2.Transvenous closure of secundum atrial septal defects with atrial Amplatzer septal ocluder

Objective Our purpose was to report Initial clinical application experience and assess the immediate and short-term effect of transcatheter closure secundum atrial septal defects (ASD) using Amplatzer atrial septal ocluder.Methods Procedures were perfomed under fluoroscopy and transesophageal echocardiography monitoring. Nine patients (3 male, 6 female) underwent attempted transcatheter closure of a ASD using the Amplatzer atrial septal ocluder device with 10F, 11F or 12F long sheath at a mediant age of 33.0?5.2 years (range 8 to 52 years) and weighed 22 kg or more. Other congenital cardiac anomanies which require surgery were excluded. The mediant diameter of ASD at its narrowest segement were mensureted with the balloon catheters was 23.3?6.2 mm (ranger 11 to 30 mm). Systolic pressures of pulmonary arteries were 24.4?5.5 mm Hg (24-46 mm Hg) with catheterization mensuration. After the procedure, TEE were perfomed immediately to find whether there any residual shunt retained. Follow-up evaluation was color flow mapping at 24 h, 1, 3 and 6-months after closure. Results Nine patients had successful device placement. Transesophageal echocardiography showed that 8 patients had complete immediate closure and one had a small residual shunt after the operation, and could get up in the next day. The complication of cerebral throumbus embolism occurred in one woman patient during the procedure. She accepted immediately thrombolysis therapy with urokinase and recovered after several days. Conclusion Transcatheter closure of secondum ASD using Amplatzer atrial septal occluder device is an effctive nonsurgical therapy method. The operation has specialities of simple, safe with a high succes rate of placement and a fine occlusion effect. Further clinical trias are underway.
3.Pharmacoeconomics Evaluation of CYP2C19 Genetic Test Guiding Antiplatelet Therapy for Acute Coro-nary Syndrome Patients in China

OBJECTIVE:To evaluate the feasibility of using CYP2C19 gene to guide the use of antiplatelet agents in acute cor-onary syndrome(ACS)patients undergoing interventional operation in China from the viewpoint of pharmacoeconomics. METH-ODS:Based on global PLATO trial data,the patients were divided into 3 groups according to treatment strategies(clopidogrel group,ticagrelor group,genetic test group). By applying Treeage Pro 2011 software,short-term decision tree model and long-term Markov model were established,and related data were imported to evaluate cost-effectiveness of different treatment strategies. RE-SULTS&CONCLUSIONS:For ACS patients,according to the level of GDP in China,conventional use of ticagrelor is of pharma-coeconomic advantage for in Shanghai and uninsured patients insured patients;the use of antiplatelet agents guided by CYP2C19 gene test is of pharmacoeconomic advantage among nationwide insured patients.
4.Clinic observation on shenqi 11 flavor capsule as adjuvant treatment in advanced NSCLC patients during chemotherapy

Objective To evaluate the clinical effects and immune function of shenqi 11 flavor capsule as an adjuvant treatment in advanced NSCLC patients during chemotherapy.Methods 58 cases of advanced NSCLC were divided into two groups by mean of digital random table method,with 29 patients in each group.GP scheme chemotherapy was applied to patients in group B,using gemcitabine (1000 mg/m2) on the 1st and 8th day intravenously (procedure should be finished within 30 minutes) and cisplatin (75 mg/m2) on the 1st day intravenously.Each cycle was 3 weeks,two cycles in total.Same scheme was applied to patients in group A and with an addition of shenqi 11 flavor capsule(1.65 g/time,3 times/d) throughout the whole procedure of chemothrapy.Recent curative effect,life quality,toxic reactions and changes in immune indexes of both groups were observed.Results Short-term response rate in group A was 48.27% and 41.38% in the group B (x2=0.279,P>0.05),which showed no significant differences between the two groups.Toxic reactions such as leukopenia,hematochrome decrease and gastrointestinal reactions in the group B were significantly severe compared to group A after chemotherapy(x2 were 4.678,4.549 and 4.687 respectively,P<0.05).As for immune indexes after chemotherapy,CD3+ (55.21 ± 3.28) %,CD4+ (38.84±5.13) %,CD8+ (29.86±4.83) %,CD4+/CD8+( 1.29± 0.17) and NK cells (20.12± 2.11 ) %in the group B indicated significant differences compared to those in group A[(62.96±4.12)%、(45.21±3.43)%、(25.23±2.79)%、(1.82±0.21)、(25.78±3.36)%],(P<0.05).The life quality was 58.62% in group A and 31.03% in the group B,which showed significant difference (x2=4.462,P<0.05).Conclusion Shenqi 11 flavor capsule as an adjuvant treatment in advanced NSCLC patients during chemotherapy can not only improve life quality of patients,alleviate the symptoms,but also enhance their immune function.
5.Short-term effects of Aidi injection adjuvant GP methods chemotherapy in treatment for advanced non-small cell lung cancer

Objective:To study the short-term effects,toxic side effects and immune function of Aidi injection adjuvant GP methods chemotherapy for advanced non-small cell lung cancer(NSCLC).Methods:130 patients with advanced NSCLC were divided into study group and the control group according to treatment methods.The study group were given Aidi injection +GP,and the control group only treated with GP.The short-term effects,toxic side effects gradeⅢ-Ⅳand immune function changes were compared between the two groups.Results:The overall response rate was 40.28%in study group,higher than 37.93% in control group(P>0.05).The two groups showed differences in the rates of leucocyte decrease,gastrointestinal reaction and platelets decrease(P<0.05).The levels of CD4,CD8 and CD4/CD8 didn′t changed obviously in study group(P>0.05),while in the control group the levels of CD4,CD8 and CD4/CD8 changed more significantly than before(P<0.05).The two groups also produced differences in CD4,CD8 and CD4/CD8 after treatment( P<0.05 ) .Conclusion: The short-term effects of Aidi injection adjuvant GP chemotherapy regimen for advanced NSCLC is similar to only GP chemotherapy,but it can obviously reduce the incidence of toxic side effects and prevent the immune func-tion.
6.Filling effect of autologous skin fibroblasts: a study of two-photon fluorescence microscopy

