2.Anesthesia experience for the elderly patients with total hip replacement

Objective To discuss the anesthesia experience for the elderly patients undergoing total. Methods All 50 cases received the epidural anesthesia, during the operation, MAP,HR and SPO2 were monitored. The blood glucose was monitored for each 45 minutes for the diabetes patients. Results All patients satisfied with the effect of epidural anesthesia, blood pressure dropped to varying degrees, the appropriate colloidal solution was used to expand capacity, prepared baek-ephedrine and alropine. These measures help maintain a stable blood pressure, iinprove the blood indexes, save blood transfllsion, reduce the ineidenee of lung infection, also provide a good postoperative epidnral analgesia. reduce Ihe occurrence of postoperative complications. Conclusion Continuous epidural anesthesia in ehterly patients with total hip replacement might help reduce postoperative complications and improve the quality of rehabililtation.
3.Application of gene chip technology to medicinal plant researches

Recently,gene chip technology has become a rapidly developed biotechnology.It contains so many advantages including large-scale, high flux,and parallelism that it has been widely applied in many fields.In this paper,the updated advances on applications of gene chip technology to medicinal plant researches are discussed and the contents are ranged from isolation of differentially expressed genes,discovery of new genes,research on functional genomics,identification of Chinese materia medica,detection of genetically transformed medicinal plants,and the molecular mechanisms of medicinal plant pharmacology and their diseases as well.Some problems and prospects related to the technology are also briefly presented.
4.Preparation and process optimization of HCPT-loaded PLGA microvesicles by ultrasound

Objective To investigate the effects of different technical conditions on the microvesicle size,envelopment rate,morphology of surface and so on in order to select the best technical conditions to prepare HCPT(10-hydroxy-camptothecin)-loaded PLGA microvesicles.Methods Microvesicles were prepared by a water/oil/water emulsion and solvent evaporation method.Various factors related to the envelopment rate and micmparticle size were studied,such as the ratio of water phase and oil phase,ultrasound power,and time,stirring time and so on.Single factor experiments and orthogonal design testing was carried out to optimize the technology of microvesicles preparation.Results The best processing conditions for microvesicles preparation were as following: HCPT 25 mg,PLGA 1.875 g,the ratio of internal and external phases 1∶15,and the PVA concentration 3%.These preparative variables produced global,smooth and glossy microvesicles.Electric charges were probably between-40 to 0 mV,microvesicles sizes ranged from 500 to 1 000 nm,envelopment rate was 79.33%,drug loading to the microvesicles was 0.478 3%,and ultrasonic imaging was clear in vitro.Conclusion Our optimize technical conditions can prepare injectable microvesicles by ultrasound.
5.Clinical research of bilevel non-invasive mechanical ventilation in the treatment of chronic obstructive pulmonary disease with respiratory failure

Objective To explore the application of bilevel non-invasive mechanical ventilation in the treatment of chronic obstructive pulmonary disease(COPD) with respiratory failure.Methods 126 COPD patients with type Ⅱ respiratory failure were selected and divided into control group and research group,63 cases in each group.The control group received regular treatment,and the research group received bilevel non-invasive mechanical ventilation on the basis of regular treatment.,The results of blood gas analysis,respiratory frequency(RR),heart rate(HR) and expiratory flow(PEF) were observed and compared between the two groups.Results The indicators of pH,PaCO2,PaO2 in the research group after treatment were (7.41±0.08),(51.26±12.13)mmHg,(77.28±15.34)mmHg,which in the control group were (7.26±0.07),(67.21±13.05)mmHg,(69.26±14.31)mmHg,compared with before treatment,pH and PaO2 of the two groups were increased,PaCO2 was decreased after treatment,and the three indicators of the research group were changed significantly better than those of the control group,the differences were statistically significant(t=11.2,7.11,3.03,all P<0.01).After treatment,HR and PR of the two groups were decreased,PEF was increased.The three indicators of the research group were also changed significantly better than those of the control group,the differences were statistically significant(t=3.13,8.58,5.45,all P<0.01).Conclusion Bilevel non-invasive mechanical ventilation in the treatment of COPD with respiratory failure has remarkable clinical effects,can significantly improve the patients' blood gas analysis index,HR,RR and PEF,and it is worthy of clinical application.
6.Is operation necessary in idiopathic hyperaldosteronism?

The clinical data of 39 cases of idiopathic hyperaldosteronism (IHA) confirmed by operation were analyzed. The indications of operation consisted of (1) unable controlled the patients′ conditions by pharmacotherapy, (2) severe side effects due to high doses of medication and (3) difficulty in distinguishing between adenoma and nodule hyperplasia. The unilateral total adrenalectomy was suggested.
7.Relationship Between Auxotypes,Antibiotic Susceptibilities and Plasmid of Neisseria Gonorrhoeae

Objective To investigate the relationship between auxotypes,antibiotic resistance and plasmid of Neisseria gonorrhoeae. Methods 83 isolates of Neisseria gonorrhoeae from Chongqing during the period from January 1994 to November 1995 were examined.Results Eight auxotypes were detected in 83 strains. The prevalent auxotypes were proline requiring strains (51.8%) and nonrequiring (18.1%). The agar dilution method was performed to detect the susceptibility of 83 isolates to pencillin,tetracycline and spectinomycin, the results indicated that 15.6% of strains was penicillin resistant,70% was tetracycline resistant, 1.2% was spectinomycin resistant. Plasmid profile of 44 isolates,which was tested by rapid alkaline extraction method, showed that 70% of the strains carried 24.5 Md conjugative plasmid, 80% carried 2.6Md cryptic plasmid, 66% carried both 24.5Md and 2.6Md plasmids, and no plasmid was found in 16% of the strains. 8.Generalized Atrophic Benign Epidermolysis Bullosa:The First Case Report in China

Objective To report a pedigree of generalized atrophic benign epidermolysis bullosa (GABEB),a special type of junctional epidermolysis bullosa(JEB). This is the first case report in China. Methods The clinical, histologic, electron microscopic, and immunofluorescence changes of the disease were evaluated. Results Besides the symptom of inherited epidermolysis bullosa, the patient had special clinical manifestations such as atrophic alopecia and dental dysplasia. The disease was inherited in an autosomal recessive mode in this pedigree. Electron- microscopy found the fissure in lamina lucida, accompaning with dysplasia and sparse hemidesmosome. Immunofluorescence study did not show BPAg2 in the basement membrane zone, indicating that mutation in the gene encoding BPAg2 leading to the disease in this family. Conclusions The case is diagnosed as GABEB, a special type of JEB. Although GABEB has its characteristic, clinical manifestations, electron microscopy and immunofluorescence study are important for confirmative diagnosis and classification, and will guide the study of mutation detection.
9.Relation between modulation of the N-methyl-D-aspartate receptor and cyanide-induced neurotoxicity

Cyanide can activate the N-methyl-D-aspartate (NMDA) receptor by two approaches directly and indirectly. Firstly cyanide-induced depletion of energy is associated with the activation of NMDA receptors indirectly by increasing extracellular glutamate (Glu) and affecting cytosolic Ca 2+ homeostatic mechanisms. Secondly most likely as a conditioner of the NMDA receptor, cyanide enhances NMDA receptors responses by modulating redox sites of cysteine residue located in the subunit NR 1 or NR 2 of the NMDA receptor. NMDA receptor-induced neurotoxicity, initiated by the direct or indirect activation of NMDA receptor, leads to neuronal injury, apoptosis or necrosis finally. Therefore, it is believed that the activation of the NMDA receptor is presumably the key event in the mechanism of cyanide-induced neuronal injures.
