2.The value of serum glycosyl protein determination in acute cerebral vascular diseases

Serum glycosyl protein and glucose levels were concomitantly measured in 35 DACVD patients, 59 ACVD patients and in a control group of 65 healthy subjects. Our data revealed statistically significant differences in serum glucose between the ACVD group and control group (P0.05).Hyperglycosemia occurring in ACVD may be a response, or really means diabetes mellitus. Our data indicated that determination of serum glycosyl protein may be useful for the differential diagnosis of diabetic neuropathy from the reactive hyperglycosemia in ACVD.
3.Evaluation of the clinical effects of implant-supported overdenture with ball or Locator attachment

Objective:To evaluate the clinical effects of ball attachment and Locator attachment for implant-supported overdenture.Methods:67 cases with edentulous jaw were treated with 150 Straumann and Bego implants and implant-supported overdentures using ball attachments Locator attachments,respectively.All cases were followed up regularly,biological and mechanical complications were observed,the patient satisfaction after restoration was compared between groups.Statistical analysis was performed using SPSS 17.0 software.Results:Follow-up was completed in 58 cases,25 with ball attachment,26 with Locator attachment and 7 with ball changed by Locator.Biological complications comparison showed that the incidence of gingival hyperplasia in the ball group was higher than that in the Locator group(P < 0.05).There was no statistical difference of mechanical complications between the 2 groups (P > 0.05),but the incidence of all kinds of mechanical complications in the Locator group was less than those in the ball group,the average repair frequency of the ball and Locator group was 1.9 times and 0.9 times respectively.Patients'satisfaction on chewing and retention was higher in the Locator attachment group than that in the ball attachment group(P < 0.05).No statistical difference was found in patients' satisfaction between genders (P>0.05),and no correlation of satisfaction was found with patients' age and follow-up time.Conclusion:The clinical effects of implant-supported overdenture using the Locator attachment is superior to that using the ball attachment.
4.Establishment and Implementation of Management Method of Second Kind Psychotropic Drugs in Outpatient Pharmacy

OBJECTIVE:To establish scientific and rational management of second kind psychotropic drugs.METHODS:The account book of second kind psychotropic drugs was set up according to specification of narcotic drugs and first kind psychotropic drugs.Daily radix management was carried out by exclusive person,account book and counter,as compared with traditional management.RESULTS & CONCLUSIONS:In this management method,checking time was decreased to 20 min from 50 min.The rate of consistency between account and drug was up to 98% from 95%.The method is superior to traditional one.
5.The effect of doctor-nurse-patient collaborative nursing model on patients′ self-management ability and cardiovascular events after PCI

Objective:To investigate the effect of doctor-nurse-patient collaborative nursing mode on long-term self-management ability and cardiovascular events of patients after percutaneous coronary intervention (PCI).Methods:Totally 160 patients who received PCI from the department of cardiology of the First Affiliated Hospital of Chengdu Medical College were randomly assigned to control group ( n=80) and experimental group ( n=80). The traditional nursing model was adopted in the control group. However, the nursing mode of doctor-nurse-patient collaborative was adopted in experimental group. The self-management ability and cardiovascular events in patients after PCI at 6 months and 12 months in two groups were both measured by chronic disease management software, self-behavioral management scale, Minnesota Living Scale, self-made medical care work assessment scale and self-made satisfaction questionnaire. Results:The rates of adverse habits management, symptom management, emotional cognition, emergency management, disease knowledge management, general life management, and therapy compliance management in the experimental groups, 6 months after operation, were 96.3%(77/80), 73.8%(59/80), 80.0%(64/80), 91.3%(73/80), 85.0%(68/80), 81.3%(65/80), respectively. In the control group, the above indexes were 85.0%(68/80), 56.3%(45/80), 62.5%(50/80), 75%(60/80), 75%(60/80), 65%(52/80). The difference between the two groups was statistically significant ( χ 2 value was 5.375-7.530, P<0.05). In the experimental group, 12 months after PCI, the rates of the above indexes were 91.3%(73/80), 78.8%(63/80), 82.5%(66/80), 93.8% (75/80), 87.5%(70/80), 86.3%(69/80), 93.8%(75/80). While, the ratios in the control group were 77.5%(62/80), 52.5%(42/80), 66.3%(53/80), 78.8%(63/80), 66.3%(53/80), 65.0%(52/80), 75.0%(60/80), there was significantly difference between two groups ( χ 2 value was 5.542-12.218, P<0.05). Furthermore, there were 12 and 38 cases of cardiovascular events at 6 months and 12 months after operation, and 28 and 60 cases in the control group, respectively. Meanwhile, the number of re-hospitalizations events in experimental group were 13, and 27 in the control group, after 12 months operation, there was significantly difference between two groups( χ 2 value was 8.533,12.745, 6.533, P<0.05). The quality of life scores and medical work quality score in the experimental group in different months were 31.26±8.39 (6 months), 26.39±4.76 (12 months), 77.44±8.21 (6 months) and 71.50±5.73(12 months), respectively. However, the scores in the control group were 40.51±8.38, 35.39±4.89, 65.90±5.62, 63.55±5.62, there was significant difference between two groups ( t value was -10.373-11.804, P<0.01). And the overall satisfaction of patients in the experimental group [96.3%(77/80)] was significantly higher than the control group [85.0% (68/80)]. The difference between the two groups was statistically significant ( χ 2 value was 5.959, P<0.05). Conclusion:The doctor-nurse-patient collaborative nursing mode can improve the long-term self-management ability, reduce the occurrence of cardiovascular events and the number of re-hospitalizations in patients after PCI, and improve the quality of life and satisfaction of patients.
6.Construction and identification of a lentiviral vector mediating RNA interference of tyrosine kinase B gene

