The incidence of gastric cancer has declined worldwide in the past decades. However, due to the large number of population in mainland of China, the absolute number of newly-diagnosed patients in this developing country is still very high. Most of the major clinical centers have established multi-disciplinary team, but the strategy in treating this life-threatening disease still needs to be optimized, including the precise preoperative staging by various techniques, perioperative chemotherapy for locally advanced diseases, role of minimally invasive surgical approaches in treating patients with different stages, translational research to fill the gap between clinical trials and basic research, as well as the individualized treatment for gastric cancer. We believe that the biological behaviors and molecular events of gastric cancer in eastern countries are different from that of Western countries, so the clinical evidences derived from Eastern countries need to be strengthened to enrich the unmet demands in the clinical practice in China.

Objective To assess the feasibility of evaluation of coronary flow reserve (CFR) for left anterior descending (LAD),left circumflex (LCx),and right coronary posterior descending artery(PDA) as well as diagnostic accuracy for patency of each branches by transthoracic Doppler echocardiography (TTDE). Methods CFR of main coronary artery branches at the distal sites were studied by TTDE with adenosine infusion (140 μg · kg-1 · min-1) in 439 consecutive patients with suspected coronary heart disease. The relations between CFR and stenosis rates of coronary bifurcations were analyzed. Results Twenty-eight of LAD,12 of LCx and 21 of PDA were occluded in 439 patients. The success rate of CFR assessment of LAD, LCx and PDA were 96.8% (395/411), 74.9% (320/427), 83.0% (347/418)respectively. Then CFR≤2.0 and CFR≤1.7 were chosen as the criteria of coronary stenosis (≥50 %) and severe stenosis(≥75%). The sensitivity and specificity of detection of LAD,LCx and PDA stenosis were 81% and 87% ,64% and 82%, 79% and 85%, respectively. The sensitivity and specificity of diagnosis of LAD, LCx and PDA severe stenosis were 86% and 81%, 70% and 85%, 81% and 82%, respectively.Conclusions CFR evaluation of main coronary artery branches has high feasibility and can provide valuable information for relevant vessel patency.

Objective To explore modification of islet cell function in type 2 diabetes by the treatment of insulin detemir and insulin aspart.Methods A total of 68 patients with diagnosed type 2 diabetes were randomized into two grouops,the control group (insulin glargine combined with insulin aspart)and observation group (insulin detemir combined with insulin aspart),each group had 34 cases.Before and after treatment,fasting insulin and 2 h postprandial insulin,glycated hemoglobin (HbA1c),C peptide and fructosamine (FMN)levels were compared in two groups,and the treatment safety was observed.Results After treatment,fasting insulin and C -peptide levels in the two groups were significantly increased than before treatment(P <0.05 ).In observation group,2 h postprandial insulin level (29.97 ±9.18)mIU /L was significantly higher than before treatment (20.79 ±7.49)mIU /L(t =4.52,P =0.00), which was better than the control group (24.67 ±7.69)mIU /L,the difference was significant (t =2.58,P =0.01). After treatment,postprandial 2h C peptide levels in the two groups had no significant differences compared with before treatment (P >0.05).In the observation group,except for 2h postprandial insulin levels,other indicators had no significant differences compared with the control group (P >0.05 ).After treatment,FMN and HbA1c in the two groups decreased significantly compared with before treatment(P <0.05 ),but there was no significant difference between the two groups (P >0.05).During treatment,both two groups observed hypoglycemia,but there were no severe adverse reactions.Conclusion The clinical application of insulin detemir combined with insulin aspart in the treatment of type 2 diabetes can significantly improve the insulin -producing cells and accelerate functional recovery, and the clinical effect is better than the united aspart insulin glargine,with no significant adverse reactions,it can be recommended as a clinical type 2 diabetes treatment drug of choice.

Objective To evaluate the importance of clinical screening and follow up by direct colonoscopy for colorectal cancer at an early and curable stage. Methods There were 2 196 elderly people aged between 60 to 89 years. The clinical screening by direct colonoscopy was performed according to the protocol. 1 740 of 2 196(79.2%) patients were followed up every year. Results Fifty two elderly persons were found to be colorectal cancer patients by colonoscopy, with the detectable rate being 2.4%. Nineteen were diagnosed early stage colorectal cancer, accounting for 36.5% of the detected colorectal cancer. Nine among the followed up cases were detected early colorectal cancer, accounting for 45 0% of the detected colorectal cancer. The resectable rate and the 5 year survival rate was 97 7% and 80 9% for colorectal cancer, respectively. 98 9% of the cecum intubation cases was successful. The incidence of complication for colonoscopy was 0 05%. Conclusions By clincal colonscopy screening and follow up study for colorectal cancer and precancerous changes in the elderly, the patients with adenomatoid polyps were early diagnosed and treated, so it raised the detectable rate of early colorectal cancer and the level of grade prevention of colorectal cancer.

