1.Detection of EGF in Tears from Patients with Herpes Simplex Virus Keratitis

To study the secretion of epidermic growth factor (EGF) in tears from patients with herpes simplex virus keratitis (HSVK), radio-immunoprecipitation was used to detect the value of EGF in human reflex tears from 40 cases as control group (group A), 49 cases of acute HSVK (group B) and 31 cases of convalescence HSVK (group C). Results showed that the value of EGF in reflex tears from the patients in the group B was obviously increased as compared with that in the group A and group C (P<0.001). The value of EGF in tears in the group C was higher than in group A (P<0.05). It was concluded that the value of EGF in reflex tears from HSVK patients was higher than that from normal people.
2.Nerve growth factor modulate proliferation of cultured rabbit corneal endothelial cells and epithelial cells.

In order to investigate the effect of nerve growth factor (NGF) on the proliferation of rabbit corneal endothelial cells and epithelial cells, the in vitro cultured rabbit corneal endothelial cells and epithelial cells were treated with different concentrations of NGF. MTT assay was used to examine the clonal growth and proliferation of the cells by determining the absorbency values at 570 nm. The results showed that NGF with three concentrations ranging from 5 U/mL to 500 U/mL enhanced the proliferation of rabbit corneal endothelial cells in a concentration-dependent manner. 50 U/mL and 500 U/mL NGF got more increase of proliferation than that of 5 U/mL NGF did. Meanwhile, 50 U/mL and 500 U/mL NGF could promote the proliferation of the rabbit corneal epithelial cells significantly in a concentration-dependent manner. However, 5 U/mL NGF did not enhance the proliferation of epithelial cells. It was suggested that exogenous NGF can stimulate the proliferation of both rabbit corneal endothelial and epithelial cells, but the extent of modulation is different.

4.Construction of core competence evaluation index system for intravenous therapy specialized nurses

Objective To construct the core competence evaluation index system for intravenous therapy specialized nurses.Methods Using literature research,group discussion,and two-round Delphi expert consultation,the core competence evaluation index system for intravenous therapy specialized nurses was constructed.Results The response rates for two rounds of expert consultation were both 100%,authority coefficients were 0.93 and 0.94,and coordination coefficients at all levels ranged from 0.252 to 0.418.The core competence evaluation index system for intravenous therapy specialized nurses consisted of 8 first-class indexes,35 second-class indexes and 93 third-class indexes.Conclusion The enthusiasm,authority,concentration degree and coordination degree of experts were comparatively high.It can provide references for training,assessment,certification,utilization and management of intravenous therapy specialized nurses.
5.Expression and significance of matrix metalloproteinase-1,9, tissue inhibitor of metalloproteinase-4 and extracellular matrix metalloproteinase inducer in the myocardium of congestive heart failure in patients with rheumatic heart diseases.

OBJECTIVE:
To investigate the expression of the matrix metalloproteinase-1, -9 (MMP-1,9), and tissue inhibitor of metalloproteinase-4 (TIMP-4), extracellular matrix metalloproteinase inducer (EMMPRIN) in the myocardium of congestive heart failure in patients with rheumatic heart diseases.
METHODS:
The papillary muscle specimens of the left ventricle were obtained from 18 patients with heart failure during rheumatic heart valve replacement, and the normal specimens were obtained from the autopsy of 10 adults without heart disease. The specimens were stained to examine the expression of MMP-1, MMP-9, TIMP-4, and EMMPRIN by the EnVision immunohistochemical assays.
RESULTS:
The expressions of MMP-1, MMP-9, and EMMPRIN in the myocardium of the patients with the cardiac function classes III and IV (NYHA III and IV) were significantly higher than those the normal cardiac function (NYHA I) (P<0.05) except MMP-1 expression between NYHA III and I. The TIMP-4 level was significantly lower in patients with NYHA III and IV than that of the NYHA I ( P<0.05). The expression of MMP-1, MMP-9, and EMMPRIN was significantly higher in patients with atrial fibrillation than those in the control group with regular sinus rhythm (P<0.05), whereas the TIMP-4 level was obviously lower in patients with atrial fibrillation than that in the control group with regular sinus rhythm (P<0.05). The expressions of MMP-1, MMP-9, and EMMPRIN were positively correlated with each other, but were negatively correlated with TIMP-4.
CONCLUSION
MMPs, TIMP, and EMMPRIN were significantly unbalanced in the myocardium of congestive heart failure patients with rheumatic heart diseases. They may play an important role in congestive heart failure through myocardial remodeling.

