Objective To evaluate the effect of fluid load on the prognosis of severe hand , foot and mouth disease in children. Methods The patients with severe hand foot and mouth disease in the emergency department of PICU in our hospital were enrolled as the research object. We would collect demographic characteristics , labora-tory tests and clinical data: age, gender, focus, basic disease, and simplified acute severity score (SAPS)Ⅱ and record the cumulative amount of fluid balance at 24, 48, 72 hours after admission. Results There was a signifi-cant difference on fluid balance at 48 and 72 hours between the survival group and the death group , the death group appeared the positive liquid balance , and there were significant differences in PICU retention time , mechani-cal ventilation rate, MODF involved organs, mortality and other prognostic indicators between the negative fluid balance group and the positive fluid balance group. Conclusion Fluid balance is an important treatment for severe hand foot and mouth disease, and positive liquid balance is related to mortality and other adverse prognosis.
3.Mechanism of anti-tumor effect of HIF-1alpha silencing on cervical cancer in nude mice.

OBJECTIVETo observe the anti-tumor effect of silencing the expression of HIF-1alpha on cervical cancer in nude mice and to explore its mechanism of action.
METHODSHuman cervical cancer cell line Siha cells were divided into 3 groups: mock control group, control group transfected with scrambled sequence plasmid, and experimental group transfected with pU-HIF-1alpha-shRNA eukaryotic expression plasmid. Cultured cells of the three groups were inoculated in nude mice to establish cervical cancer-bearing nude mice. HIF-1alpha RNAi assay was performed to evaluate the tumor-suppressive effect of HIF-1alpha silencing on cervical cancer-bearing nude mice. Immunohistochemistry and Western blot were used to observe the distribution and protein expression of HIF-1alpha and GLUT1, while RT-PCR was adopted to detect the gene expression of HIF-1alpha, GLUT1 and HKII. The product of glycolysis (lactic acid) and apoptosis in tumor cells were detected by colorimetry and semi-quantitative TUNEL staining, respectively.
RESULTSThe tumor growth in experimental group was significantly slower than that in the two control groups (P < 0.05). On the 50th day after transplantation, the tumor weight in the experimental group was (1.90 +/- 0.28) g, significantly lower than (2.95 +/- 0.77) g in the control group and (2.54 +/- 0.56) g in the mock group (P < 0.01). In the experimental group, the gene and protein levels of HIF-1alpha were 0.45 +/- 0.04 and 1.25 +/- 0.92, and the levels of GLUT1 were 0.32 +/- 0.02 and 1.25 +/- 0.48, respectively. Both indicators in HIF-1alpha and GLUT1 were lower than that in the two control groups (P < 0.05). The expression levels of HKII gene and lactic acid in the experimental group were lower than that in the two control groups (P < 0.05), but the apoptotic cells were much more numerous in the experimental group than that in matched control groups (P < 0.01).
CONCLUSIONThe gene therapy by siRNA targeted silencing of HIF-1alpha may down-regulate its downstream genes GLUT1 and HKII expression, therefore, to reduce the tumor glycolysis activity and promote tumor cell apoptosis, and exert a tumor-suppressing effect in vivo.

4.Relationship between NF-kappa B binding activity and expression of PPAR gamma-mRNA in the livers of rats with fatty liver disease.

