1.Effect of Flexion Angle of Knee Joint on Co-contractions of Agonist and Antagonist Muscles

Objective To test the effect of different flexion angles of knee joint on the myoelectric activity and torque of quadriceps femoris and hamstring muscle when these muscles contracting.MethodsElectromyographic activities and isometric torque measurements were performed on 10 healthy subjects at 30°, 60° and 90° of knee joint flexion.ResultsThe results indicated that the greatest maximal voluntary isometric contracture-torque of quadriceps femoris occurred during knee extension at 60° of knee flexion ( P<0.01). However, no significant relationship of maximal voluntary isometric contracture-electromyography and maximal voluntary isometric contracture-torque of quadriceps femoris were found in 10 subjects. But when knee joint was at 90° flexion, hamstring muscle had a greatest neuro-myoelectric activity.ConclusionThe exercise of maximal voluntary isometric contraction of quadriceps femoris and hamstrings muscles at certain knee joint angle can help to maintain the stability of knee joint.
2.The effect of β-elemene combined with irradiation on DNA damage and repair in A549 cells

Objective To study if β-elemene can increase radiation-induced deoxyribonucleic acid (DNA) damage and decrease the damage repair.Methods Exponentially growing human lung adenocarcinoma cells (A549) were exposed to 10 or 20 μg/ml β-elemene for 24 h before irradiation.The effect of β-elemene on the in vitro radiosensitivity of A549 cells was evaluated using clonogenic assay.DNA damage and repair were evaluated using comet assay.Results Exposure to β-elemene before irradiation increased the radiosensitivity of A549 cells.The SERD0 for 10 μg/ml and 20 μg/ml β-elemene was 1.55 and 1.64, respectively.The SERDq for 10 μg/ml and 20 μg/ml β-elemene was 1.43 and 1.75, respectively.Combined treatment, comparing to irradiation or β-elemene treatment alone, induced higher levels of DNA damage and slower rate of damage repair.A549 cells exposed to 20 μg/ml β-elemene followed by irradiation showed a higher levels of tail moment (TM) than those exposed to irradiation or β-elemene alone at 0 h,2 h,6 h and 24 h after irradiation.The TM of the three groups at 0 h,2 h,6 h and 24 h after irradiation was 7.16±2.61,0.95±0.65 and 1.81±1.23(F=231.24,P<0.01), 3.65±2.06,0.11±0.07 and 1.58±1.40(F=90.22,P<0.01), 2.09±0.83,0.1±0.05 and 0.45±0.25(F=238.44,P<0.01), 1.45±1.37,0.11±0.08 and 0.60±0.40(F=38.94,P<0.01), respectively. Conclusions β-elemene can enhance the radiosensitivity of A549 cells through the enhancement of DNA damage and the inhibition of DNA damage repair.
3.Determination of 9 Residual Organic Solvents in Blonanserin by Headspace Gas Chromatography

