Chinese Journal of Postgraduates of Medicine 2012;35(18):10-12
Objective To investigate the clinical effect of uterine arterial embolization combined with hysteroscopes in the treatment of cesarean scar pregnancy(CSP).Methods Seventy CSP patients were divided by willingness of patients and their familiality into treatment group and control group with 35 cases each. The treatment group was treated with uterine arterial embolization combined with hysteroscopes, while the control group was treated with uterine curettage after crug therapy. The intraoperative and postoperative situations of two groups were observed. Results The intraoperative blood loss, erythrocyte suspension transfusion, plasma transfusion, preparation time before curettage, hospital stay and pestoperative serum β- human chorionic gonadotropin in treatment group were all obviously lower than those in control group [(420.7 ± 18.9) ml vs.(1893.9 +39.8) ml,(2.1 + 1.3) U vs.(11.1 ±1.4) U,(400.2 ±23.6) ml vs.(1700.3 ± 31.4) ml,(3.9 ± 2.3) d vs.( 18.8 ± 6.7) d,( 14.32 ± 3.89) d vs.(37.83 ± 8.98) d,(312 ± 54) U/L vs.(532 ± 85) U/L,P< 0.01].There was 1 case with massive hemorhage in treatment group, while 3 cases in control group. Conclusions Uterine artery embolization combined with hysteroscopes is a safe and effective treatment with less trauma for CSP.It is worthy of clinical promotion and application.

OBJECTIVE:To provide a reference for the management of the facilities and equipment of the pharmacy intrave-nous admixture service(PIVAS). METHODS:The experience in the management of the facilities and equipment of PIVAS in our hospital was introduced in respect of system establishment, archives management and the maintenance of important equipment, and the performance of core purification equipment was tested to evaluate management effect. RESULTS & CONCLUSIONS:The PIV-AS in our hospital established a practical system by developing and improving 7 management systems,9 sets of operational proce-dures and 5 contingency plans, increased the awareness of archives management by perfecting 27 archive files and standardized and conducted the maintenance of key equipment. In 2014 and 2015,after implementing management, the passing rates of all test items of the purification equipment were 100%. It is suggested that facilities management should focus on active preparation of an intelli-gent monitoring system, the improvement of the information system and the training of professional management personnel.

Type 1 diabetes mellitus (T1D) is an immune-mediated disease. The autoreactive T cells in T1D patients attack and destroy their own pancreatic cells. In order to systematically investigate the potential autoreactive T cell receptors (TCRs), we used a high-throughput immune repertoire sequencing technique to profile the spectrum of TCRs in individual T1D patients and controls. We sequenced the T cell repertoire of nine T1D patients, four type 2 diabetes (T2D) patients and six nondiabetic controls. The diversity of the T cell repertoire in T1D patients was significantly decreased in comparison with T2D patients (P = 7.0E08 for CD4+ T cells, P = 1.4E04 for CD8+ T cells) and nondiabetic controls (P = 2.7E09 for CD4+ T cells, P = 7.6E06 for CD8+ T cells). Moreover, T1D patients had significantly more highly-expanded T cell clones than T2D patients (P = 5.2E06 for CD4+ T cells, P = 1.9E07 for CD8+ T cells) and nondiabetic controls (P =1.7E07 for CD4+ T cells, P= 3.3E03 for CD8+ T cells). Furthermore, we identified a group of highly-expanded T cell receptor clones that are shared by more than two T1D patients. Although further validation in larger cohorts is needed, our data suggest that T cell receptor diversity measurements may become a valuable tool in investigating diabetes, such as using the diversity as an index to distinguish different types of diabetes.

Objective:To investigate the clinical value of magnetic resonance imaging (MRI) in the early diagnosis of ankylosing spondylitis involving hip joint. Methods:One hundred and twenty-eight patients with ankylosing spondylitis involved in the hip joint were selected who were treated in the People′s Hospital of Longhua District from January 2017 to February 2020. The patients were divided into computed tomography(CT) group (64 cases) and MRI group (64 cases) according to the examination method of CT and MRI were performed respectively, and the incidence of abnormal hip joints were analyzed, and the prognostic treatment effects of the two groups were compared after following up.Results:Twenty-seven cases of fat deposition, 43 cases of subchondral bone marrow edema and 31 cases of inflammatory changes of tendon and ligament attachment were detected in MRI group, while CT didn′t showed these changes. Thirty-four cases of hip joint lesions were detected in CT group, the detection rate was 53.1%(34/64), and 56 cases of hip joint lesions were detected in MRI group, the detection rate was 87.50(56/64), the difference was statistically significant ( P<0.05). After treated for 3 months, the cure rate of the MRI group was higher than that of the CT group: 65.6%(42/64) vs. 34.4% (22/64), the difference was statistically significant ( χ2=18.11, P<0.05). Conclusions:MRI is an important imaging method for the early diagnosis of ankylosing spondylitis involving the hip joint, and its diagnostic sensitivity is better than that of CT.
