2.Experience of Carrying Out Pharmaceutical Care and Considerations on Clinical Pharmacists' Working Pattern

OBJECTIVE: To discuss the working pattern and key points for pharmacists to carry out pharmaceutical care.METHODS: The working contents and working pattern of pharmacists were summarized by reviewing the experience of pharmacists in carrying out pharmaceutical care in certain departments in our hospital. RESULTS & CONCLUSION: To carry out clinical pharmaceutical care service,clinical pharmacists should actively participating in clinical work by giving monitoring on prescriptions,drug interaction,ADR and nutritional support,and communication with patients etc.
3.Analysis on Blood Concentration Monitoring of Vancomycin and Clinical Administration Behavior

OBJECTIVE:To analyze the blood drug monitoring results of vancomycin and clinical administration behavior and then to provide a clinical reference for its rational utilization. METHODS:We retrospectively collected the blood drug monitoring results of vancomycin and drugrelated information from July 2007 to October 2008 in our hospital. These data were from the electronic clinical pharmacy workstation and initial written case history. RESULTS:Among the 76 cases,non-clinical significant results due to doctor’s irregular manipulation in collecting blood sample accounted for 32.89%. In renal failure group,the rate of peak concentration above the effective blood concentration range was 44.46% . Every patient was averagely administered 21.85 kinds of drugs mainly by intravenous injection during vancomycin application. Clinical response rate of vancomycin was 65.00%. CONCLUSION:During the blood concentration monitoring of vancomycin,we should take clinical need into consideration,collect blood samples rightly,consider the factors such as special pathophysiologic status of patients,drug combination,and adjust the dosage timely.
4.Isolation and identification of lung cancer stem cells from NCI-H1650 cell line

Objective To isolate and identify the lung cancestem like cellfrom the cell line NCI-H1650 .MethodThe NCI-H1650 cellwere continuously cultured in serum-free medium with human insulin ,recombinanepidermal grow th facto(EGF) ,recombinanbasifibroblasgrowth facto(bFGF) and bovine serum albumin(BSA) ,and paclitaxel waadopted to induce the stem cell.Flow cytometry and immunofluorescenstaining were used to analyze the expression levelof lung cancestem cell markeCD133 and CD326 .The expressionof stem cell related markeOct4 ,Nanog and Sox-2 were detected by RT-Pcand West-ern blo.ResultThe NCI-H1650 cellcontinuously cultured by serum-free medium and induced by combining paclitaxel induction showed the spherical suspension growth .Compared with NCI-H1650 cellcultured by serum ,CD133 and CD326 had significantly high expression and the expression levelof Oct4 ,Nanog and Sox-2 were significantly increased (P<0 .05) .Conclusion Adopting the serum-free culture in vitro combined with paclitaxel foinducing NCI-H1650 cellcan effectively isolate the stem cellamong them.
5.Progress of high density lipoprotein in resisting atherosclerosis

Coronary atherosclerotic heart disease is the leading cause of death in the world of non-infectious diseases.High density lipoprotein ( HDL ) plays an important role in the progression of anti-atherosclerosis.Recent studies have found that HDL can carry microRNA, and the anti-inflammation function of endothelial cell is achieved by the transport of microRNA.The specific functions of different HDL subtypes in the anti-atherosclerosis have been gradually recognized.At the same time, a series of traditional HDL anti-atherosclerotic functions also have been newly discovered, such as reverse cholesterol transport, antioxidant, anti-inflammatory and the protection of vascular endothelium.All of these advances provide a new basis for the clinical application of HDL.
6.Analysis of Drug Utilization by 98 Cases of Renal Transplant Inpatients During 2005

Objective:To analyze the situation of immunosuppressants utilization of renal transplantation inpa- tients in our hospital.Method:The records and relevant data of 98 case of renal transplant inpatients during 2005 were col- lected,and analyzed according to drug cost and defined daily dose(DDDs)ranking.Result:The average age was(42.50?10.56)a,the average drug cost was(18745.36?7603.86)yuan,the average inmunosuppressant cost was 13456.58 yuan,and all the drug utilization indicators(DUI)were≤1.Conclusion:The analysis shows that the inmunosuppressant utilization rate was high and the inmunosuppressant utilization by renal transplant inpatients was basically rational.
7.In vitro metabolism of forscolin isolated from Coleus forskohlii.

This paper is to report the study of the metabolism of forscolin in plasma and liver microsomes for guiding clinical therapy. Forscolin was quantified by HPLC-MS/MS. The metabolic stability of forscolin in rat, Beagle dog, monkey and human plasma and liver microsomes, mediated enzymes of forscolin and its inhibition on cytochrome P450 isoforms in human liver microsomes were studied. Results showed that forscolin was not metabolized in plasma of the four species but metabolized in liver microsomes of the four species. The t1/2 of forscolin in rat, Beagle dog, monkey and human liver microsomes were (52.0 +/- 15.0), (51.2 +/- 5.9), (6.0 +/- 0.2) and (11.9 +/- 1.8) min; CL(int) were (75.6 +/- 18.7), (60.9 +/- 6.8), (513.8 +/- 14.3) and (176.2 +/- 25.6) mL x min(-1) x kg(-1); CL were (34.8 +/- 4.5), (23.3 +/- 1.0), (40.3 +/- 0.5) and (17.9 +/- 0.3) mL x min(-1) x kg(-1), respectively. Forscolin was metabolized by CYP3A4 in human liver microsomes. There was definite inhibition on CYP3A4 at the concentrations of forscolin between 0.1 ng x mL(-1) and 5 microg x mL(-1). Therefore, forscolin is rapidly excreted from liver microsomes. Attention should be paid to the drug interaction when forscolin was used along with other drugs metabolized by CYP3A4 in clinics.
8.Pharmacists' Practice in Carrying out Drug-interaction-related Pharmaceutical Care

10.The changes of PECAM-1,Bcl-2 and Bax after focal cerebral ischemia in rats

Objective To observe the changes of PECAM-1,Bcl-2 and Bax expressed by ischemic cerebrum of adult rats after focal cerebral ischemia. Methods Middle cerebral artery occlusion (MCAO) models were made by operation with Longa suture method in Wistar rats. The expression levels of PECAM-1,Bcl-2 and Bax at the 6th,12th,18th,24th,48th,72nd hour after MCAO were detected by immunohistochemistry staining. Results The expression levels of PECAM-1,Bcl-2 and Bax in cerebrum were significantly increased after MCAO. The expression levels of Bcl-2 we、re up to the peak at the 12th hour after MCAO, while the levels of Bax and PECAM-1 were up to the peak at the 24th and 48th hour after MCAO. At the 72nd hour after MCAO, the expression levels of PECAM.1.Bcl-2 and Bax were still higher than that in the control group(all P<0.001).Conclusions PECAM-1,Bcl-2 and Bax participate in the different pathological stages of focal cerebral ischemia.
