2.Application of Low Tidal Volume Ventilation in Children with Acute Respiratory Failure

Objective To study the therapeutic and protective effect of low tidal volume(LTV) ventilation in children with acute respiratory failure.Methods Forty-eight patients with respiratory failure were enrolled in our study.The patients were suffered from severe infection,trauma or after major operation.Besides general treatments,they were all given ventilation with LTV.Blood gas,ventilation parameters and complications related to ventilation were also measured.Results No significant difference was found between the patients with high tidal volume(HTV) and those with LTV in the results of improving respiratory acidosis.But the incidence of(ventilated-)induced lung injury(VILI) was lower in LTV patients.Conclusion LTV ventilation is preferable for children with acute respiratory failure and beneficial for alleviation of VILI.

Hair follicle stem cells located bulge region of hair follicle,had characteristics of all adult stem cells,including slow-cycling,undifferentiated.They also had the abilities of self-renew and proliferation in vitro.CD34,K15,K19 and Nestin might be the makers of the hair follicle stem cells.They could be induced to differentiate to neurons,glial cells,keratinocytes,smooth muscle cells,melanocytes In vitro and neurons,melanocytes in vivo.There were many signals in regulating the hair follicle stem cells involved Wnt signal,BM Psignal and NFATc1 etc.
5.Discussion on Protective Effects of Histone Modiifcations for Myocardial Ischemia by Acupuncture

Protective effects of acupuncture on myocardial ischemia in each stage have been confirmed increasingly, while there was no in-depth research in terms of histone modifications. Histone modification is an important part of epigenetics. It is also the focus of molecular biology and basic medical research. In this paper, the relationship of histone modifications and gene expression of heart was first explained. Then, the recent research progress of modified histones, which was the direct alteration of gene expression induced by histone modifications on the degree and prognosis of myocardial ischemic injury, was reviewed. This investigation introduced epigenetic regulation in the protection mechanism of myocardial ischemia by acupuncture. It provided new ideas and directions for the study of molecular mechanism of acupuncture efficacy.
6.Inflammatory reaction characteristics of perihematoma brain tissue in patients with acute hypertensive intracerebral hemorrhage

48 hours.The expressions of leukocyte common antigen(LCA) and intercellular adhesion molecule-1(ICAM-1) in the brain tissues collected during the surgery were detected by immunohistochemistry staining method.Results LCA and ICAM-1 immunopositive cells and microvessels were verified in all 29 cases of ICH groups,and the expressions were more than control group(all P48 hours group were higher than that in 6~24 h group(both P48 hours group.Conclusions Acute inflammatory reaction occurs in acute period of ICH.Expression of ICAM-1 reinforces gradually during early 48 hours since the onset of ICH.
7.Diagnosis and treatments of craniomaxillofacial deformities with OSDB.

Obstructive sleep disordered breathing (USDB) is a common diseases which caused by upper airway(UA) occlusion, muscle tone problems and collapse of upper airway etc. The article introduces how to select surgical treatment protocol. First, it is necessary of PSG and upper airway evaluation. Then, it is key to master surgical indications. For the children with hypertrophy of tonsillectomy or adenoidectomy, it may first step of tonsillectomy or adenoidectomy, they needed followed up by orthodontic doctor or orthognathic surgeon for tooth and maxillomandibular malformation. For the adult OSDB patients, there are two method, soft tissue reduction/mass excision around UA or maxillomandibular advancement(MMA), and MMA is recommended to the patients such as failure from UPPP and other surgical treatment, but for severe obesity patients, bariatric surgery is commended, it is very effective for soft tissue reduction around UA and weigh loss. For the OSDB patients with craniaomaxillomandibular deformities, such as craniosynostosis syndrome/micrognathia. Finally, it is important of the UA management for the surgical patients around perioperative period. All in all, it is necessary of the multidisciplinary cooperation for the OSDB patients, surgical treatment is useful method but it is not commend to all OSDB patients.