6.Laparoscopy combined with transperineal extralevator abdominoperineal excision for locally advanced low rectal cancer.

OBJECTIVETo evaluate the laparoscopy combined with transperineal extralevator abdominoperineal excision (TP-ELAPE) for locally advanced low rectal caner.
METHODSClinical data of 12 patients with locally advanced low rectal cancer undergoing laparoscopy combined with TP-ELAPE in our department from May 2013 to March 2015 were retrospectively analyzed. There were 8 male and 4 female patients with median aged of 63 (46 to 72) years. The median distance from tumor lower margin to anal verge was 3.5(2.0 to 4.0) cm. A self-made transanal suit for minimally invasive operation was used to make a sealed lacuna outside the sphincter, thus laparoscope can be applied to perform transperineal operation.
RESULTSAll the patients underwent operations successfully without conversion to open abdominal operation. The median operating time was 206 (180 to 280) minutes with perineal operating time 95(80 to 120) minutes. The median intraoperative blood loss was 120(50 to 200) ml. The median postoperative hospital stay was 12(9 to 18 ) days. Postoperative pathology revealed that all circumferential margins (CRM) were negative. The area of sample horizontal section was (2 824±463) mm(2), and of outer muscularis propria was(2 190±476) mm(2). Postoperative complications included chronic sacrococcygeal region pain in 2 cases, urinary retention in 3 cases, perineal wound infection in 1 case. No perineal seroma, perineal hernia, wound dehiscence and sinus tract formation were observed. Among 8 patients with preoperative normal sexual function, sexual dysfunction occurred in 2 patients. There was no local recurrence and metastasis during a median follow-up of 21(12 to 34) months.
CONCLUSIONLaparoscopy combined with TP-ELAPE has the potential to simplify the operation procedure for low rectal cancer, can ensure the radical treatment and safety of operation, and may be carried out in experienced centers.

7.Application value of biological mesh in the pelvic floor reconstruction of extralevator abdominoperineal excision for advanced low rectal cancer

Objective To investigate the application value of biological mesh in the pelvic floor reconstruction of extralevator abdominoperineal excision (ELAPE) for advanced low rectal cancer (RC).Methods The retrospective cohort study was conducted.The clinicopathological data of 228 patients with advanced low RC who underwent ELAPE in the Beijing Chaoyang Hospital of Capital Medical University between August 2008 and December 2016 were collected.Of 228 patients,174 using biological mesh closure and 54 using primary closure were respectively allocated into the biological mesh group and primary closure group.Observation indicators:(1)intra-and post-operative situations;(2) postoperative complications (including short-term and long-term complications);(3) follow-up and survival situations.Follow-up using outpatient examination and telephone interview was performed to detect postoperative long-term complications,tumor recurrence or metastasis and overall survival up to December,2017.Measurement data with normal distribution were represented as( x) ±s,and comparison between groups was analyzed using the independent-sample t test.Measurement data with skewed distribution were described as M (range),and comparison between groups was analyzed using the nonparametric test.Comparisons of count data were evaluated by the chi-square test or Fisher exact probability.Results (1)Intra-and post-operative situations:all the patients underwent successful ELAPE.The perineal operation time,time of indwelling perineal drainage-tube and hospital expenses were respectively (60 ± 50)minutes,(11.6 ± 2.4) days,(57 781± 11 337) yuan in the biological mesh group and (50±21) minutes,(8.9± 1.7) days,(53 714± 13 395)yuan in the primary closure group,with statistically significant differences between groups (t =3.327,7.691,-2.203,P<0.05).The total operation time and duration of postoperative hospital stay were respectively (242±53) minutes,(13.0±5.0) days in the biological mesh group and (228±51) minutes,(12.0±5.0) days in the primary closure group,with no statistically significant difference between groups (t =1.701,1.309,P>0.05).(2) Postoperative complications:26 and 19 patients in the biological mesh group and primary closure group had respectively perineal wound complications (1 patient combined with multiple complications),showing a statistically significant difference between groups (x2 =10.660,P<0.05).The perineal wound infection,perineal hernia and disruption of perineal wound were respectively detected in 20,6,1 patients in the biological mesh group and 12,7,3 patients in the primary closure group,showing statistically significant differences between groups (x2 =3.931,5.282,P<0.05).(3) Follow-up and survival situations:174 patients in the biological mesh group were followed up for 64 months (range,13-112 months),and 54 patients in the primary closure group were followed up for 51 months (range,23-76 months).The local recurrence rate,distal metastasis rate and overall survival rate were respectively 5.17% (9/174),20.11% (35/174),77.59% (135/174) in the biological mesh group and 7.41%(4/54),24.07%(13/54),79.63%(43/54) in the primary closure group,with no statistically significant difference between groups (x2 =0.080,0.389,0.101,P>0.05).Conclusions The biological mesh in the pelvic floor reconstruction of ELAPE for advanced low RC is safe and feasible.Compared with primary closure,biological mesh closure will extend perineal operation time and time of indwelling perineal drainage-tube,and increase hospital expenses,but doesn't affect total operation time and duration of postoperative hospital stay,meanwhile,it can also reduce the overall perineal wound complications,especially in perineal wound infection,perineal hernia and disruption of perineal wound.
8.Physiological and enrichment characteristics of Paulownia fortunei seedlings under zinc, cadmium and their combined stress.