Objective To construct a tyrosine kinase B(TrkB) targeted RNA interference (RNAi) lentiviral vector.Methods Four oligonucleotides targeting rat TrkB gene were synthesized and cloned into lentiviral vector pXZRNAi 1.0 to construct recombinant lentiviral vectors pXZRNAi-shTrkB-1,2,3,4.Neural stem cells prepared from rat hippocampus were infected with these high-titer viruses.Real-time PCR was employed to detect the TrkB mRNA expression and western blot was used to assess the gene silencing efficacy of these recombinants.Results Enzyme digestion and DNA sequencing results demonstrated that these shRNAs were correctly inserted into lentiviral vectors and the four recombinants were constructed successfully with the titer of 8.6 × 105cfu/ml.The infection efficiency of the letivirus on neural stem cells reached 80%.Compared with the uninfection group,the expression levels of TrkB mRNA in neural stem cells decreased significantly after transfected with pXZRNAi-shTrkB-3 and 4((66.7 ± 5.5) % and(76.8 ± 4.9) % respectively,P < 0.05) ; and the protein expression levels were also significantly decreased ((68.5 ± 4.3)% and (78.2 ± 5.1)% respectively,P < 0.05).Conclusion The lentiviral vectors for TrkB have been successfully constructed with high yield of lentivirus,which provides versatile method for assessing gene function in neural stem cells.
7.Indinavir Resistance Evolution in One Human Immunodeficiency Virus Type 1 Infected Patient Revealed by Single-Genome Amplification

Human Immunodeficiency Virus Type 1 exists in vivo as quasispecies, and one of the genome's characteristics is its diversity. During the antiretroviral therapy, drug resistance is the main obstacle to effective viral prevention. Understanding the molecular evolution process is fundamental to analyze the mechanism of drug resistance and develop a strategy to minimize resistance. Objective: The molecular evolution of drug resistance of one patient who had received reverse transcriptase inhibitors for a long time and had treatment which replaced Nevirapine with Indinavir was analyzed, with the aim of observing the drug resistance evolution pathway. Methods: The patient, XLF, was followed-up for six successive times. The viral populations were amplified and sequenced by single-genome amplification. All the sequences were submitted to the Stanford HIV Drug Resistance Database for the analysis of genotypic drug resistance. Results: 149 entire protease and 171 entire reverse transcriptase sequences were obtained from these samples, and all sequences were identified as subtype B. Before the patient received Indinavir, the viral population only had some polymorphisms in the protease sequences. After the patient began Indinavir treatment, the variants carrying polymorphisms declined while variants carrying the secondary mutation G73S gained the advantage. As therapy was prolonged, G73S was combined with M46I/L90M to form a resistance pattern M46I/G73S/L90M, which then became the dominant population. 97.9% of variants had the M46I/G73S/L90M pattern at XLF6. During the emergence of protease inhibitors resistance, reverse transcriptase inhibitors resistance maintained high levels. Conclusion: Indinavir- resistance evolution was observed by single-genome amplification. During the course of changing the regimen to incorporate Indinavir, the G73S mutation occurred and was combined with M46I/L90M.

