlab FXF-N modules were successfully adapted to synthesize18F-SFB, with the radiochenmical yield (76.41 ±4.00)% (n = 10), the corrected yield (45.43 ±5.90 ) % and the radiochemical purity about 95%. The whole procedure for labeling 18 F-SFB was about 87 min.From PET/CT imagings, significant uptake was found in the tumor after chemotherapy, but no obvious up-take was found in heart, lungs and liver. HE staining demonstrated large number of apoptotic bodies within the tumor tissues. Conclusions 18 F-SFB can be automatically synthesized. 18F-FB-C2A-GST might be useful for the detection of apoptosis in tumor after chemotherapy.
4.Correlation of serum albumin with short-term functional outcome of acute ischemic stroke

Objective No consensus has yet been achieved on the relationship of serum albumin with the functional out-come of acute ischemic stroke.The aim of our study was to determine whether the serum albumin level was associated with the short-term functional outcome of acute ischemic stroke in well-nourished patients. Methods Totally, 113 patients with first-ever acute ischemic stroke were recruited from Nanjing Stroke Registration Program between January and June 2015.Baseline data including de-mographic and body parameters, vascular risk factors, and laboratory results were collected.The NIH Stroke Scale ( NIHSS) was used to evaluate the severity of neurological deficits and the modified Rankin Scale ( mRS ) employed to assess the short-term functional outcome.According to the mRS at discharge, the patients were divided into a good outcome group ( mRS<3 ) and a poor out-come group ( mRS≥3 ) .The independent predictors of the short-term functional outcome were evaluated by multivariate logistic regression analysis. Results Of the 113 acute ischemic stroke patients included, 52 (46.0%) were in the good outcome group, and 61 (54.0%) in the poor outcome group.Those in the former group had a significantly higher BMI, lower serum LDL-C, lower WBC count, and lower NIHSS at admission than those in the latter .Multivariate logistic regression analysis showed that low serum albumin, NIHSS at admission, and arteriole occlusion were independent predictors of the poor short-term functional outcome ( OR=0.684, 95% CI:0.490-0.956, P=0.026). Conclusion Low serum albumin is an independent predictor of poor short-term functional outcome in acute ischemic stroke patients in well-nourished status.
5.Advances in antiviral research of adaptor-associated protein kinase 1 (AAK1) inhibitors

As one of the major sources of infection, viruses could infect all organisms including bacteria, plants, animals, and humans. Infectious diseases caused by viruses pose a great threat and damage to human health and economic activities all over the world. Adaptor-associated protein kinase 1 (AAK1) is a member of the Ark1/Prk1 family of serine/threonine kinases and a specific key kinase regulating the phosphorylation of AP-2 protein μ2 subunit T156. In the past, AAK1 has been regarded as a feasible biological target for the treatment of nerve pain. Recently, scientists have found that inhibiting AAK1 can regulate endocytosis and inhibit virus invasion into cells. Therefore, AAK1 could be the potential target of anti-virus therapy. This paper reviews the research progress of small molecule AAK1 inhibitors in the field of antiviral, analyzes the future research directions and challenges, and provides new ideas for the development of antiviral drugs targeting AAK1.
6.Progress on Detection of Metals Ions by Functional Nucleic Acids Biosensor

Functional nucleic acids are natural or artificialnucleic acid sequences with specific functions and special structures. Special metal ions are essential trace elements for human health, but excessive metal ions will be harmful to human health. The functional nucleic acids are widely used in the detection of metal ions because of their advantages such as easy modification, low price, high stability and strong specificity. This review describes the function of functional nucleic acid and metal ions, mainly including cutting type, link type, metal ion-mediated base pairing, click chemical type, conformation change type, and other types. Then, biosensor of functional nucleic acid combined with different signal output is introduced. Finally, the research significance and problems of functional nucleic acid in metal ion detection are discussed, and the future development direction and application prospect of functional nucleic acid biosensor are prospected.
7.Effect of morphine postconditioning on myocardial ischemia-reperfusion injury in rabbits.

OBJECTIVETo investigate the protective effects of morphine postconditioning on myocardial ischemia-reperfusion (I/R)injury and the potential mechanisms in rabbits.
METHODSThirty-two New Zealand male white rabbits were randomly assigned into 4 groups: Group 1 (Sham), Group 2 (I/R), Group 3 (ischemic postconditioning), Group 4 (ischemia and morphine postconditioning). Group 1 was perfused for 160 min; Group 2 underwent 40 min ischemia and 120 min reperfusion; Group 3 underwent three cycles of 30 s reperfusion and 30 s left anterior descending coronary artery re-occlusion immediately after 40 min ischemia and before 120 min reperfusion; Group 4 was given morphine 1.0 mg/kg immediately after 40 min ischemia in 1 min and before 120 min reperfusion. Blood samples were taken from arterial line at 20 min before occlusion, 20 min after occlusion, 40 min after occlusion, 1 h after reperfusion and 2 h after reperfusion for determination of the plasma levels of cardiac troponin I (cTnI). At the end of the reperfusion, infarct size (IS) and area at risk were defined by Evans and TTC staining. Plasma SOD activity and MDA were determined at the end of reperfusion.
RESULTThe levels of cTnI were significantly lower during reperfusion in the two postconditioning groups than those in I/R group. The plasma MDA content was significantly lower and SOD activity was significantly higher in the two postconditioning groups than those in I/R group, but there was no difference between two postconditioning groups. Morphine significantly reduced infarct size of the left ventricular area at risk as compared with I/R group (P<0.05).
CONCLUSIONMorphine postconditioning is as effective as ischemic postconditioning in the protection of myocardium against I/R injury in rabbits. Decrease in oxygen free radicals and increased antioxidant activity might be involved in its mechanism.

