1.The clinical significance of changes of platelet count in children with bronchial pneumonia

Objective To explore the changes of platelet count in children with bronchial pneumonia and its clinical significance in the disease.Methods 320 cases of bronchial pneumonia were retrospectively analyzed after hospitalization.According to the number of platelet count,the cases were divided into increased platelet count group (>400 ×109 /L)and normal platelet group(100 -400 ×109 /L),and the clinical data of two groups were statistically analyzed,such as age,gender,dyspnea,treatment,white blood cell count,hemoglobin,C -reactive protein.Results In 320 cases,62 cases'platelet count increased,258 cases were normal.Compared with the normal platelet group,the cases in increased platelet count group had the following characteristics:younger(<1 year old mainly,accounted for 46.77%;the former was between 1 and 3 years old mainly,accounted for 40.70%;Z =2.08),treatment time pro-longed (7.37 ±1.90)d,the former was (6.79 ±1.51)d,white blood cell count increased [(10.71 ±3.91)×109 /L,the former was (9.37 ±3.56)×109 /L,t =2.61,P <0.05];the incidence rate of dyspnea was higher (74.19%, the former was 52.33%,χ2 =9.73),hemoglobin decreased [(104 ±14)g/L,the former was (111 ±13)g/L,t =3.34],C -reactive protein increased [(24.42 ±12.73)mg/L,the former was (18.31 ±8.86)mg/L,t =3.58,P <0.01].There was no significant difference in gender between the two groups.Conclusion Children with bronchial pneumonia who platelet count increased are younger and their diseases are more serious.Observation the platelet count change can help to rightly judge and effective treat.
2.Drug Analysis and Literature Review of One Patient with Heart Failure and Gouty Attack

Objective: To investigate the principles of rational drug use for the patients suffering from heart failure complicated with diuretic-induced gouty arthritis.Methods: The clinical pharmacist participated in the diagnosis, treatment and drug adjustment of one patient suffering from heart failure complicated with furosemide-induced gouty arthritis.Taking the disease symptoms and potential gouty-inducing drugs into full consideration, clinical pharmacist suggested the withdrawal of related-drugs and the adjustment of treatment plan.Results: Considering the characteristics of the patient, clinical pharmacist adjusted the rational drug treatment.After the adoption of the suggestions, the gouty symptoms were relieved, heart failure was controlled and the condition was improved.Conclusion: Loop diuretics can easily trigger an acute attack of gout, especially for the patients with heart failure and gout history.Clinical pharmacist can make full use of professional advantages to optimize the treatment plan in order to reduce adverse reactions and promote the rational drug use by participating in clinical practice.
3.Clinical observation of treatment of chronic hepatitis B patients with lamivudine

Objective To appraise the clinical therapeutic effects of lamivudine in patients with chronic hepatitis B, and the recurrence of the disease after withdrawal of the drug. Method 122 patients with chronic hepatitis B treated with lamivudine were followed-up for over half a year after withdrawal. Result It was shown that, among the 9 cases in immunological tolerance stage and received lamivudine treatment, 4 showed effective result and ineffective in 5. All the 4 patients with effective result recurred within 6 months after lamivudine withdrawal. Among the 113 patients in non-immunological tolerance, 13 showed notable effective result (11.50%), 90 effective (79.65%) and 10 showed ineffective result (8.85%), respectively. The disease in 11 patients showing effective result recurred during the treatment. The rate of recurrence was 53.85% and 87.34% respectively in patients with notable effective and effective results. The level of ALT before lamivudine treatment showed no significant difference between the patients with abrupt and gradual withdrawal of lamivudine treatment, but it was significantly different after lamivudine withdrawal. The rate of recurrence showed no significant difference between patients with abrupt and gradual withdrawal of the drug. Conclusion The therapeutic effect of lamivudine is poor in immunological tolerant patients. Patients with chronic hepatitis B are likely to recur after lamivudine withdrawal. Lamivudine should be given for a long-term to inhibit persistent HBVDNA replication.
4.Therapeutic effect of glucocorticoid on nephrotic syndrome and its psychological intervention