OBJECTIVETo investigate the changes of NF-kappa B binding activity, the expression of PPARr and their correlation in the liver of rats with fatty liver disease (FLD) induced by different pathogenic factors and to investigate the molecular mechanism of the inflammation in FLD.
METHODS40 Wistar rats were randomly divided into 4 groups of ten each: normal group, alcohol group, fat-rich diet group, alcohol adding fat-rich diet group. The rats were sacrificed at the end of the 16th week from the starting day of the experiment. Serum and liver specimens were collected. Histological specimens were stained with HE, SudanIV, and Masson and then studied microscopically. The ultrastructural changes were also checked under an electron microscope. NF-kappa B binding activity and the expression of PPARr mRNA were determined by electrophoretic mobility shift assay (EMSA) and RT-PCR respectively. The correlations between NF-kappa B binding activity and the expression of PPARr and the biochemical indexes were analyzed.
RESULTSSteatosis, inflammation, necrosis and fibrosis were present in livers of the rats of all the experimental groups, and were most severe in the alcohol adding fat-rich diet group. NF-kappa B binding activity was markedly increased in the livers of the alcohol group (142+/-16.32) and of the alcohol adding fat-rich diet group (238+/-19.14) in comparison to the livers of the normal (73+/-9.24, F = 6.36, 17.93) and those of the fat-rich diet group (84+/-10.38, F = 5.96, 16.20). Binding activity was higher in the alcohol adding fat-rich diet group than that in the simple alcohol group, but there was no difference between those of the fat-rich diet and normal groups. The level of PPARr mRNA was lower in the livers of the alcohol, fat-rich diet, alcohol adding fat-rich diet groups (0.2530+/-0.069, 0.3647+/-0.082, 0.1226+/-0.054) than that of the controls (0.8097+/-0.094) (F = 15.43, 7.24, 21.45). NF-kappa B binding activity was correlated positively with the level of serum TNF alpha (r = 0.527, 0.639) and the content of MDA in the liver homogenates (r = 0.723, 0.537), but negatively with the expression of PPARr in the livers of the alcohol and the alcohol adding fat-rich diet groups (r = -0.568, -0.891).
CONCLUSIONThe enhanced nuclear factors NF-kappa B binding activity and decreased expression of PPARr play a pivotal role in the inflammatory response of FLD induced by alcohol and fat-rich diet. It may provide a new idea for treating FLD effectively.

5.Effect of electro-scalp acupuncture on cerebral dopamine transporter in the striatum area of the patient of Parkinson's disease by means of single photon emission computer tomography.

OBJECTIVETo probe the mechanism of electro-scalp acupuncture in treatment of Parkinson's disease (PD) by single photon emission computer tomography (SPECT).
METHODSFive cases of PD received electro-scalp acupuncture at Dingnie Qianxiexian (MS 6), Epangxian III (MS 4), Dingpangxian I (MS 8), Dingpangxian II (MS 9) and Zhenxia Pangxian (MS 14). Contralateral points were selected for pathologic change on one side and bilateral points were selected for pathologic lesion on both sides. All the patients received 99mTc-TRODAT-1 SPECT examination before and after acupuncture treatment of 6 weeks. And activities of dopamine transporter (DAT) were analyzed by the ratio of striatum/occipital lobe (ST/OC), which was evaluated by means of technique of regional of interesting (ROI).
RESULTSThe ratio of ST/OC on the same side of the affected extremity before and after treatment were 1.19 +/- 0.15 and 1.24 +/- 0.31 respectively. And on the other side were 0.90 +/- 0.12 and 0.95 +/- 0.25 respectively. They were increased after treatment (P > 0.05).
CONCLUSIONElectro-scalp acupuncture can decrease the loss of DAT and improve the activities of DAT in the striatum of the patient of PD.

6.Expression of zinc finger protein X-linked in childhood B lineage acute lymphoblastic leukemia.

