1.Clinical study on renal transplantation in polycystic kidney disease

ObjectiveTo study the outcome of renal transplantation in polycystic kidney disease (PKD) and its influencing factors.MethodsFrom 1978 to 2002, 46 PKD patients undergoing renal transplantation (PKD group) and 46 cases of other diseases (non-diabetic nephropathy;control group) were retrospectively analyzed. Patient/renal survival rate (1, 3 and 5 years after re nal transplantation) and postoperative complications were evaluated.ResultsA comparable overall patient (1, 3 and 5 years: 95.7?% , 91.3?% , 91.3 ?% PKD vs 97.8?% , 95.7?% , 93.5?% controls) and transplant sur vival rate (1, 3 and 5 years: 93.5?% , 89.1?% , 87.0?% PKD vs 95 .7?% , 89.1?% , 87.0?% controls) was found in both groups. Infectious complications with the exception of urinary tract infections (UTIs: PKD 43.4 ?% vs 10.9?% ) were diagnosed in similar frequency in the graft recipien ts. In patient with primary biliary cirrhosis,the metabolism of calcium and vitamin D could be affected and osteomalacia and secondary hyperparathyroidism might occurr.Besides,hypoalbuminemia may mask the real level of serum calcium and thus lead to misdiagnosis of coexisting parathyroid adenoma.Therefore,a rare case of parathyroid adenoma associated with primary biliary cirrhosis was herewith presented to call attention to the effect of hypoalbuminemia on serum calcium.

e: Objective:To establish a useable animal model for the purification and immunotherapy of HSP with the observation of the growth rule of S180 sarcome tumor tissue and the immunohistochemical expression of different HSP in mice. Methods:The tumor incidence?the role of growth?the survival time and immunohistochemical expression of different HSP were observed after the S180 sarcome cells were inoculated at Balb/C mice back subcutaneous in different dose. Results:Sarcoma were formed in 100%.There are significant difference between vary dose in survival time, The expression was positive in HSP60?70?90? and grp94, and HSP70 expression was enhanced whereas HSP90? expression was decreased after heat shock treated. Conclusion:The use of S180 sarcome in mice is a good model to observe the expression of different HSP and the tumor tissue is suitable to purify. The model can be also used in the library study of HSP tumor vaccine.
4.Comparative imaging study of mediastinal lymph node from pre-surgery dual energy CT versus post-surgeron verifications in non-small cell lung cancer patients.

OBJECTIVE:
To validate the value of dual energy CT (DECT) in the differentiation of mediastinal metastatic lymph nodes from non-metastatic lymph nodes in non-small cell lung cancer (NSCLC).
METHODS:
In the study, 57 surgically confirmed NSCLC patients who underwent enhanced DECT scan within 2 weeks before operation were enrolled. Two radiologists analyzed the CT images before operation. All mediastinal lymph nodes with short diameter≥5 mm on axial images were included in this study. The morphological parameters [long-axis diameter (L), short-axis diameter (S) and S/L of lymph nodes] and the DECT parameters [iodine concentration (IC), normalized iodine concentration (NIC), slope of spectral hounsfield unit curve (λHU) and effective atomic number (Zeff) in arterial and venous phase] were measured. The differences of morphological parameters and DECT parameters between metastatic and non-metastatic lymph nodes were compared. The parameters with significant difference were analyzed by the Logistic regression model, then a new predictive variable was established. Receiver operator characteristic (ROC) analyses were performed for S, NIC in venous phase and the new predictive variable.
RESULTS:
In 57 patients, 49 metastatic lymph nodes and 938 non-metastatic lymph nodes were confirmed by surgical pathology. A total of 163 mediastinal lymph nodes (49 metastatic, 114 non-metastatic) with S≥5 mm were detected on axial CT images. The S, L and S/L of metastatic lymph nodes were significantly higher than those of non-metastatic lymph nodes (P < 0.05). The DECT parameters of metastatic lymph nodes were significantly lower than those of non-metastatic lymph nodes (P < 0.05). The best single morphological parameter for differentiation between metastatic and nonmetastatic lymph nodes was S (AUC, 0.752; threshold, 8.5 mm; sensitivity, 67.4%; specificity, 73.7%; accuracy, 71.8%). The best single DECT parameter for differentiation between metastatic and nonmetastatic lymph nodes was NIC in venous phase (AUC, 0.861; threshold, 0.53; sensitivity, 95.9%; specificity, 70.2%; accuracy, 77.9%). Multivariate analysis showed that S and NIC were independent predictors of lymph node metastasis. The AUC of combined S and NIC in the venous phase was 0.895(sensitivity, 79.6%; specificity, 87.7%; accuracy, 85.3%), which were significantly higher than that of S (P < 0.001) and NIC (P=0.037).
CONCLUSIONS
The ability of quantitative DECT parameters to distinguish mediastinal lymph node metastasis in NSCLC patients is better than that of morphological parameters. Combined S and NIC in venous phase can be used to improve preoperative diagnostic accuracy of metastatic lymph nodes.