Objective To investigate the glucocorticoid treatment of nephrotic syndrome curative effect and the effect of psychological intervention on the patients. Methods From June 2015 to May 2016 in our hospital treated 110 cases of patients with nephrotic syndrome, patients were randomly divided into two groups, control group patients with routine treatment and nursing measures, observation group of patients on the basis of conventional treatment nursing glucocorticoid treatment and nursing intervention measures for nursing, completion of treatment, compared two groups of patients with therapeutic effect and recurrence. Results In terms of treatment, observation group of patients treatment effectiveness 94.55%, control group patients treatment effective rate was 81.82%, the observation group of patients treatment effectiveness is significantly higher than patients with the control group (P<0.05); In terms of relapse, observation group after treatment in patients with recurrence rate was 3.64%, the control group after treatment in patients with recurrence rate 9.09%, observation group of patients with recurrence rate was significantly lower than patients with the control group (P<0.05). Conclusion By glucocorticoid treatment of nephrotic syndrome curative effect and psychological intervention on the patients, can significantly improve the effect of treatment of nephrotic syndrome patients, reduce the occurrence of patients relapse, help patients recover better, worthy of popularization and application in clinic.
5.Bibliometric Analysis of the Clinical Literature of Acupoint Thread-embedding for Simple Obesity

Objective To analyze the bibliometry of the literature of acupoint thread-embedding in treating simple obesity, and to provide references for clinic.Method By both computer and manual retrieval, the relevant literatures over the past 30 years were collected from the Chinese Journal Full-text Database (CJFD), Vip database, WanFang database. By using the SPSS 17.0 software for statistical analysis, the distribution (province or city), publication year, and journal of the literatures, supporting fund, sample size, research method, material of the thread, use frequency of the involved acupoints, and the meridians of the selected acupoints were summarized.Result The included eligible 175 articles were distributed over many provinces and cities; the publication amount had been increasing year by year; there was a variety of thread materials; the literature with supporting funds only occupied a small percentage, while the research with a small sample size occupied a comparatively large percentage, and the randomized controlled trials (RCTs) also had a small percentage.Conclusion As a common Chinese external therapy in treating simple obesity, acupoint thread-embedding has been paid more and more attention for its practical value and bright future. However, the relevant governmental department hasn’t paid enough attention to this therapy, and the low clinical research quality also makes it difficult to scientifically evaluate its clinical efficacy and safety. Therefore, more attention is required, RCTs of multicenter and large sample size are expected, and clinical research quality also needs improvement, to provide references for clinic.
6.Acupuncture and stress.

The relationship between acupuncture and stress is discussed from three aspects, including is it possible for acupuncture stimulation to be a stressor, whether acupuncture will start stress reaction, and whether acupuncture effects contain some stress factors. It is believed that correct acupuncture manipulation will not cause stress response, however, under some circumstances, such as inaccurate manipulation, improper treatment or patients who are very nervous but do not receive effective intervention, acupuncture is likely to cause stress response. Acupuncture-induced stress response is totally different from acupuncture anti-stress. The possible stress factors in acupuncture effect are explored, which can provide a new angle for the research on action mechanism of acupuncture. From the view of stress to review acupuncture treatment, there are three enlightenments: emphasizing on communication between doctors and patients, avoiding over-pursuit of deqi and focusing on analysis of the body constitution.

