Objective To describe the clinical and imaging manifestations of patients with synovitis acne pustulosis hyperostosis osteomyelitis (SAPHO) syndrome, and to analyze the diagnostic importance of different clinical and imaging manifestations for SAPHO syndrome. Methods Seventeen patients (7 males and 10 females) with SAPHO syndrome were recruited in this study.Age ranged from 36 to 67 years with a mean age of (48 ± 8) years. All patients fulfilled the diagnostic criteria of Benhamou. Serum HLA B27 antigen records were reviewed for all patients. Imaging data of the abnormal bone sites were collected by conventional radiograph in all patients,CT in 13 patients as well as MR in 3 patients.Average time to take for a definite diagnosis of the syndrome was 3.7 years (ranged from O.5 to 13 years). Results Serum HLA B27 antigen was positive in all patients. Both skin and bone abnornalities were found in all patients.Ten patients had skin palmoplantar pustulosis and two patients had acne. Involving sites of bone and joints include sacroiliac joints, anterior chest and limbs.Sacroiliac joints were asymmetrically involved with imaging features in all patients.Eight patients exhibited anterior chest wall involvement. Five patients had osteomyelitis at limbs. For all images of 17 patients,CT was superior to conventional radiography in detecting abnormal changes of bone erosion and soft tissue swelling.MR imaging was able to depict edema changes that was not detectable by CT and radiography. Conclusion SAPHO syndrome is a rare disease, but for patients with skin and bone-joint abnormalities, especially with skin palmoplantar pustulosis, acne as well as with imaging features at the sacroiliac joint and anterior chest wall,SAPHO syndrome should be taken into a diagnostic consideration.

The present study aims to predict the action targets of antidepressant active ingredients of Xiaoyaosan to understand the "multi-components, multi-targets and multi-pathways" mechanism. Using network pharmacology, the reported antidepressant active ingredients in Xiaoyaosan (saikosaponin A, saikosaponin C, saikosaponin D, ferulic acid, Z-ligustilide, atractylenolide I, atractylenolide II, atractylenolide III, paeoniflorin, albiflorin, liquiritin, glycyrrhizic acid and pachymic acid), were used to predict the targets of main active ingredients of Xiaoyaosan according to reversed pharmacophore matching method. The prediction was made via screening of the antidepressive drug targets approved by FDA in the DrugBank database and annotating the information of targets with the aid of MAS 3.0 biological molecular function software. The Cytoscape software was used to construct the Xiaoyaosan ingredients-targets-pathways network. The network analysis indicates that the active ingredients in Xiaoyaosan involve 25 targets in the energy metabolism-immune-signal transmutation relevant biological processes. The antidepressant effect of Xiaoyaosan reflects the features of traditional Chinese medicine in multi-components, multi-targets and multi-pathways. This research provides a scientific basis for elucidation of the antidepressant pharmacological mechanism of Xiaoyaosan.

Chinese Journal of Integrated Traditional and Western Medicine 2003;23(12):922-925
OBJECTIVETo observe the anti-liver-fibrosis effect of Guzhang Tablet (GZT) and to explore its mechanism.
METHODSFifty female rats, weighing 200-220 g were randomly divided into the blank control group, the model group, the small dosage GZT group, the large dosage GZT group and the Biejiajian Pill (BJJP) group, 10 rats in each group. The changes of related serum enzymes, liver-fibrosis marker, oxygen free radical and liver tissue pathology were observed after 8 weeks of treatment.
RESULTS(1) Compared with the model group, the serum levels of alanine transaminase (ALT), aspartate aminotransferase (AST), glutamyltranspetidase (GGT), alkaline phosphatase (AKP) were improved in the GZT groups, better effect was got in the large dosage GZT group. (2) Compared with the blank control group, GZT showed no effect on serum total protein (TP) but with raising albumin (ALB) effect. (3) Compared with blank control group, the levels of hyaluronidase (HA), laminin (LN), collagen type IV (IV-C) and procollagen type III (PC III) in both GZT groups were lower, especially in the large dosage GZT group. (4) Small and large dosage of GZT showed effect in reducing malonyldialdehyde (MDA), Glutathione-peroxidose(GSH-Px) and nitric oxide (NO) content, but the effect on increasing superoxide dismutase (SOD) similar to that of BJJP with no significant different. (5) GZT, both small and large dosage, had obvious anti-liver fibrosis action, which was superior to that of BJJP.
CONCLUSIONIn the genetic and developing processes of liver fibrosis in rats, GZT could protect the liver cells, inhibit the synthesis and reduce the content of collagens, the mechanism might be related with its action in antagonizing peroxidation injury, indicating that GZT could effectively prevent liver fibrosis formation.
