1.Assessment of the Function of Disabled Athletes Using Disability Assessment Schedule Ⅱ(WHO-DAS Ⅱ)

ObjectiveTo evaluate the reliability and validity of the of Disability Assessment Schedule Ⅱ of World Health Organization(WHO-DASⅡ) applied in assessment of the function of disabled athletes.Methods56 disabled people,26 wheelchair basketball athletes and 30 patients,were sampled and completed WHO-DASⅡ questionnaire.The test and re-test had been administrated in 10 days.Barthel Index(BI) and Sports Classification Test had been implemented for the functioning assessment.ResultsThe test-retest reliability for WHO-DASⅡ in six dimensions were: 0.978,0.807,0.938,0.877,0.998,0.935,0.986 and all were in very significant level(P<0.01).There were negative correlation between WHO-DASⅡ scores and BI in total and in every dimension,the correlation coefficients were:-0.77,-0.17,-0.71,-0.82,-0.33,-0.73,-0.61.The correlation coefficients between WHO-DASⅡ total and six dimensional scores and the grade of sports classification were:-0.741,-0.378,-0.806,(-0.541),-0.293,-0.510,-0.677.ConclusionWHO-DASⅡ can be used in assessing the function of disabled athletes.
2.The feasibility of breath control management guided stereotactic ablation body radiotherapy of liver tumors by volumetric modulated arc therapy

Objective To investigate the feasibility and plan quality of the image-guided volumetric modulated arc therapy (VMAT) based voluntary deep exhale breath-holding technique in the stereotactic ablative body radiotherapy (SABR) for liver tumors.Methods Fifteen patients with liver tumors were involved in this study.All patients were immobilized with voluntary deep exhale breath hold (vDEBH) combined with real-time position management (RPM) respiratory gating system.Treatment was planned using VMAT with 2 modified partial arc and re-planned using intensity modulated radiation therapy (IMRT) technique for comparison.Dosimetric parameters were calculated for plan quality assessment.Quality assurance studies included absolute dose and multiple planar dose verifications,total monitor units and delivery time analysis.Daily cone beam computed tomography imaging was used to verify the motions.Results There were no significant dosimetric differences between VMAT and conventional IMRT plans (P >0.05).Both techniques were able to minimize doses to organs at risk including normal liver,kidneys,spinal cord,and stomach.However,the average monitor units with VMAT were significantly lower 28.1% than those with IMRT(t =3.064,P <0.05).The average beam-on time in VMAT plans was 31.6% shorter than that in IMRT plans(t =2.278,P < 0.05).Conclusions The utilization of VMAT in the treatment planning of SABR for liver tumors under breath control mode has better dosimetrics.In comparison to conventional IMRT plans,VMAT plans have higher efficiency and feasibility.
3.Relationship between EGFR and KRAS mutations and clinicopathologic features of non-small cell lung cancers

Purpose To explore the relationship between the mutations of epidermal growth factor receptor ( EGFR) and KRAS genes and clinicopathological characteristics in patients with non-small cell lung cancers (NSCLC). Methods Clinical samples from 431 NSCLC patients were obtained for EGFR and KRAS gene analysis. PCR based direct DNA sequencing was used to investigate mutations in exon 18-21 of EGFR gene and codon 12 and 13 of exon 2 of KRAS gene. Results The overall EGFR mutation rate of primary NSCLC was 53. 6% (231/431) in this study cohort and eight cases showed double EGFR mutations. Mutation rates in female and male were 65. 2% (122/187) and 46. 9% (98/209), respectively. The mutation rate was higher in patients with non-smokers and adeno-carcinoma and adenosquamous carcinoma subtypes than in their counterparts (P<0. 05), with the percentage of 57. 2% (124/216), 60. 3% (199/330), 42. 9% (6/14), respectively. In squamous cell carcinomas and other subtypes, EGFR mutation rates were 11. 6% (5/43) and 11. 1% (1/9), respectively. The EGFR mutation types included exon 18 point mutations (4. 0%, 9/227), exon 19 deletion mutations (4. 5%, 101/227), exon 20 insert or point mutations (9. 7%, 22/227) and exon 21 point mutations (41. 4%, 94/227). Activating mutations of KRAS gene were detected in 7. 8%(31/396) of NSCLC. Twenty-eight patients showed codon 12 mutations ( G>T, G>A, G>C) , and three patients had codon 13 mutations ( G>A, G>T) . Most of these mutations were G to T transversion (64. 5%, 20/31). Conclusion Polymerase chain reaction-direct sequencing is a reliable and effective method for the detection of the EGFR and KRAS gene mutation in NSCLC patients. The mutation rate of EGFR is higher in Chinese patients, especial-ly in non-smoking female patients with adenocarcinoma.
4.Advances in Application of Blue Laser Imaging in Diagnosis of Early Upper Gastrointestinal Cancer

Blue laser imaging (BLI) is a new endoscopic system equipped with the laser beam emitting two different wavelengths.It produces bright and high resolution images for observation of microvascular and microsurface patterns of esophageal and gastric mucosa, helping the diagnosis of early upper gastrointestinal cancer.Compared with the existed endoscopic techniques, BLI shows its unique advantages.In this article, advances in application of BLI in diagnosis of early upper gastrointestinal cancer were reviewed.
5.Reliability and validity of WHO ICF-Clinical Checklist for patients with spinal cord injuries

ObjectiveTo explore the reliability and validity of WHO ICF-Clinical Checklist for patients with spinal cord injuries.MethodsClinical testing, case records and check had been implemented for 50 patients with spinal cord injuries with ICF-Clinical Checklist, ADL and ASIA.Results and ConclusionThere were high test-retest reliability and validity in WHO ICF-Checklist for patients with spinal cord injuries.
6.Methyl 3, 4-dihydroxyphenylacetate prevents enterovirus 71 proliferation in rhabdomyosarcoma cells.

