Objective:To evaluate the changes of maxillary sinus density after mid-facial fractures.Methods: A retrospective review was performed on patients with mid-facial fractures from 1995 to 2006.The changes of maxillary sinus density were evaluated by Water's view.Results: There were 219 cases,including 165 male and 54 female patients,who showed changes of maxillary sinus density on Water's view.Their ages ranged from 3 to 78 years.78 cases had follow-up plain radiograph.Among them,51 cases had normal maxillary sinus density,the density of sinuses of 19 cases became lower and the density of sinuses of 8 cases did not change or became higher.Conclusion: The maxillary sinusitis is a late complication and develops in some maxillary sinus after treatment of mid-facial fractures.Attention to the maxillary sinus density should be paied when following-up.

Objective There are many methods to treat adult humeral shaft fracture. Plates fixation and intramedullary locking nail are two important methods. Though plate fixation are "gold criterion", there were many defects limited its development. With humeral intramedullary nail widely used, this study is to analysis and investigates the complications of locked nailing in humeral shaft fractures. Methods Between January 2000 and January 2004, 90 humeral shaft fractures (74 acute fractures and 16 delayed unions or nonunions) in 89 patients were treated with humeral intramedullary locking nails and followed up for an average of 25.4 months(12-34 months). There were 56 males and 33 females, with an average age of 39.3 years. Acute fractures included 62 closed, 7 Gustilo type Ⅰ, 3 type Ⅱ and 2 type Ⅲa open fractures. 12 nonunions had previous operations. 8 patients had radial nerve palsy. In general, acute fractures were treated with closed nailing and nonunions were treated with open nailing with bone grafting. 53 proximal humeral fracture were treated by antegrade nail, the others with distal humeral fracture were treated by retrograde nail. Results In total, 17 patients had 18 significant complications. 5 of them were persistent nonunions. There was no statistics difference between antegrade nail and retrograde nail. The others were protruded screws, fracture gap, shoulder impairment, elbow impairment, angular malunion, deep infection and postnailing radial nerve palsy. Conclusion The risk of operative comminution was significantly higher in retrograde nailing, and operative comminution resulted in a significantly higher risk of nonunion. Many complications of humeral locked nailing can be prevented by improving the implant design or surgical techniques.

Objective To evaluate the applicability of Mini-Nutritional Assessment(MNA),Short-Form Mini-Nutritional Assessment(MNA-SF)in the malnutrition screening of patients with Alzheimer's disease(AD).Methods MNA and MNA-SF were performed in 267 AD patients.The correlation between the screening results were compared.Results According to MNA,the rates of malnutrition,potential malnutrition,and normal nutritional status were 59.7%,20.9%,and 19.4%,respectively.The sensitivity was 73.4%and the specificity was 60.1%.According to MNA-SF,the rates of malnutrition and normal nutritional status were 70.4%and 29.6%.The sensitivity was 50.6%and the specificity was 92.4%.The screening results of these two tools were significantly correlated(r=0.924,P<0.01).Conclusions MNA and MNA-SF are reliable tools for evaluating the nutritional status of AD patients.While MNA-SF is more convenient and feasible,the combination of these two tools may be helpful.

OBJECTIVE To investigate the clinical distribution and drug resistance of the nonfermenters between 2000 and 2004 in our hospital. METHODS The bacteria were identified by ATB,drug resistance test was then performed by K-B method. RESULTS A total of 875 strains of nonfermentative Gram-negative bacteria were isolated from various clinical specimens during Jan 2000-Dec 2004.The dominant strains were Pseudomonas aeruginosa((313 strains)),Acinetobacter baumannii(287 strains) and Stenotrophomonas maltophilia(180 strains).They happened most commonly in the respiratory system.The positive rate from sputum and pharyngeal probe was 73.7% and 6.2%.The resistance rate of P.aeruginosa and A.baumannii to imipenem was the lowest,accounted for 17.6% and 7.7%.These nonfermenters were highly resistant to cefalotin,ceftriaxone and sulfamethoxazole/trimethoprim.The resistance rate of S.maltophilia to sulfamethoxazole/trimethoprim was lower than the others,the rate being 7.8%. CONCLUSIONS The drug resistance of nonfermenters to frequently-used antibacterials is higher.Due to the high resistant rates,the drugs should be chosen according to the result of the drug susceptibility test.

Objective To analyse the alterative characteristics of electrophysiology by detecting somatosensory evoked potential(SEP) during cerebral remodeling after transfer of healthy C7 nerve root both in patients and normal individuals. Methods From 2002 to 2003, 12 cases of the left total brachial plexus nerve root avulsion were surgically treated by transferring healthy C7 nerve root to impaired median nerve through bridged ulnar nerve. The waveform of SEP of bilateral cortical expressions were recorded after stimulating median nerve of impaired upper extremity at the wrist and elbow. Meanwhile, latent period and amplitude of wave of SEP, and nerve conduction velocity from the level of wrist to elbow were exhibited and compared. Results There were differences of the latency and amplitude of SEP in normal individuals between two sides of cerebrum to some extent. In the healthy sides (first group), difference of latency of SEP on two sides of cerebral cortex had no statistical significance but in the reverse for the amplitude of SEP. For the diseased sides(second group), difference of amplitude of SEP for both sides of cerebral cortex between the patients and the nonmal individuals were of statistical significance, but on the contrary for the change of latency of SEP. Conclusion Cerebral cortex dominating affected extremity is able to bring up functional remodeling electrophysiologically after nerve transfer of healthy C7 nerve root. Shortened latency and augmented amplitude discrepancy of SEP recorded on both sides of cerebral cortex were preliminary electrophysiological criterions. After contralateral C7 nerve root transfer, the examination of SEP can offer data of corresponding cortex "reinnervation", which provides objective evidence for further treatment to promote functional recovery and cortical plasticity.

Objective To explore effect of immune function of children with EB virus infectious mononu-cleosis (IM). Methods From Sep. 2011 to Jun. 2013, a total of 62 cases of children with IM were analyzed retrospectively. The proportion of T-lymphocyte subsets ,B-lymphocytesand and NK cells in EBV-DNA group with positive and negative were compared. Results The numbers of IM EBV-DNA-negative cases were 18 (29.03%), the numbers of IM EBV-DNA-positive cases were 44(70.97%), the averages of EBV-DNA in positive group is 6.1 × 103/mL. CD3+,CD8+cell ratios in IM children are significantly higher than control group (P<0.01), CD4+, CD19+cell ratios are significantly lower than control group (P<0.01), NK cells are not significantly different (P>0.05). In the IM children, CD4+, CD19+cells in DNA-positive group are significantly less than DNA-negative group (P<0.01), CD8+cells are higher than DNA-negative group (P<0.05), NK cells are not significantly different (P>0.05). Conclusions Positive blood EBV-DNA is closely related to changes in T-lymphocyte and B-lymphocyte, but isn′t obviously related in NK cells. So effect of EB virus in IM child have certain clinical values to judge the severity and prognosis.
