1.Ethics of the donation after cardiac death

Donation after cardiac death (DCD) means organ harvest and donation cannot be carried on until the requirement of death criteria using cardiopulmonary arrest is reached.Great success has been achieved since DCD was promoted widely in China.However,some problems were encountered during the implementation of DCD and there also exists many ethical controversies.The ethical principles of informed consent and no harm mentioned inDonation after Cardiac Death Work Instructions of China should be strictly adhered to,while justice,fairness and openness must be ensured in organ allocation.Appropriate compensation should be given to DCD donor and his family,and the utilization of DCD should be further improved.All of these ethical principles will help to promote the healthy development of organ donation and transplantation in China.
2.Clinical analysis of dual-port video-assisted thoracic surgery for secondary spontaneous pneumo-thorax

Objective To evaluated clinical effects and effectiveness of dual-port video-assisted thoracic surgery(VATS)for secondary spontaneous pneumothorax.Methods The clinical data of 48 pa-tients with secondary spontaneous pneumothorax who underwent open thoracotomy(n =18)or dual-port VATS(n =30)were reviewed retrospectively.Mean operation time,mean postoperative drainage period and mean postoperative hospital stay between the two groups were compared.Results All surgeries were suc-cessfully performed without reoperation and severe complications.No patient was converted to thoracotomy in the VATS group.There were significant differences in mean operation time [(67.9 ±7.2)min vs (73.3 ±6.4)min],mean postoperative drainage period [(3.2 ±0.9)d vs(5.0 ±1.3)d],and mean postoperative hospital stay [(7.2 ±1.4)d vs(8.7 ±1.5)d]between the dual-port VATS and open thora-cotomy(P <0.05 ).Conclusion Dual-port video-assisted thoracic surgery can reduce postoperative drainage period and postoperative hospital stay and increase satisfaction.It has little influence on upper limb movement and improves postoperative recovery.
3.The Clinical Effect of Ilium Grafting Combined with Duhuojishengtang on Patients with Delayed Fracture Healing and the Changes of Hemorheology of Patients

Objective To investigate the clinical effect of ilium grafting combined with Duhuojishengtang on the delay fracture healing and explore the effect on hemorheology. Methods Eighty patients with delayed fracture healing were randomly divided into two groups. Forty patients in control group were treated by ilium grafting and 40 patients in observation group were treated by ilium grafting combined with Duhuojishengtang. The clinical effect was observed after treatment and indexes of hemorheology were recorded after different treatment time. Results The cure rate of observation group was 62.5%, which was significantly higher than that of control group 35.0%, the effective rate of observation group was 92.5%, which was significantly higher than that of control group 72.5% (P<0.01) . There were no significant differences between two groups in indexes of hemorheology after ilium grafting (P>0.05) . After the first, second, third course, the indexes of hemorheology of observation group were better than those of control group, with statistically significant differences (P<0.05) . Conclusions Ilium grafting com-bined with duhuojishengtang can improve the indexes of hemorheology, improve the cure and effective rate and also has high security. It has high clinical value and worth of popularization and application.
4.Effects of hemorrhagic shock and sodium cyanide intoxication on rabbit serum oxidative stress

Objective To investigate the effects of sodium cyanide (NaCN) and hemorrhagic shock on the level of serum oxidative stress in rabbits. Methods A total of 15 Japanese white rabbits were randomly divided into 5 groups: sham operation, hemorrhagic shock (HS), NaCN intoxation, HS+NaCN and HS+NaCN+4-dimethy laminophenol (4-DMAP). The rabbits were anesthetized for left arteria carotis communis cannulation, and connected with three-way tube and RM6240 electronic physiological signal recording system. The models of hemorrhagic shock and/or sodium cyanide intoxation (blood pressure was controlled at 5.3 kPa) were established. At 0.5, 1 and 2 h after the intraperitoneal injection of 2.5 mg/kg NaCN and/or 3.2 mg/kg 4-DMAP, the blood were drawn. 5.Antiviral treatment status and progress of chronic hepatitis C in children