7.Effects of electroacupuncture at points selected by orthogonal experiment on the extracellular signal regulated kinase signal pathway in a rat model of cerebral ischenia-reperfusion injury

BACKGROUND:Acupuncture point matching is little reported in the electroacupuncture for cerebral ischemia-reperfusion injury.
OBJECTIVE:To explore the influence of electroacupuncture acupoints selected by orthogonal experiment on rats with cerebral ischemia reperfusion and extracel ular signal regulated kinase (ERK) signal pathway.
METHODS:150 Sprague-Dawley rats were randomly divided into five groups:sham operation group rats were exposed the middle cerebral artery only;models of cerebral ischemia reperfusion were established in the rats in model and orthogonal groups. Then rats in the orthogonal groups 1 (Baihui (DU 20), Quchi (LI 11), Zusanli (ST 36), and Chize (LU 5)), 2 (Baihui (DU 20), Zusanli (ST 36), Chize (LU 5), and Sanyinjiao (SP 6)) and 3 (Hegu (LI 4), Zusanli (ST 36), Chize (LU 5) and Sanyinjiao (SP 6)) were given electroacupuncture at acupoints selected by orthogonal experiment, respectively.
RESULTS AND CONCLUSION:Compared with the model group, in al orthogonal groups, the neurological function deficit scores were decreased, infarct volume was smal er, the number of microvessel density in the ischemia region and the p-ERK expression level in the cortex and hippocampal CA1 region of ischemic hemisphere were increased, and significant changes were found in the orthogonal group 3. These findings indicate that electroacupuncture at points of Hegu (LI 4), Zusanli (ST 36), Chize (LU 5) and Sanyinjiao (SP 6) selected by orthogonal experiment can significantly reduce the infarct volume of cerebral ischemia-reperfusion injury in rats possibly by regulating the ERK signaling pathway.
8.Application of via-anal ileus tube in the therapy of left-sided acute malignant colonic obstruction

Objective To investigate the effect of via-anal ileus tube used as a drainage in left-sided acute malignant colonic obstruction.Methods Forty-seven cases of left-sided acute malignant colonic obstruction were divided into control group (25 cases) and tube group (22 cases).The patients in control group received intraoperative colonic lavage for one-stage surgery.The patients in tube group received via-anal ileus tube for decompression before the surgery.Compared the difference of bowel movement recovery time,exhaust time,fasting time,postoperative hospitalization time,hospitalization costs and complication rate between two groups.Results The technically successful rate in tube group was 90.9% (20/22),and one-stage surgery were performed.No anastomotic leakage or postoperative stenosis occunred after operation.There were 23 cases in control group who performed coloclysis in operation and one-stage surgery.The bowel movement recovery time,exhaust time,fasting time,postoperative hospitalization time and hospitalization costs in tube group were significantly lower than those in control group [(12.78 ± 2.07) h vs.(18.01 ±3.42) h,(78.76 ± 11.43) h vs.(96.38 ±13.09) h,(3.18 ±1.76) d vs.(5.51 ±2.95) d,(10.23 ± 2.33) d vs.(15.86 ± 6.74) d,(25 437.43 ± 2 343.67) Yuan vs.(31 051.32 ± 2 542.73) Yuan](P < 0.01 or < 0.05).After operation,there were 2 cases of stomas fistula and 2 cases of peritoneal cavity infection in control group,and none of them in tube group,the complication rate in control group was significantly higher than that in tube group [17.4% (4/23) vs.0,P < 0.05].Conclusions The via-anal ileus tube used as a drainage in left-sided acute malignant colonic obstruction is effective and safe,and can improve the rate of one-stage anastomoses,decrease the complication rate,promote the promote.It has important clinical value.
9.Pharmaceutical Care on an H Hypertension Case with MTHFR Gene Mutation

Objective To explore the role of clinical pharmacist in individualized treatment of hypertension.Methods A patient withH hypertension receiving pharmaceutical care from clinical pharmacists was retrospectively analyzed.Results Patient's MTHFR (C677T) gene type was TT homozygous.Clinical pharmacist suggested doctor modify treatment,and then patient's plasma homocysteine dropped from 61.5 to 16.0 μmol·L-1,and blood pressure dropped from 173/ 111 mmHg(1 mmHg =0.133 kPa) to 130/80 mmHg.Conclusion Clinical pharmacist provides individualized treatment for patient with hypertension to ensure the safety and effectiveness of the drug by genotyping.
10.The study on pathological rhythm characteristics of chronic fatigue syndrome