5.An analysis on 200 randomized clinical isolates of Mycobacterium in Sichuan

Objective To find out the resistant situation and drug of Mycobacteria patients in Sichuan and offer foundation for clinical.Methods Two hundred randomized clinical isolates of Mycobacterium were determined by Roche drug sensitivity and minimum inhibitory concentration (MIC) method.Results Of the 200 clinical isolates,192 stains were Mycobacterium tuberculosis(MTB) (96.0%),8 strains (4.0%) were non-tuberculosis mycobacterium(NTM).Of the 192 MTB strains,108( 57.3% ) sensitive strains and 84 (43.7%)stains were resistant to one or more than one drugs.Among these 84 resistant strains 23 were multi-drug resistant ( MDR,12.0% ),4 were extensively drug resistant( XDR,2.1% ).The anti-TB drug resistance rates were:SM(16.7%),INH(20.8%),RFP(17.2%),EMB(10.9%),PI(16.1%),LFX(8.8%),AMK ( 16.7% ),CPM ( 6.2% ),PTA ( 33.3% ),respectively.Conclusion The resistance rate of tuberculosis keeps at a high level in Sichuan,especially the resistance rate of multiple (≥4) drug,we should oar attention.
6.Study on EB virus infection, LMP1 and Bcl-2 expression in lung cancer patients.

OBJECTIVETo investigate the Epstein-Barr virus (EBV) infection, the expression of EBV latent membrane protein 1 ( LMPl) and oncogene bcl-2 in lung cancer patients.
METHODSEBERI in 108 cases of lung cancer were detected with in situ hybridization. EBV positive and negative lung cancer tissues were analysed for the expression of LMP1 and Bcl-2 by immnohistochemistry. The average area (AA) and integral optical density (IA) of each sample was measured with the digital medical image analyzing system.
RESULTSIn 108 cases of lung cancer, 36 cases were EBER1 positive and 7 cases were LMP1 positive. The expression of Bcl-2 was higher in EBV positive lung cancer tissues than that in EBV negative. The AA value was 58014.23 +/- 6918.45 and 38156.22 +/- 4096.79, while the IA value was 11.00 +/- 1.48 and 8.03 +/- 0.78 respectively. No statistic difference was fund in the expression of Bcl-2 betwen LMP1 positive and negative lung cancer tisssues.
CONCLUSIONEBV infection in lung cancer increased the expression of bcl-2, which may play a role in the occurrence or development of lung cancer. The increased expression of Bcl-2 may not be induced by LMP1. The exact mechanism need further study.

8.Diagnostic value of serum cysteine leukotriene levels for obstructive sleep apnea hypopnea syndrome in children

Objective To assess the clinical value of the measurement of cysteine leukotriene (CysLTs) concentration in serum as a diagnostic predictor to screen the obstructive sleep apnea hypopnea syndrome (OSAHS) in children.Methods Serum CysLTs concentration was measured with ELISA kit in 75 patients with snoring before per.forming polysomnography (PSG).Subsequently,all the subjects underwent PSG test.Forty-one subjects were diagnosed as OSAHS,and 34 subjects didn't have OSAHS.These 34 non-OSAHS subjects were served as control group.Among the OSAHS group,25 subjects were the Han nationality and 12 subjects were the Uyghur nationality.According to apnea hyponea index(AHI),the OSAHS group was divided into 3 groups,which were light,moderate and severe groups.And the serum CysLTs level of the OSAHS group was taken for correlation analysis with the sleep respiratory parameters.Compared with the PSG results,the clinical value of the measurement of CysLTs concentration in serum was taken as a diagnostic predictor to screen patients with OSAHS in children.Results 1.The serum CysLTs level of OSAHS group [(683.102 ±89.825) ng/L] was significantly higher than that in the control group [(461.985 ± 84.951) ng/L] (P < 0.05).And it was correlated positively with AHI,longest apnea time,sleep apnea low aeration time (r =0.417,0.422,0.208,all P < 0.05),but correlated negatively with the lowest oxygen saturation and the mean oxygen saturation (r =-0.192,-0.255,all P < 0.05).2.The serum CysLTs level [(773.118 ± 92.015) ng/L] in severe OSAHS group was significantly higher than that in the moderate OSAHS group [(712.144 ± 41.331) ng/L] (P < 0.05),and much more significantly higher than that in the light OSAHS group [(642.206 ± 77.250) ng/L] (P < 0.05).3.When the serum CysLTs level was 560.872 ng/L,there was the best critical point with the highest sensitivity rate (92.7%)and specificity rate (94.1%),and the lowest misdiagnosis rate and the false dismissal rate,as diagnose accordance rate was 93.3%.4.The serum CysLTs level of Han [(704.417 ± 77.149) ng/L] was higher than that of Uygur [(628.053 ± 105.443) ng/L],and there was significant difference between the 2 groups (P < 0.05).Conclusions Serum CysLTs level is closely related with the severity of OSAHS.There is difference in serum CysLTs level between different nationalities.The serum CysLTs level of the Han nationality is higher than the Uygnr nationality.The serum level of CysLTs may be used as a predictor in screening 0SAHS children and a biological marker of the severity of OSAHS children.
9.Association of polymorphism of β3-adrenergic receptor gene and the leptin gene with Kazak obese children in Xinjiang

