1.Clinical study on endoscopic variceal ligation combined with endoscopic variceal sclerosis for treating patients with esophageal variceal bleeding

Objective To investigate the clinical efficacy and safety of endoscopic variceal ligation (EVL) combined with endoscopic variceal sclerosis (EVS) for treating patients with esophageal variceal bleeding (EVB).Methods Ninety patients with EVB were divided into observation group and control group by treatment methods with 45 patients each.The observation group was given EVL combined with EVS and the control group was given EVL.The elimination of varicosity after the first treatment,time and times required to eliminate,rehaemorrhagia,varicosity recurrence,complication and prognosis of the 2 groups were observed.Results The elimination rate of varicosity of observation group was 97.8%(44/45),the control group was 93.3%(42/45),there was no significant difference (P> 0.05).The elimination rote of varicosity after the first treatment of observation group was significantly higher than that of control group,the time and times required to eliminate were significantly lower than those of control group [71.1%(32/45) vs.48.9%(22/45),(17.24±5.65) d vs.(36.01 ± 11.81) d,(1.42 ±0.47) times vs.(1.87 ±0.61) times,P < 0.05 or <0.01].The rates of early-onset rehaemorrhagia,delayed rehaemorrhagia and varicosity recurrence of observation group were significantly lower than those of control group [4.4%(2/45) vs.17.8%(8/45),6.7%(3/45) vs.22.2% (10/45),2.2% (1/45) vs.22.2% (10/45),P <0.05].The rate of complication of observation group was significantly lower than that of control group [11.1% (5/45) vs.42.2% (19/45),P <0.01].Conclusion The EVL combined with EVS is an effective method for emergency hemostasis and preventing rehaemorrhagia in the patients with EVB.
2.Determination of Stilbene Glucosides in Yening Granules by RP-HPLC with Pre-column Derivation

Objective:To establish a RP-HPLC method with pre-column derivation for determining the content of stilbene glyco-sides in Yening granules. Methods:DNS-Cl was used as the derivation reagent, a Wondasil-C8 column (250 mm × 4. 6 mm, 5 μm) was used as the stationary phase and 30mM ammonium phosphatere gulation (pH 3. 5)-acetonitrile (78∶22) was applied as the mobile phase, a fluorescence detector (λex=370 nm,λem=560 nm) was used, the flow rate was 1. 0 ml·ml-1 , the column temperature was 30℃ and the injection volume was 20 μl. Results: There was no interference from the other substances in the preparation. The linear range of the two stilbene glycosides was 0.071-27.280 mg·ml-1(r=0.999 9). The average recovery was 99.04%(RSD=0. 1%, n= 9). Conclusion:The method is simple, quick, sensitive and accurate, which can be used for the quality control of Ye-ning granules.
3.Application of computer-assisted instruction for Case-based teaching in medical imaging

Objective A study was performed to better define the teaching outcome of computers in teaching medical imaging. Methods 150 4rd-year students were randomly assigned to four groups and experimental group exposed to 20 radiology cases as well as to a voluntary weekly radiology lecture. Group A used computer-based cases with interactive elements; group B used computer-based cases without interactive elements; group C used paper-based cases with interactive elements; and group D was not exposed to the cases and served as a control group. Results On a multiple-choice question test,groups A,B,and C showed significant improvemen(t+10.6%,+15.7%,+13.5%,respectively),whereas group D did not(+2.7%). 4.Effect of dexmedetomidine on patients′ inflammation during the CPB and the effect on patients′ renal and hepatic function

Objective To observe the effect of dexmedetomidine on patients′ inflammation during CPB and protective effect on kidney and liver. Methods 60 cases undergoing cardiac valve replacement under CPB were randomly divided into NS group and Dex group. Blood samples were taken before induction , before ascending aorta blocked, end of CPB, 24, 48 and 72 hours after operation. The serum level of HMGB-1, TNF-α, IL-6, BUN, Cr and ALT are tested. Blood WBC and N% are also counted. Results WBC, N% and HMGB-1, TNF-α, IL-6, BUN, Cr in Dex group significantly decreased at time point T2 ~ T6 (P < 0.05) compared with NS group. But ALT in Dex group only decreased at time point T 2 and T5 compared with NS group (P < 0.05). Conclusion Dexmedetomidine can significantly decrease inflammatory factor during CPB and improve renal function after surgery.
5.Establishment of an animal model of distraction osteogenesis in the irradiated dog mandible