Children with hepatitis C virus RNA positive were considered to start antiviral treatment.Alpha interferon or pegylated interferon plus ribavirin in children achieved satisfactory sustain virology response.Although there were many adverse events,some can be relieved by communicating with the guardian,bringing care to the patient and giving treatment.Clinical trials in children with hepatitis C by the direct-acting antiviral agents were in progress,safety and efficacy have no differences with the adult.There were still lots of work to do in view of the particularity of children.
6.The effect of pinacidil ischemic postconditioning protects against myocardial injury in rats

Aim To observe the effect of pinacidil ischemic postconditioning on rat ventricular myocytes during ischemia/reperfusion.Methods Rat acute myocardical anoxia-reperfusion model was used.The cardiac function and electrophysiological properties were surveyed by isolated perfusion and patch-clamp technique,respectively.Results Compared with those in the control groups,these results demonstrated that anoxia-reperfusion groups in rats could increase PSP and-dp/dtmax markedly while LVDP,+dp/dtmax and HR decreased slightly.Significant difference was found in A/R group;while pinacidil group showed the better recovery of cardiac function after ischemia/reperfusion. using patch-clamp technique,significant difference was found in pinacidil group,including increasing mean open time and conductance value.Conclusion Ischemic postconditioning by Pinacidil(PCOS of KATP)exerts an cardioprotection effect on acute myocardial anoxia-reperfusion injury.

10.Biomechanical evaluations of three anterior thoracolumbar internal fixation devices

BACKGROUND: The anterior thoracolumbar internal fixation devices can be divided into two main categories: the stick system such as kaneda,and the plate system such as Z-plate. Both categories have satisfactory biomechanical properties, but the devices of these types are all imported and so expensive that they exceed the affordability of domestic patients:Based on various anterior throacolumbar internal fixation devices and the spinal characteristics of the Chinese people, the author designed a new brand of anterior thoracolumbar internal fixation device for Chinese people:New Ti-plate (NTP).OBJECTIVE: Biomechanical properties of three anterior thoracolumbar internal fixation devices were compared in an attempt to provide a scientific and theoretical basis for clinical applications.DESIGN: A sampling investigation.SETTING: The Department of Orthopaedics of the Second Affiliated Hospital of Soochow University and Department of Radiology of Nantong Hongqiao Hospital.PARTICIPANTS: This study was carried out at the Biomechanical Engineering Research Institute of Shanghai University between May and August 2003. Fifteen fresh spinal samples from the healthy adults (T12-S1) were used in this study.METHODS: Fifteen spinal (T12-S1) specimens were obtained from fresh bodies to induce injuries to anterior and middle columns, which were fixed with Dunn Ⅲ, NTP and Z-plate respectively to compare the strength, rigidity, and the stability of the spine.MAIN OUTCOME MEASURES: Changes of load-straining and loaddisplacement of the three groups of specimens under the conditions of axial decompression, anteflection, post-extention, and lateral flexion.RESULTS: Fifteen fresh adult spinal specimens were involved in this study and all entered the stage of result analysis. Under the maximal load of 500 N,the average straining for Z-plate, NTP and Dunn Ⅲ was 410 uε,430 uε, 477 uε respectively. The average longitude displacement for the above three devices was 2.23 mm, 2.38 mm and 2.85 mm respectively.The thoracolumbar fracture was fixed with three anterior internal fixation devices and it was proved that Z-plate and NTP fixations were better than fixation with Dunn Ⅲ in respect of spinal strength, rigidity and stability (P < 0.05), but the difference between Z-plate and NTP had no statistical significance (P > 0.05).CONCLUSION: NTP and Z-plate devices are conformed to the spinal biomechanical principles and were capable of reconstructing spinal biomechanical stability effectively, and it is a preferable anterior spinal internal fixation device.
