Objective:To establish the quality standard for Qiyeling granules. Methods:Astragali Radix and naringin were identi-fied by TLC. The content of astragaloside A was determined by HPLC. Results:The qualitative identification was with high resolution and without interference from the negative substances. The linear range of astragaloside A was 0. 166-1. 104 μg(r=0. 999 1), and the average recovery was 97. 27%(RSD=1. 3%,n=6). Conclusion:The method can be used in the quality control of Qiyeling granules.
2.Peripheral neuropathy in multiple sclerosis:a clinical and electrophysiologic study

Objective To study the clinical and electrophysiological characteristics of peripheral neuropathy in patients with multiple sclerosis(MS).Methods As a total of 84 MS patients,the clinical and electrophysiological data were analyzed in 16 MS patients with the symptoms of peripheral neuropathy(PNMS group)and in 68 MS patients without peripheral neuropathy(NPNMS group).Results The incidence of peripheral neuropathy was 19.0%(16/84)in this group.The clinical symptoms included extremity numbness in 14 cases(87.5%),limb inertia in 12 cases(75.0%),and radicular pain in 4 cases(25.0%).Signs included decrease of tendon reflex in 15 cases(93.7%),periphery sensory disability in 13 cases(81.2%),and myatrophy in 5 cases(31.2%).The mean age in the PNMS group was(44.6?12.5)years vs.(32.2?11.5)years in the NPNMS group.The course of disease was(39.3?18.3)months vs.(31.6?17.2)months in the NPNMS group,there were significant differences in two groups(P
3.Detection and subtype analysis of VP1 gene of enterovirus 71 strains isolated from children with hand,foot and mouth disease in Shanghai area during the first half year of 2009

Objective To understand the characteristics of molecular epidemiology of enterovirus 71(EV71) in children with hand, foot and mouth disease (HFMD) in Shanghai area during the first half year of 2009. Methods Seventy-three throat swabs and 38 stool samples were collected from 95 hospitalized children with clinical diagnosis of HFMD in Children's Hospital of Fudan University during April to May 2009. TaqMan real-time reverse transcription-polymerase chain reaction (RT-PCR) and nest RT-PCR were used to detect EV71 VP1, followed by gene sequencing analysis. Results Six of the 73 throat swabs were EV71 positive with the detection rate of 8.2%. In the 38 stool samples, 24 were EV71 positive with the detection rate of 63.2%. Twenty-eight nested RT-PCR positive samples were sequenced and the genetic analysis showed that 27 were C4 subtype,which were absolute dominant strain and the other one was C2 subtype. The isolated strain from a fatal case was C4 subtype and there was no obvious mutation found in VP1 region. Conclusions EV71 is an important pathogen in HFMD children in Shanghai area during April to May 2009. C4 subtype strains are absolutely dominant, and accompanied by epidemic strains of subtype C2.

As a member of nuclear receptor superfamily, farnesoid X receptor (FXR) has been shown to regulate numerous metabolic pathways, which include playing an important role in bile acid metabolism, maintaining lipid and glucose homeostasis when FXR is activated. With the prevalence of the glucose and lipids disorder, FXR attracts increasing attention. It may be a potential target for the treatment of type 2 diabetes mellitus and lipid disorders.

6.Prevalence and risk factors of sexual dysfunction in patients with Parkinson's disease

Objective To investigate the prevalence and related factors of sexual dysfunction (SD) in patients with Parkinson's disease.Methods A total of 411 patients with Parkinson' disease in Nanjing Brain Hospital were analyzed retrospectively.The ICD-10 diagnostic criteria for SD was used to evaluate SD.The Unified Parkinson's Disease Rating Scale (UPDRS) part Ⅲ, Hoehn and Yahr (H&Y) staging, Non-motor Symptoms Questionnaire (NMS-Quest), Parkinson's Disease Sleep Scale (PDSS), Hamilton Anxiety Rating Scale (HAMA), Hamilton Rating Scale for Depression (HRSD) and Montreal Cognitive Assessment (MoCA) were used to evaluate patients.Results SD was found in 145 (35.3 %) patients and the prevalence was higher in male than in female.Patients with SD had older age, longer duration, severer motor symptoms, and higher scores of UPDRS-Ⅲ, HAMD, HAMA and NMS-Quest (all P<0.05).In male patients, SD was positively associated with age, duration of disease, UPDRS-Ⅲ , H-Y stage, HAMD, HAMA and NMS (r=0.127, 0.303, 0.240, 0.236, 0.181, 0.221 and 0.302, all P<0.05).In female patients, SD was positively associated with duration of disease, HAMD, HAMA and NMS (r=0.194, 0.163, 0.189 and 0.178, all P<0.05).The forward binary Logistic regression analysis indicated that female gender (OR=0.539, P<0.05), duration of disease (OR=1.133, P<0.05), NMS (OR=1.104, P<0.05) and history of exposure to heavy metals (OR=5.268, P<0.05) were associated with SD.Conclusions SD is a common complication in patients with Parkinson's disease, and a great attention should be paid to its screening and diagnosis.
7.The application of Omaha system to seniors with chronic disease in medical nursing home

