1.Medical coping strategies and impact factors of patients with coronary heart disease in Wuhan

Objective To explore the medical coping strategies and impact factors of patients with coronary heart disease in Wuhan.Methods Both medical coping strategies and impact factors of 90 patients with coronary heart disease in Wuhan were analyzed by using Spearman coefficient and multiple linear regression analysis,and compared with the norm of 701 cases.Results Compared with the norm,patients with coronary heart disease in Wuhan preferred less facing coping strategies and more avoidance coping strategies:(17.89±2.67) points vs.(19.48±3.81) points,(16.95±2.54) points vs.(14.44±2.97) points,and there were significant differences (t=2.146,P=0.035;t=1.994,P=0.048).The yielding coping strategies was indicated insignificant differences (t=1.758,P=0.173).The facing coping strategies were correlated with age (r=-0.212,P=0.036),education levels (r=0.190,P=0.041),family monthly income (r=-0.106,P=0.049) and disease course (r=0.327,P=0.017).The avoidance coping strategies were correlated with marital status (r=-0.304,P=0.013).The yielding coping strategies were correlated with length of disease(r=0.294,P=0.023)and therapy methods (r=0.308,P=0.017).Conclusions Based on patients' characteristics and impact factors,nursing staff should guide and encourage patients to use positive medical coping styles.
2.Establishment of a eukaryotic expressing system for human decay accelerating factor

Objective:To construct recombinant expressing vector pcDNA3-DAF and to develop the NIH3T3 cell model expess human complement regulatory protein decay accelerating factor(DAF,CD55)stably after transfected.Methods:Human membrane complement regulatory protein(hCRP) DAF cDNA containing the full-length of encoding region was cloned into expressing vector pcDNA3.After identification by restriction enzyme digestion,PCR and sequencing,the recombinant plasmid was transfected into NIH3T3 cells with calcium phosphate-DNA precipitate method.A stably-transfected cell line was established by G418 selection.Extraneous gene integration was identified by PCR.Expression of DAF at both mRNA and protein levels was analyzed by RT-PCR,Western blot and indirect immunofluorescence microscopy.Results:The eukaryotic expression vector pcDNA3-DAF was successfully constructed and the DAF gene was transfected stably into NIH3T3 cells,a stably-transfected cell line was established and DAF was efficiently expressed on the surface of transfected NIH3T3 cells.Human DAF cDNA was integrated into NIH3T3 pcDNA3-DAF genomic DNA after continuous 30 times passages,indicating that NIH3T3 pcDNA3-DAF was stable cell line.Conclusion:The establishment of the stably-transfected cell line and the expression of the target gene provide a base for further studies on the function of the DAF and the cooperative fashion among different human complement regulatory proteins in alleviating the complement-mediated cytolysis.
3.Co-expression and synergic effect of human complement regulatory proteins DAF and MCP.

Recombinant expression vector pcDNA3-DAFMCP-DP containing human membrane complement regulatory proteins (hCRPs) decay accelerating factor (DAF) and membrane cofactor protein (MCP) cDNA was constructed by using two independent promoters. After transfected into NIH3T3 cells by calcium phosphate-DNA precipitate method, NIH3T3 pcDNA3-DAFMCP-DP transfectants were obtained by G418 selection. Extraneous genes integration was identified by PCR. The co-expression of human DAF and MCP at both mRNA and protein levels was confirmed by using RT-PCR and Western blot analysis. Human DAF and MCP cDNA were integrated into NIH3T3 pcDNA3-DAFMCP-DP genomic DNA after continuous 30 times passages, indicating that NIH3T3 pcDNA3-DAFMCP-DP were stable cell lines. Human C-mediated cytolysis assays showed that NIH3T3 cells transfected stably with pcDNA3-DAF, pcDNA3-MCP, and pcDNA3-DAFMCP-DP were protected from C-mediated damage and co-expressed human DAF and MCP provided more excellent protection against C-mediated attack, which was compared with either DAF or MCP alone. These results suggest that the dicistronic vector could improve the efficiency of multi-gene delivery and benefit the synergic effect of human membrane complement regulatory proteins DAF and MCP.

