目的探讨非完全同步影像尿动力学检查的临床应用意义。方法采用Laborie尿动力学检查仪和Toshiba放射检查床,对128例患者进行非完全同步影像尿动力学检查(一般取截石位及斜坐位),其中脊髓损伤92例(71.9%)、骶裂5例(3.9%)、尿失禁8例(6.2%)、排尿困难2例(1.6%)、前列腺增生21例(16.4%)。灌注液中泛影葡胺含量为15%,灌注速度10—20ml/min,检查方法同普通尿动力学检查,记录膀胱压力(Pves)、直肠压力(Pabd)、逼尿肌压力(Pdet)、尿流率(Q)和肌电图等参数,采用膀胱储尿期及排尿期同步透视,点拍摄放射片4次,记录并保存影像。结果发现膀胱输尿管返流6例(4.7%)、膀胱结石18例(14.1%)、膀胱憩室35例(27.3%)、膀胱颈开放78例(60.9%)、逼尿肌膀胱颈协同失调23例(18.0% )、逼尿肌尿道外括约肌协同失调59例(46.1%)、外括约肌痉挛31例(24.2%)。结论非完全同步影像尿动力学检查能够利用现有设备进行影像尿动力学检查,对全面了解神经原性膀胱尿道功能障碍及LUTS患者的下尿路病理生理改变有重要价值。

Objective To observe the influence of chitosan on the skin and soft tissue expansion.Methods Twenty-five patients were selected,who were suitable to be embedded soft tissue expanders in the face,a 100-milliliter expander was implanted in one side of the face,and other side was used as control.A 100-milliliter expander was implanted in each group,and a slender silicon duct was embedded between the expander and subcutaneous tissue in the experimental group.About five to seven days after the operation,the negative drainage tube was removed,and then two-milliliter medical chitosan injected with the silicon duct in the experimental group,but not in the control group.Two groups were injected with normal saline in the second day.The center of expanded skin was pressed and skin elasticity and relaxation were compared between the two groups during the injection interval.The time of injection interval,the quantity of normal saline inside the expanders after two weeks and three weeks and the total time of expansion to 100 milliliters were recorded.After injection was completed in the two groups and maintained for two weeks.In the stage Ⅱ operation,the expanders were taken out,1 cm × 1 cm fibropeplos was removed from the center of the expanded skin flap from the two groups,and pathological section was prepared to measure the thickness of fibropeplos,average gray scale of collagen and the quantity of blood capillaries.The fibroblasts,collagen fiber and capillaries were observed and compared under light microscope.A matched-pairs t analysis was used to analyze the data.Results Compared with the control group,the quantity of normal saline inside the expanders in the experimental group was increased at the same time; the water injection period was shorten and tissue expansion was significantly accelerated.The number of fibroblasts in the fibropeplos decreased with the influence of chitosan.The fibroblasts were restrained to mature period and collegan decreased.The fibropeplos became thinner but the capillaries were not affected.Conclusions Chitoson can inhibit fibroblast proliferation and collagen production,and the effect of accelerating tissue expansion is significant and deserves to be recommended.

Objective:To compare the effectiveness of fire needle versus Western medicine in the treatment of herpes zoster. Methods:Randomized controlled trials comparing fire needle with Western medicine in the treatment of herpes zoster were identified using 8 databases. A meta-analysis was performed using RevMan 5.3 software. Results:Eight trials involving 569 patients were included in this meta-analysis, and the results showed that fire needle was superior to Western medicine comparing the effective rate [risk ratio (RR)=1.13, 95％ confidence interval (CI): 1.06 to 1.20;P=0.0002], the visual analog scale (VAS) score [mean difference(MD)=–7.95, 95％ CI: –10.71 to –5.20;P<0.00001], time of pain disappearance (MD=–7.61, 95％CI: –9.38 to –5.84;P<0.00001), time of blister-stop (MD=–1.34, 95％CI: –1.51 to –1.18;P<0.00001), time of crusted scab (MD=–2.92, 95％CI: –3.62 to –2.23;P<0.00001), and time of scab off (MD=–4.64, 95％CI: –5.83 to –3.46;P<0.00001). In addition, a significantly lower incidence of postherpetic neuralgia was found in the fire needle group in 30 d (RR=0.23, 95％CI: 0.11 to 0.51;P=0.0002) and 60 d (RR=0.33, 95％CI: 0.12 to 0.91; P=0.03) after treatment. Conclusion:Fire needle has a favorable effect in increasing the effective rate, relieving pain, recovering skin lesions and decreasing incidence of postherpetic neuralgia in the treatment of herpes zoster. However, considering the limitations in this study, the findings should be interpreted cautiously.

