1.Strategies to improve the surgical treatment of portal hypertension

The surgical treatment for portal hypertension (PHT) aims to control and prevent the gastroesophageal variceal bleeding.The choices of surgical timing and procedures are dependent on the liver reserve function.Except for Child-Pugh classification and model for end-stage liver disease scoring system,the future liver remrant and pre-albumin are the important evaluation indexes,meanwhile,the choice of surgical procedures would be dependent on portal hemodynamics that can reduce incidence of rebleeding of postoperative hepatic encephalopathy.Hepatic venous pressure gradient is the most important objective index forecasting bleeding risk and severity of PHT.
2.Verification of Microbial Limit Test Methods for Three Kinds of Preparations with Heavy Metals

Objective To establish the methods of microbial limit test for three kinds of preparations with heavy metals, such asDingxian Pills,Pizhi Lotion andJiawei Huangqin Ointment.Methods According to Chinese Pharmacopoeia 2010, the recovery rates of bacteria, fungus and yeast treated by three preparations were detected. And the methods of testing control bacteria were also validated.Results Culture medium dilution method was proved to be applicable forDingxian Pills. Culture medium dilution method combined with pre-filtration method was proper for Pizhi Lotion. And the extraction method was adopted forJiawei Huangqin Ointment. The recovery rates of these five validation strains reached 70% by appropriate methods. And the same methods were used for validation of the control bacteria.Conclusion The methods of microbial limit test for these three different preparations were established through this study.
3.Methods for identification of citrullinated proteins in tissue cells

Citrullination is a post-translational modification of arginine caused by peptidyl arginine deiminase in the presence of calcium ions.Up-regulation of citrullination plays a significant role in the progress of numerous disorders,and it has important physiologic and pathological significance.High-throughput and quantitative methods with high specificity and sensitivity are still needed to identify the citrullinated sites in tissue cells.With the development of chemical derivatization and mass spectrometry, new rapid and accurate proteomics technology still has a good prospect of development and application.This article aimed to provide new ideas and directions to identify the citrullinated sites in the complex biological samples by introducing strategies available to identify citrullinated peptides, such as color development reagent analysis, immunodetection and mass spectrometry.
4.Perspectives on Clinical Pharmacists' Participating in Consultation on the Use of Antibiotics in 227 Cases

OBJECTIVE:To discuss the approach and role of clinical pharmacists' participating in clinical consultation. METHODS: The consultation data of clinical pharmacists' participating in clinical consultation from Feb. 2004 to Dec. 2007 including patients' basic condition,reasons for consultation,pathogens,past medication history,whether the consultative suggestions were adopted or not,feedback of patient's condition etc were analyzed. RESULTS: Clinical pharmacists participated in clinical consultation for 7 cases on the use of antibiotics in 2004,up to 123 cases in 2007. Of which,the patients from internal medicine department accounted for no less than 42.8%;the ratio of male to female was about 2:1. Objective To analyze the clinical characteristics of acinetobacter baumannii infection and drug resistance tendency of our hospital in 2014 ,so as to promote the clinical rational use of antibiotics .Methods The study was a retrospective review ,the re‐sults of clinical distribution and drug resistance of acinetobacter baumannii isolated from our hospital in 2014 were analyzed .The an‐timicrobial susceptibility testing(AST) of acinetobacter baumannii was determined by K‐B disk diffusion method and minimal inhib‐itory concentration(MIC) test ,respectively .The AST was performed as recommended by CLSI 2010 .Results A total of 299 strains of acinetobacter baumannii were isolated from clinical specimens throughout the year .Of the 299 Acinetobacter baumannii isolates , 268 strains(89 .63% ) were isolated from sputum ,165 strains(55 .18% ) of Acinetobacter baumannii were from intensive care unit (ICU) and 52 strains(17 .39% ) were from neurosurgery .The resistance rate of Acinetobacter baumannii to cefoperazone/sulbactam was 5 .02% while its resistance to Imipenem and Meropenem significantly increased to 52 .17% and 56 .86% ,respectively .And the resistance rates ofβ‐lactams ,fluoroquinolones and aminoglycosides were higher than 60% .Conclusion The isolation rate of Acine‐tobacter baumannii is increasing in recent year in our hospital ,as well the resistance rate to the common Antibiotics .Monitoring the resistance of Acinetobacter baumannii should be strengthened for preventing resistant bacteria from spreading .