Objective:To establish an animal model of bone distraction in the irradiated dog mandible.Methods:1 0 Chinese dogs were used.8 dogs received a unilateral irradiation of 60 Co(Group R)in the mandible with a total dose of 22.8 Gy in four 5.7 Gy frac-tions (biologically equivalent to 50 Gy/25 fractions).The other 2 dogs without irradiation served as the controls(Group C).Bilateral corticotomy was made 3 months after completion of irradiation.After a 1 -week latency period,bone distraction was activated at a rate of 0.5 mm twice daily for 1 0 days,followed by a consolidation phase of 8 weeks.New bone was evaluated by radiographic,histological and single photon emission computed tomography(SPECT)analysis.Results:One dog was excluded from the study due to anaesthetic death in group R.After 8 weeks of consolidation,no difference was found between the percent area of new bone of both groups.New bone was more mature and organized in Group C than that in Group R.SPECT analyses showed that there was active osteogenic activity in Group R.Conclusion:Distraction osteogenesis can be achieved in the irradiated dog mandible.
6.Adverse Drug Reactions of 6 Kinds of Quinolones

OBJECTIVE To analyze the adverse drug reactions of 6 kinds of quinolones in order to provide information for clinically rational use of drugs.METHODS The adverse drug reactions of quinolones such as norfloxacin,ofloxacin,levofloxacin,ciprofloxacin,fleroxacin and rufloxacin used in outpatients during Feb to May 2007 were collected and retrospectively analyzed.RESULTS The main adverse drug reactions were seen in gastrointestinal system,nervous system,immune system,respiratory system,urinary system etc.From them the symptoms of gastrointestinal system and nervous system were prominent.CONCLUSIONS In use of quinolones it should think highly to their adverse drug reactions.
9.Uses of half-dose gonadotropin-releasing hormone agonist with different treatment cycles after conservative laparoscopic surgery in the treatment of endometriosis

To explore the efficacy of using gonadotropin-releasing hormone agonist (GnRH-a) with different cycles after conservative laparoscopic surgery in the treatment of endometriosis.Research was conducted on 108 patients with endometriosis Ⅲ or Ⅳ diagnosed by conservative laparoscopy from January 2010 to June 2011.Among them,57 patients (group A) were managed with GnRH-a postoperatively for 3 months and the others (group B) 6 months.Clinical efficacy,relapse rate and pregnancy rate after withdrawal were compared between two groups.The clinical efficacy rate of groups A and B were 73.7% and 90.2% respectively.And there were significant statistical differences between them.However,the relapse rate of groups A and B were 14.0% and 5.9% and the pregnancy rate of two groups 45.0% and 4/12 respectively.And there were no statistical differences.A 6-month regimen of GnRH-a can reduce and delay the recurrence of endometriosis and improve the clinical efficacy.A short-term dosing of GnRH-a is indicated for those patients with fertility requirements.
10.Application of whole brain angiography via radial approach in young patients for the diagnosis of ischemic cerebrovascular disease

Objective To assess the feasibility and safety of transradial puncture route for digital subtraction angiography (DSA)in young patients with ischemic cerebrovascular disease. Methods One hundred young patients with cerebrovascular disease who underwent whole brain angiography at the Department of Neurology,the 81 st Hospital of PLA were enrolled. They were divided into either a radial group (n =50)or a femoral group (n = 50)using a computer random number method. The time of puncture,success rate of puncture,success rate of selective angiography,exposure time,and incidence of complications were compared and analyzed. Results (1)The puncture time of the radial group was significantly longer than that of the femoral group,they were 3. 00 ± 0. 50 min and 1. 50 ± 0. 25 min respectively. There was significant difference (t = 18. 97,P < 0. 05). There were no significant differences in the success rate of puncture (98% vs. 100%),success rate of selective angiography (100% vs. 100%),and exposure time (5. 3 ± 2. 2 vs. 4. 8 ± 1. 7 min)between the two groups (all P > 0. 05). (2)The incidence of complications of the radial group (4%,n = 2)was significantly 1ower than that of the femoral group (18%,n = 9 ). There was significant difference between the two groups (χ2 = 5. 01,P < 0. 05 ). Conclusion Transradial route puncture for whole brain DSA in young patients with cerebrovascular disease is safely and feasible.
