3.Surgical treatment for rotationally and vertically unstable pelvis fracture.

OBJECTIVETo study the clinical results of surgery for rotationally and vertically unstable pelvis fracture.
METHODSThirty-four patients with rotationally and vertically unstable pelvis fractures were treated. There were 23 male and 11 female,with the average age of 36 years ranging from 13 to 56 years. There were 9 cases of type APC III, 14 cases of type LC III, and 11 cases of type VS according to Young-Burgess Classifiction. All patients' pelvis were treated with temporary external fixation after hospitalization, and were treated with open reduction and internal fixation through anterior approach after stabilization of body condition.
RESULTSAll patients were followed up for 12 to 48 months (average 21 months). All the incisions healed well, and the fractures got union for 3 to 6 months. According to the Majeed evaluation, the results were excellent in 21 cases, good in 10, fair in 3. All patients were not remained deformity of rotation and dislocation. But 3 patients remained lameness, 4 remained low back pain, 3 remained both leg and feet numbness.
CONCLUSIONIn the management of the rotationally and vertically unstable pelvis fractures, a stable pelvis can be reconstructed by effective open reduction and internal fixation through the anterior approaches, so that further sequelae can be reduced.

4.Efficacy Analysis of Combination Drug Therapy for Chronic Prostatitis:Report of 253 Cases

Objective:To explore the efficacy of combination drug therapy for chronic prostatitis .Methods:According to National Institute Health Chronic Prostatitis Syndrome Index (NIH‐CPSI) combined with the pre‐ and post‐ massage test (PPMT) and expressed prostatic secretion (EPS) routine examination ,the NIH classification and efficacy evaluation was conducted for 253 patients with chronic prostatitis .All the 253 cases were administered quinolone antibiotics for 2 to 4 weeks , and meanwhile were administered alpha blocker and prostat for 4 to 6 weeks .Results:According to NIH‐CPSI ,there were 23 (9 .1% ) cases of type Ⅱ ,109(43 .1% ) cases of type Ⅲ A and 121(47 .8% ) cases of type Ⅲ B among the 253 patients .After 6 weeks of treatment ,the patients were evaluated by CPSI and the results showed that the curative rate ,the remarkable effective rate ,the improving rate ,the ineffective rate ,the total effective rate was 18 .6% ,45 .5% ,14 .6% ,21 .3% ,and 77 .7% , respectively .The average WBC count in EPS was (5 .4 ± 1 .6)/HP after the treatment ,while it was (24 .6 ± 3 .8)/HP before the treatment .And the difference was statistically significant (P< 0 .05) .After the treatment ,23 patients with chronic bacterial prostatitis underwent bacterial culture by using PPMT ,19 of which turned negative with a turning negative rate of 82 .6% .Conclusions:NIH‐CPSI is a reliable method for efficacy evaluation .The combination of drugs ,including quinolone , alpha blockade and prostat ,shows satisfactory effect for the treatment of chronic prostatitis .
5.Comparison between anti-ouabain egg yolk(IgY) and rabbit antibody(IgG) in enzyme-linked immunosorbent assay.

AIMTo improve specificity and accuracy of endogenous ouabain measurement assay.
METHODSAnti-ouabain polyclonal antibody egg yolk (IgY) and anti-ouabain rabbit antibody (IgG) were prepared respectively. In the presence of two kinds of antibody, then the specificity and accuracy of enzyme-linked immunosorbent assay (ELISA) were compared.
RESULTSThe ELISA, in the presence of IgY, provided a sensitivity of the average intraassay coefficient of variation(CV) was 2.03%, and the inter-assay CV was 2.34% respectively. In contrast, IgG were 2.83% and 3.29%. No significant interferences were observed with hydrocortisone and dexamethasone. There was 3.45% vs. 5.95%, 3.20% vs. 5.20% of crossreaction with cedilanid and digoxin.
CONCLUSIONThe specificity and accuracy of ELISA, in which IgY was used, were more better than IgG.

6.Aberrant methylation of hMLH1 gene promoter in papillary thyroid cancer and its clinical significance.

