5.Clinical consensus and progress of prophylactic cranial irradiation
Journal of International Oncology 2017;44(3):217-220
Prophylactic cranial irradiation is an effective means of dealing with brain metastasis of cancer, which can reduce the incidence of brain metastasis by killing the micrometastasis and then improve the prognosis. The effectiveness has been verified in small cell lung cancer, but the application is still in the exploration in non-small cell lung cancer (NSCLC) and breast cancer.Prophylactic cranial irradiation is expected to become the conventional treatment for patients with NSCLC of ⅢA-N2 stage after operation and adjuvant chemotherapy or breast cancer of epithelial growth factor receptor-positive after receiving trastuzumab.
6.Allogeneicversus heterogeneous bone marrow mesenchymal stem cells for laryngeal cartilage repair
Chinese Journal of Tissue Engineering Research 2016;20(7):966-971
BACKGROUND:Because chondrocytes have no regeneration ability, to select suitable seed cels is the primary problem to repair cartilage defects.
OBJECTIVE:To investigate the effect of alogeneicversus heterologous bone marrow mesenchymal stem cels (BMSCs) in repairing laryngeal cartilage defects after chondrogenic induction.
METHODS:BMSCs from human and rabbits were isolated and cultured. Passage 3 cels were cultured in chondrogenic induction medium containing transforming transforming growth factor beta 1 and bone morphogenetic protein, and then were dropped onto a poly(lactic-co-glycolic acid) (PLGA) scaffold. Thirty New Zealand rabbits were randomly assigned into three groups: blank control group, human BMSCs group, rabbit BMSCs group. Animal models of laryngeal cartilage defects were made in the three groups. After modeling, saline-soaked PLGA scaffold, PLAG scaffold with human BMSCs or with rabbit BMSCs were implanted respectively into the rabbits in the normal blank, human BMSCs and rabbit BMSCs groups. The expression of type II colagen in the larynx and its surrounding tissues was detected by immunohistochemistry at 4 and 8 weeks postoperatively.
RESULTS AND CONCLUSION: The animals in each group breathed normaly with no presence of wheezing, and their eating and activity were good. Moreover, there was no purulency or infection in the three groups. At 4 and 8 weeks after operation, the positive rates of type II colagen in the two BMSCs groups were significantly higher than that in the blank control group (P < 0.05). There was no significant difference between two BMSCs groups (P > 0.05). These results show that both alogeneic and heterologous BMSCs have good therapeutic effects on the repair of laryngeal cartilage defects in rabbits.
7.Clinical studies of adult epilepsy patients with attention-deficit hyperactivity disorder
Chinese Journal of Neurology 2015;48(12):1052-1056
Objective To investigate the incidence of adult epilepsy patients with attention-deficit hyperactivity disorder (ADHD) and the characteristics of ADHD subtypes, and to explore the correlations between several epilepsy-related factors with ADHD.Methods According to corresponding inclusion criteria and exclusion criteria, 156 adult epilepsy patients who visited to Neurology Clinic of Zhejiang Hospital and the First Affiliated Hospital of Anhui Medical University from March 2014 to October 2014 were enrolled as a study group to enter this test.Eighty-six nomal adults were enrolled as the control group who matched for the age, gender, inteligence quotient with the study group.Using the chi-square test and Logistic regression analysis and the other statistical methods to analyze the phenomenon of ADHD in adults with epilepsy and the factors that may contribute to the phenomenon. Results The detection rate of ADHD in 156 adult patients with epilepsy was 26.9% (42/156), significantly higher than that in the heathly control group which was 4.7% (4/86, x2 =17.862, P < 0.05).Symptomatic epilepsy and the electroencephalogram showed multifocal discharge had higher detection rate of comorbid ADHD and the difference was statistically significant (The detection rate of idiophathic epilepsy was 22.1% (21/95), symptomatic epilepsy was 46.7% (14/30), cryptogenic epilepsy was 22.6% (7/31), x2 =7.362, P =0.025.The detection rate of frontal epileptiform discharges was 30.6% (11/36), temporal area was 25.9% (14/54), central area was 18.2% (4/22), multifocal epileptiform discharges was 46.2% (12/26), the normal was 1/18, x2 =10.187, P =0.037).The results of binary Logistic regression analysis showed that epilepsy age of onset (B =-0.889, P =0.049) was possibly an important factor for epilepsy with ADHD.Conclusions The risk of ADHD occurred in adults with epilepsy is higher than that in the healthy adults and attention-deficit is the common subtype. The etiology, age of onset and electroencephalogram features of adults with epilepsy may be associated with the occurrence of ADHD.
8.Clinical study of high-dose glucocorticoid on early active rheumatoid arthritis
Chinese Journal of Postgraduates of Medicine 2014;37(24):1-4
Objective To investigate the effects and adverse reactions of high-dose glucocorticoid on early active rheumatoid arthritis patients. Methods The early active rheumatoid arthritis patients were randomly divided into three groups:high dose group (16 patients, with large doses of glucocorticoid therapy),low dose group (17 patients, with small doses of glucocorticoid therapy),and control group (41 patients, with no glucocorticoid therapy).Compared the differences among the three groups. Results The changes in the three groups were different in joint pain, joint swelling, morning stiffness, erythrocyte sedimentation rate, rheumatoid factor and C-reactive protein (P < 0.05).The high dose group was significantly better than the other two groups in the above variables alleviating,in the 1 week after the treatment. The changes in the three groups had no significant difference of bone mineral density,Larsen hip score (P> 0.05).The NSAIDs days in the control group was significantly longer than the high dose group, low dose group [(51.17 ± 2.56) d vs.(14.16 ± 0.71) d,(26.50 ± 1.59) d],the difference was statistically significant (P < 0.05).Rate of weight gain in the high dose group, low dose group was significantly higher than the control group [5/16,4/17 vs.4.9% (2/41)],the difference was statistically significant (P < 0.05).Body weight increased in the high dose group and low dose group occurred mainly in the following 1.5 months of therapy, with the discontinuation of cortices-teroids,weight gradually fell to normal.Gastrointestinal discomfort incidence in the control group was significandy higher than that in the high dose group, low dose group [51.2% (21/41) vs.2/16,3/17],the difference was statistically significant (P< 0.05).Conclusions High-dose glucocorticoid can rapidly relieve clinical symptoms and inflammation in early active rheumatoid arthritis patients.High-dose corticosteroids do not bring too many side effects to patients but weight gain, and it reduces the patients' gastrointestinal discomfort greatly, because of the decrease use of NSAIDs.
