1.Effects of different frequency microvibrations in the vascular endothelial growth factor expression and permeability of vascular endothelial cell.

OBJECTIVEThis study aimed to evaluate the vascular endothelial growth factor (VEGF) expression and permeability of vascular endothelial cell under microvibration.
METHODSHuman umbilical vein endothelial cell (HUVEC) were cultured, randomly vibrated under low frequency of 0.2, 0.5, 2, 5 Hz, 30 min per day. The VEGF mRNA level was detected by Tagman probe real-time fluorescence quantitative polymerase chain reaction (PCR), and the VEGF protein expression level was detected by Western blot. The permeability of vascular endothelial cell was evaluated.
RESULTSCompared with the blank control group, the mRNA and protein expression level of VEGF were significantly increased under 0.2, 0.5 Hz thelial, and increase the permeability microvibration (P<0.05), and decreased under 2, 5 Hz microvibration (P<0.01). The vascular endothelial permeability in creased under 0.2, 0.5 Hz microvibration (P<0.01), whereas the permeability decreased under 2, 5 Hz microvibration (P<0.01).
CONCLUSION0.2-0.5 Hz microvibration can up-regulate the expression of VEGF mRNA and protein in vascular endothelial, and increase the permeability.

2.Construction of new coding system for medical supplies and its application in supply chain management

Coding system is the basis of delicacy and informatization management of medical supplies, and is the link between enterprise production and clinical use. Rational construction of coding system can realize dynamic analysis of big data, guide clinical use, improve the use efficiency, and form a traceability system. Coordinating with medical security standardization work guidance, the authors analyzed the characteristics of current classification, and constructed a new medical supplies coding system by combining the clinical characteristics, price supervision, level of risk, properties of medical charge and insurance, production date and traceability, etc. This new medical supply coding system could contribute to analyzing the usage structure and strengthening price supervision, medical supplies classification and traceability management, which could be appropriate for the monitoring of the whole supply chain, from production, logistics, procurement, supplies usage, charge, medical insurance to traceability management.
3.ReABLE study on the efficacy and long-term safety of recombinant human tumor necrosis factor-α receptor Ⅱ IgG Fc fusion protein with methotrexate in active rheumatoid arthritis

ObjectiveTo evaluate the clinical and radiographic efficacy and safety of the combination of recombinant human tumor necrosis factor-αt receptor Ⅱ IgG Fc fusion protein (rhTNFR:Fc) and methotrexate (MTX) in patients with rheumatoid arthritis (RA). MethodsThirty patients with highly active RA were treated with rhTNFR:Fc (25 mg subcutaneously twice weekly) and oral MTX (up to 15 mg weekly). Clinical efficacy was assessed using ACR response criteria and the disease activity score in 28 joints (DAS28).Radiographs of the hands and wrists were assessed with the modified Sharp score. Chi-square test, Fisher is exact test and paired t-test were performed. ResultsAt week 52, ACR20, ACR50 and ACR70 responses were achieved by 90%, 87% and 67% respectively. At week 52, mean DAS28 was 3.4±1.1 compared to 6.4±0.6 at base-line(P<0.01), with 23% patients achieving clinical remission and 17% patients in low disease activity. Similarly, the HAQ was improved significantly, declining from 1.18±0.56 at base-line to 0.25t±0.34 at week 52 (P<0.01). No radiographic progression was found in 22 cases. Adverse events were mild in general.ConclusionTreatment with rhTNFR:Fc plus MTX has shown good efficacy throughout 52 study period in reducing disease activity, improving function, and retarding radiographic progression. Combination therapy for 52 weeks can achieve disease remission and no radiographic progression, which are the two goals of therapy for RA.
4.Influences of vibration on rapid osteogenic response of osteoblasts.

OBJECTIVEThis study investigated the rapid response of osteoblasts, which were derived from low-magnitude high-frequency vibration (LMHFV). Refractory period-derived memory response was also observed.
METHODSMC3T3-E1 cells were incubated and received LMHFV stimulation (0.49 g, 40 Hz) for 30 min. After application of LMHFV, mRNA levels of earlier osteogenic differentiation markers Runt-related transcription factor 2 (Runx2), collagen typeⅠ(Col-Ⅰ), and alkaline phosphatase (ALP) were immediately detected by real-time fluorescence quantitative polymerase chain reaction in the absence or presence of antioxidant. Simultaneously, concentrations of mitochondrial reactive oxygen species (ROS) and average mitochondrial length were also measured.
RESULTSOsteoblasts in the vibration group showed decreased gene expressions of Runx2, Col-Ⅰ, and ALP (P<0.01) and increased levels of mitochondrial ROS (P<0.01) and shortened mitochondria (P<0.01), whereas antioxidant treatment resulted in recovery from changes in the above indicators (P<0.01).
CONCLUSIONSLMHFV can downregulate mRNA levels of early osteogenic differentiation markers, promote ROS generation, and mitochondrial fission. .

