OBJECTIVETo compare the efficacy of two surgical techniques for controllong nasal width after Le Fort I osteotomy.

METHODSFifty-five patients who received the Le Fort I osteotomy have been included in this study. They were randomly divided into 2 groups. The experimental group received extraoral ABS, and the control group received traditional intraoral ABS. 3D photos of the patient's face were taken before operation and at postoperative 3 months. Alar width was measured on the 3D photos. Data was reported as means and standard deviations, and statistic analysis was done by using student t test.

RESULTSCompared with presurgical data, G. lat-G. lat increased by (2.66 +/- 1.47) mm, Al-Al increased by (2.20 +/- 1.22) mm and Sbal-Sbal increased by (1.30 +/- 1.33) mm in experimental group. G. lat-G. lat increased by (1.38 +/- 1.29) mm, Al-Al increased by (1.06 +/- 0.95) mm and Sbal-Sbal increased by (0.36 +/- 1.33) mm in the control group. There was significant difference between two groups.

CONCLUSIONSThe surgical technique of ABS is the most important factor for determining the postoperative alar width. Both techniques have better effect on the Sbal-Sbal width control than the G. lat-G. lat and Al-Al width control. Traditional intraoral ABS can more effectively control the alar width. Both techniques cannot completely control the alar base widening after Le Fort I osteotomy.

Objective To discuss the technical specifications of current high-frequency electrotome detection,to avoid the hidden danger of high-frequency electrotome power detectors,and to measure the leakage current of different kinds of highfrequency electrotome accurately.Methods The power and leakage current of the high frequency electrotome were measured by FLUCK QA-ES Ⅱ high frequency electrotome analyzer.The safety of the two methods was compared before and after the improvement of the power measurement.Four parameters of leakage current were repeatedly measured with the ways of high frequency earthing and high frequency isolation respectively.The maximum measurement of leakage current was recorded.Results The improved connection method was safe in the power measurement.For the high-frequency electrotome in the model of high frequency earthing,the values of leakage current were restrained within the range of error with two ways of monopolar loading operation electrode and neutral electrode.For the high-frequency electrotome in the model of high frequency isolation,the values of leakage current were limited within the range of error withtwo ways of monopolar empty operation electrode and neutral electrode.Conclusion The improved high-frequency electrotome power detection method is safe for detectors.The data obtained from the leakage current detection method using the national standard correction method reflect the actual state of the high-frequency electrotome,when the electrotome with earth as the reference is used to detect the leakage current with loading or the insulated electrotome is applied to measuring the leakage current with no loading.

The two-dimensional model of cell culture is an important method in the study of testicular development and spermatogenesis but can not effectively mimic and regulate the testicular microenvironment and the whole process of spermatogenesis due to the lack of relevant cell factors and the disruption of a three-dimensional spatial structure. In the past 20 years, the development and optimization of the in vitro model such as testis organotypic culture and in vivo model such as testis transplantation achieved a transformation from two- to three-dimension. The maintenance and optimization of the testicular niche structure could mimic the testicular microenvironment and cell types including Leydig, Sertoli and germ cells, which showed similar biological behaviors to those in vivo. Besides, the cell suspension or tissue fragment floats in the gas-liquid interface so that the development of somatic and germ cells is well maintained in vitro whilst the feedback linkage between grafted testis tissue and hypothalamus-pituitary of the host rebuilt in the in vitro model provides an endocrinological basis for spermatogenesis, which serves as an effective methodology to better understand the organogenesis and development of the testis as well as testicular function regulation, advancing the concept of treatment of male infertility. Al- though each of the methods may have its limitations, the progress in the processing, freezing, thawing, and transplantation of cells and tissues will surely promote their clinical application and present their value in translational medicine.

