Thyroid-like papillary adenocarcinoma is an extremely rare neoplasm in the nasopharynx. We present a case with a pedunculated tumor in the nasopharyngeal vault. Complete tumor resection via nasal endoscopy was performed subsequently. Postoperative histopathological examinations and immunohistochemical studies confirmed the diagnosis of thyroid-like papillary adenocarcinoma. This rare neoplasm demonstrated a distinct etiology and nature from typical nasopharyngeal carcinomas and should be managed mainly by surgical resection.

An international symposium on the standard surgical treatment of morbid obesity and type 2 diabetes mellitus was held in Florida, USA in March 2011. An expert panel from all over the world attended the meeting. The expert panelists discussed and established International Sleeve Gastroectomy Expert Panel Consensus Statement : best practice guidelines based on experience of more than 12,000 cases, which made a very commendable attempt in standardized surgical pathway of sleeve gastroectomy. Based on clinical experience over 10 years and the understanding of the consensus , this paper discusses the learning curve, preoperative preparation, indications/contraindications, surgical technique, management and prevention of complications, and post operative management.

5.The value of the combined test for the diagnosis of thalassemia and iron deficiency anemia

Objective To investigate the value of the combined test in the diagnosis of thalassemia and iron deficiency anemia (IDA) .Methods From August 2013 to February 2015 ,36 patients with thalassemia ,34 patients with IDA ,and 30 healthy people who had undergone physical examination were enrolled in the study .The blood samples of those people were tested ,and items of the combined test included content of red blood cell volume(MCA) ,red blood cell volume distribution width(RDW) ,hemoglobin(Hb) electrophoresis and red blood cell(RBC) .Results The patients with IDA or thalassemia had reduced MCA and RBC fragile ,pa‐tients with IDA had increased RDW ,and patients with thalassemia had increased HbA2 ,compared with the control group the differ‐ences were all statistically significant(P<0 .05) .The sensitivity of RDW to IDA was 70 .6% ,and to thalassemia was 77 .8% ,the difference was not statistically significant (P> 0 .05) .The sensitivities of MCV ,Hb electrophoresis ,RBC fragility to thalassemia were 97 .2% ,88 .9% ,77 .8% respectively ,and the specificity were 80 .0% ,90 .0% ,83 .3% respectively .The sensitivities of MCA test combined with Hb electrophoresis ,hemoglobin electrophoresis combined with RBC brittleness test and parallel combined test of MCA ,Hb electrophoresis and RBC brittleness to thalassemia were 100 .0% ,97 .2% ,100 .0% respectively ,which were significantly higher than those of single tests ;the specificities were 96 .7% ,100 .0% and 100 .0% respectively ,which were significantly higher than those of single tests ;the differences were statistically significant(P<0 .05) .Conclusion The application of hematological pa‐rameters ,Hb and RBC in the diagnosis of anemia and IDA could significantly improve the diagnostic accuracy ,and it is worthy of popularization and application .
6.Performance evaluation of the one self-developed device for transmitting negative pressure in RNA extraction

Objective To evaluate the self‐developed device for transmitting negative pressure with large capacity before propa‐gating .Methods Totally 67 ribonucleic acid(RNA) of WBC > 2 .0 × 109 /L fresh whole blood were extracted with the intrinsic de‐vice(control) and the self‐developed device for transmitting negative pressure with large capacity(test) in parallel .The difference of the performance including efficiency ,concentration ,purity and integrity of RNA extraction were evaluated between the two groups . Results In 67 specimens of RNA extraction ,efficiency ,concentration ,purity and integrity of the control device were 0 .97(portion/minute) ,(248 .8 ± 21 .4)μg/mL ,(1 .995 ± 0 .095) (OD260/OD280) ,(2 .020 ± 0 .082) (OD260/OD230) ,1 .964 - 2 .025 (95% con‐fidence interval for mean) (OD260/OD280) ,2 .001 - 2 .040 (95% confidence interval for mean) (OD260/OD230) and complete . Those of test device were 1 .63 portion/minutes ,(260 .3 ± 21 .8)μg/mL ,(2 .093 ± 0 .092) at OD260/OD280 ,(2 .071 ± 0 .120) at OD260/OD230 ,2 .075 - 2 .113 for 95% confidence interval for mean at OD260/OD280 ,2 .044 - 2 .103 for 95% confidence interval for mean at OD260/OD230 and complete .The t value was 24 .570 (P< 0 .001) in paired t‐test ,and the linear regression equation was Y = 0 .950X + 0 .039 ,R2 = 0 .903 for the two groups data of RNA concentration .Conclusion The self‐developed device for transmitting negative pressure with large capacity is excellent in the work efficiency ,concentration ,purity ,integrality and of the RNA in extracting .It is better than the intrinsic device in the clinical value for situation of lab .
7.Application of ultrasound guided autobiopsy technology in interventional diagnosis of pulmonary peripheral disease