OBJECTIVETo study the expression of zinc finger protein X-linked (ZFX) in bone marrow mononuclear cells (BMMCs) of children with B lineage acute lymphoblastic leukemia (B-ALL) and its relationship with prognosis.
METHODSThe expression of ZFX in human leukemia cell lines (REH, HL-60, NB(4) and K562) was measured by Western blot. ZFX gene was cloned by PCR from one patient and DNA sequencing technology was used to confirm it. Real-time PCR was used for detecting ZFX mRNA expression in the BMMCs of 82 children with newly-diagnosed B-ALL, 24 children with complete remission (CR) after induction therapy and 64 control children (fracture or congenital heart disease patients). According to the presence of bone marrow or central nervous system relapse during a follow-up of 3 years, the patients were identified as having a good or poor prognosis. Their ZFX mRNA levels in BMMCs at diagnosis were compared.
RESULTSZFX protein was expressed in human leukemia cell lines REH, HL-60, NB(4) and K562. ZFX mRNA expression was significantly higher in the newly-diagnosed ALL group than in the control group (P < 0.01). ZFX mRNA expression in the ALL CR group was significantly reduced compared with the newly-diagnosed ALL group (P < 0.01). Children with a poor prognosis had significantly higher ZFX mRNA levels at diagnosis than those with a good prognosis (P < 0.05).
CONCLUSIONSZFX is over-expressed in children with B-ALL and its levels are higher in those with a poor prognosis than those with a good prognosis, which suggests that ZFX is important in the prognosis evaluation of B-ALL.

BACKGROUND: Peak bone mass and standard deviation (SD) in different regions are varied, which have great influence on diagnosis of osteoporosis. To establish a complete database can provide accurate evidence for osteoporosis diagnosis. OBJECTIVE: To investigate the influence of bone mineral densities (BMD) and their SD of young people on the detective rate of osteoporosis in general population. DESIGN, TIME AND SETTING: Investigation analysis was performed at Beijing, Shanghai, Guangzhou, Nanjing, Jiaxing and Chengdu between January 1997 and December 1999. PARTICIPANTS: 11418 subjects from related 6 centers of BMD reference database in China were investigated and analyzed using prospective and retrospective methods, including 3 666 males, and 7 752 females aged 20-90 years. Of them, 2385 were from Beijing, 1178 from Guangzhou, 1404 from Shanghai, 2938 from Nanjing, 1425 from Chengdu, and 2088 from Jiaxing. The subjects were selected from community investigation, physical examination volunteers. METHODS: BMD of the lumbar spine (L2-4) and the hip in 11, 418 subjects from the related 6 centers in China was measured with GE-Lunar dual energy X-ray absorptiometry (DXA) and the BMD reference database was established. The accuracy rate of the inner machine was 0.3%-0.7%, and the accuracy of different machines averaged 1.1%. MAIN OUTCOME MEASURES: Lumbar BMD distribution of different age groups from 6 centers; influence of young people's BMD and its SD on detective rate of osteoporosis. RESULTS: Different BMD and SD were found in the individual subject from 6 centers, and the maximum differences were 0.098 g/cm2 and 0.027 g/cm2. With mean BMD and SD of the individuals from 6 centers as references, different T-scores and the detective rates of osteoporosis derived from the T-scores were found in the same group. The detective rate increased by 1.6% when BMD of the young people increased by 0.01 g/cm2 (positive correlation), but the detective rate decreased by 4% when SD increased by 0.01g/cm2 (negative correlation). CONCLUSION: Changes in BMD and SD of the young people can influence the detective rate of osteoporosis. To achieve comparability for the detective rate of osteoporosis in different centers, the specific reference database should be established for the same race, the same area, and the same bone densitometry machine. The T-score should be determined with the normal BMD and SD of the young, people as the reference database.
8.Investigation on pharmacokinetics of helicid in rats.

