1.Recent advances in congenital cataract surgery

Congenital cataract is a common eye disease which leads to children low vision. Surgery is the main treatment. Because of children's preoperative evaluation, operation, postoperative management is more difficult than adult, the outcome of surgery is not so well. Therefore, we review the relevant congenital cataract surgery researches in recent years in order to discuss the problems and further developments in this area.
2.The relationship of adolescents' family satisfaction scale and internet addiction disorder of college students

Objective:To explore the relationship of adolescents' family satisfaction scale and internet addiction disorder(IAD)of college students.Methods:256 college students were assessed with adolescents' family satisfaction scale questionnaire(AFSS)and internet addiction impairment index.Results:(1)the prevalencerate of IAD among college students was 12.1%.(2)the differences in the scores of family circumstance,the way of families fostering the child,communication and understanding of families between IAD and normal groups were significant.(3)the IAD of college students were correlated with the scores of family circumstance,the way of families fostering the child,communication and understanding of families.Conclusion:The prevalencerate of IAD among college students was serious.The IAD of college students are closely related to family satisfaction scale.As a result,prevention intervention and cure of lAD need to be carried out in family aspects.
3.Application of delta-shaped anastomosis in totally laparoscopic distal gastrectomy

Objective To observe the technical feasibility and safety of delta -shaped (DS)anastomosis in totally laparoscopic distal gastrectomy (TLDG).Methods A retrospective analysis of 9 cases underwent TLDG with D2 lymphadenectomy and DS anastomosis.Results All the patients underwent TLDG with D2 lymphadenectomy and DS anastomosis successfully by the same doctor and his team.The total operative time was (178 ±43)minutes,and the DS anastomosis time was (32 ±8)minutes.All the patients achieved microscopic cancer -free margin and the number of lymph nodes harvested was (27 ±8)per patient.The average time to flatus,time to fluid diet were (3.8 ± 1.7)days and (3.9 ±1.8)days respectivly.One patient developed to fat liquefaction of incision,and one developed to urinary tract infection after the operation.No anastomotic bleeding,anastomotic leakage,anastomotic stenosis or other anastomotic complications happened among all patients.Conclusion DS anastomosis is safe and convenient.It is an ideal choice for reconstruction in TLDG with D2 lymphadenectomy.
4.Prevention, Diagnosis and Treatment of Biliary Tract Complications after Liver Transplantation

Objective To investigate the causes of biliary tract complications after liver transplantation, and to put forward effective measures of prevention, diagnosis and treatment. Methods The literatures of recent years were reviewed and summarized. Results The causes of biliary tract complications after liver transplantation are very complex, and there are no standard preventive measures. Treatment differs according to causes. Conclusion One of the most important causes leading to biliary complications is preservative and ischemic injury. Poorly operative techniques and blood supply to biliary tract are also disastrous. Improving T tube placement can reduce the incidence of biliary complications related with T tube. To prevent biliary complications, it is crucial to completely wash the biliary tract, avoid damaging the blood supply to donor biliary tract and manage perfect biliary mucosa-to-mucosa anastomosis without tension. T tube cholangiography combined with noninvasive MRCP enables accurate depiction of the biliary tree and diagnosis of biliary complications. Doppler ultrosonography should be routinely applied postoperatively. Timely application of interventional radiological technique is a valuable nonoperative procedure for treatment of biliary complications. Meanwhile, biliary sludge or cholestasis and mixed infections of biliary tract should be handled actively and properly.
5.Cost-effectiveness Analysis of Three Drugs in the Treatment of Chronic Hepatitis B

Objective:To compare the cost-effectiveness ratio among lamivudin(LMV),adefovir dipivoxil(AFD) and entecavir(ETC)in the treatment of chronic hepatitis B to find the best scheme.Method:1026 patients with chronic hepatitis B from 9 reports were given LMV(359 patients),SFD(345 patients)and ETC(322 patients)for 48 weeks.Then a cost-effectiveness analysis was carried out.Result:The cost for LMV was 5 231.52 yuan,the cost for AFD was 5006.4 yuan,and the cost for ETC was 12 559.68 yuan.The therapeutic effects were 42.90,51.59 and 76.09,respectively.The cost-effectiveness analyses of LMV,AFD and ETC were 121.95,97.04 and 165.06,respectively.Conclusion:AFD AFD is the best scheme in the treatment of chronic hepatitis B.
6.Effect of Reduning Injection combined with spleen aminopeptide oral solution on children with hand-foot-and-mouth disease and its regulative effect on myocardial enzyme and inflammatory level

