3.The value of somatosensory evoked potentials in early diagnosis of cervical spondylotic radiculopathy

Objective To investigate the value ofsomatosensory evoked potentials (SEP) in early diagnosis of cervical spondylotic radiculopathy (CSR). Methods The median nerve SEP (SEPm) was examined in 65 cases of CSR (experimental group), and was compared with 30 controls(control group). All the patients were examined by MRI of the cervical spine which were compared with SEPm. All cases were followed up 12 months, at the same time MRI and SEPm were examined repeatedly. Results The experimental group with CSR had significantly prolonged peak latency (PL) of N11 and N13 wave and interpeak latency(IPL) of wave N9-NI3 within 3 months [( 12.92 ± 0.97), (14.81 ± 1.16), (4.45 ±0.65)ms] and at the 12th month [( 14.20 ± 1.06), ( 16.35 ± 1.75), (5.62 ± 0.88) ms] compared with those of control group[(11.65 ± 0.69), (13.26 ± 0.78), (3.56 ± 0.31 ) ms](P< 0.05). The PL of Nll and N13 wave and IPL of wave N9-N13 significantly prolonged at the 12th month compared with those within 3 months.SEPm provided more sensitive modality for the diagnosis[87.69%(57/65 )] of CSR within 3 months than that of MRI [70.77% (46/65)] ( X2 =4.35,P <0.05). While there was no significant difference in sensitive between SEPm [90.77%(59/65)] and MRI [81.54%(53/65)] at the 12th month (X2= 1.56,P>0.05).Conclusion SEP is valuable for the early diagnosis of CSR and may be clinically utilized as a sensitive indicator of CSR.

6.Comparison of transcatheter intervention and surgical operation in pulmonary atresia with intact ventricular septum

Objective To compare the results of transcatheter intervention (TI) and surgical operation (SO) in the pri-mary treatment for pulmonary atrcsia with intact ventricular septum (PA/IVS). Methods From January 2006 to May 2009, 25 patients (20 male,5 female) with PA/IVS were treated. The age at treatment was from 2 days to 8 months. The body weight was from 2.1 kg to 6.7 kg. All patients had mild to moderate hypoplasia of the right ventricle (the Z-valvue of the tricuspid valve: from -2 to 1.5) with tripatite right ventricle and without coronary artery-right ventricular fistula. Eight patients under-went TI and 17 patients underwent SO. Results The primary procedure was successful in 7 patients (88%) in TI group and in 16 patients (94%) in SO group. There were 2 deaths (1 in each group). There was no significant difference regarding re-sidual pulmonary stenosis between two groups. The ventilation time and the days of hospital stay were shorter in TI group than in SO group. All the survivors were followed up for 3-36 months. One patient required repeat balloon dilation in TI group. While in SO group, one required reoperation and one required balloon dilation for PS, and 2 patients were waiting for balloon dilation. Twenty-one patients have achieved complete biventricular circulation. Two patients were scheduled for Glenn shunt operation later (1 in each group). Conclusion For PA/IVS patients with mild or moderate right ventricle hypoplasia, tran-scatheter intervention is a better alternative than surgical operation in the primary treatment.

Objective To study the natural developing characteristics of language skill of the autistic children. Methods 300 untreated autism children were included. Their developmental characteristics of individuals were evaluated by the Psycho-Educational Profile-Revised (PEP-R). Results The developmental quotient of most children (89.33%) was less than 70. And the developmental quotient gradually decreased with age (P<0.01). The scores of language performance and language verbal were deviation (P<0.01). Conclusion Language characteristics of autistic children without treatment is abnormal in development progress and order, and the situation would be worse with age.
8.Case-control study on the treatment of humerus supracondylar fracture of Gartland III in children by manipulative reduction and Kirschner wire percataneous internal fixation.