OBJECTIVETo study the effects of modified Huopo Xialing Decoction (HXD) in treating patients of Pi-Wei dampness-heat syndrome (PWDHS) and its impact on epithelial cell apoptosis of tongue.

METHODSFifty patients with PWDHD were randomized depending on their visiting sequence into two groups, the 30 patients in the treatment group treated with HXD and the 20 in the control group treated with Domperidone. The therapeutic course for both groups was 2 weeks. Besides, a normal control group consisting of 30 healthy volunteers was set up. The changes of tongue fur, grade of major syndromes in different groups were observed and scored before and after treatment to evaluate the therapeutic effectiveness, and the apoptotic index (AI) was estimated using TUNEL technique.

RESULTSAI in the two patient groups before treatment was obviously lower than that in the normal group (P < 0.01 or P < 0.05); after treatment, AI in the treatment group increased significantly (P < 0.01), with no difference from that in the normal group (P > 0.05), while in the control group, it only showed a slight trend of increasing. Similar outcome of tongue fur grades was seen in the two groups, it was improved significantly (P < 0.01) in the treatment group but insignificantly in the control group (P > 0.05). After treatment, syndrome scores were both lower in the two groups (P < 0.01), but the decrement in the treatment group was more significant than that in the control group (P < 0.01). Comparison between groups showed that the total effective rate in the treatment group was significantly superior to that in the control group (90% vs 65%, P < 0.01).

CONCLUSIONHXD has good clinical efficacy in treating PWDHS, and has good effect in alleviating the greasy yellow tongue fur in patients with PWDHS by accelerating the tongue epithelial cell apoptosis.

BACKGROUND:Previous studies have confirmed that ethanol can promote adipogenic differentiation of bone marrow mesenchymal stem cells (BMSCs), and up-regulate the expression of PPARγ and aP2 in the tumor necrosis factor α (TNF-α) signaling pathway. As a member of the ZIP protein family, Zrt/Irt-like protein 1 (ZIP1) is closely related to bone metabolism and osteogenic differentiation.OBJECTIVE:To study the effect of BMSCs transfected by ZIP1 on TNF-α signaling pathway in the process of adipogenic differentiation.METHODS:The BMSCs from rabbits were isolated and cultured under different concentrations of alcohol (0.03, 0.09,0.15, 0.21 mol/L), followed by transfection by ZIP1 siRNA and ZIP1 expression vector.RESULTS AND CONCLUSION:After culture in alcohol, the expression levels of aP2 and PPARγ proteins were both significantly increased (P < 0.05), and the level of triglyceride was increased in all alcohol groups except for 0.03 mol/L alcohol group (P < 0.05). After siRNA transfection, the expression levels of aP2 and PPARγ as well as the level of triglyceride were increased significantly in all the alcohol groups (P < 0.05); however, ZIP1 transfection decreased the expression levels of aP2 and PPARγ proteins (P < 0.05). To conclude, ZIP1 siRNA could promote the adipogenic differentiation of BMSCs through the activation of TNF-α signaling pathway.

BACKGROUND: With the increasing number of patients undergoing knee arthroplasty in China, the complications caused by the mismatch of prosthesis with anatomical parameters arouse extensive attentions.OBJECTIVE: To measure the anatomical parameters of the knee joints in healthy Zhuang ethnic group from Guangxi Zhuang Autonomous Region, China, using three-dimensional (3D) reconstruction technology, thus providing reference for the design and placement of the hip prosthesis.METHODS: Sixty-eight individuals, including 30 females and 38 males, were selected from 217 healthy people undergoing CT examination. All data were imported into Mimics 15.0 software used for 3D reconstruction, and then the knee parameters were then measured and analyzed.RESULTS AND CONCLUSION: There was no significant difference in the parameters of the bilateral femur and tibia (P > 0.05). The femoral parameters had significant differences between genders (P < 0.05). The width of tibial platform, sagittal length of lateral tibial platform, distance between medial tibial plateau and fibular head, and posterior slope of tibial plateau showed no significant differences between genders (P > 0.05), while there were significant differences in the width of medial/lateral tibial platform, and sagittal length of medial tibial platform between genders (P < 0.05). The sagittal length of medial femoral condyle was negatively correlated with age, and all femoral parameters were positively correlated with height; while only the width of medial and lateral tibial platform were positively correlated with height. To conclude, 3D reconstruction technology is available for research on the morphology of the knee joint in the Zhuang ethnic group from Guangxi Zhuang Autonomous Region, which provides references for prosthesis design and research appropriate for the Zhuang ethnic group.