OBJECTIVETo report the pharmacokinetic parameters of helicid HD in rats after intravenous administration at different doses.
METHODFifteen Wistar rats were randomly assigned as three groups (n=5, male) to be given helicid solution via tail vein injection at a single dose of 2.23, 4.46, 6.70 mg x kg(-1), respectively. Blood samples were collected via tail vein at time intervals after HD injection. 6% perchloric acid was to precipitate plasma protein. Separation was achieved on a reversed-phase C18 column with UV detection at 270 nm.
RESULTThe calibration curves were linear ranging from 43.8 to 43,800 microg x L(-1). The intra-and inter-day precisions were no more than 6%. The average recovery of helicid was more than 87% from plasma. With minimum akaike information criterion (AIC) values, a two-compartment open pharmacokinetic model was proposed and validated through the DAS 2.0 to explain the apparent diphasic phenomenon of (HD) in rat blood after intravenous administration. Helicid appeared to be distributed rapidly into a highly perfused central compartment (first compartment), with a less rapidly elimination. The diphasic phenomenon could be observed from the compartment model parameters t(1/2alpha) and t(1/2beta). The least squares regression analysis indicated that the plasma concentrations and AUC of helicid in rat plasma were proportional to the administrated doses. At the administrated doses of 2.23, 4.46, 6.70 mg x kg(-1), the values of distribution half-life (t(1/2alpha)) were 4.582, 5.097, 4.727 min, respectively. And the values of elimination half-life (t(1/2beta)) were 23.945, 26.508 and 25.396 min, respectively. The volume of distribution from these three different doses were 0.036, 0.035, 0.035 L, respectively. The AUCo(0-->t) (area under the concentration-time curve) increase was proportional to the administrated dose.
CONCLUSIONIn the range of the doses examined, the pharmacokinetics of helicid in rats is based on linear dynamics.

9.Characteristics of drug resistance and molecular typing for Salmonella in diarrhea patients from four hospitals in Shenzhen

We analyzed the antimicrobial resistance and pulsed field gel electrophoresis (PFGE) patterns of Salmonella strains of infectious diarrhea isolated four hospitals in Shenzhen during 2012-2016.A total of 137 Salmonella strains collected in 4 hospitals in Shenzhen were identified by serotyping,micro-dilution method was used to detect the antimicrobial susceptibility of the isolates.Pulsed field gel-electrophoresis(PFGE) was used for molecular typing and cluster analysis of S.typhimurium and S.enterica strains.Results showed that 137 strains of Salmonella were divided into 30 kinds of serotype.S.typhimurium was the predominant serotype.The drug susceptibility test indicated that the antibiotic resistance of the strains to chloromycetin,ampicillin and amoxicillin were more serious than other drugs,the resistance to penicillins,cephalosporins,aminolycosides and macrolides showed increase trends year by year.Of 49 strains of S.typhimurium were divided 38 molecular patterns by PFGE,with similarity ranged from 65.5% 100%,TY18 was the main PFGE pattern.The 27 strains of S.enterica were divided into 7 molecular patterns by PFGE,with similarity ranged from 61.8%-100%,EN7 was the main PFGE pattern.The status of drug resistance of infectious diarrhea isolates was rather severe,PFGE patterns showed diversity,suggesting the diarrhea cases were distributed sporadically.Partial PFGE patterns and its corresponding antidrug spectrum have certain aggregation relationship.
10.A study on genetic characteristics of Enterovirus type 71 of hand-foot-mouth disease in Shaoxing city

Objective To undersyand the genetic characteristics of Enterovirus Type 71 (EV 71 )isolated from clinical specimens of children with hand-foot-mouth disease (HFMD)in Shaoxing city from 2012 to 2013.Methods RNA was extracted from specimens collected from stool,throat swab and vesicle from children with HFMD,and EV 71 were identified by reverse transcriptase polymerase chain reaction (RT-PCR).Ten positive samples were isolated and were amplified by segmented PCR,then,they were spliced into a full-length after sequenced according to the reference strains. Homology analysis on region VP1 and phylogenetic tree were carried out based on nucleotide sequence and amino acid sequence.Results The results indicated that the ten isolated strains were all EV 71 positive.Sequence alignment of VP1 showed that the nucleotide and amino acid homogeneity of these 10 strains were related to C4a genotypes,with 95.06%-97.87% and 93.60%-98.99% respectively.Phylogenetic tree based on VP1 indicated that these 10 strains all belonged to C4a genotype.Conclusion The identified EV 71 strains isolated from HFMD children in Shaoxing city belong to genotype C4a.