OBJECTIVETo investigate the aberrant promoter methylation of hMLH1 gene promoter and its clinical significance in papillary thyroid cancer (PTC).
METHODSmethylation of hMLH1 gene promoter in the cancer tissue and matched tumor-adjacent normal tissue of 152 PTC patients were detected by real-time methylation specific PCR (qMSP). The relationship between the methylation of hMLH1 gene promoter and clinicopathological features was analyzed.
RESULTSThe methylation rate of hMLH1 gene promoter in cancer tissues was 37.5% (57/152), of which 33 cases were totally methylated and 24 cases were partially methylated. The methylation rate of adjacent normal tissues was 5.3% (8/152)(all were partially methylated). The methylation rate of PTC tissues was significantly higher than that in the tumor-adjacent normal tissue (P < 0.01). The promoter methylation of hMLH1 gene in PTC was significantly correlated with age, size and number of the primary lesion, local invasion, T stage and lymph node metastasis (P < 0.05) , but not correlated with gender and clinical stage (P > 0.05).
CONCLUSIONPromoter methylation of hMLH1 gene is a common molecular event in PTC tissue, and it is significantly correlated with the progression of PTC.

7.Efficacy and safety of sodium valproate in a higher dose for treating nocturnal and early-morning seizures

Objective To explore the efficacy and safety of sodium valproate in a higher dose in the evening for treating nocturnal and early-morning seizures.Methods A total of 69 newly diagnosed pediatric patients with nocturnal or early-morning seizures were enrolled.They were randomly distributedinto experimental group (35 cases) and control group (34 cases) and treated with sodium valproate tablets.The initial dose was little.It was gradually increased to the effective maintenance dose.With sodium valproate given twice a day,the post meridiem(PM) dose was twice the ante meridiem(AM) dose in the experimental group,while the PM/AM dose was equal in the control group.All patients were at least been followed up for 6 months.Results In the experimental group,28 cases were seizure free (80.0%),and the total effective rate was 85.7%.In the control group,20 cases were seizure free (58.8%),and the total effective rate was 64.7%.The difference in the total effective rate between the 2 groups was significant (P < 0.05).No severe adverse effect was found among all patients.The incidence of daytime sleepiness (0/35 cases) in the experimental group was lower than that in the control group (2/34 cases).Conclusions A higher dose of sodium valproate in the evening for nocturnal and early-morning seizures led to better seizure control,better nocturnal sleep,less daytime somnolence,and the side effects are slight.
8.Choice of surgical approaches for salvage surgery of primary lesion recurrence and residual cases of nasopharyngeal carcinoma

Objective The choice of surgical approaches for salvage surgery based on the location and invasion of recurrent and residual lesions of nasopharyngeal carcinoma (NPC),surgical results,complications,and survival were assessed.Methods Thirty-seven cases with recurrent and residual lesions of NPC underwent salvage surgery between March 1991 and January 2005 were analysed retrospectively.Of 37 patients,23 were men and 14 women,with a median age of 46.5 years (26 -57 years) ;4 were at stage Ⅰ,10 at stage Ⅱ,14 at stage Ⅲ,and 9 at stage Ⅳ; 5 cases were with cervical metastasis,including 3 cases of N1 and 2 cases N2.All recurrent and residual lesions of NPC were determined by biopsy.On the location and invasion of recurrent and residual lesions of NPC,8 cases underwent endoscopic resection of lesions,12 cases of the palate nasopharyngectomy,5 cases of maxillary swing,4 cases of maxillary swing plus preronal approach,2 cases of lateral rhinotomy plus coronalflap approach,and 6 cases transfacial plus nasal pyramid swing approach.Five cases with cervical metastasis received neck dissection in addition to the operations for recurrent and residual lesions of NPC. Postoperatively 31 cases received radiotherapy with dosage of 60 Gy,among them 15 cases with concurrent chemoradiation therapy,and 6 cases with clear surgical margin did not received radiotherapy or chemotherapy. The cases were followed up for 12 - 72 months,with a median of 45 months.Results Total resection for the recurrent and residual lesions of NPC acounted for 91.8% (34/37) and subtotal resection for 8.2% (3/37). The accidence of perioperative complications was 24.3% (9/37). The 3- and 5-year overall disease-free survival rates (DFSR) were 62.1% and 43.3%,respectively. The 3- and 5-year overall survival rates (OSR) were 72.9% and 51.3%,respectively.The 5 year DFSR of cases at stage Ⅰ - Ⅳ were 100%,40%,28% and 11% (χ2 =10.0,P <0.01=,respectively.The 5 year OSR were 100%,70%,35% and 28% (χ2 = 11.5,P <0.01),respectively.Conclusions Salvage surgery is a justified treatment for the recurren and residual lesions of NPC,by which some patients with recurren and residual lesions of NPC can be salvaged.
9.Overexpression of angiotensin-converting enzyme 2 inhibits angiotensin Ⅱ type 1 receptor expression and signal transducer and activator of transcription 3 phosphorylation in cultured smooth muscle cells