Objective To evaluate the toe flap repair of finger pulp defect of the clinical effects.Methods Of finger pulp defect using the first or second toe in 25 cases of free flaps, the flaps were cut size of 2.0 cm x 3.0 cm-3.5 cm x 4.5 cm, to toe at the end of artery-means the artery, subcutaneous veins-ve-nous anastomosis reconstruction of dorsal skin flap blood circulation, plantar digital nerve-refers to the inherent sensory nerve reconstruction.Results Twenty-five patients with flaps all survived, after vascular crisis occurred in 2 cases,surgical exploration and re-anastomosis of vascular survival, 3 months after flap reconstructive surgery in 12 cases.All patients were followed up for 2 months to 2 years, an average of 10 months,the fingers were satisfied with function and appearance, pulp full, two- point discrimination was 4-6 mm.Conclusion The toe plantar free flap repair of finger pulp defect may be a better clinical effects.
9.The value of combined detection of HbA2 and HbF for the screening of thalassemia among individuals of childbearing ages.

OBJECTIVE:
To assess the application value of combined detection of HbA2 and HbF for the screening of thalassemia among a population of childbearing age in Quanzhou, Fujian, and determine the optimal cut-off values for the region.
METHODS:
Capillary hemoglobin electrophoresis and genetic testing for α and β globin gene mutations were simultaneously carried out on 11 428 patients with suspected thalassemia. Statistical methods were used to analyze the distribution of various types of thalassemia and compare the performance of HbA2 and HbF measurement for the screening of various types of thalassemia. The optimal cut-off values for HbA2 and HbF were determined with the ROC curves.
RESULTS:
4591 patients with α, β, and αβ compound thalassemia were identified by genetic testing. The most common genotypes for α and β thalassemia included --SEA/αα and β654/βN, β41-42/βN, and β17/βN. The ROC curves were drawn to compare the performance of HbA2 screening for α-, β-, αβ-compound, static α-, mild α-, and intermediate α-thalassemia, and the maximum area under the curves was 0.674, 0.984, 0.936, 0.499, 0.731, 0.956, and the optimal cut-off values for HbA2 were 2.45%, 3.25%, 3.65%, 2.95%, 2.55%, 1.75%, respectively.
CONCLUSION
HbA2 is an efficient indicator for identifying intermediate types of α-, β-, and αβ compound thalassemia. The combination of HbA2 and HbF measurement can effectively detect carriers for β-thalassemia mutations.

10.Evaluation of an in-house method for HIV-1 drug resistance genotyping test

ObjectiveTo evaluate the sensitivity and accuracy of an in-house detecting method of HIV-1 genotypic drug resistance system. MethodsTotally 130 serum specimens from Henan and Guangxi province were collected from April 2004 to October 2008 and tested in the Military HIV Testing Center of China. ViroSeqTM v2.0 (Abbott, Switzerland), a US FDA approved HIV genotypic drug resistance detecting system was utilized as the reference method. All the specimens were detected by the novel in-house method and the reference method to validate the difference in amplifying efficiency, drug resistance mutation detection and drug resistance report. ResultsConcerning the 14 850 known drug resistance mutation sites,14 752 (99. 3% ) mutations can be detected by both of the two methods. Rates of concordance of detection in the regions of protease inhibitors-, reverse transcriptase inhibitors- and both two classes inhibitors-resistance were99.7% ( Kappa =0. 909 9 , P <0. 01 ) , 99. 0% (Kappa=0.952 1, P<0. 01) and99.3% (Kappa=0. 948 8, P < 0. 01 ) respectively. Drug resistance reports from these two systems showed similar results (Kappa = 0. 637 4, P < 0. 01 ). The in-house detecting system identified 34 novel mutations besides the ViroSeqTM drug resistance mutation database ( ViroSeqTM software v2. 7). Two mutations, V179F and K238T,had significant effect on HIV drug resistance. ConclusionsThe in-house genotyping system is an accurate,cost-effective method and has a high concordance with commercial ViroSeqTM genotyping system. Database from the in-house assay was superior to this of the ViroSeqTM assay.