In order to study the distribution of virulence genes of Listeria monocytogenes (Lm) in Hebei Province,29 virulence genes of Lm,including Listeria monocytogenes pathogenicity islands Ⅰ (LIPI-Ⅰ:prfA,plcA,plcB,hlyA,mpl and actA),10 internalins genes (inlA,inlB,inlC,inlD,inlE,inlF,inlG,inlH /C2,inlI and inlJ) and the other 13 virulence-associated genes (bsh,srtA,iap,sigB,virR,mprF,dltA,dltB,dltC,dltD,srtB,fbpA and hpt) were detected by PCR.Results showed that in the 91 Lm strains,the detection rate of 23 virulence genes were 100％.The 29 virulence genes of 26 Lm strains were all detected,and 65 Lm strains had different deletion of 6 virulence genes inlD,inlF,inlG,inlH /C2,inlJ and mpl.The deletion rate of inlG and inlF were 60.44％ and 54.95％,respectively,following by mpl gene,with a deletion rate of 19.78％.According to the absence of virulence genes,91 strains could be divided into 10 subtypes,and the dominant virulence subtypes was type Ⅰ with all 23 virulence genes.The deletion rate of virulent genes in Shijiazhuang was higher than that in northern Hebei.It is suggested that the rate of virulence gene of food-borne Lm in Hebei Province is high,and the virulence gene deletion patterns has diversity and regional differences.

OBJECTIVETo explore the mechanical behavior of lumbar spine loaded by stress and provide the mechanical basis for clinical analysis and judgement of lumbar spine fracture classification, mechanical distribution and static stress.

METHODSBy means of computer simulation method, the constructed lumbar spine three-dimensional model was introduced into three-dimensional finite element analysis by software Ansys 7.0. The lumbar spine mechanical behavior in different parts of the stress loading were calculated. Impact load is 0-8000 N. The peak value was 8000 N. The loading time is 0-40 minutes. The values of the main stress, stress distribution and the lumbar spine unit displacement in the direction of main stress were analyzed.

RESULTSThe lumbar spine model was divided into a total of 121 239 nodes, 112 491 units. It could objectively reflect the true anatomy of lumbar spine and its biomechanical behavior and obtain the end-plate images under different stress. The stress distribution on the lumbar intervertebral disc (L(3)-L(4)) under the axial, lateral flexion and extension stress, and the displacement trace of the corresponding processus articularis were analyzed.

CONCLUSIONIt is helpful to analyze the stress distribution of lumbar spine and units displacement in static stress loading in the clinical research of lumbar spine injury and the distribution of internal stress.

AIM: To investigate the effect of indwelling time of lacrimal silicone drainage tube on chronic dacryocystitis and its effect on visual acuity.?METHODS: The clinical data of 92 patients ( 110 eyes ) with chronic dacryocystitis treated with lacrimal drainage tube in our hospital from January 2013 to December 2016 were retrospectively analyzed. According to the indwelling time, the patients were divided into 6wk group (42 patients 50 eyes ) and 12wk group ( 50 patients 60 eyes). Comparison of two groups on clinical efficacy, vision, overflow symptoms, complications and recurrence was taken.?RESULTS:The effective rate was significantly higher in the 6wk group than in the 12wk group (P<0. 05). The visual acuity and incidence of epilepsy symptoms at 3mo after extubation was not signifcantly different (P<0. 05). The incidence of complication was 14. 0％ (7/50) at 3mo after extubation in 6wk group and significantly lower than that in the 12wk group which was 38. 3％ (23/60, P<0. 05). The recurrence rate was 2. 0％ (1/50) in the 6wk group, which was significantly lower than that in the 12wk group (18. 3％, 11/60;P<0. 05).?CONCLUSION: The clinical efficacy of lacrimal silicone drainage tube for 6wk and 12wk is similar to patients with chronic dacryocystitis. There is also little difference in the visual acuity of patients with dacryocystitis, but with the prolongation of the lacrimal duct drainage tube, rinse patency rate, increase the difficulty of extubation, complications and recurrence rate, the clinical situation should be based on the patient as much as possible to shorten the lacrimal duct silicone drainage tube retention time.
