1.Significance of C-myc Expression in Renal Tissue in Children's Primary Nephrotic Syndrome

Objective To observe the correlation between the C-myc expression and pathologic lesions of kidney to explore the role of C-myc re-expression in children's primary nephrotic syndrome(CPNS). Methods The C-myc expression of renal tissue in 30 cases of CPNS was detected using immunohistochemical method, and the correlation between C-myc expression and renal pathological lesions was analyzed. Results There were various degrees of C-myc positive staining in the renal tissue of all patients with CPNS, while no C-myc positive expression in the renal tissue of control group. C-myc expression was mainly located in podocytes and less in endothelial cells of glomeruli. There was high level of C-myc expression in nephric tubules, especially in proximate tubules. There was no C-myc expression in the Henle's loop,tubulous matrix and vessel areas. There was no obvious difference in C-myc expression level in the podocytes and proximate tubules among the different pathological types of CPNS. The level of focal segmental glomerulosclerosis(FSGS) in proximate tubules obviously reduced compared with mesangil proliferative glomerulonephritis(MsPGN) and minimal change disease(MCD)(P
2.Applicated effect of self -made Dahuang paste on preventing bloating and constipation after lung cancer endoscopic surgery

Objective To explore the effect of self -made Dahuang paste on the prevention of postoperative constipation after lung cancer endoscopic surgery by pasting Shenque point and beating Tianshu point .Methods From January 2015 to April 2015,120 patients with lung cancer underwent thoracoscopic surgery in Zhejiang Cancer Hospital were selected and divided into study group and control group according to the principle of single and double numbers,with 60cases in each group.All patients were treated with intravenous analgesic pump after operation ,with 0.2mg fentanyl hydrochloride and 0.9%saline 100 mL.Dahuang paste was made by the hospital preparation center. The control group was treated with enema or cork ,the study group was given Dahuang massage.The effect on the prevention of postoperative constipation was compared.Results The total effective rate of postoperative prevention of constipation in the study group was 95.00%,which was significantly higher than 58.33%in the control group (χ2=22.547,P=0.000).Conclusion Dahuang paste has obvious effect on the prevention of postoperative constipation by pasting Shenque point and beating Tianshu point ,which is worthy of clinical promotion.
3.Expressions and Significance of Interleukin-22,Matrix Metalloproteinase-9 and Macrophage Migration Inhibitory Factor in Peripheral Blood of Patients with Inflammatory Bowel Disease

Background:The incidence of inflammatory bowel disease(IBD)is increasing recently. However,the pathogenesis has not been fully clarified. Aims:To investigate the expressions and significance of interleukin-22( IL-22),matrix metalloproteinase-9(MMP-9)and macrophage migration inhibitory factor(MIF)in peripheral blood of patients with IBD. Methods:A total of 80 patients with IBD admitted from May 2011 to Nov. 2014 at the First Affiliated Hospital of Soochow University were enrolled,in which 43 cases were Crohn’s disease(CD),37 cases were ulcerative colitis(UC). Forty healthy subjects were served as normal controls. Peripheral levels of IL-22,MMP-9 and MIF were detected by ELISA. Multivariate Logistic regression model was used to analyze IL-22,MMP-9 and MIF in active CD and UC and ROC curve was used to evaluate the diagnostic performance of these markers for screening of active CD and UC. Results:Compared with normal control group,peripheral levels of IL-22,MMP-9 and MIF increased significantly in CD and UC groups(P <0. 05),while no significant difference was found between CD and UC groups(P > 0. 05). Peripheral levels of IL-22, MMP-9 and MIF in active CD and UC were significantly higher than those in remission stage(P < 0. 05). For screening of active IBD,the area under ROC curve(AUC)of combined detection of IL-22 and MMP-9(0. 853 for CD,0. 867 for UC) was superior to that of IL-22,MMP-9 or MIF only(0. 747,0. 770 and 0. 699 for CD,0. 774,0. 815 and 0. 761 for UC). Conclusions:Peripheral levels of IL-22,MMP-9 and MIF increase markedly in IBD patients,which are correlated closely with the activity of IBD. Combined detection of IL-22 and MMP-9 might greatly increase the accuracy for screening of active IBD.
4.Gene mutations and prenatal diagnosis in families with maple syrup urine disease

Objective Maple syrup urine disease (MSUD) is a rare metabolic disorder caused by deficiency of the activity of branched-chain 2-keto acid dehydrogenase complex.The complex contains E1α,E1β and E2 subunits which are encoded by BCKDHA,BCKDHB or DBT genes respectively.Mutation in any gene will cause MSUD.The aim of this study was to analyze the gene mutations of four cases with MSUD and carry out prenatal diagnosis for these four families for MSUD.Methods From 2005 to 2010,four neonates (two males and two females) were diagnosed as MSUD at 2,5,10and 26 days of life.The coding regions of BCKDHA gene and BCKDHB gene in the above four cases were amplified by polymerase chain reaction and analyzed by direct DNA sequencing.During the second pregnancy of the same mother,the amniotic fluid was drawn out at 16-20 weeks for gene mutation analysis after the amniocytes were cultured.Results Mutation analysis revealed six mutations in four patients,including four novel mutations (c.308T>C,c.562G>T,c.1279C>G and c.1280-1291de112) and two previously reported mutations.Five mutations (c.308T>C,c.562G >T,c.868G>A,c.1279C>G and c.1280-1291de112) were detected on BCKDHA gene in three patients.While one mutation (c.853C>T) was found on BCKDHB gene in one patient.Only one mutation was found in the amniocytes of each patient's mother at their second pregnancies suggesting a MSUD heterozygous fetus.Conclusions Analysis of BCKDHA and BCKDHB allowed preliminary understand of gene mutations in the four MSUD families,and made prenatal diagnosis possible,which helped in consultation in the second pregnancy.