Triple negative breast cancer (TNBC) is a special subgroup of breast cancer with more aggressive biological behavior and clinicopathological characteristics,and the therapy of TNBC has always been the research difficulty and hot spot.Currently,surgery and adjuvant radiotherapy are still the main method in local treatment,amd the research of traditional chemotherapy and targeted therapy have been made some progress.Meanwhile,new drugs continuously appear in recent years.
7.Transabdominal gastric fundusectomy plus distal esophagectomy for the treatment of portal hypertension

ObjectiveTo evaluate the effect of transabdominal gastric fundusectomy plus distal esophagectomy in patients suffering from portal hypertension. MethodsEighteen portal hypertension patients with a history of bleeding esophageal varix were treated by this procedure on elective basis.ResultsAll cases were followed up for an average of 46 months. There was no recurrent hemorrhage and no mortality during the follow-up.ConclusionsThis procedure is well tolerated by the patients with definite immediate effect and low recurrent bleeding rate.
8.Data dimensionality reduction for hospital efficiency measurement

A comparison was made for the correlation and application scope of the statistical methods commonly used by hospitals for their efficiency measurement.Hospital data processed with PCA (principal component analysis)for dimension reduction were used in a correlation analysis for the results of ratio analysis (RA),stochastic frontier analysis(SFA)and data envelopment analysis(DEA).The authors hold that the RA can expediently display the order of hospital efficiency,the SFA demands a stricter premise yet presents more stable results,while the DEA boasts greater relative advantages and thus suitable for processing hospital efficiency measurement tasks of multi-input and multi-output indexes.
9.Temporoparietooccipital and parietooccipital disconnection in patients with intractable epilepsy

Objective: To assess the surgical techniques and surgical outcomes of temporoparietooccipital and
parietooccipital disconnection. Methods: The authors conducted a retrospective review of clinical,
neuropsychological, EEG, imaging data in 16 patients with intractable epilepsy who underwent
temporoparietooccipital and parietooccipital disconnection between April 2008 and October 2011.
Of the 16 cases, 12 were males and 4 were females. The age of seizure onset was from 0.1 to 27
years (average 6.6) and disease duration of 0.1 to 18 years (average 7.5). The surgery was performed
between the age of 3 and 37 years (average 14.1). Nine patients underwent temporoparietooccipital
disconnection, 5 patients parietooccipital disconnection and 2 patient parietooccipital disconnection
and temporal lobotomy. Results: After a mean follow-up of 1.6 years (range 0.5–3.9 years), 13 patients
(81%) were seizure free, 1 patient had Engel Class II seizure outcome and 2 patient had Engel class
III outcome. Two patients exhibited severe brain swelling and one patients had second resective
operation with good recovery. None of the patients developed new motor defi cits postoperatively and
there was no mortality.
Conclusion: Temporoparietooccipital and parietooccipital is a safety and effective epilepsy surgery
procedure for patients with epileptic zone localization to the posterior quadrant on one side. The results
of surgical disconnection for posterior quadrantic epilepsy have yielded excellent seizure outcomes
in 81% of the patients, with no mortality or major morbidity.
10.Determination of serum prealbumin and C-reactive protein in 68 burn patients and its clinical significance

Objective To investigate the change of serum prealbumin (PA) and C-reactive protein (CRP) in burn patients and its clinical significance.Methods Sixty eight cases of burn patients were divided into two groups,group Ⅰ [n=38,with total burn surface area less than 15% of total body surface area (TBSA)] and groupⅡ(n=30,with total burn surface area more than 50% of TBSA). Thirty healthy people were set as healthy control group. Particle enhanced immunoturbidimetic assay was applied to detecting the serum levels of PA and CRP for all subjects on the day 1 after burns.Results The CRP level was much higher and serum level of PA was much lower in group Ⅱ than that of group Ⅰ,respectively. There existed significant negative correlation between PA and CRP level.Conclusion The level of serum PA decreases significantly along with total burn surface area expansion. PA can be used as a sensitive marker for estimation of inflammation degree after burns.
