1.Pseudomyxoma peritonei: report of 8 cases

Objective To investigate the diagnosis and treatment of pseudomyxoma peritonei(PMP).Methods[WT5”BZ] Retrospective study was made on 8 cases of PMP treated in our department.Results Main clinical presentations included abdominal pain,distention,mass and wasting.Ultrasonography showed heterogeneous ascites in 2 cases and elevated CEA level in 4 cases.All patients underwent surgery.Primary tumour was identified in the appendix in 7 cases and in the colon in 1 case.Surgical procedure included resection of primary tumour, omentectomy ,removal of mucinous ascites and combined resection of organs involved.Postoperative adjuvant chemotherapy and /or radiotherapy was added to 6 cases.All patients were followed up and 5 year survival rate was 50%(4/8),10 year survival rate was 13%(1/8).Conclusions 1 .PMPisarareconditionanddifficulttodiag nosisbeforesurgery ,ultrasonographyandCEAmayhelpinmakingadiagnosis .2 .Beingalow gradecarcinomaand infrequentmetastasis,PMPisoftenresponsivetorepeateddebulkingfollowedbyadjuvanttherapy,hencehasafa vorablelong termsurvival.
2.SURGICAL MANAGEMENT OF PRIMARY HYPERPARATHYROIDISM

To study the surgical management of primary hyperparathyroidism, we analysed 30 patients with primary hyperparathyroidism operated in our hospital from 1981 to 1990. A low collar transverse incision was done in all patients. A single parathyroid adenoma was found in 28 patients,and in 3 cases there were ectopic glands (1 in the mediastinum 2 intrathyroid). The adenoma was removed in all patients.In 1 case there was parathyroid hyperplasia, but only one gland was involved and it was excised. 1 case was diagnosed as parathyroid carcinoma which was located in the tracheo esophageal groove and had invaded the trachea and esophagus. An en bloc resection and tracheostomy were performed. 25 cases were followed up(including 1 case with parathyroid hyperplasia and 1 case with carcinoma) from 8 months to 19 years, there was no recurrence.These results suggest surgical management is the most effective treatment for primary hyperparathyroidism. Removal of the parathyroidoma and unilateral cervical exploration are adequate for the patients with parathyroid adenoma. Bilateral exploration is necessary when parathyroid hyperplasia is found. An en bloc resection is necessary for parathyroid carcinoma.
3.ANALYSIS OF RARE CAUSES OF OBSTRUCTIVE JAUNDICE IN CHILDREN

To investigate some rare causes of obstructive jaundice in children. Three school children with obstructive jaundice caused by uncommon causes were analyzed. Despite pre operative physical examination, B Ultrasonic and CT examinations, the diagnosis was not established. The main clinical manifestation of three patients was obstructive jaundice. Postoperative pathological examination proved that one patient was suffering from cryptococcus neoformans infection of the common bile duct, another embryonal rabdomyosarcoma of common bile duct, and the third primary hydatid cysts in the common bile duct. They were all treated with surgical removal of the lesions, followed by reconstruction of the bile duct and other adjunctive treatment with satisfactory result. The results suggest that obstructive jaundice in children may be caused by some unusual diseases. Comprehensive analysis and corresponding treatments were recommended.
4.Immunogenicity of a novel vaccine with VLP as vector for Wilm's tumor

Objective To develop a vaccine for Wilm's tumor in order to lay a foundation for effective treatment and substitute for the radiotherapy and chemotherapy of this tumor. Methods DNA fragments encoding the WT1 antigens (including, HLA-A * 2402 and HLA-A * 2402x), and DNA fragments encoding the couple antigens HLA-A * 2402-HBc and HLA-A * 2402x-HBc were cloned and inserted into the eukaryotic expression vector pcDNA3.1(+) in sense orientation and baculovirus expression pFactbac respectively, then transfected into COS-7 cells and Sf 9 cell respectively. The expression of the target proteins in the cells were identified by CPE, SDS-PAGE and electron microscopy. Virus-like particles were observed under electron microscopy. Results The fusion gene of HLA-A * 2402 and HLA-A * 2402x and HBc express virus-like particle protein. Conclusions This preliminary result shows a hopeful future in developing an effective immunotherapy to Wilm's tumor.
5.A preliminary exploration for the proteome of children neuroblastoma

Objective To screen the differential expression proteins in the children with neuroblastoma (NB) by proteomics tools.Methods Three specimens were collected from the patients diagnosed Ⅳstage NB by biopsy at Department of Pediatric Surgery of Chinese PLA General Hospital in Beijing from July to December,2011.Another three specimens were acquired from the same patients underwent tumor excision.Average age was 3.17 years.Proteins in neuroblastoma before and after chemotherapy were separated by two dimensional gel electrophoresis,analyzed by high performance liquid chromatography-eleetrospray tandem MS (Nano-UPLC-ESIMS/MS).Results After two dimensional gel electrophoresis,we obtained the maps about tissues before and after chemotherapy.There were 7 differential protein spots identified by using the Image Master two dimensional gel electrophoresis software,in which 2 were up-regulated,including Nm23 protein and neuropolypeptide h3,5were down-regulated after chemotherapy,including stathminl,heat shock protein 27,mitochondrial short-chain enoyl-coenzyme A,peroxiredoxin 1 and peroxiredoxin 3.Conclusion The differential expression proteins of children neuroblastoma before and after chemotherapy were successfully identified by two dimensional gel electrophoresis and Nano-UPLC-ESI-MS/MS.
