Objective To evaluate the clinical value of ulinastatin on the treatment of systemic inflammatory response syndrome in severe acute panereatitis. Method Eighty-four patients with severe a-cute pancreatitis were randomly divided into two groups. In the treatment group (42 cases),on the base of routine treatment, ulinastatin was administered intravenously for seven days after hospitalization, while in the control group only routine treatment was given (42 cases) to. Inflammatory factors in serum, the change of liver function and renal function were measured in two groups before and after the treatment, and the clinical efficacy were observed. Results There was significant difference, in the serum level of tumor necrosis factor-α, interleukin-1, interleukin-6, alanine aminotransferase, aspartate aminotransferase, blood urea nitrogen and creatinine on the 7th day between two groups (P < 0.05 or < 0.01 ) ,there were significant differences in the incidence of complications, hospitalization time, incidence of multi-organ failure between two groups [14.3%(6/42) vs 38.1%(16/42), (29.4 ± 1.5)d vs (34.4 ± 1.8)d, 28.6%(12/42) vs 47.6%(20/42), P<0.01 or <0.05 ]. Conclusions Ulinastatin can suppress the secretion and release of some inflammatory factors, protect the important viscera functions and reduce the incidence of complications, and it has favorable clinical efficacy.

Journal of Interventional Radiology 2001;0(06):-
Objective To evaluate imaging diagnosis, differential diagnosis, surgery and interventional therapy of hepatic angiomyolipoma associated with tuberous sclerosis. Methods Clinical features, imaging appearances and interventional therapy for 2 cases of HAML in TS were retrospectrively analyzed. Results The features of HAML in TS are as following:(1) Two cases were female with no history of hepatitis and hepatocirrhosis, liver function test and alpha feto protein were normal. (2) the tumor was shown as a multiple hyperechoic mass with sharp margin on ultrasonography; well defined, fat density and intensity mass on computerized tomography (CT); hypervascular mass with early drainage of hepatic vein and tumor staining on angiography. (3) One case underwent interventional therapy. After 42 month follow up, the tumor was found to be diminished in size. Another case was under monitored with only the enucleating of left renal mass. The hepatic tumor showed no change after 10 months. Conclusions The imaging appearances of HAML in TS are relatively characteristic. Superselective arterial embolization is an effective treatment for HAML in TS.

Chinese Journal of Trauma 2003;0(11):-
Objective To explore the cytological mechanism of bi ological reaction of prosthetic loosening mediated by wear particles from joint prosthesis. Methods Three special particles of UHMWPE, Ti- 6Al-4V and Co-Cr-Mo with the same average size of 2.5 ?m were injected respe ctively into rabbit knee joint to induce synovitis, in which the changes of syno vial cells and its cytoplasm as well as ultrastructure were observed by transmis sion electron microscopy. Results Three kinds of particles mainly induced type A synovial cell reaction with proliferation of subsynovial h istocytes. Wear particles and lysosomes existed in the cytoplasms of the synovia l cells and histocytes. Co-Cr-Mo particles could also accelerate infiltrating reaction of regional cytoplasms. Conclusions The biological effect of wear particles inducing prosthetic loosening is mainly non-immunolog ic inflammatory reaction. Certain immunologic factors may involve in biological reaction induced by Co-Cr-Mo wear particles.

Objective To appraise the clinical therapeutic effects of lamivudine in patients with chronic hepatitis B, and the recurrence of the disease after withdrawal of the drug. Method 122 patients with chronic hepatitis B treated with lamivudine were followed-up for over half a year after withdrawal. Result It was shown that, among the 9 cases in immunological tolerance stage and received lamivudine treatment, 4 showed effective result and ineffective in 5. All the 4 patients with effective result recurred within 6 months after lamivudine withdrawal. Among the 113 patients in non-immunological tolerance, 13 showed notable effective result (11.50%), 90 effective (79.65%) and 10 showed ineffective result (8.85%), respectively. The disease in 11 patients showing effective result recurred during the treatment. The rate of recurrence was 53.85% and 87.34% respectively in patients with notable effective and effective results. The level of ALT before lamivudine treatment showed no significant difference between the patients with abrupt and gradual withdrawal of lamivudine treatment, but it was significantly different after lamivudine withdrawal. The rate of recurrence showed no significant difference between patients with abrupt and gradual withdrawal of the drug. Conclusion The therapeutic effect of lamivudine is poor in immunological tolerant patients. Patients with chronic hepatitis B are likely to recur after lamivudine withdrawal. Lamivudine should be given for a long-term to inhibit persistent HBVDNA replication.