5.Meta-analysis of early endoscopic retrograde cholangiopancreatography combined with endoscopic sphincterotomy versus conservative treatment in acute biliary pancreatitis

Objective To evaluate the efficacy and safety of early endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic sphincterotomy (EST) in patients of acute biliary pancreatitis (ABP).Methods Databases including PubMed,EMBASE,Cochrane library,CBM,CNKI,WanFang data and VIP data were searched with " Endoscopic retrograde cholangiopancreatography" "Endoscopic sphincterotomy" "ERCP" "Pancreatitis" "Biliary pancreatitis" resection up to September 2017.Randomized controlled trails (RCTs) conternming the comparison of ERCP with EST versus conservative management in ABP patients were enrolled in the study.Two reviewers independently screened literature,extracted data,and assess the risk of bias of the included studies.The Meta-analysis was conducted by RevMan 5.3 software.Results A total of 10 RCTs involving 1 752 cases were included in the Metaanalysis.The results of Meta-analysis showed that compared with the control group,early ERCP with EST reduced the systemic complication rate of ABP (RR =0.66;95% CI:0.47-0.93;P < 0.05).However,there were no significant differences in overall mortality,local complication rate,and ERCP related complication rate in ABP between early and selected ERCP with EST treat (P > 0.05).Subgroup analyses indicated that early ERCP with EST therapy reduced the mortality of severe acute biliary pancreatitis (SABP),ABP with biliary obstruction and within 24 hours of admission (RR =0.48,0.35,0.25;95% CI:0.24-0.96,0.17-0.74,0.09-0.73;P < 0.05);decreased the local and systemic complication rate of SABP,ABP with biliary obstruction (RR =0.56,0.54,0.51,0.51;95% CI:0.37-0.86,0.33-0.91,0.32-0.81,0.32-0.80;P < 0.05);reduced the local and systemic complication rate of SABP (RR =0.42,0.47;95% CI:0.25-0.70,0.25-0.87;P <0.05) and shortened the hospital stay of SABP (MD =-11.22 d;95% CI:-16.09--6.36;P < 0.01).However,there were no significant differences of overall complication rate in ABP without biliary obstruction and hospital stay in MABP.Conclusion Early ERCP with EST therapy appears to be safe and effective for ABP patients.

Objective To establish new classification criteria for early rheumatoid arthritis (E-RA) based on large samples of early inflammatory arthritis patients and to evaluate the value of this criteria in China.Methods Patients who had arthritic complaints with disease duration less than one year were enrolled.They were divided into RA group and non-RA group according to the clinical diagnosis by experienced rheumatologists.The clinical and laboratory parameters were analyzed and those with high sensitivity or specificity were selected as the new classification criteria.Statistical analysis was carried out by using t test,x2 test and Logistic regression.Results ① A total of 803 patients with early inflammatory arthritis were included in this study.Five hundreds and fourteen patients were diagnosed as early RA and 251 were diagnosed as other rheumatic diseases,and the diagnosis of 38 patients remained unestablished by the end of follow-up.② New E-RA classification criteria were established based on the parameters with high sensitivity and/or specificity.The sensitivity of the new E-RA criteria was 84.4%,which was higher than 1987 ACR criteria (58.0%),while the corresponding specificities were similar,which were 87.4% and 93.6% respectively.③ Compared with the complex scoring system of 2010 ACR/EULAR criteria,the E-RA criteria was more simple and practical.The diagnostic sensitivity and specificity of E-RA criteria were higher than those of 2010 ACR/EULAR criteria reported in the literatures.④ New classification criteria based on scoring system using Logistic regression analysis was established.The sensitivity of this criteria was 86.4%,which was higher than 1987 ACR criteria (58.0%).Conclusion The diagnostic value of the E-RA criteria developed in this study for early RA is better than 1987 ACR criteria,and is more simple than 2010 ACR/EULAR criteria.It may be used as a new classification criteria for early RA diagnosis.

ObjectiveTo examine the clinical features of fractures and related risk factors in patients with rheumatoid arthritis(RA) in China.MethodsSix hundred and eighty-one RA patients were randomly selected from department of rheumatology of 18 hospitals of China.Data were obtained from the questionnaire,including age,sex,disease duration,the involvement of joints,treatment regimen,features of fractures etc.The possible risk factors of fracture in patients with RA were analyzed with a multi-variate Logistic regression analysis.Results① In 681 RA patients of the survey,48 patients had 54 fractures,and the incidence of fractures was about 8%.② Fractures occurred at various sites.Foot/ankle,femur,spine and wrist were the mostfrequent sites.③ The Logistic regression analysis showed that several factors increased the risk of fracture in RA patients,including long disease duration (OR:1.245,95%CI:0.987-1.570,P=0.065),male gender(OR:0.433,95%CI:0.199-0.942,P=0.035),more deformed joints(OR:1.042,95%CI:1.006-1.079,P=0.023),family history of RA (OR:2.201,95%CI:0.984-4.923,P=0.055),and high scores of SF-36(OR:1.017,95%CI:1.002-1.033,P=0.028).④ According to the degree of correlation from strong to weak,the risk factors of fracture were disease duration,SF-36,sex,number of deformed joints and family history of rheumatoid arthritis.ConclusionThe incidence of fracture is high in patients with rheumatoid arthritis.Several factors could increase the risk of fractures in RA patients,including long disease duration,male gender,more deformed joints,and family history of RA.In order to prevent the occurrence of fractures,cautions should be taken to prevent the development of fractures and treat the disease aggressively to suppress the disease activity of RA.