OBJECTIVE:To establish a method for the determination of 9 residual organic solvents in blonanserin as methanol, alcohol,isopropyl alcohol,acetonitrile,dichloromethane,hexane,ethyl acetate,tetrahydrofuran and methylbenzene. METHODS:Headspace gas chromatography was adopted. The determination was performed on DB-624 capillary column using temperature pro-gramming. The inlet temperature was 150 ℃,and flame ionization detector was used with temperature of 250 ℃. High purity nitro-gen was used as carrier gas with flow rate of 2.8 mL/min. The split ratio was 1:1,and headspace sample size was 1 mL. Head-space heating temperature was 90 ℃,and equilibration time was 35 min. RESULTS:The linear ranges of methanol,alcohol,iso-propyl alcohol,acetonitrile,dichloromethane,hexane,ethyl acetate,tetrahydrofuran and methylbenzene were 6-1500 μg/mL(r=0.9998),10-2500 μg/mL(r=0.9999),10-2500 μg/mL(r=0.9998),0.82-205 μg/mL(r=0.9994),1.2-300 μg/mL(r=0.9995), 0.58-145 μg/mL(r=0.9994),10-2500 μg/mL(r=0.9999),1.44-360 μg/mL(r=0.9996),1.78-445 μg/mL(r=0.9995),respec-tively. The limits of quantitation were 17.71,6.02,3.17,7.45,1.53,0.69,0.93,1.01,0.22 μg/mL;the limits of detection were 5.89,1.90,1.05,2.48,0.51,0.23,0.31,0.33,0.07 μg/mL,respectively. RSDs of precision and stability tests were all lower than 3.0%,and isopropanol was found in repeatability test (RSD=2.1%). The average recoveries ranged 96.67%-102.66%(RSD=1.9%,n=9),96.00%-101.83%(RSD=1.9%,n=9),97.17%-101.50%(RSD=1.4%,n=9),96.97%-102.44%(RSD=2.2%,n=9),95.83%-103.33%(RSD=2.5%,n=9),95.83%-100.28%(RSD=1.9%,n=9),98.17%-101.25%(RSD=1.0%,n=9),96.55%-102.30%(RSD=1.9%,n=9),96.30%-102.22%(RSD=1.8%,n=9),respectively. CONCLUSIONS:The method is simple,rapid,accurate and suitable for simultaneous determination of 9 residual organic solvents in blonanserin as methanol,alco-hol,isopropyl alcohol,acetonitrile,dichloromethane,hexane,ethyl acetate,tetrahydrofuran and methylbenzene.
4.Application of modular cementless femoral stems in complex revision hip arthroplasty.

OBJECTIVETo investigate and clarify the key surgical techniques and evaluate clinical effectiveness of modular cementless femoral stems(LINK MP modular stem) in complex revision hip arthroplasty.
METHODSFrom January 2002 to December 2013, 470 patients in Chinese PLA General Hospital received revision hip arthroplasties using MP stems. Among them, 246 patients were followed for an average of 5.7 years (ranged, 0.5 to 11 years). There were 148 males and 98 females, with an average age of 62.5 years old (ranged from 26 to 83 years old). The most common reason for revision was aseptic loosening of femoral stems (107 cases, 43.5%), followed by prosthetic joint infection (104 cases, 42.3%). Other reasons included unexplained hip or thigh pain, periprosthetic femoral fracture, prosthesis wear and dislocation. According to Paprosky classification of femoral deficiency, there were 171 cases of type IIIA (69.5%), 14 cases of type I, 39 cases of type II, and 22 cases of type IIIB. The average preoperative Harris hip score (HHS) was 37.4 ± 7.5. In addition, radiographic presentation, degree of patients' subjective satisfaction, and clinical effectiveness were assessed as well.
RESULTSThe average Harris score was im- proved to 87.8 ± 4.2 after operation,with significant statistical difference when compared to preoperative score (t = 92.13,P = 0.00). Analysis of patients' subjective satisfaction for leg length discrepancy, stability and overall degree of satisfaction showed that the number of patients with great satisfaction were 27, 60, 61 respectively. The most common complication was the periprosthetic fracture (intraoperatively 21 cases and postoperatively 8 cases). Other complications included infection (7 cases), dislocation (5 cases), numbness possibly caused by incomplete sciatic nerve injury (6 cases) and thigh swelling (3 cases). The X-ray taken at the final follow-up revealed that prosthesis subsidence occurred in 4 cases, 2 of which needed a second revision.
CONCLUSIONThe application of modular cementless femoral stems (LINK MP modular stem) in complex revision hip arthroplasty (especially in cases with the serious defect of proximal femoral bone) shows good results at both the short-term and long-term follow-up, especially in the revision for prosthetic infection with the high success rate. In order to avoid complications and improve clinical outcomes,the surgeons should carefully select proper surgery candidates,perform the procedure with care, and master the key surgical techniques.