8.Experimental study of the effect of deferasirox on the micro-angiogenesis in narrow pedicle flap through epithelial-mesenchymal transition.

OBJECTIVETo investigate the effect of Deferasirox on the micro-angiogenesis in narrow pedicle flap through Epithelial-Mesenchymal Transition.
METHODS32 male rats were randomly divided into group I and II which were subdivided into Ia and Ib, IIa and IIb, 8 rats in each group. The rats were administrated intragastrically for 7 days with Deferasirox 100 mg/kg in group Ia and IIa, with the same dose of N. S. in group Ib and IIb. After that, narrow pedicle flaps were formed on the rats back. In group I, the subcutaneous vascular network was observed intraoperatively. The flap survival rate was recorded. In group II , specimens were collected at the distal end of flaps 3 days after operation. IHC and Western Blot were done to examine the expression of CD34, E-cadherin, Vimentin. The microvessel density was also calculated.
RESULTSThe subcutaneous micro-angiogenesis in group Ia was more exuberant than that in group Ib. The narrow pedicle flaps in group Ia survived completely, while the survival rate was 62.5% in group Ib (P < 0.05). The percentage of flap survival area for Ia and Ib was (100 +/- 0.00) % and (84.06 +/- 4.42)% (P < 0.05). The expression of E-cadherin in IIa was lower than that in IIb, while the expression of Vimentin and CD34 were higher in IIa, showing statistically difference (P < 0.05).
CONCLUSIONDeferasirox can improve the flap micro-angiogenesis through inducing epithelial-mesenchymal transition, so as to improve the survival rate of narrow pedicle flap.

9.Antimicrobial susceptibility pattern and epidemiology of blood stream infections in China, 2012

Objective To investigate the distribution and changing pa-ttern of susceptibility in bloodstream infection.Methods The blood cul-ture isolates from 557 tertiary hospitals and 232 second class hospitals were routinely isolated and identified.Antimicrobial susceptibility of these isolates were tested routinely.Results A total of 76483 strains were isolated from blood specimens in the tertiary hospitals during 2012.Gram-positive cocci and gram-negative bacilli accounted for 44.35%and 55.65%, respectively.A total of 6261 strains were isolated from blood specimens in the second-class hospitals during 2012.Gram-po-sitive cocci and gram -negative bacilli accounted for 48.89% and 51.11%, respectively.The most frequent gram-negative bacteria from the tertiary hospitals and the second -class hospitals were Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Acinetobacter baumannii and Enterobacter cloacae, the most fre-quent gram-positive bacteria were Coagulase-negative Staphylococus, Staphylococus aureus, Enterococcus faecium and Enterococcus faecalis.700 strains of Streptococcus pneumoniae ( 0.8%) , 28 strains of Hemophilus influenza (<0.01%) and 152 strains ofβ-hemolytic Streptococcus(0.18%) were isolated.Prevalence of methicillin-resistant Staphylococcus aureus ( MRSA) and methicillin-resistant coagulase negative staphylococcus( MRSCN) among Staphy-lococus aureus and Coagulase-negative Staphylococus were 44.4%and 78.0% in the tertiary hospitals,but the ratios were 32.5%and 71.4%in the second-class hospitals, respectively.All the isolates were sensitive to Vancomycin and Linezolid in the Staphylococcus spp.The sensitivity rates of Staphylococcus spp to Amikacin, Rifampicin and Mino-cycline were high ( >80.0%).The sensitivity rates of Enterococcus faecalis to Pencillin and Ampicillin were high (>80.0%).Escherichia coli was sensitive to not only Ampicillin/Sulbactam and Piperacillin/tazobactam (>90.0%) , but also Amikacin and Cefoxitin ( about 80.0%).Pseudomonas aeruginosa was sensitive to all of the common antibiotics almost.But it was just the opposite to Acinetobacter baumannii.Polymyxin B and Cefoperazone/Sulbactam were sensitive to Acinetobacter baumannii in the tertiary hospitals and the second-class hospitals, respec-tively(97.6%and 76.7%).Conclusion The datas of 2012 indicated that gram-negative bacilli, especially Esche-richia coli, played an important role in blood stream infections.A large number of Coagulase-negative Staphylococus, Staphylococus aureus were isolated, so we should carry out steps according to the standard criterion strictly to reduce the contamination rate.Because of different results to the common antibiotics for the different microorganism, antibiotics should be prescribed according to bacterial resistance results reasonably.