Objective To describe the distribution of medication costs of rheumatoid arthritis patients, and to analyze the factors that may affect the costs. Methods Data were obtained from a 12-month retrospective investigation of patients with rheumatoid arthritis (RA) across China. Department of Rheuma-tology of 18 hospitals were randomly selected. The data about their social conditions, clinical conditions, medications associated with RA such as disease-modifying antirheumatic drugs (DMARDs), non -steroidal anti -inflammtory drugs (NSAIDs), steroids, biologic agents were collected, and the costs of drugs were calculated. A non-parameter test and multivariate logistic regression analysis were performed. Results Six hundred and forty six patients were enrolled into the study, 435 completed data were chosen for analysis. The results demonstrated that the average costs per patient for medications in the past year was 8018 . The total medication costs were further subdivided into the following parts: DMARDs, (represented 20% of the total costs), biologic drugs (49%), NSAIDs (4%), herbal drugs (22%), steroids (1%). Data analysis showed that patients with higher education and higher incomes, with medical insurance,better health function status and outpatients paid more on DMARDs. Extra-articular manifestations increased the odds of the high-cost group (OR: 2.180, 95%CI: 1.335~3.558, P=0.002), while poor health function status increased the probability of paying high costs (OR: 1.373, 95%CI: 1.012~1.863, P=0.041). Conclusion High medication costs in RA do exist in RA patients. The costs of medication is associated with health function status and the presence of extra-articular manifestations.
9.Retrospective analysis of adverse reactions and related factors of resin cement dentin adhesive

Objective:
To investigate the adverse reactions of resin cement used for dentin bonding and its influencing factors.
Methods:
Patients with dental defects treated with resin cement dentin adhesive for direct composite resin repair or full crown indirect repair were selected as the research objects. The occurrence and causes of adverse reactions, such as dental pulp lesions, soft tissue adverse reactions, and restoration loosening and falling off 7 days, 1 month, 3 months, 6 months, and 1 year after treatment, were analyzed retrospectively.
Results :
Among the 14 776 teeth of 5 971 patients who used resin cement dentin adhesive, 580 cases (3.93%) had adverse reactions. Univariate analysis showed that the incidence of adverse events was highest in teeth with the "full crowns (fixed partial dentures)" restoration type at 7 days, 1 month, and 12 months after treatment. At 7 days after treatment, the incidence of adverse events was the highest in teeth with a preparation depth of "near pulp after preparation". At 7 days and 3 months after treatment, the incidence of adverse events was the highest in teeth with " dentin conditioner-adhesive-resin" treatment of the bonding surface. Multivariate analysis indicated that pulp perforation and pulp capping after tooth preparation were risk factors for adverse events 7 days after treatment (OR=2.610), and the “dentin primer-adhesive-resin” bonding surface treatment method was a protective factor for adverse events 7 days and 3 months after treatment (OR<1).
Conclusion
affect the occurrence of adverse events. pulp perforation, and pulp capping after tooth preparation, and self-etching bonding may contribute to adverse reactions.
10.Application of socket-shield technique for immediate implant placement and provisionalization in esthetic zone: a case report and literature review

Objective:
To investigate the effect of the socket-shield technique (SST) concurrent with immediate implant placement and provisionalization (IIPP) in the aesthetic restoration of anterior teeth.
Methods:
A case of maxillary anterior tooth stumps with a thin labial bone wall was treated with SST for preservation of labial soft and hard tissue fullness, combined with an immediate implant placement and immediate provisional crown for restoring the shape of the tooth and gingival molding
Results:
Immediate implant placement and provisionalization restored the morphology and function of the affected tooth in the shortest possible time. The patient's labial soft and hard tissue contours in the affected tooth area were well preserved in the 18-month follow-up after the application of the SST, which presented a better aesthetic result. The literature review indicates that the indications for SST are unrestorable maxillary anterior teeth, whose dental, periodontal and periapical tissues are healthy and intact. In the esthetic zone, root shielding is effective in maintaining the soft and hard tissue contour on the labial side of the implant. However, there is no consensus on the technical details of SST, such as the ideal coronal height and thickness of the shield, and the management of the gap between the shield and the implant. Thus, more clinical studies and histologic evidence are needed to provide a reference for clinical decision-making. In addition, digital technology can improve the accuracy of implant placement and shield preparation.
Conclusion
The correct application of SST combined with IIPP in the esthetic zone can ensure esthetic results. However, more high-quality evidence-based medical evidence is needed for its long-term efficacy, and indications should be strictly controlled during clinical application.