4.An evaluation study on the randomized controlled trial of preoperative skin preparation for emergency percutaneous coronary intervention based on GRADE system

Objective To systematically evaluate and analyze the evidence level of randomized controlled trials (RCT) of preoperative skin preparation for emergency percutaneous coronary intervention (PCI), and to understand the current research status and evidence level of preoperative skin preparation randomized controlled trials for emergency PCI, and provide reference for skin preparation for emergency PCI. Methods PubMed, EMbase, The Cochrane Library, CINAHL, JBI, CBM, CNKI, Wanfang DATA were searched by computer from inception to March, 2018 for emergency PCI preoperative skin preparation randomized controlled trials. Two evidence panel members searched and selected articles independently and the quality was assessed in accordance with Cochrane Manual. The articles were analyzed with Review Manager 5.3, and the evidence quality was assessed with GRADE profiler 3.6.1 software. Results A total of 5 RCTs were included, of which the number of RCTs with grade A quality was 1 and the number of RCTs with grade B was 4. The results of the Meta analysis showed that there was no significant difference in the incidence of skin infections at the postoperative puncture site by conventional methods for routine removal of surgical wild hair and no removal of hair prior to emergency PCI (P<0.05). In addition, regular removal of hair before surgery may result in prolonged preoperative preparation and may cause psychological discomfort to the patient. After the GRADE system rating, the quality of the evidence body was of a lower level. Conclusions It is more beneficial to shorten the treatment time without routine removal of all the hair at the puncture site and the surrounding patients. The evidence included is not yet certain whether the incomplete removal of hair can reduce the infection rate. In the future, more large-scale, multi-center, high-quality research should be carried out to provide more credible evidence for this study.
5.Construction of sensitive quality indicators for cardiovascular nursing based on RAND-UCLA expert consensus method

Objective:To establish the sensitive quality indicators for cardiovascular nursing based on the RAND Corporation University of California, Los Angeles (RAND-UCLA) expert consensus method, so as to lay the foundation for scientific and standardized quality management of cardiovascular nursing.Methods:Relevant literature was searched on China National Knowledge Infrastructure, WanFang Data, VIP, China Biomedical Literature Database, Cochrane Library, PubMed, EMbase and CINAHL. The search period was from database establishment to August 31, 2021. We used the Johns Hopkins evidence-based nursing practice model for evidence-based analysis and summary, and referred to documents on cardiovascular nursing quality indicators to form an indicator pool. From May 23 to July 12, 2022, 17 experts were selected to apply the RAND-UCLA expert consensus method to inquire about the indicators.Results:The effective response rates of the first and third rounds of the questionnaire were both 100.00% (17/17) , and the authority coefficients of the experts were 0.87 and 0.88, respectively. Among the 18 indicators in the third round of expert consultation, the consistency degree of importance and operability of all indicators was less than 1.00. Finally, 18 sensitive quality indicators for cardiovascular nursing were established.Conclusions:The consistency and coordination of experts in this study are good, and the nursing sensitive indicators formed have scientific characteristics, cardiovascular specialty characteristics, and good operability.
6.Effect of ulinastatin on the expression of serum inflammatory mediators in patients with acute myocardial infarction (AMI) after thrombolysis

Objective To investigate the effect of ulinastatin on the expression of serum inflammatory mediators in pa tients with acute myocardial infarction (AMI) after thrombolysis. Methods 84 patients with AMIin our hospital from January 2017 to December 2017 were selected and randomly divided into the control group and the experimental group with 42 cases in each group. The control group received recombinant streptokinase intravenous thrombolytic therapy, while the experimental group was given intravenous infusion of ulinastatin in the foundation of thrombolytic therapy. Two groups of patients were extracted venous blood to measure the expression levels of inflammatory mediators including hs-CRP, IL-1, IL-10 and TNF-a before treatment, 24 h and 72 h after treatment. Results 24 h after treatment, the expression levels of hs-CRP, IL-1, IL-10 and TNF-a in the two groups were higher than those before the treatment, and the difference was statistically significant (P<0. 05). However, the increase in the experimental group was lower than that of the control group (P<0. 05). 72 h after treatment, the levels of inflammatory mediators in the two groups were significantly lower than those before and 24 h after the treatment(P<0. 05). The decrease in the experimental group was higher than that of the control group and the difference between the two groups was statistically significant(P<0. 05). There was no significant difference in the incidence of adverse reactions and adverse cardiovascular events between the two groups during hospitalization after thrombolysisP>0. 05). Conclusion The use of ulinastatin before and after thrombolytic therapy can effectively inhibit the expression levels of inflammatory mediators and relieve myocardial ischemia reperfusion injury in patients with AMI.