Objective To investigate the association between C2549A polymorphism of leptin gene and the Trp64Arg polymorphism of the β3-adrenergic receptor gene(β3-AR gene) and obesity in Kazak school-age children.Methods Totally 92 obese children and 71 healthy controls were selected from 6 to 12 years old in Kazak school-age children from the area around Urumqi.Genotype of the C2549A polymorphism of leptin gene and the Trp64Arg polymor phism of β3-AR gene were determined by polymerase chain reaction and restriction fragment length polymorphism methods.Results 1.The difference in distribution of the β3-AR gene Trp64Arg genotype of obesity and healthy controls of Kazak children was statistically significant (x2 =10.472,P < 0.05),but the difference in distribution of the alleles was not statistically significant (x2 =3.541,P > 0.05).2.The differences in distribution of the leptin C2549A genotype and alleles of the 2 groups were all statistically significant,and the odds ratio of the alleles(0.608) and 95% CI 0.380-0.972 suggested that the mutation occurrence of obesity might have certain protective effect.3.The difference in distribution of the genotype of β3-AR gene Trp64Arg polymorphism and leptin gene promoter area C2549A polymorphism in the same individual joint action of 2 groups was statistically significant (x2 =15.978,P < 0.05),but the difference in distribution of the alleles in the same individual joint action of 2 groups was not statistically significant (x2 =6.362,P > 0.05).Conclusions 1.There were distributions of the Trp64Arg polymorphism of β3-AR gene and the C2549A polymorphism of leptin gene in Kazak school-aged children in Xinjiang.2.These results suggested that C2549A mutation of leptin gene might have certain protective effect in Kazak children obesity,and the Trp64Arg polymorphism of β3-AR gene might be associated with Kazak children obesity.3.These results suggested that the Trp64Arg polymorphism of β3-AR gene and the C2549A polymorphism of leptin gene in the same individual joint action might be associated with Kazak children obesity in Xinjiang.
10.Clinical study on adverse reactions of peginterferon alfa-2a in treatment of chronic hepatitis in adults and children.

OBJECTIVETo investigate the scope and degree of short-term adverse reactions of peginterferon alfa-2a in treatment of chronic hepatitis in adults and children to provide basis for anti-viral treatment in clinical practice.
METHODSA prospective study was conducted in adults and children with chronic hepatitis treated with peginterferon alfa-2a. Meanwhile, the reactions in the patients were recorded with a table designed by ourselves and statistically analyzed.
RESULTSThe short-term adverse reactions included increase in body temperature and aching pain in joints and muscles. The increase in body temperature was the major reaction and accounted for 54.11%. The increase in body temperature began to appear in 47.6% of the patients. The body temperature was 37.3 degrees C-38.9 degrees C in most of the patients and mediate and low increase was found in 85.4% of the patients, which was decreased to 70% in the 4th week. However, the percentage of patients with high temperature was increased from 14.5% in the 1st week to 30% in the 4th week. The increase of body temperature began to appear in 9-12 h and 3-5 h after injection of peginterferon alfa-2a in the 1st and later, respectively. The duration of fever was 3-4 h in most of the patients. It appeared once in 1 week after the rejection in most of the patients. For management of fever, cooling with medication was conducted in 45.5 % of the patients.
CONCLUSIONThe short-term adverse reactions in patients with chronic hepatitis treated with peginterferon alfa-2a include the increase in body temperature etc. The severity of the adverse reactions gradually reduces with continuation of the treatment. Of the adverse reactions, the increase in body temperature is the major (47.6%) and others only account for 1%-16.9%. The increase in body temperature is mainly transient and no management is needed in 50% of the patients. Since the "ladder-type" dose-adding method is used for administration of peginterferon alfa-2a in this group of patients, the adverse reactions are low in number and mild in degree.