Objective To investigate the effect of Reduning Injection (RI) combined with spleen aminopeptide oral solution on children with hand-foot-and-mouth disease and its regulative effect on myocardial enzyme and inflammatory level.Methods Totally 184 cases treated in Children's Hospital of Kaifeng City from September,2014 to September,2016 were randomly divided into observation group and control group with each group of 92 cases.Control group was given conventional therapy,on the basis of which,observation group was given RI combined with spleen aminopeptide oral solution.Clinical effect and levels of inflammatory cytokine and myocardial enzyme were compared between the two groups.Results The total effective rate in observation group was 93.48%,which was significantly higher than that in control group (80.43%) (P < 0.05).After therapy,the levels ofhs-cTnT,CK-MB,CK,TNF-α,IL-6,CRP,CD3+,CD4+,CD8+,IgA,IgM,and IgG in observation group showed no statistical difference compared with healthy group.Index mentioned above of control group were significantly improved,however there were still statistical difference compared with healthy group (P < 0.05).Conclusion RI combined with spleen aminopeptide oral solution could effectively decrease the levels of inflammatory cytokine as well as myocardial enzyme,which deserves clinical expansion.
7.On the Roles of Nurses Engaged in Community Health Service

In the implementation of community health service,the working efficiency of the community nurses becomes increasingly important to the overall service standards of the service centers as the local residents have begun to take it into consideration when evaluating the service quality.By illustrating the roles of nurses in all the six major functions played by the community health service,this paper intends to arouse the attention from the concerning authorities to the training of the nurses so that the nurses can be developed in an all-round way and provide better services for the community.

10.Clinical characterisis and prognosis analysis of 107 adult onset Still's disease

Objective To analyze the clinical characteristics, laboratory results, treatment and prognosis of adult onset Still's disease (AOSD). Methods The clinical and laboratory data of 107 AOSD patients were retrospectively analyzed. SPSS 22.0 was used for statistical analysis. Categorical data were described as frequency and percentage, and numerical data were described using x±s deviation. Chi-square test was used to compare categorical data between groups and analysis of variance test was used to compare numerical data between groups. K-W test was used to compare skewed distributed data between groups and J-T test was compared in pairwise comparison. Logistic regression was applied to examine prognostic factors. Results The average age of 107 patients (12 male and 95 female) was(39±16) years and the average disease course was (1.4±2.7) years. The patients were classified into 3 types based on their disease course: monocyclic (38 cases, 35.5%), polycyclic (29 cases, 27.1%) and chronic arthritis (40 cases, 37.4%). The clinical characteristics were high fever (107 cases , 100%), rash (97 cases, 90.7%) and arthralgia/arthritis (86 cases, 80.4%). The most common involved joints were knee (24 cases, 23.3%), wrist (19 cases, 18.4%) and proximal interphalangeal joints (14, 13.7%). The laboratory results were elevated including white blood cells count (88/105, 83.8%) and elevated serum ferritin (107, 100%). Seventy-three out of 84 patients (86.9%) had negative rheumatic factor and 9 out of 91 (9.9%) had positive ANA, which were low titre in general. Eleven patients (10.3%) had joint erosions suggested by X-ray at diagnosis. Intergroup analysis revealed that patients with polycyclic type were less likely to have abnormal liver function tests at disease onset (χ2=9, P<0.05). Twenty-five patients (23.8%) in total had complications, which were mainly pulmonary interstitial fibrosis (9 cases ) and hemophagocytic syndrome (7 cases). Male patients (n=6, χ2=5.93, P=0.015) were more likely to have complications. One hundred and two (95.3%) patients were treated with glucocorticoid and among them, 86 (84.3%) were combined with immunosuppressive agents. After 4 weeks of treatment, 89 patients (89.2%) achieved complete remission. The number of patients with glucocorticoid dependence or treatment failure was less in monocyclic group, although the differences were not significant. The analysis of prognostic factors founded that patients with Tmax>39.5 ℃ (P=0.030) were mostly fell into the monocyclic group. Compared with monocyclic type, patients with polycyclic or chronic arthritis type were less likely to have thrombocytosis (P=0.019, P=0.004). However, when compared with chronic arthritis type, patients with monocyclic or poly - cyclic type were less likely to have arthralgia/arthritis (P=0.030, P=0.000), especially the involvement of large joints (P<0.05). Conclusion AOSD mainly presents as high fever, rash, arthralgia and elevated inflammatory markers. Joint erosion may occur during the course of disease. The treatment of glucocorticoid and immunosup - pressive agent combinations is effective and most patients could achieve remission during 4 weeks of treatment. Patients with thrombocytosis at disease onset might be monocyclic type, while patients with arthritis, especially involvement of large joints at disease onset might be chronic arthritis type.
