3.Differences in clinical features of interstitial lung disease between polymyositis and dermatomyositis

Objective To assess if there are differences in clinical features and prognosis of intersti-tial lung disease(ILD) between polymyositis(PM) and dermatomyositis(DM). Methods Medical records of 114 patients with PM/DM(31 PM-ILD, 83 DM-ILD) were reviewed retrospectively to analyze the demograph-ics , clinical manifestations, laboratory findings, high-resolution computed tomography (HRCT), pulmonary function tests (PFTs), blood gas analysis, treatments and prognosis. The differences of measured data were detected by descriptive statisitical analysis, and rates were detected by four-fold table Chi-Square test and Fisher′s exact test. Results The incidence of PM/DM-ILD was 35.8%(114/318) in this study, and DM was more prone to have ILD (χ2=5.019, P=0.025). There were significant difference in sex ratio between PM-ILD and DM-ILD(χ2=4.929, P=0.026). Arthralgia/arthritis was more common in DM-ILD than PM-ILD(χ2=7.756, P=0.005). In PM-ILD, ILD was often present before the diagnosis of PM (χ2=15.555, P<0.01),while it was opposite in DM-ILD(χ2=7.002, P=0.008). The frequency of anti-Jo-1 antibody was higher in PM-ILD than in DM-ILD(χ2=11.395, P=0.001). On HRCT, ground-glass opacity(χ2=7.940, P=0.005) and pericardial effu-
sion (χ2=6.322, P=0.012) were more frequently observed in PM-ILD, while patchy shadows were more frequent in DM-ILD (χ2=5.105, P=0.024). There was no difference in PFTs and blood gas analysis between the two groups. With the similar therapeutic regimen, the prognosis of DM-ILD was significantly worse than in PM-ILD (χ2=7.595, P =0.006). Conclusion There are significant differences in sex ratio, clinical manifestations, HRCT imaging findings, and prognosis between PM-ILD and DM-ILD. We propose that the difference in the immunopathological processes of PM and DM leads to different clinical features of ILD between PM and DM.
4.Rehabilitation effect of lower limb rehabilitation training robot combined with task-oriented training on walking ability after stroke

Objective To investigate the effect of lower limb rehabilitation training robot combined with task-oriented training on walking ability after stroke. Methods From February 2014 to August 2015,74 consecutive patients with post-stroke who received rehabilitation therapy and met the inclusion criteria admitted to the Department of Rehabilitation Medicine,Xuanwu Hospital,Capital Medical University were collected prospectively. They were all the patients with the first-ever stroke for 1 to 12 months. They were divided into either an observation group (n = 39)or a control group (n = 35)according to whether they were treated with the lower-limb rehabilitation robot. The patients of both groups received task-oriented training,2 times a day,once for 20 min,5 days a week for 12 weeks. The observation group was also treated with the lower-limb rehabilitation training robot,1 time a day,once for 30 min,5 days a week. Berg balance scale,Fugl-Meyer assessment (FMA),timed up-and-go test (TUG)and knee flexion active range of motion (KFAROM)were used to assess the efficacy. Results (1)After treatment,the Berg scale and FMA scale scores were increased in the observation group and the control group compared with before treatment. There was significant difference (Berg scale:28 ±9 vs. 22 ±9,29 ±9 vs. 24 ±9;FMA scores:47 ± 8 vs. 36 ± 8,40 ± 6 vs. 36 ± 7;all P < 0. 01),however,there was no significant difference between the two groups (P <0. 05),and there was significant difference in FMA scores between the 2 groups (P < 0. 01 ). The differences of Berg scale scores in the observation group and the control group were 10. 75 + 0. 30 and 4. 71 + 0. 14 respectively before and after treatment. There was no significant difference between the 2 groups (t = 0. 95,P = 0. 345). The differences of FMA scores in the observation group and the control group were 5. 8 ±0. 6 and 4. 9 ±0. 8 before and after treatment (t =5. 16,P <0. 01). (2)After treatment,the tug test and KFAROM of the observation group and the control group were better than those before treatment. There were significant differences (TUG test:35 ± 13 s vs. 56 ± 18 s,53 ± 17 s vs. 58 ± 18 s;KFAROM:82 ± 24° vs. 60 ± 23°,63 ± 23° vs. 57 ± 26°;all P < 0. 01),and there were significant differences between the 2 groups (all P < 0. 01). The differences of the TUG test in the observation group and the control group before and after treatment were 21. 5 ± 5. 0 and 4. 6 ± 0. 6 s respectively. There was significant difference between the 2 groups (t = 9. 55,P < 0. 01);the differences of KFAROM in the observation group and control group before and after treatment were 5.8 ±0.6° vs. 4.9 ±0.8° respectively. There was significant differences between the 2groups (t =4.17,P <0. .01). Conclusion Lower limb rehabilitation training robot combined with task-oriented training may improve the lower extremity motor function,walking ability,knee flexion joint activity of the patients after stroke,but the improvement effect of the lower limb balance is not obvious.