Objective To investigate the effects of Omaha system which is applied to seniors with chronic disease in medical nursing home.Methods 48 seniors with chronic diseases in medical nursing home were chosen through convenience sampling methods,the Omaha system was applied to assess nursing problem,carry out nursing intervention,score outcome before and after the intervention,SPSS13.0 was used to enter data for statistical analysis.Results Seniors with chronic disease in medical nursing home had 29 nursing problems,accounting for 69.05%,4 intervention categories was used,69 of targets selected,accounting for 90.79%,after the intervention,KBS score was (3.85 ±0.89),(3.92±0.83),(4.05 ±0.77),higher than (3.07±0.83),(3.16±0.75),(3.61±0.77) before the intervention,and the difference was statistically significant.Conclusions The application of Omaha system can improve the level of cognition and behavior and state of seniors with chronic disease in medical nursing home.
8.Application of via-anal ileus tube in the therapy of left-sided acute malignant colonic obstruction

Objective To investigate the effect of via-anal ileus tube used as a drainage in left-sided acute malignant colonic obstruction.Methods Forty-seven cases of left-sided acute malignant colonic obstruction were divided into control group (25 cases) and tube group (22 cases).The patients in control group received intraoperative colonic lavage for one-stage surgery.The patients in tube group received via-anal ileus tube for decompression before the surgery.Compared the difference of bowel movement recovery time,exhaust time,fasting time,postoperative hospitalization time,hospitalization costs and complication rate between two groups.Results The technically successful rate in tube group was 90.9% (20/22),and one-stage surgery were performed.No anastomotic leakage or postoperative stenosis occunred after operation.There were 23 cases in control group who performed coloclysis in operation and one-stage surgery.The bowel movement recovery time,exhaust time,fasting time,postoperative hospitalization time and hospitalization costs in tube group were significantly lower than those in control group [(12.78 ± 2.07) h vs.(18.01 ±3.42) h,(78.76 ± 11.43) h vs.(96.38 ±13.09) h,(3.18 ±1.76) d vs.(5.51 ±2.95) d,(10.23 ± 2.33) d vs.(15.86 ± 6.74) d,(25 437.43 ± 2 343.67) Yuan vs.(31 051.32 ± 2 542.73) Yuan](P < 0.01 or < 0.05).After operation,there were 2 cases of stomas fistula and 2 cases of peritoneal cavity infection in control group,and none of them in tube group,the complication rate in control group was significantly higher than that in tube group [17.4% (4/23) vs.0,P < 0.05].Conclusions The via-anal ileus tube used as a drainage in left-sided acute malignant colonic obstruction is effective and safe,and can improve the rate of one-stage anastomoses,decrease the complication rate,promote the promote.It has important clinical value.
9.Detection of methicillin-resistant S . aureus from respiratory specimens of ICU patients by conventional sputum culture compared with rapid polymerase chain reaction assay

Objective To evaluate the performance of polymerase chain reaction (PCR)‐based rapid detection of methicillin‐resistant S .aureus (MRSA) from the respiratory specimens of ICU patients in comparison with conventional sputum culture . Methods We collected 3 respiratory specimens from each patient simultaneously for bacterial culture ,fungal culture and PCR‐based detection of MRSA .The performance of the established PCR method was compared with the results of sputum culture in terms of sensitivity ,specificity ,positive predictive value (PPV) and negative predictive value (NPV) .Results The sensitivity , specificity ,PPV and NPV of PCR assay were 88 .9% ,73 .6% ,36 .4% ,and 97 .5% ,respectively ,which were lower than the reported results of PCR assay with nasal swabs .Conclusions The utility of PCR assay for active screening of MRSA from respiratory specimens still needs further supportive data .However ,PCR assay is somewhat informative in excluding MRSA infection or colonization because of its high NPV .
10.Effect of Protein Kinase C Inhibitor Enzastaurin on Growth of Gefitinib-resistant Human Non-small Cell Lung Cancer Cell Lines

Objective: To observe the effect of a novel targeted agent enzastaurin alone or in combination with gefitinib on ge-fitinib-resistant human non-small cell lung cancer cells to explore the rational drug combination. Methods: CCK-8 assay was used to measure the cell proliferation. Combination index ( CI) was calculated by Chou-Talalay method to assess the efficacy of the combination therapy. The flow cytometry (FCM) was used to analyze the change in the cell cycle. Results:In 72h, the IC50 value of gefitinib and enzastaurin for the lung cancer NCI-H460 cells was 6. 99μmol·L-1 (95%CI:3. 55-13. 79μmol·L-1 ) and 7. 25μmol·L-1 (95%CI:4. 77-1. 02 μmol·L-1), respectively. The inhibition effect on the cell proliferation was stronger in the combination treatment than that in the monotherapy (P<0. 05), and the simultaneous treatment showed the most significant inhibition effect (P<0. 01). The IC50 value of gefitinib for H460 cells in the simultaneous administration group, the sequential administration group with gefitinib used first and the sequential administration group with enzastaurin used first was 0.006 μmol·L-1(95%CI:0.002-0.020μmol·L-1), 0.02μmol·L-1(95%CI:0.011-0.037 μmol·L-1) and 0.085 μmol·L-1(95% CI:0.042-0.170μmol·L-1, respectively. The CI of the simultaneous administration group was lower than one when the gefitinib concentration was above 0. 05μmol·L-1 . The cell cycle distribution result indicated that the simultaneous administration group had significantly increased G0/G1 proportion (P<0. 05) and induced cell cycle arrest at G1 phase. Conclusion:Protein kinase C inhibitor enzastaurin combined with EGFR inhibitor gefitinib shows a synergistic effect, suggesting that the combination treatment of the two drugs might be a new strategy for the follow-up therapy of gefitinib-resistant non-small cell lung cancer.