Objective To investigate the survival profile of the intradermally injected mouse autologous skin fibroblasts and the changes of the collagen fibers by using green fluorescent protein labeling and two-photon fluorescence microscopy. Methods The cultured cells were transfected by EGFP lentivirus, and then the cells were injected into the corresponding mouse skin. Biopsy was taken from the animals after 1 and 2 months. The specimens made serial frozen sections, the survival profile of the injected cells and the changes of the collagen fibers were observed by two-photon fluorescence microscopy. The collagenic area and dermal thickness were measured with image analysis software, and statistical analysis was also carried out. Results Two-photon fluorescence microscopy showed clear images of the injected cells and collagen fibers. Both the area of collagen fibers and the dermal thickness were significantly increased in injected cells after 2 months (P<0.05), however, there were no difference between injected cells and control after 1 mouth (P>0.05). Conclusions Autologous cultured fibroblasts could survive in a long time after transplantating into the skin, and collagen could be newly produced, the depth of dermis increases, which provides a possibility to treat mini-defects of the tissue.
8. Research on drug use rules of ancient and modern medical prescriptions for differentiation and treatment of digestive system tumors based on data mining

To investigate the rule of prescription medication in the treatment of digestive system tumors. All the digestive system cancer prescriptions in Cancer Prescription Daquan were entered into Microsoft Excel 2010, the database was established and the frequency, drug pairs and categories of traditional Chinese medicine were counted. IBM SPSS Modeler 14.1 was used for correlation analysis, and IBM SPSS Statistics 21 was used for clustering and factor analysis. A total of 501 prescriptions for digestive system tumors were studied, involving 651 kinds of Chinese materia medica. Poria, Citri Reticulatae Pericarpium, Aucklandiae Radix, and Angelicae Sinensis Radix were the most frequently used drugs, and Poria-Atractylodis Macrocephalae Rhizoma, Sparganii Rhizoma-Curcumae Rhizoma, Citri Reticulatae Pericarpium-Aucklandiae Radix were the most frequently used drug pairs; Chinese herbal medicine categories of heat-clearing drugs, deficiency-tonifying drugs, blood-activating and stasis-removing drugs were the most frequently used. The highest frequency of Sparganii Rhizoma-Curcumae Rhizoma and Poria-Atractylodis Macrocephalae Rhizoma was found in correlation network diagram; Cluster analysis found 12 Chinese herbal medicine combinations; Six common factors were extracted from the analysis, and the highest loads in sequence were Atractylodis macrocephalae Rhizoma, Sparganii Rhizoma, and Rhei Radix et Rhizoma. The prescriptions of digestive system tumors contained in Cancer Prescription Daquan are based on the core of dispersing symptoms and resolving nodules. Its usually change and use flexibly drugs from three angles of regulating qi, removing dampness, promoting qi and reducing adverse reactions to disperse turbidity, activating blood circulation and benefiting qi, and eliminating blood stasis; clearing away heat, fighting toxins and activating blood circulation to resolve accumulate.
9.The influence of ginkgo dipyridolum injection on blood-rheological and coagulation function in pationts with AECOPD

ObjectiveTo evaluate the influence of ginkgo dipyridolum injection on blood-rheological and coagulation function in pationts with AECOPD.Methods78 cases of patients with AECOPD were divided into group A (38 cases)and group B (40 cases)by mean of digital random table method.The routine treatment including oxygen inhalation,spasmolytics and anti-infection was given to patients in both groups while ginkgo dipyridolum injection (30 ml/d)was additionally added to patients in group B for two weeks.The blood-rheological and coagulation function were evaluated.ResultsAs for blood-rheological after the treatment,whole blood high shearing viscosity(5.25 ± 1.24)mPa · S,low shearing viscosity (11.12 ±2.43) mPa · S,plasma viscosity (2.06± 0.14 ) mPa · S and hematocrit (45.52 ± 2.78) % in the group A indicated significant differences compared to those in group B [ (4.83 ± 1.42)mPa· S,(8.78± 3.02) mPa · S,(1.73 ±0.21) mPa · S,(39.05 ± 3.41) %],(P<0.05); as for coagulation function after the treatment,PT (13.14± 1.31 ) S,APTT (30.85±5.24)S,FIB (4.99±1.04)S,D-D (1.42±0.23)mg/L in the group A indicated significant differences compared to those in group B [ (14.78 ± 3.13) S,(36.67 ± 8.12)S,(3.81 ± 0.42) S,(0.84 ±0.39) mg/L],(P<0.05).ConclusionFor the patients with AECOPD,the ginkgo dipyridolum injection can decrease hood-viscosity,and obviously improve hypercoagulabale state.
10.Discussion on Tongxie Yaofang for Treatment of Constipation

Tongxie Yaofang is a classic formula in the treatment of diarrhea caused by liver wood restricts earth, but the clinical application is not confined to diarrhea with pain. According to TCM thought of treating different diseases with the same method, it can also be used for the treatment of constipation caused by liver and spleen discord. Combining modern study and linking clinic, this article discussed the mechanism of this formula for the treatment of constipation, with a purpose to inspire thought and expand its clinical application range.
