Pharmacometabonomics, as an emerging branch of system biology, has been increasingly used in personalized medicine and showed broad prospects. By means of metabonomics, the complicated and detailed metabolic profile of the patient is described, thus providing more detailed description of the disease phenotype. With this understanding, response of different individuals to the drugs are predicted or evaluated through inherent genetic information of the individual combined with the environmental factors. As a result, appropriate drugs and dosage are chosen, which greatly promotes the realization of the individualized therapy goals. This article describes the emerging field of pharmacometabonomics, and the research results of personalized medicine based on the pharmacometabonomics in recent years are reviewed in detail.

Objective To ecaluatc the diagnostic value of digital subtraction angiography (DSA) and carotid artery ultrasound for transient ischemia attack (TIA). Methods Among the 74 patients with TIA, 45 had internal carotid artery (ICA)-TIA and 29 had vertebrobasilar artery (VBA)-TIA. DSA examination was performed in order to detect intracranial and extracranial arterial stenosis in the above two systems. Cervical artery ultrasound examination was used to understand the distribution of arterial plaques. Results DSA shoved that the detection rate of vascular stenosis in patients with ICA-TIA was 84. 4% (n = 38),and the patients with serious, moderate and slight stenoses were 31.1% (n = 14), 26. 7% (n = 12) and 11.1% (n =5), respectively. Of those, intracranial arterial stenosis was 44.4% (n =20), and it was significantly higher than 22.2%(n = 10) in extracranial arterial stenosis (P <0. 001 ); the detection rate of vascular stenosis in patients with VBA-TIA was 65.5% (n = 19), and the patients with serious, moderate and slight stcnoscs were 17. 2% (n = 5), 27. 5% (n = 8), and 20. 7% (n = 6), respcctivegly. Of those, cxtracranial arterial stenosis was 44.8% (n = 13), and it was significantly higher than 13.8% (n = 4) in intracranial stenosis (P < 0. 001 ). Carotid artery ultrasound shoved that the detection rate of ICA plaque was 44.4% (n = 20), and it was higher than 24, 1% (n =7) in patients with VBA-TIA; the detection rate of the plaques in the initial segment of subclavian artery in patients with VBA-TIA was 44. 8% (n = 13), and it was significantly higher than 13.3% (n = 6) in patients with ICA-TIA (P < 0.001 ). Conclusions There were differences between the intracranial and extracranial vascular lesions and the distribution of atherosclerotic plaques in patients with ICA-TIA and VBA-TIA. 1he former was more common in intracranial lesions, and the latter was more common in extracranial lesions.

Chinese Journal of Ultrasonography 1993;0(02):-
ObjectiveTo observe the rabbit cardiac siz e and cardiac function change before and after bone-marrow stem transplanting(BMST) by echocardiography.MethodsThirty-four rabbits were divided into 3 groups randomly: group control, group myocardial infarction(MI) and group BMST. The left anterior coronary artery in group MI and group BMST were ligated to produce the myocardial infarction model and injected bone-marrow stem cell to the infracted area in the later group. The size and the systolic function of the heart were measured before and after infarction and transplantation. The left ventricular systolic pressure (LVSP) and the end-diastolic pressure(LVEDP) were also measured in the 3 groups at the end of experiment. ResultsThe left ventricle diameter of group BMST was smaller than that of the group MI, and the ventricular function increased, and almost reached the normal level. And more, LVSP increased and LVEDP decreased statistically in the group BMST. ConclusionsBone-marrow stem transplantation can release the enlargement of left ventricle and improve cardiac function after myocardial infarction.

Objective To investigate the clinical characteristics of skin lesions of tsutsugamushi disease. Methods The skin lesions and clinical characteristics of 17 hospitalized patients with tsutsugamushi disease were analyzed. Results Of the patients, 88.2% (15/17) developed eschar and skin ulcer, 64.7% (11/17) had skin rashes. The patients often developed solitary, painless and nonpruritic eschar 4 to 10 days after the occurrence of fever, with indefinite locations and average diameter of 1.2 cm (range: 0.5 - 3.0 cm). The eschar was covered with a black and dry surface and surrounded by a ring-shaped erythematous halo, and usually shedded 5 to 9 days after the appearance with the formation of shallow ulcer. Superficial lymphadenectasis was observed near the eschar in 80% of the patients with eschar, and no eschar was observed at the first visit in 66.7% of the patients. Skin rashes mainly included nonpruritic congestive papules (45.5%) and maculopapular rashes (54.5%), and often subsided 2 to 5 days after emergence. Conclusions The skin lesions of tsutsugamushi disease include eschar and skin rashes. Eschar is an important characteristic and highly suggestive of this entity.

Objective To detect viable myocardium in patients with old myocardial infarction(OMI) by using quantitative tissue velocity imaging(QTVI) in low dose dobutamine stress test(LDDSE).Methods(Twenty-five) patients with OMI were accepted QTVI examination including 2-chamber,4-chamber apical view.QTVI indices included peak velocity in isovolumic contraction(V_(IVC)),peak velocity in systolic(V_S) and time of isovolumic contraction(T_(IVC)).Results Two hundred segments were divided into the normal(group N,72 segments),viable myocardium(group V,77 segments) and non-viable myocardium(group NV,51 segments). At baseline, compared with group N,V_(IVC),V_S of group V and group NV decreased significantly(P

Objective To investigate the surgical efficacy for the treatment of ruptured anterior communicating artery aneurysms with different dome projections in acute phase via supraorbital lateral approach. Methods From January 2014 to March 2017,the clinical data of 79 patients with acute ruptured anterior communicating artery aneurysm with different dome projections in acute phase treated via supraorbital lateral approach in the First Affiliated Hospital of Soochow University were retrospectively analyzed. The operative efficacy was analyzed. The Glasgow outcome scale (GOS) score was used to evaluate the prognosis at 3-36 months after procedure. Results In the 79 patients,34 were superior dome projections,37 was inferior dome projections, and 8 were complicated dome projections. All the anterior communicating artery aneurysms were successfully clipped. The patients were followed up for 3-36 months after procedure.GOS showed good prognosis in 70 cases (88.6%),of which 26 were superior dome projections,36 were inferior dome projections, and 8 were complicated dome projections;9 suffered mild disability (11.4%),of which 8 were superior dome projections and 1 was inferior dome projection. There were no severe disability, persistent vegetative state, and death. The prognosis in patients with inferior dome projection was better than that of superior dome projection (97.3% [36/37] vs.76.5% [26/34]).The difference was statistically significant (χ2=5.19,P<0.05).Conclusions The lateral supraorbital approach is an effective option for treating ruptured ACoAA with different dome projections in acute phase. The prognosis in patients with inferior dome projection is better than that of superior dome projection.

Aim To construct and express the fused gene of anti-glioma single chain Fv(39ScFv)/ human tumor necrosis factor α (TNF-α ). Methods The mature peptide cDNA of TNF-α was fused to 3′ -terminus of ScFv gene. The 39ScFv-TNFα fusion gene was cloned into expression vector pET20b(+ ) and induced to express in E.coli. Results The fusion protein expressed in E.coli account for 20% of the total bacterial protein. It displayed a single band of Mr 44 000 by reducing SDS-PAGE and Western blot, and retained both of its anti-glioma immunoreactivity and biological activity of TNF. Conclusion The fusion gene was constructed and expressed successfully in E.coli. The fusion protein possessed bifunctional activities, it may prove useful in targeting immunotherapy against glioma.
