2.Application of Low Tidal Volume Ventilation in Children with Acute Respiratory Failure

Objective To study the therapeutic and protective effect of low tidal volume(LTV) ventilation in children with acute respiratory failure.Methods Forty-eight patients with respiratory failure were enrolled in our study.The patients were suffered from severe infection,trauma or after major operation.Besides general treatments,they were all given ventilation with LTV.Blood gas,ventilation parameters and complications related to ventilation were also measured.Results No significant difference was found between the patients with high tidal volume(HTV) and those with LTV in the results of improving respiratory acidosis.But the incidence of(ventilated-)induced lung injury(VILI) was lower in LTV patients.Conclusion LTV ventilation is preferable for children with acute respiratory failure and beneficial for alleviation of VILI.

OBJECTIVE:To establish culture management concepts in hospital pharmacy professions and to standardize the behaviors among pharmacists,patients,colleagues and other health professionals.METHODS:Theories about the corpo?ration management development,definition of corporation culture,the3levels of corporation culture management and the operation of corporate culture construction were briefly introduced;the hospital pharmacy profession culture management principle was established with a reference of the corporate culture management concept at all times and in all over the world.RESULTS:Harmonious,friendly,kindly and placatory atmosphere can be established through the culture construction in material,system,behavior and spiritual levels;pharmacists should be urged to win the trust of patients so as to achieve the best therapeutic effects for the patients.CONCLUSION:The establishment of hospital pharmacy profession culture management principle can inspire pharmacists'passion and enthusiasm for work;thereby the development of pharmaceutical enterprises can be promoted.
5.Chemopreventive effects of celecoxib on rat colon aberrant crypt foci induced by dimethylhydrazine

Objective An investigation was conducted to assess the chemopreventive effects of celecoxib, a selective cyclooxygenase(COX)-2 inhibitor, on a rat colon carcinogenesis model induced by dimethylhydrazine (DMH). The results were compared with those of sulindac. Methods Thirty-two 8-week-old female Sprague-Dawley rats were randomly divided into four groups (n=8 each):Group A rats were treated with DMH(120 mg/kg wt, single subcutaneous injection) alone and group B rats were treated with saline alone. Group C rats were pre-treated with sulindac (320 mg/kg feedstuff) and group D rats pre-treated with celecoxib (1500 mg/kg feedstuff) for 7 days before DMH initiation. The animals were killed at the end of the experiment (week 5) and the aberrant crypt foci(ACF) and aberrant crypt(AC) of the colonic mucosa were assessed after being stained with methylene blue. Results In group A(DMH only), the average numbers of ACF and AC were 182.4?93.43 and 262.8?197.8 respectively. In group B (saline group) rats, no ACF was found. In group C (sulindac group) rats, the average numbers of ACF and AC were 91.25?48.98 and 139.60?68.52 respectively, both of them were decreased significantly as compared with those in Group A (P
6.Species Distribution and Drug-resistance of 137 Strains of Infecting Urinary System Epiphytes:An Analysis

OBJECTIVE To study and analyze the detected rate,distribution of fungal species and drug resistance of 137 strains of infecting urinary system epiphytes in local area over the last three years to provide foundation for the reasonable clinical use of anti-epiphyte medicines.METHODS The epiphytes isolated from epiphyte culture of midstream urine were tested with API 20C AUX and their drug sensitivity was tested with Rosco disk diffusion method.RESULTS Of 137 strains isolated in the experiment,57 were Candida albicans,accounted for 41.6%;48 were C.tropicalis,accounted for 35.0%;and 19 were C.glabrata,accounted for 13.9%.And their drug resistances to fluconazole were 5.2%,8.3% and 36.8%,respectively.All tested epiphytes were sensitive to amphotericin B.CONCLUSIONS The results indicated that the isolation rate of epiphytes in local area is high.The majority of tested epiphytes are C.albicans and C.tropicalis,which are comparatively sensitive to zole medicines.C.glabrata is comparatively resistant to zole medicines.Amphotericin B is the most effective medicine to cure epiphytic infections of urine system.
7.Optimization of expression conditions of an induction strategy for improving liver targeted interferon (IFN-CSP) production in E. coli.

