Objective To develop a clinically useful assay for detecting the mutations of HBV pre-C/BCP based on the pyrosequencing and accuracy, reproducibility and reliability of this assay was evaluated. Methods The pyrosequencing primers for HBV pre-C/BCP mutation were designed through the cluster analysis among one hundred HBV gene sequences. After the amplification of the fragment of pre-C/BCP with the template of pre-C/BCP mutation plasmids, the pyrosequencing method for pre-C/BCP detection was initially set up with this standard sample. The accuracy, reliability and reproducibility of the pre-C/BCP pyrosequencing were confirmed through the pre-C/BCP plasmids as a standard sample when compared with Sanger/genechip sequencing method pre-C/BCP pyrosequencing assay was applied for detecting pre-C/BCP mutation types of 60 chnical serum samples in HBV patients. Results The pre-C/BCP mutation detection assay based on pyrosequencing has been established in our study. The coincidence rate between pyrosequencing and Sanger squencing was 100%. The coincidence rate between the result of pyrosequencing and of genechip method was 91.7%. The reproducibility of this assay was 97. 8%. It indicates the pre-C/BCP pyrosequencing is a high-accurate method with, good-reproducibility and high-reliability. And multi-site detection can be achieved by pyrosequencing one time. A rare mutation T1758C was also detected. Conclusion Pyrosequencing for pre-C/BCP mutations assay is high-throughout method for simultaneous detection of multi-site mutation.

Objective:To explore the effect of the operation process query system and the psychological impact on the surgical patient's family members. Methods:From April to October 2019, 198 family members of patients whose operation time> 1 hour were selected from the Shanghai East Hospital, Tongji University. The patients were divided into a control group and an observation group according to the order of operation, with 99 cases in each group. Both groups implemented preoperative visits, preoperative assessments, and preoperative education. The control group performed routine nursing of Operating Room and did not specifically inform family members of the information about the operation process during the operation. The family members of the observation group could view information about the operation process in the operation progress query system. The State-Trait Anxiety Inventory (S-AI) was used to assess the anxiety of the family members of the two groups of patients, and we compared the changes of pulse, respiration and blood pressure before and during the operation of the two groups of family members, and compared the satisfaction of the family members of the two groups after the operation. Results:The anxiety scores and vital signs of the family members of the two groups of patients during the operation were higher than those before the operation. The family members of the observation group had an intraoperative anxiety score of (49.01±7.91) , pulse (92.97±8.76) times/min, respiration (20.13±1.64) times/min, systolic blood pressure (122.06±11.69) mmHg, diastolic blood pressure (82.86±6.45) mmHg, all lower than those of the control group, the differences were statistically significant ( t=20.494, 6.749, 26.784, 4.387, 8.139; P<0.01) . The total satisfaction of the family members of the observation group after the operation was 95.96% (95/99) , and that of the control group was 63.64% (63/99) , and the difference was statistically significant (χ 2=34.932, P<0.01) . Conclusions:The application of the operation query system can effectively alleviate the psychological anxiety of the family members of the surgical patients and improve the satisfaction of the family members of the patients with the operation.

Objective? To investigate and analyze the nursing service demand and satisfaction of the operation patients in department of general surgery outpatient. Methods? A total of 516 operation patients were selected from the outpatient surgery in Eastern Hospital Affiliated to Tongji University from July to December 2017 were selected by convenience sampling method. Nursing service demand questionnaire and satisfaction questionnaire were applied in the investigation. Results? The average score of demand for nursing services of 516 patients was (3.72±0.62), which was between need and general. The highest requirement before operation was to know the time of the operation and the time needed for the operation. The highest requirement during operation was that I would not feel pain and discomfort. The highest requirement after operation was to tell me how to seek help if I got home uncomfortable. The average score of nursing satisfaction was (4.04±0.48), which was between great satisfaction and satisfaction. The most unsatisfactory thing for patients is that the waiting time is more than 30 minutes. There were significant differences in the needs of pre-operative, intraoperative and post-operative nursing services among patients of different ages (P<0.05). Conclusions? There are many demands for nursing services in outpatients in department of general surgery. Operating rooms should cooperate with multiple departments to provide targeted nursing services to meet the needs of patients. Hospitals should constantly optimize the process of patients' medical treatment, adopt the method of time-division appointment, shorten the waiting time of patients, and improve patients' satisfaction.