Objective To investigate the value of ultrasound guided autobiopsy technology in interventional diagnosis of pulmo-nary peripheral disease .Methods 117 cases of pulmonary peripheral disease proved by X-ray or CT ,confirmed by ultrasound ,but could not be identified the nature by the examination above ,were biopsied under the guidance of ultrasound .Results All the 117 ca-ses were biopsed successfully .115 cases of them had definite pathological results ,the pathological positivity rate was 98 .3% ,in which contained 20 cases of chronic inflammation ,14 cases of tuberculosis ,30 cases of squamous cell carcinoma ,45 cases of adeno-carcinoma ,6 cases small cell undifferentiated carcinoma .No complication ,such as pneumatothorax ,hemoptysis ,bleeding occurred during the operation .Conclusion The ultrasound guided autobiopsy technology in interventional diagnosis of pulmonary peripheral disease is a safe and effective technique ,the tissues obtained from ultrasound guided autobiopsy were adequate to pathological slide which can provide the basis of the clinical diagnosis .
8.Drug Resistance Analysis of Pseudomonas aeruginosa in Intensive Care Unit and Molecular Mechanism of Its Resistance to Fluoroquinolones

OBJECTIVE To study the Pseudomonas aeruginosas drug resistance in intensive care unit and its fluoroquinolone-resistant molecular mechanism,and provide scientific basis for rational employment of antibiotics in clinic. METHODS E test was used to determine the minimal inhibitory concentrations(MIC) of 13 antibiotics against 83 P. aeruginosa strains. Twenty-eight fluoroquinolone-resistant strains were selected with standard sensitive strain-ATCC27853 as control. The quinolone resistance-determining region(QRDR)of the gyrA and parC genes was amplified by PCR and sequenced. RESULTS The positive rate of P. aeruginosa in sputum specimen was the highest from 83 strains (71.08%). gyrA Genes of all resistant strains had an ACC to ATC mutation in codon 83,leading to the amino acid substitution of threonine for a an isoleucine and 11 high level resistant strains also showed a GAC to GGC mutation in codon 87,leading to the substitution of an aspartic acid a glycine for. In addition,14 resistant strains also had an TCG to TTG mutation in codon 87 of parC gene,leading to the amino acid substitution of a serine for a leucine. We didn't find parC gene mutation existing independent in fluoroquinolone-resistant P. aeruginosa. CONCLUSIONS Meropenem remains highly active against P. aeruginosa. But the abuse of imipenem and other fluoroquinolones leads to rise in their drug-resistance rate. Fluoroquinolone-resistance has increased rapidly,the mechanism of resistance is gene mutant. It displayed that gyrA and parC gene mutation is associated with fluoroquinolone resistance.

10.Effect of blocking p38MAPK signal pathway on activity of rat hepatic stellate cells and c-myc protein expression

Objective To study the effect of p38MAPK on the activity and c-myc protein expression in rat acetaldehyde-induced hepatic stellate cell(HSC),and to investigate the alcoholic liver fibrosis related mechanism.Methods The different concentrations of SB203580 as the p38 specific blocker was adopted to conduct the intervention on rat acetaldehyde-induced HSC.The cellular mor-phological change was observed by the microscope.The cell proliferation was detected by MTT,the cell cycle was analyzed by flow cytometry(FCM),and the expression of c-myc protein was examined by the SABC method.Results (1)after acetaldehyde stimula-tion,HSC was increased in size and proliferated rapidly,but with the added SB203580 concentration increase,the cellular prolifera-tion was slowed down,the cells size was diminished and the deformed cells were increased.(2)The proliferation of acetaldehyde-in-duced HSC was inhibited by different doses of SB203580,and the higher concentration has the more significant inhibiting effect.(3) With the SB203580 concentration increase,the cells at the phase G0 and G1 were increased,while the cells at the phase S were de-creased,at the same time the expression positive rate of c-myc protein was decreased.Conclusion Blocking p38MAPK pathway ac-tivity could inhibit the proliferation of acetaldehyde-induced HSC,which may be related to the down-regulation of C-myc protein ex-pression and blocking the DNA synthesis in cells entering from G0/G1 phase to S phase.