OBJECTIVETo study the clinical effect of the Gartland III humerus supracondylar fractures in children by manipulative reduction and Kirschner wire percataneous internal fixation.
METHODSFrom July 2010 and July 2013, 60 patients with Gartland III humerus supracondylar fracture were selected and divided into treatment group and control group. In the treatment group 32 patients were treated with traditional bone setting tetradeca-manipulative reduction and percataneous Kirschner wire internal fixation,included 18 males and 14 females with an average age of (7.8 +/- 2.7) years old ranging from 5 to 11; in the control group 28 patients were treated with open reduction and Kirschner wire internal fixation,included 16 males and 12 females with an average age of (7.2 +/- 3.0) years old ranging from 4 to 12. The motion range of the elbow joint,the time of fracture clinical healing, and the effect after 6 months of Flynm clinical functional assessment standards were observed and compared.
RESULTSThe average fracture healing time of the control group (5.01 +/- 0.43) weeks was longer than that of the treatment group (4.29 +/- 0.29) weeks (t = 7.49, P = 0.00). At 6 months after treatment,the elbow motion range of the treatment group (146.02 +/- 2.28) was more than that of the control group (140.76 +/- 4.42) (t = -5.67, P = 0.00). At 6 months after treatment, according to Flynn evaluation, in the control group,there were 7 cases as excellent, 16 as good, 4 fair, 1 poor; in the treatment group, excellent in 21, good in 9, fair in 2 (U = 3.09, P = 0.002).
CONCLUSIONManipulative reduction and Kirschner wire percataneous internal fixation for treatment of children's Gartland III humerus condyle fractures can shorten fracture clinical healing time and the clinical curative effect is better.

9.Evaluation of cardiopulmonary allograft function for a combined heart-lung transplantation patient survived 5 years

Objective To evaluate the cardiopulmonary allograft function and to analyze key factors for long-term survival of heart-lung transplantation in a patient survived more than 5 years. Methods On December 17th, 2003 at Zhongshan Hospital of Fudan University, a homologous heart-lung transplantation was performed on a female who diagnosed with cardiopulmonary failure secondary to congenital atrial septal defect with severe pulmonary hypertension. Heart-lung allograft was preserved with 1 000 mL UW solution and 4 000 mL HTK solution.Postoperative immunosuppressive therapies were managed with Zenapax, cyclosporine A (or tacrolimus), mycophenolate mofetil and corticosteroids. Cyclosporine A maintained with serum trough levels of 100-200 μg/L and tacrolimus with serum trough levels of 8-20 μg/L. Cardiopulmonary allograft functions were evaluated by echocardiogram, pulmonary function test and thoracic CT periodically. Results The patient survived operation and experienced normal daily life with NYHA cardiac function of class Ⅰ-Ⅱ during the follow-up of 5 years and 6 months. Echocardiogram showed left ventricular ejection fraction of 65% to 86%. Pulmonary function test exhibited with nearly normal oxygen exchange, meanwhile, small airway obstruction was detected from one year after operation and keeping stable from then on. Two episodes of severe pneumonia were complicated and treated with antibiotics and fhconazob, no severe acute allograft rejection episode was experienced. Conclusions Heart-lung transplantation proves to be a reliable therapy modality for terminal cardiopulmonary failure. Excellent donor organ preservation, accurate balance of the risk between acute allograft rejection and infection, and strict preventive measures against infection are key factors for long-term survival of heart-lung transplantation.

Objective To deplore the immunoregulatory function changes of mesenchymal stem cells(MSCs)from multiple myeloma(MM)patients and its effects on the pathogenesis of myeloma bone disease.Methods MSCs from MM patients and normal controls were isolated and the immunophenotype was detected.Real-time PCR was performed to detect the expressions of TGF-β1,TGF-β2,TGF-β3,IL-6,IL3,TNF-α,FasL and RANKL of MSCs.Transwell coculture systems were performed between MSCs and T cells.Lymphocyte proliferative assay was employed to detect the effect of MSCs on T cell proliferation.The effect of MSCs on T cell cycle and T cell activation markers CD25 and CD69 expression were analyzed by flow cytometry.Cleaved caspase 3 protein by western blot and hoechst 33258 staining were employed to detect the apotosis of T cells.Influence of T cells on the osteogenesis potential of MSCs were detected by Von kossa stain,real-time PCR and Western blot.Results MSCs from both MM patients and normal controls possessed similar morphology and immunophenotypes.MM derived MSCs exhibited increased expressions of TGF-β1,IL-6,IL-3,TNF-α and RANKL and decreased expression of TGF-β2,TGF-β3 and FasL.The inhibitory effect of MM derived MSCs on T cell proliferative ability was attenuated compared to control MSCs.MSCs from normal controls silence more T cells in Go/G1 phase than those from MM patients.The daupening effect of MM derived MSCs on activation-induced T apoptosis seemed to be enhanced.Expression of T cell activation markers were significantly inhibited by MSCs from normal controls.Both T cells cocultured with MM deprived MSCs and T cells directly from MM patients inhibited osteogenesis potential of MSCs from normal controls.Conclusion MSCs from MM patients showed impaired immunoregulatory capability on T cells.The activated T cells,in turn,inhibited the osteogenesis potential of MSCs.This may participate in the pathogenesis of myeloma bone disease.