5.Hydroxysafflor yellow A reduces anoxia/reoxygenation-induced injury in rat cardiomyocytes

Objective To observe the protective effect of hydroxysafflor yellow A(HSYA) on anoxia/reoxygenation (A/R) injury of neonatal primary cardiomyocytes, and its relationship with phosphoinositide 3-kinase/protein ki-nase B/glycogen synthase kinase 3β(PI3K/Akt/GSK3β) signaling pathway. Methods Primary cardiomyocytes of neonatal rats were isolated from the rats and incubated for 48 hours. The cells were adhered to each other and then divided into five groups:control group (Con group), anoxia/reoxygenation group (A/R group),HSYA treatment group(A/R+H group),PI3K inhibitor (LY294002)treatment group(A/R+L group)and HSYA+LY294002 treat-ment group (A/R+H+L group),then to collect the supernatant fluid of each group to measure LDH.The flow cy-tometry was used to measure the apoptotic cells. The protein levels of Bcl-2,Bax,Akt,p-Akt (Ser473),GSK3β, p-GSK3β (Ser9) were evalated by Western blot. Results A/R increased LDH release,the apoptosis rate (P<0.001),and the expression of pro-apoptotic protein Bax (P <0.001) with the decrease of anti-apoptotic protein Bcl-2,p-Akt(Ser473), p-GSK3β(Ser9)(P<0.001) as compared with the control group. HSYA treatment de-creased LDH release,the apoptosis rate (P<0.001),and the expression of Bax (P<0.001) and increase the ex-pression of Bcl-2,p-Akt(Ser473),p-GSK3β(Ser9)(P<0.001). Compared with the A/R+H group,the expres-sion of Bax was increased (P<0.001),while the expression of Bcl-2, p-Akt(Ser473), p-GSK3β(Ser9)was de-creased (P<0.001) in the A/R+H+L group. Conclusions HSYA protects rats'cardiomyocytes from anoxia/reoxy-genation injury by regulating PI3K/Akt/GSK3β signaling pathway.

Objective To observe the difference in treament of thoracolumbar vertebral bodies fractures be- tween AF nail and Dick nail.Methods From March 1998 to March 2007,85 cases of thoracolumbar vertebral bod- ies fractures were followed up.20 cases were fixed with Dick nail,and 65 cases with AF nail.Results The mean,fol- low-up period was 12 months.By comparison of the operating rime,bleeding amount,the recovery rate of vertebral height,the reduction of Cobb angle and capacity of vertebral canal,AF nail was much better than Dick nail.But there was no marked difference in the recover of nerve function.Conclusion AF nail has more power to reduce vertebral height and is easier to set than Dick nail.It will be worthy of more and wider application in basic level hospitals.
7.Impact on postoperative outcomes after red blood transfusion in coronary artery bypass graft surgey: a meta-analysis of current evidence

Objective To systemically assess impact on postoperative outcomes after red blood cell transfusion(RBCT) in coronary artery bypass graft surgey.Methods A meta-analysis was performed on the comparison and synthesis of findings from included studies published from January 1980 to January 2014.Pooled odds ratio(OR) and 95 % confidence interval(CI) were calculated using RevManS.3 software.Sensitivity analysis was conducted and possible publication bias was tested as well.Results Seven retrospective studies including 71 228 patients(33 872 RBCT cases,37 356 control cases) were eligible for inclusion.The pooled analysis revealed difference in the 30-day mortality OR =1.85 (95% CI:1.35-2.54),1-year mortality OR =2.02 (95 % CI:1.44-2.84),shock OR =2.92 (95 % CI:1.96-4.35),renal dysfunction OR =7.67 (95 % CI:1.44-40.94),mediastinitis OR =2.26 (95 % CI:1.72-2.97),and myocardial infarction OR =3.53 (95 % CI:2.89-4.29).Conclusion Perioperative RBCT can incresase the risk of postoperative mortality and complications in coronary artery bypass graft surgey.High-quality randomized case cohort studies are still needed for the further proof of the risk.
8.Histone deacetylase inhibitors for the treatment of myelodysplastic syndrome.