Objective To investigate the MRI and echocardiography manifestations of noncompaction of ventricular myocardium(NVM)and assess the role of MRI in the diagnosis of NVM by comparing it with echocardiography.Methods Fourteen cases of NVM diagnosed by echocardiography were examined with MRI,including scanning of black-blood sequences,double inversion recovery fast spin echo (DIRFSE)and triple inversion recovery fast spin echo(TIRFSE),and white blood sequence:fast imaging employ steady state acquisition(FIESTA).Scanning plane includes short axis view,four-chamber view and long axis view.Results Both MRI and echocardiography displayed involvement of left ventricles in thirteen cases and involvement of double ventricles in one case.Apexes of heart and the intermedius are commonly affected.MRI showed 54 segments and echocardiography showed 53 segments affected,and there is no significant difference between the capability of MRI and echocardiography(P=1.000).The affected myocardium consisted of two layers:subendoeardial noncompacted myocardium and epicardial compacted myocardium,and the ratio measurement of N/C by MRI was 3.37?0.89 and it was 3.19?0.82 by echocardiography.Noncompacted myocardium was characterized by prominent and excessive myocardial trabeculations and deep intratrabecular recesses,in which the blood flow was communicated with the ventricle.One case was complicated with ventricular aneurysm,and coronary arteriography was performed with unremarkable findings.One case underwent heart transplantation because of progressive heart failure, Gross findings demonstrated prominent muscular trabeculations with deep intratrabecular recesses,which coincided well with MRI findings.Conclusion The MRI manifestation of NVM is characteristic,and MRI with multiple series and planes is helpful in the diagnose of NVM.Compared with echoeardiography,MRI could display the pathological cardiac muscle more clearly,because of its high soft-tissue resolution and spatial resolution.

Objective To design and implement an automatic analysis and evaluation system for the image parameters of medical MRI quality testing.Methods The system was developed and debugged by the study on MRI image quality parameters,the image denoising,integration,extraction and etc by MATLAB processing platform as well as the comparison and comparative calculation of the obtained data.Results The system replaced manual operation by auto processing and parameters analysis of MRI quality inspection image.Conclusion The system enhances the efficiency and avoids artificial error,and has a promising prospect in the future.

OBJECTIVETo study the application of distraction osteogenesis (DO) of the mandibular ramus in hemifacial microsomia.

METHODSFrom 1999 to 2006, 15 patients with hemifacial microsomia were treated with DO of the ramus, including 12 cases of children and juveniles, 3 cases of grown-ups. Maxillary Le Fort I osteotomy, unilateral sagittal split ramus osteotomy, genioplasty and free scapular flaps were selected to perform on the first or secondary stage of treatment.

RESULTSSuccessful distraction was achieved in all patients with an average distraction distance of 21.25 mm. The distractor was removed after consolidation proved by clinical examination and X-ray. Facial appearance and and occlusal relationship were improved greatly. The occlusal plane was almost normal. There was no infection, malunion or permanent injury of inferior alveolar nerve.

CONCLUSIONSDO of the ramus is very effective for the correction of severe hemifacial microsomia. It has much better therapeutic effect than traditional orthognathic procedures.

OBJECTIVETo investigate the intra-vaginal ejaculation latency time (IELT) of varicocele patients, the influence of spermatic vein ligation on IELT, and the relationship of Visual Analogue Score (VAS) with IELT.

METHODSWe selected 112 males who had regular sexual life after spermatic vein ligation and conducted follow-up visits for 6 months. According to preoperative IELT, we divided the patients into an IELT < or = 2 min group and an IELT > 2 min group, and compared their IELT, VAS and Chinese Index of Sexual Function for Premature Ejaculation-5 (CIPE-5) scores before and 6 months after operation.

RESULTSFollow-up was accomplished in 81 of the patients, 18 in the IELT < or = 2 min group and 63 in the IELT >2 min group. Compared with the baseline, IELT was significantly prolonged postoperatively in both the IELT < or = 2 min group ([1.26 +/- 0.37] vs [4.53 +/- 1.69] min, P < 0.01) and the IELT >2 min group ([5.14 +/- 2.03] vs [7.69 +/- 4.51] min, P < 0.05); the postoperative CIPE-5 scores were remarkably improved in the former (11.27 +/- 3.52 vs 15.64 +/- 2.37, P < 0.05) but insignificantly in the latter group (20.42 +/- 4.65 vs 21.83 +/- 5.49, P > 0.05); the postoperative grades of the CIPE-5 scores showed significant differences in both groups (chi2 = 6.353, P = 0.042 and chi2 = 3.910, P = 0.048); the postoperative VAS was markedly increased (3.18 +/- 0.92 vs 1.56 +/- 0.83 and 3.24 +/- 0.95 vs 1.74 +/- 0.79, P < 0.05), with significant differences in the grades of VAS in both groups (chi2 = 4.433, P = 0.035 and chi2 = 10.088, P = 0.001). The variation of VAS was negatively correlated with that of IELT in both groups (r = -0.572, P < 0.01 and r = -0.465, P < 0.05).