8.Immunohistochemical analysis of OTUB1 in 90 cases of ovarian mucinous tumors

Background and purpose:The ovarian mucinous tumor is one of the major subtypes of the ovari-an epithelial cancer. Ubiquitination is one of the main post-translational modifications, which has proven to be involved in tumorigenicity. Deubiquitinase is the protein enzyme that could reverse the process of ubiquitination to affect the initiation and progression of malignancies. This study aimed to analyze the expression and clinical application of deubiquitinase OTUB1 (OTU deubiquitinase, ubiquitin aldehyde binding 1) in ovarian primary mucinous tumors. Methods:This study collected 90 cases of ovarian primary mucinous tumors during 2010-2015 in Obstetrics and Gynecology Hospital of Fudan University, and then collected the clinicopathological information and performed the immunochemistry.Results:Fourteen out of 90 cases were ovarian primary mucinous cystadenoma, 17 were borderline mucinous tumor (intestinal type and intraepithelial carcinoma), and 59 were ovarian primary mucinous carcinoma. The expression rate and intensity of OTUB1 were much higher in malignant cases than those in benign ones (P<0.05). The expression rate and intensity of OTUB1 were much higher in cases with mucinous intraepithelial carcinoma than those in cases with intestinal type borderline mucinous tumor (P<0.05). The expression rate and intensity of OTUB1 increased with the advance of FIGO staging (P<0.05). The expression rate and intensity of OTUB1 were much higher in cases with involved fallopian tubes than those in cases without involved fallopian tubes (P<0.05). The expression rate and intensity of OTUB1were much higher in cases with involved uterus and omentum than those in cases with-out involvement (P<0.05). The expression rate and intensity of OTUB1were much higher in cases with lymph node metastasis than those in cases without involvement (P<0.05).Conclusion:There is significant difference in OTUB1 expression between ovarian primary mucinous carcinoma and benign mucinous cystadenoma. It is highly correlated to FIGO staging and invasion and metastasis of tumor. OTUB1 could be used in differential diagnosis and in monitoring the tumor initiation and progression in ovarian mucinous carcinoma.
9.Clinical study on renal retransplantation

Objective To summarize the experiences of kidney retransplantation from 1978 to Dec. 2002.Methods The clinical data of 86 cases subject to first renal transplantation and 86 cases to second renal transplantation were retrospectively analyzed. The main causes of failure of the first renal transplantation were CAN, while those of the second renal transplantation were HAR, and other complications such as AR, graft rupture and serious graft T.B. PRA or CDC in 31 cases was positive before retransplantation. The clinical data such as antibody-inducing therapy, the protocols of immunosuppression and the survival rates of patients and grafts in these patients were analyzed.Results Survival rates at 1, 3, 5 year of the patient/graft of retransplantation group were 84.8 %/61.6 %, 79.1 %/45.3 % and 58.1 %/41.9 % respectively, while those at 1, 3, 5 years of the patient/graft with the first renal transplantation were 89.5 %/79.1 %, 81.4 %/74.4 % and 67.4 %/58.1 % respectively. 10.Spongiform encephalopathies in the elderly: a case report of clinicopathological and literature review

Objective To explore the clinical features of spongiform encephalopathies occurred in the elderly and causes of misdiagnosis. Methods Clinicopathological diagnosis of a 79 year old man with acute spongiform encephalopathy was reported. Clinical, EEG and MRI characteristics of eight old patients with spongiform encephalopathy in China confirmed by neuropathology were reviewed. Results Elder patients with spongiform encephalopathies had an acute onset and a rapid developing course, and all patients presented with progressive dementia accompanied by various clinical manisfestations of extrapyramidal diseases. EEG recordings with typical periodic sharp wave complexes (PSP) was shown in six cases, and abnormal T2 signal in both basal ganglia by MRI in one case. Three cases were misdiagnosed as cerebral vascular disease and one cse as encephalitis herpes cimplex. Conclusions Spongiform encephalopathy in the elderly should be paid more attention to since their early clinical symptoms are similar to that of the acute cerebral vascular diseases. It is suggested that early diagnosis of spongiform encephalopathy could be made basing on the combination of the typical clinical characteristics and dynamic EEG findings, and brain biopsy if necessary, to avoid nosocomial spreading.
