2.Analysis of associated factors of ICU delirium after cardiac surgery

Objective To analyze the associated factors of ICU delirium after cardiac surgery,and propose the proper treatment strategy.Methods 124 patients after cardiac surgery were observed.The basic information including gender,age,blood type,respirator assistant time and ICU hospitalizing time was recorded.SPSS software was used for statistical analysis.Results Among the 124 patients,the incidence of ICU delirium was 17%.There was significant difference between patients with ICU delirium and without ICU delirium on age,respirator assistant time and ICU hospitalizing time.The ICU delirium incidence was dramatically different among different blood types,and the incidence of blood type A was much more higher than blood type B and O.Univariate Logistic regression analysis showed that age,mechanical ventilation time,ICU time,gender and blood types were the related factors of delirium in ICU.Conclusions Age,respirator assistant time,ICU hospitalizing time,and blood type are all influencing factors of ICU delirium.Patients who are male,older,or with longer usage of respirator,or with longer ICU hospitalizing time are more likely to develop ICU delirium.The incidence of ICU delirium is quite different among different blood types,blood type A is more easily to occur ICU delirium than blood type O and B.

4.An ecologistic view on the theories and practices of psychotherapy

This article adopts an ecologistic perspective to analyze the regularity of psychological phenomena,examine the mechanisms of psychological problems and mental disorders,and discuss the process of psychological counseling and therapy.The author proposes that the practice of counseling and psychotherapy should be directed with an ecological view.A practitioner needs to not only focus on the problems and symptoms,but also consider the restoration and development of the personality foundation of the individuals who seek for help.A practitioner should not only pay attention to visitors'psychological defects,but also try to assist the development of their positive characters which could alleviate symptoms,and promote the construction of their outer environments that are beneficial for psychological rehabilitation.
5.Development of Positive Perfectionism Questionnaire

Objective: To develop a questionnaire which measures positive perfectionism. Methods: Initial items of the questionnaire were compiled according to theoretical analysis and interviews. 407 college students were recruited to respond to the items. The explorative factor analysis was used to decide the final items and dimensions. Another 309 college students were recruited to respond to the decided questionniares and several criterion questionnaires. Results: The positive perfectionism questionnaire consisted of three dimensions: Positive Self-Expectations(PSEX), Positive Organization (POR), and Positive Self-Evaluations (PSEV). Internal consistency (Cronbach’s ?) of the three dimensions were 0.87、0.76 and 0.79. The two-week test-retest reliabilities were 0.79、0.84 and 0.81. The cumulative explanation of the variance was 50.2%. A confirmative factor analysis revealed that the three-dimension model was proper (?2/df =1.71,GFI=0.89,CFI= 0.92,IFI=0.92,RMSEA=0.048). The Positve Perfectionism Questionnaire was negatively related to anxiety and depression. Conclusion: The positive perfectionism questionnaire has satisfactory psychometric characteristics.
6.Relationship between muscle tension and recovery of upper limbs motor function of stroke patients

ObjectiveTo explore relation of muscle tension on recovery of upper limbs motor function of patients with hemiplegia after stroke.Methods102 hemiplegia patients were divided into 4 groups according to the degree of muscle tension,and treated with routine rehabilitation training. After one month,upper limbs motor ability and activities of daily living (ADL) of patients were assessed.ResultsAfter treatment,upper limbs motor ability and ADL of all patients were raised,but patients with light muscle tension had a better effect compared with those with serious muscle tension (P<0.01).ConclusionExcessive muscle tension will block the motor functional recovery of patients with hemiplegia after stroke.
8.Efficacy and risk factors of carotid artery stenting in patients with unilateral and bi-lateral carotid stenosis

Objective To investigate the efficacy and risk factors of carotid artery stenting ( CAS) in patients with uni-lateral and bilateral carotid stenosis .Methods We retrospectively studied 312 cases of patients with CAS diagnosed in xinqiao Hospital between January 2005 and January 2011 .According to the carotid artery lesions , patients were divided in-to the unilateral carotid stenosis group .Periprocedural data and those after one-month follow-up and one-year follow-up were collected in two groups and analysed by statistics .Results A total of 312 patients were enrolled in this study , including 136 patients with bilateral lesions and 176 patients with unilateral lesion .303 of these patients were followed up after one month, and 224 cases after one year.All the 312 patients were implanted with 391 stents.The two groups of patients with transient ischemic attack ( TIA) were different in the incidence of perioperative TIA (1.2%for bilateral lesions and 7.6%for unilateral lesions;P<0.01).Compared with bilateral carotid lesions group , the incidence of early postoperative adverse events (death, stroke, perioperative TIA,and perioperative myocardial infarction ) in unilateral carotid lesion group was lower (5.2%vs 11.4%), but of no statistical significance (P>0.05).The incidence of medium-term adverse events (death, stroke, perioperative TIA, perioperative myocardial infarction, and restenosis) in the bilateral carotid lesions group was significantly higher than in the unilateral group (21.8%vs 9.7%, P<0.01), mainly driven by a higher inci-dence of death, periprocedural TIA, and restenosis (P<0.05).Multivariate analysis showed that left ventricular dysfunc-tion, masculinity , bilateral carotid artery disease , renal insufficiency , and cerebral ischemic symptoms within the last 6 months before intervention , and low-density lipoprotein cholesterol ( LDL-C ) levels were the independent risk factors for medium-term adverse events .Conclusion Patients with bilateral carotid artery disease treated by CAS are more prone to adverse events than those with unilateral disease .
9.Multivariate analysis of risk factors with placental abruption in preeclampsia

Objective To investigate the risk factors with placental abruption in patients with preeclampsia.Methods Retrospective analysis on medical documents of 219 patients treated in Peking University Third Hospital from Jan.1994 to Dec.2008.Patients were divided into 3 groups, including 52 cases with severe preeclampsia terminated following placental abruption, 130 cases only with severe preeclampsia and 37 cases with unexplained placental abruption without preeclampsia.One hundred and multivariate regression analysis were used to identify the risk factors for placental abruption in patients with preeclampsia.Results (1) When compared with those in control group, univariate analysis showed that gravidity, parity, history of preeclampsia, second and third trimester pregnancy loss, history of autoimmune disease, chronic hypertension disease, lack of regular antenatal care, fetal growth restriction (FGR) and raises of umbilical artery Doppler resistance were risk factors associated with placental abruption.Logistic regression analysis showed that lack of a regular antenatal care ( OR = 45.348, 95% CI: 17.096 - 120.288,P = 0.000 ), FGR ( OR = 27.087, 95% CI: 5.585 - 131.363, P = 0.000 ) and second/third trimester pregnancy loss( OR = 16.068, 95% CI: 1.698 - 152.029, P = 0.015 ) were independent risk factors with placental abruption.(2) When compared with those in preeclampsia patients without placental abruption,the history of preeclampsia ( OR = 3.715,95% CI: 1.096 - 12.596, P = 0.035 ) and lack of a regular antenatal care( OR = 2.509,95% CI:1.173 -5.370,P =0.018) were risk factors for placental abruption in preeclampsia.Conclusion Lack of regular antenatal care, FGR, history of preeclampsia and second/third trimester pregnancy loss were risk factors associated with placental abruption in patients with preeclampsia.