CONCLUSIONVaricocele may be one of the causes of premature ejaculation, and some of the varicocele patients with IELT < or = 2 min may benefit from spermatic vein ligation. Improved VAS is negatively correlated with prolonged IELT. The relationship between varicocele and premature ejaculation deserves further studies.

To study the effect of alpha1-acid glycoprotein 1 (ORM1) polymorphism on the concentration of free nortriptyline in serum, genotyping analysis was employed in ORM1 by sequencing. Eighteen unrelated male adults were chosen and given a single dose of 25 mg nortriptyline orally, then the blood samples were taken at 0, 1, 2, 3, 4, 6, 8, 12, 24, 32, 48, 72, 96 and 168 hours after drug administration. Nortriptyline and 10-OH-nortriptyline in serum and ultrafiltrate were detected for the total and free concentration by using HPLC-MS/MS. Pharmacokinetic parameters were compared among different ORM1 genotypes. No significant differences were shown in the pharmacokinetic parameters of total nortriptyline and 10-OH-nortriptyline. The mean AUC(0-infinity) of free nortritpyline in ORM1 * F/ * F1 subjects was significantly higher than that in ORM1 * F1/ * S and ORM1 * S/ * S subjects [(119.1 +/- 74.4) ng x mL(-1) x h vs (51.4 +/- 23.2) ng x mL(-1) x h and (42.4 +/- 11.6) ng x mL(-1) x h]. The percentage of protein binding in subjects with ORM1 * F1/ * F1 genotype at 2, 3, 4, 6, 8 and 12 h after administration was slightly lower than in those with ORM1 * F1/ * S and ORM1 * S/ * S genotypes while the distinct difference was shown at 4 h (P < 0.05). Different ORM1 genotypes might affect the protein binding percentage and the concentration of serum free nortriptyline. The ability binding to the drug was higher in subjects with ORM1 * S/ * S genotype than in those with other two genotypes, so as to cause the lower concentration of free nortriptyline.

Rab1A is a member of the RAB family as a small GTPase and a mammalian target of rapamycin complex 1 (mTORC1) Activator,which has been well established to mediate vesicular trafficking from the endoplasmic reticulum (ER) to the Golgi apparatus. With increasingly deep research of Rab1A,many researchers discover that Rab1A protein is also involved in mediating signal transduction,cell migration and regulation of autophagy. Meanwhile,aberrant expression of Rab1A has been linked to a range of human diseases as well,including Parkinson's disease,cardiomyopathy,and aspirin-exacerbated respiratory disease. Recently Rab1A research has gradually shifted to its role in occurrence and development of tumor,and researchers also discover that Rab1A was abnor-mally expressed in many malignant tumors such as tonguesquamous carcinoma,breast cancer,human lung cancer,hepatocellular carci-noma,colorectal cancer,gastric cancer and cervical cancer. Besides that,over expression of Rab1A plays a significant role in the pro-gression of different tumors. This article summarizes the research progress of Rab1A intumor development and signaling pathways.

Objective To evaluate tension-free hernia Onlay repair with premuscular positioning of the prosthesis for the treatment of ineisional henia of the abdominal wall.Methods In this study 126 patients with incisional henia were treated with a tension-free manner of hernia repair by using synthetic material ONLAY between September 1999 and June 2007.Results All operations were successful.There was no hospital death or severe postoperative complications.The average age was 58.5 years old ranging from 28 to 89.There were 67 patients in which the abdominal defect ranged from 5～10 cm,and 59 patients with abdominal defect≥10 cm.The mean operating time was 95(70～120)min,and the average intraoperative blood loss was 80 ml(60～250 ml).The mean postoperative hospitalization was 14.5 days(10～28 d). Patients were followed-up from 3 to 96 months,and 3 patients suffered from hernia recurrence(2.38%). Conclusions The ONLAY repair of ineisional hernia of the abdominal wall with synthetic material mesh was a safe procedure,especially for those with large abdominal wall defects.