Objective To explore the effects of recombinated lentiviral angiotensin-converting enzyme 2 ( ACE2 ) vector transfer on the expression of angiotnsin Ⅱ type 1 ( AT1 ) receptor in cultured vascular smooth muscle cells (VSMCs).Methods VSMCs were divided into 7 groups:( 1 ) Control:serumfree culture medium; (2) Lentiviral-GFP vector group:Lentiviral-GFP vector ( MOI =10) ; ( 3 ) Ang Ⅱ group ( 10-7 mol/L) ;(4)Ang Ⅱ ( 10-7 mol/L) + Lentiviral-ACE2 ( MOI =10) group; (5)Ang Ⅱ ( 10-7 mol/L) +Irbesartan (10-7 mol/L) group ; (6) Ang Ⅱ ( 10-7 mol/L) + irbesartan (10-7 mol/L) + Lentiviral-ACE2 ( MOI =10) group ; ( 7 ) Lentiviral-ACE2 ( MOI =10 ) group.Ninety-six hours later,the proliferation of VSMCs was determined with CCK-8 Kit.AT1 receptor mRNA and protein expressions were detected with quantitative real-time PCR and Western blot,the signaling pathway of signal transducer and activator of transcription 3 (STAT3) was also detected.Results ACE2 gene transfer significantly inhibited the VSMCs proliferation in the absence or presence of Ang Ⅱ.AT1 receptor mRNA and protein expressions were also significantly downregulated in the absence or presence of Ang Ⅱ.Similar to AT1 receptor mRNA and protein expression changes, STAT3 phosphorylation was also significantly inhibited by ACE2 overexpression.Conclusion Our results suggest that overexpression of ACE2 gene could inhibit the VSMCs proliferation by downregulating AT1 receptor expression and STAT3 phosphorylation. ACE2 could also directly inhibit AT1 receptor in cultured VSMCs.
10.Neutrophil-to-lymphocyte ratio may help predict pediatric testicular torsion in chlidren with acute scrotal pain before surgery

Objective To explore the feasibility and accuracy of neutrophil-to-lymphocyte ratio(NLR)in the prediction of testicular torsion(TT)in children with acute scrotal pain.Methods A retrospective case-control study was performed on 158 pediatric patients with ultrasound suspicion of TT who underwent surgical testicular examination during Jan.2017 and Jan.2024.The patients were divided into TT group and non-TT group.Clinical data and laboratory data at admission were analyzed.Sensitivity and specificity of NLR to TT were determined with the area under the curve(AUC)represented on the receiver operating characteristic(ROC)curves.Results There were with no statistically significant differences in clinical data between the two groups(P>0.05).The NLR was significantly higher in the TT group than in the non-TT group[(4.82±2.37)vs.(2.85±0.75),P<0.05].The optimal cut-off value of TT predicted by NLR was 2.07,the AUC was 0.809(95%CI:0.709-0.909),and the sensitivity and specificity were 97.9%and 93.3%,respectively,which were significantly higher than other factors.Conclusion For suspicious ultrasound diagnosis of pediatric acute scrotal pain cases,NLR can be used to predict the possibility of TT and may help to evaluate the urgent surgical treatment in these patients.