6.Complications of radiofrequency ablation for liver cancer in high-risk locations and their prevention

Radiofrequency ablation (RFA) is one of the most important methods for the treatment of liver cancer and has the advantages of small trauma,simple operation,and repeatability.However,for tumors in high-risk locations within 5 mm of the first and second branches of the hepatic portal vein,near the hepatic vein,the inferior vena cava,or the gallbladder,within 5 mm of the intestinal tract,under the Glisson's capsule,and in the diaphragm,RFA has the issues of a low complete ablation rate,a high local recurrence rate,and serious complications.This article introduces the complications of RFA for liver cancer in high-risk locations and their prevention and points out that with the promotion of individualized and standardized RFA,liver cancer in these high-risk locations is no longer a contradiction for RFA.

8.Evaluation on radiation protection and performance test for 192Ir brachytherapy apparatus

Objective: To evaluate the radiation protection and performance test for the 192Ir brachytherapy apparatus. Methods: According to the national standard, radiation protection and its various properties of the 192Ir brachytherapy apparatus were tested. The radiation leakage dose of the 192Ir brachytherapy apparatus and the radiation scattered dose of the surrounding environment were tested with the ion chamber survey meter 451P. The activity of 192Ir radiation source was calibrated with the well chamber HDR1000 Plus. Results: The radiation leakage and scattered dose of the 192Ir source storage device and the surrounding environment are far below the national standard. The deviation of the activity calibration for 192Ir source is-2.07%, accordance with national standard±5%. Conclusion: Radiation protection and its various properties of the 192Ir brachytherapy apparatus should be regularly monitor for assuring the quality of the brachytherapy.
9.Hidden Horizontal Tears of the Posterior Horn of the Medial Meniscus:a Report of 14 Cases

Objective To investigate the clinical features of hidden horizontal tears of the posterior horn of the medial meniscus and clinical efficacy of arthroscopic partical meniscectomy. Methods A total of 14 cases of hidden horizontal tears of posterior horn of the medial meniscus from May 2011 to May 2013 were enrolled.The knee arthroscopy was carried out through anteromedial and anterolateral approaches.The lesion of tears was exposed after the inner edge of posterior horn of the meniscus was bitten away.The bottom of the posterior horn of meniscus was found instable during the operation, which was then removed to conduct a partial meniscectomy.Afterwards the arthroscopic meniscus plasty of the posterior horn was performed. Results Arthroscopic photographs showed normal appearance in 6 cases and tears underside meniscus in 8 cases, all of which were confirmed to be horizontal medial meniscus posterior horn tear during operation.The mean time of operation was 32 min (range, 26-40 min), and the mean hospitalization time was 6 days ( range, 3-8 d) .There was no complications, such as infections or stiffnesses.All the patients were followed up for 2 -3 years.Subjective symptoms improved significantly after arthroscopic partial meniscectomy.According to the Lysholm knee scoring scale, the scores were (71.1 ±6.6) points preoperatively and (92.0 ±3.4) points postoperatively, with significant difference (t=10.530, P=0.000). Conclusion Diagnosis of hidden horizontal tears of the medial meniscus posterior horn is often difficult, because most patients have osteoarthritic knees.Careful physical examination and MRI are critical for making a correct diagnosis.Arthroscopic partial meniscectomy can help patients obtain better results.
10.Construction of recombinant modified vaccinia virus Ankara with Ag85A and ESAT-6 gene and examination of their immunogenicity in mice

Objective To construct five types of recombinant modified vaccinia virus Ankara (MVA) carrying genes encoding antigen 85A (Ag85A), early secretory antigenic target (ESAT-6) or IL-2 and to investigate the immunogenicity of these recombinant MVA in mice. Methods The genes encoding Ag85A and ESAT-6 were amplified by PCR from Mycobacterium tuberculosis H37Rv genomic DNA. The am-plified DNA fragments were sub-cloned into vector p18. The recombinant plasmids were introduced respec-tively into MVA by homologous recombination. The five types of recombinant MVA clone were selected in the MPA selection medium and their expressions of TB antigens were confirmed by Western blot. The recombi-nant MVA were used to immunize BALB/c mice. Ag85A-ESAT-6 fusion protein induced proliferation of spleno-lymphecytes from immunized mice was detected by MTT test. The concentration of IFN-γ in superna-tant of spleno-lymphocytes cultures was also analyzed by ELISA. The delayed type hypersensitivity (DTH) response to tuberculosis antigens was measured after injection of tuberculin purified protein derivative (PPD) in hind footpads of immunized mice. Results The five types of recombinant MVA were successfully con-structed by confirming their expression of TB antigen with Western blot. After co-cultured with the fusion protein Ag85A-ESAT-6 in vitro, the spleno-lymphocytes from recombinant MVA immunized mice showed sig-nificant proliferation activity (P<0.01) and increased IFN-γ secretion (P<0.05) while the same cells showed no response when co-cultured with saline. The spleno-lymphocytes from recombinant MVA immu-nized mice also demenstrated significant proliferation activity (P<0.01) and increased IFN-γ secretion (P<0.01) under Ag85A-ESAT-6 inducement, compared with the spleno-lymphocytes from mice immunized with wild type MVA or saline. The significant DTH response to PPD was detected in recombinant MVA-immunized mice (P<0.05). Conclusion The five types of recombinant MVA carrying genes of TB anti-gens were constructed. The constructed recombinant MVA could induce specific cellular immunity against tu-berculosis antigen in mice.