This article presented the inhibitory activity of methyl 3, 4-dihydroxyphenylacetate on the enterovirus 71 (EV71) infection. The EV71 VP1 capsid protein expression levels were analyzed with Western blotting. Results revealed that the compound is able to inhibit EV71 replication in rhabdomyosarcoma (RD) cells. After being incubated with the compound at a concentration of 0.01 microg x microL(-1) for 48 h, the level of EV71 vp1 mRNA in RD cells decreased by (76.83 +/- 2.47)%. The cytotoxic activity of the compound was evaluated against RD cells by a MTT assay. The results showed that the compound had low toxicity with a CC50 of 0.072 6 microg x microL(-1). These findings suggest that methyl 3, 4-dihydroxyphenylacetate is a novel compound for antiviral therapies against EV71, which merited further investigation.

7.Terminology, Diagnosis and Classification of Intellectual Disability Using WHO ICD-11 and ICF

Objective To review the nomenclature and terminology, diagnostic criteria, classification and evaluation of intellectual disability using WHO ICD-11 and ICF.Methods The theories and approaches of ICD-11 and ICF were adopted to analyze the nomenclature and terminology, diagnostic criteria, classification and evaluation of intellectual disability.Results ICD-11 and ICF had established the nomenclature and terminology, diagnostic criteria, classification and evaluation of intellectual disability from the perspectives of content model of ICD-11 and functioning of ICF respectively. Intellectual disability had used nomenclature of disorder of intellectual development and one of sub-categories of mental, behavioral or neurodevelopmental disorders. Intellectual disability was also defined as both impairment of intellectual function (b117) and functional limitation of adaptive behaviors (d1-d9) using ICF. Occurrence age of intellectual disability was before 18 years old for diagnostic criteria of ICD-11 and ICF.Conclusion The nomenclature and terminology, diagnostic criteria and classifications of intellectual disability have been reviewed and updated using the theories and approaches of ICD-11 and ICF. It provides theoretical supports, approaches, and standardized tools for the development of rehabilitation service for people with intellectual disability.
8.Individualized Services and Support Plan for People with Intellectual Disability Based on ICF

Objective To analyze the unmet needs of services and support, and design structured, standardrized and individualized service and support plans for people with intellectual disability using ICF framework.Methods In respective of intellectual function and adaptive behavior, a structured, standardrized and individualized service and support plan had been constructed according to process of individualized plan using ICF.Results Based on ICF model of functioning and disability, the structured and standardized service and support plan had been constructed, including functional diagnosis and service needs reporting, and individualized services protocols.Conclusion With the analysis of functioning and reporting of unmet needs of service using ICF, the structured, standardrised and individualized service and support plan can be developed to promote the total rehabilitation for people with intellectual disabilities.
9.Health Conditions and Health Service for People with Intellectual Disability

Objective To analyze the health conditions, related factors and development of health services for people with intellectual disability, to provide evidence-based support for the development of health services.Methods The functioning of people with intellectual disability had been analyzed using ICF. Health conditions and related factors for people with intellectual disability had been analyzed using epidemiologic approach.Results The prevalence of intellectual disability is about 1.1% reported by US and 0.75% by China. The main health conditions for people with intellectual disability were overweight, obesity, malnutrition, oral problems and digestive diseases. The main functioning included cognitive impairment, autism spectrum disorder, visual or auditory impairment, learning disability, movement limitation, generalized developmental disorder, etc. Secondary disorders caused by intellectual disability worsened the functioning of conceptual skills, social skills and practical skills.People with intellectual disability faced health risks from biological and environmental domains.Conclusion It is important to tailor to the unmet needs and health behaviors of people with intellectual disability. It is proposed to conduct capacity building for health professionals and institutions and improve the health of people with intellectual disability.
10.Vocational Rehabilitation for People with Intellectual Disability: Theoretical Framework, Approaches and Services

Objective To establish the theoretical framework and approaches of services provision of vocational rehabilitation (VR) for people with intellectual disability (ID) to promote the decent employment of people with ID.Methods The employment difficulties of people with ID had been analyzed using ICF, and the content and the procedure of VR for people with ID had been explored.Results According to ICF, the employment difficulties of people with ID included functioning, barriers of employment and environmental factors. VR services for people with ID included assessment of employment barriers and vocational capacity, vocational counseling, vocational skill development and training, vocational guidance, support and assistance service, VR and politics development and research. The procedures of VR services for people with ID included assessing employment difficulties and vocational abilities, establishing employment setting and target, planning and implementing individualized VR program, and follow-up services.Conclusion It is necessary to use ICF to develop structured and individualized VR framework and approaches of VR for people with ID.