Expression conditions of induction strategies for the cytoplasmic inclusion bodies (IBs) production of liver targeted interferon IFN-CSP by recombinant Escherichia coli (E. coli) BL21 (DE3) were optimized in shake-flask cultures in this study. The factors of the optimized protocol included in the present study were pH, inducer IPTG (isopropyl beta-D-thiogalactoside) concentration, culture growth temperature, incubation time and induction point. The effects of those factors were investigated by 'single variable at a time' method, aimed to analyze characterization of the recombinant strain. Orthogonal experimental design was further used to optimize the above critical factors for IFN-CSP production. According to the expression optimization result, it was confirmed that the main influence factors were cell density and induction temperature. The IFN-CSP gene expression optimized conditions were: pH value of the culture medium was 6.0, culture temperature 37 degrees C, adding IPTG to final concentration 0.4 mmol/L when the recombinant strain growth density OD600 achieved 0.8 and induction time 4 h. At this point, the IBs represented 74.3% of the total cellular protein. Compared with the non-optimized condition, IFN-CSP production obtained in optimized induction strategies were increased by approx. 1.2-fold. The optimized induction strategy yielded 688.8 mg/L of IFN-CSP, providing experimental data to study the biology activity and productive technology of IFN-CSP.

Hepatitis c virus (HCV) infection has become one of the global public health problem,while there is no vaccine to prevent HCV infection, the so-called "cocktail" therapy that use a combination of drugs targeting multiple steps in the HCV infection cycle could achieve better curative effect. the process of HCV entering into host cell is the important step of drug intervention, in which HCV envelope protein El and E2, Host cell factors including Heparan sulfate(HS), CD81, scavenger receptor class B type I (SR-BI), Occludin (OCLD), Claudin (CLDN), low densitity lipoprotein receptor (LDLR), dendritic cell-specific ICAM-3-grabbing nonintegrin (DC-SIGN), Liver/lymph node specific ICAM-3-grabbing integrin(L-SIGN), trans- ferrin receptor 1 (TfR1) and so on play a important role. The virus and the host factors can be used as targets of hcv entry inhibitors many studies have shown that as novel and promising compounds, HCV entry inhibitors combinating with other drugs can be more effective in the treatment of HCV, this paper have re- viewed targets and inhibitors of HCV enterring into host cell since 1990s.

9.Preliminary study of liver targeting interferon on the immune toxicity of mice

Objective To evaluate the immunotoxicity effect of Liver targeting interferon (IFN -CSP) on mice.Methods Mice were randomly divided into five groups:low, middle and high dose of IFN-CSP, solvent control group(saline) and Positive control group (cyclophosphamide).They were injected subcutaneously for 2 weeks.Delayed hypersensitivity test was used to determine the cell immunefunction and plaque forming cell assay was used to determine the humoral immune function.Results There was no significant difference of the the index of immune organ and the ear swelling degree between IFN-CSP groups and control group.There was also no significant difference on hemolytic plaque test between them.Conclusion IFN-CSP has no significant effect on both cellular immunity function and humoral immune function of mice, this results will provides the basis for further safety evaluation.
10.Invasive Fungal Infection in ICU:A Retrospective Analysis

OBJECTIVE To have an retrospective analysis of the prevalent fungi infected in patients in ICU,and their pathologic sites and drug resistance and to find out the hazard factors.METHODS The research used API 20C AUX and Rosco disk diffusion method to test the drug resistance of fungi isolated from the infected samples collected from ICU patients in our hospital between Jan 2006 and Mar 2007.RESULTS Totally 123 fungi strains were spotted,the majority of them being Candida albicans,accounting for 34.1%;Candida glabrata 26.8% and Candida tropicalis 18.7%.All the fungi were sensitive to amphotericin B and nystatin(Mycostatin) and partially resistant to fluconazole,itraconazole and ketoconazole.Patients with long term use of wide-spectrum antibiotics and catheters and elder patients would have higher fungi infection rate.CONCLUSIONS Candida are the major pathogens of fungal infections in ICU.The major infected site is lower respiratory tract.Amphotericin B and nystatin have good sensitivity but higher toxicity.Conazole medicines also have good sensitivity to C.albicans and C.tropicalis.But they are resisted largely by C.glabrata and C.krusei.Reasonable use of bacteriophage and reduction of unnecessary diagnosis and treatment procedures,and early discovery,diagnosis and treatment are keys to prevent and cure the invasive fungal infections.