The effects of conventional treatment for myelodysplastic syndrome (MDS) are not remarkable to date, while only a minority of patients was eligible for allogeneic stem cell transplantation. As epigenetics plays a significant role during the occurrence and development of MDS, and histone deacetylase inhibitors (HDACI), a class of gene expression modulating drugs, are currently being developed for therapy of several types of solid tumor, more attention is paying to HDACI as potential therapy of MDS. This review summarizes briefly the rationale for HDACI use in MDS, the common mechanism of HDACI, the present state of the clinical efficiency, and future development in this field.

9.The design and application of the handover record based on SBAR communication mode in delivery room and neonatal department

Objective To explore the effect of SBAR communication mode in handover between delivery room and neonatal department. Methods A total of 100 cases of neonates transferred from neonatal intensive care unit from March to June 2016 were selected as the control group.The traditional oral transfer mode was used. From June to September 2016, 106 cases of newborn infants who were transferred to neonatal intensive care unit were observed in the observation group. The newborns were transferred with SBAR structural framework. The differences of the integrity, the correct rate, the proportion of adverse events,the transfer time and the satisfaction of the medical staff in the 2 groups were compared. Results The integrity, the correct rate, the proportion of adverse events, the satisfaction of the medical and nursing staff on the integrity of the handover, the accuracy of the description of the disease,the practicality,and the operability were 99.06%(105/106), 99.06%(105/106), 15.09%(16/106), 93.33%(28/30), 90.00%(27/30), 96.67%(29/30), 93.33%(28/30) in the observation group, 89.00%(89/100), 94.00%(94/100),2.00%(2/100),63.33%(19/30),60.00%(18/30),73.33%(22/30),26.67%(8/30)in the control group,the differences were significant(χ2=4.008-27.778,P<0.05 or 0.01).There was no significant difference in the transit time (P> 0.05). Conclusions The design and application of the handover record based on SBAR communication mode in the delivery room and neonatal department improve the integrity of the patient′s condition,the correct rate,can detect adverse events in time.The staff were more comprehensive,more accurate description of the disease description, this form is simple and practical, operable, by the clinical staff of the trust,it is worth in the delivery room and the promotion of the use of newborns.
10.Study on prehospital time and influencing factors of stroke patients in 15 Chinese cities.

OBJECTIVETo study the prehospital time and influence factors of stroke patients in 35 hospitals from 15 cities.
METHODSUnified questionnaires were used to investigate all of the 2270 stroke patients after at the first week of onset from June 30, 2002 to April 30, 2003. Other than general data, the patients were divided into two groups according to prehospital time within or after 6 hours and all the factors influencing prehospital time of stroke patients were analysed with Chi-Square test and logistic regression.
RESULTS(1) All patients aged 18-102 with mean 64.8 y +/- 11.9 y including 60.2% of males. 78.8% of the patients were diagnosed as ischemic stroke and 21.2% hemorrhagic stroke. (2) 27.8% of the patients arriving at hospital immediately after the onset of stroke and 27.2% made emergency calls for ambulance. Only 25% of the patients had the sense that they should go to the hospital and receiving thrombolysis therapy after the onset of symptoms. 25% of the patients were ignorant of any knowledge about stroke, did not ask for help but waiting. (3) 57.5% of the patients arriving in hospital < or =6 hours after the onset and 58.6% went to emergency room directly. Thrombylysis rate among ischemic stroke patients arrived in hospitals < or = 6 hours was 6.7%. Multivariate logistic regression analysis showed following factors were related to arriving in hospital within 6 hours with P < 0.001: whether to emergency room directly, to hospital by ambulance or not, distance between stroke onset site to hospital, whether patients know thrombolysis was immediately needed after the onset of stroke, whether more attention to be paid on stroke after the onset of symptoms, whether needs to go to hospital at once after the onset ect.
CONCLUSIONThe main reasons for 42.5% stroke patients who arrived hospital over 6 hours were: lack of stroke knowledge, less using emergency call/ambulance,over 20 km between the site where stroke onset was occured and the hospital etc. To enhance public awareness about stroke was important to shorten the patients' arriving hospital's time.