The hydroponic culture test method was used to study the physiological and biochemical responses of Paulownia fortunei seedlings under Zn stress, Cd stress, and combined Zn and Cd stress as well as changes in the enrichment and transfer characteristics of heavy metals. Under single and combined heavy metal stress, the biomass, plant height, and peroxidase (POD) activity of P. fortunei decreased as the treatment concentration increased. Combined Zn and Cd affected adversely plant height and biomass. As the concentration of Zn increased when applied alone, the chlorophyll content and catalase (CAT) activity of P. fortunei first increased and then decreased, the superoxide dismutase (SOD) activity increased, and the aboveground malondialdehyde (MDA) content first decreased and then increased. As the concentration of Cd increased when applied alone, chlorophyll content and CAT activity increased, and SOD activity and aboveground MDA content first increased and then decreased. Under both Cd and Zn, the physiological response was more complex. Cd in the seedlings of P. fortunei was concentrated in the root. In contrast, Zn was concentrated in the upper part of the ground, and its transfer coefficient was greater than 1.00. Thus, the addition of Zn promotes the transfer of heavy metals to the above-ground portions of plants. Generally, P. fortunei can effectively promote ecological restoration under complex forms of heavy metal pollution.

9.Study on activity conditions screening and mechanism of Panax ginseng-Poria cocos pair against aging

OBJECTIVE To screen the best compatibility ratio and administration conditions of Panax ginseng -Poria cocos pair against aging ,and investigate its mechanism . METHODS P. ginseng-P. cocos pair extracts with different compatibility ratios (1∶1,1∶2,2∶1,1∶4,4∶1,m/m)were prepared ;taking Saccharomyces cerevisiae as the aging model organism ,the S. cerevisiae growth curve was drawn by MTT method ,the best compatibility ratio ,administration concentration and administration time point of P. ginseng-P. cocos pair were screened out ;the activities of antioxidant related enzymes [superoxide dismutase (SOD),peroxidase (POD),catalase(CAT)],the levels of reactive oxygen species (ROS)and malondialdehyde (MDA),the content of adenosine triphosphate(ATP),and the mitochondrial membrane potential (MMP)in S. cerevisiae cells were detected ;mRNA expressions of SOD1,CTT1,GSH1,ATP1,MRS1 and CDC 19 were also detected . RESULTS The optimal ratio of P. ginseng-P. cocos pair for anti-aging activity was 1∶4(m/m),the optimal administration concentration was 220 μg/mL,and the optimal administration time point was the 28th hour . The extracts of P. ginseng-P. cocos pair(1∶4,m/m)could significantly increase the activities of SOD , POD and CAT ,ATP content ,MMP,mRNA expression of CTT 1,GSH1 and MRS 1(P<0.01),but decrease the levels of MDA and ROS ,mRNA expressions of SOD 1,ATP1 and CDC 19(P<0.05 or P<0.01). CONCLUSIONS P. ginseng-P. cocos pair(1∶4, m/m)has a good anti -aging effect on S. cerevisiae ,its mechanism may be related to the positive regulation of oxidative stress and energy metabolism of S. cerevisiae cell.