Objective: To analyze the validity of the lie scale in University Personality Inventory (UPI). Methods: Relationship between choice of lie scale and UPI score , mental health was analyzed by UPI questionnaire of 2958 freshers.Results: Average every student chosed 2.37 lie items. Relationship between UPI score and numbers of the choosed lie items showed negative correlation. Numbers of lie items that was chosen by First class students was fewer than that was chosen by second class students, and numbers of lie items that was chosen by second class students was fewer than that was chosen by third class students.Conclusion:UPI lie scale had lost validity to evaluate attitude of respondent, so UPI lie scale should be revised.

OBJECTIVE:To analysis and identify the liposoluble constituents extracted with supercritical CO 2 fluid ex-traction(SFE)from atractylodes macrocephala by gas chromatography-mass spectrometry(GC/MS).METHODS:Volatile oil was extracted from atractylodes macrocephala,to analsis and identify the chemical components in liposoluble constituents with GC/MS,DB-5MS capillary column was the chromatographic column and helium was carrier gas,the flow rate was 2.0ml/min,and the column temperature was raised from 70℃ to 250℃ at an increasing rate of 10℃/min.EI ion sources were adopted in mass spectra.RESULTS:A total of 19 chromatographic peaks were separated and identified,most of which were unsatu-rated fatty acids and esters,accounting for 96.2% of the total peak area.CONCLUSION:Using GC/MS,the chemical com-position of the liposoluble constituents in atractylodes macrocephala can be analyzed,which provides references for the further exploitation and utilization of atractylodes macrocephala.

Objective To observe the protective effect of probiotics VSL # 3 on the rats of trinitrobenzene sulphonic acid (TNBS) induced acute colitis, and the effect on the expression of tight junction proteins claudins (claudinl, 2, and 3) in colon mucosa. Methods Rat of colitis model was induced by TNBS one time enema. Since the second day after the model established, 2ml 0.9％ sodium chloride was intragastric administrated into 8 rats each day (TNBS group), 8 rats with 100 mg VSL # 3 (VSL # 3 group ), another 8 rats with 5-aminosalicylic acid (5-ASA) intragastric administration (5-ASA group), and 0.9％ sodium chloride was intragastric administrated daily into 5 no modeling rats as control group. All were treated for 7 days and then sacrificed. Fecal samples were collected for bacterial culturing and detecting the changes of colon bacteria colonies. Disease activity index (DAI was calculated according to diarrhea, hemafecia and weight. The colon tissue was taken for HE staining and the pathological changes were observed under microscope.The changes of myeloperoxidase (MPO) level in colon tissue were examined by enzyme-linked immunosorbent assay (ELISA). The expression and distribution of claudins 1, 2, and 3 in colon tissue was detected by a novel quantum dots immunofluorescence labeling technique. Results Compared with TNBS group,both VSL # 3 and 5-ASA treatment could improve colitis, as well as decreas DAI and colonic MPO level, and lower the inflammatory score of colitis. Compared with control group, the bacteria counting in the colon of colitis rats changed, the expression of Claudins 1, 2, and 3 expression was significantly increased (average density: 66.200±5.737, 71.780±6.670 and 61.300±5. 199,t＝17.237, 27.909and 21.788, all P＜0.01). Compared with TNBS group or 5-ASA group, VSL# 3 could adjust the balance of intestinal bacteria and increase the expression of Claudin 1 and 3 (Claudin 1 : 75. 550 ±8.717, 66.200±5.737 and 67.080±5.401;t＝9.348, 8. 469; all P＜0.05; Claudin-3: 68. 820±7.443, 61.300±5. 199 and 59. 830±5. 930;t＝7. 519, 8. 988, all P＜0.05), however claudin 2 expression was down-regulated (58. 740±6. 457, 71. 780±6. 670 and 66. 870±5. 791; t＝ 13. 033,P＜0.01, t＝8. 123, P＜0. 05). Conclusion Probiotics VSL# 3 may enhance the intestinal mucosal barrier function through changing the claudins expression, and play a role in the relief of inflammatory reaction in acute colitis.
