1.Diagnosis and treatment of traumatic chyloperitoneum

Objective:To investigate the proper method for treating traumatic chyloperitoneum. Methods: A retrospective analysis was performed on clinical data of 4 cases of traumatic chyloperi- toneum admitted to our hospital in recent 26 years. Results: All 4 cases were cured after conserva- tive treatment. Conclusion: Perdominantly integrated therapy including absolute diet,total parrenter- al nutrition(TPN) and abdominal cavity drainage can be the effective method for the treatment of trau- matic chyloperitoneum.
2.Biological effects of nonthermal plasma combined with X-ray irradiation on human hepatocellular carcinoma in nude mice

Objective To investigate the biological effects of nonthermal plasma (NTP) combined with X-ray irradiation on human hepatocellular carcinoma in nude mice.Methods By subcutaneous inoculation, tumor models of human hepatocellular carcinoma HepG2 in nude mice were established, and these mice were divided into control group, NTP treatment group, X-ray irradiation group, combined treatment group.The changes of tumor microenvironment were observed using hematoxylin-eosin (HE) staining.The terminal deoxynucleotidyl transferase-mediated nick end labeling (TUNEL) was used to determine the level of neuronal apoptosis in tumor tissues.The expression of metal matrix proteinase-2 (MMP-2) was detected by immunohistochemical assay.Transmission electron microscope was used to observe cell structure changes.Results The final tumor volumes of control group, NTP treatment group, X-ray irradiation group, combined treatment group were (543.96±108.45), (436.54±65.49), (351.66±56.68), (281.97±35.60)mm3, with a statistically significant difference (F=9.63, P=0.01), and the difference between X-ray radiation group and the combination treatment group was significant (P=0.05).HE staining showed that there was a larger area of necrosis in the combined treatment group compared with the other groups.TUNEL showed that the apoptotic indexes were (0.95±0.13)%, (5.82±0.26)%, (7.53±0.43)%, (11.07±0.35)% respectively, with a statistically significant difference (F=547.76, P=0.00), and the difference between X-ray radiation group and the combination treatment group was statistically significant (P=0.00).The immune scores of each group were 12, 9, 9, 2.Electron microscopic observation showed that there were more apoptotic bodies in the combined treatment group than those in the other groups, accompanied by mitochondrial edema.Conclusion NTP and X-ray irradiation therapy in the treatment of human hepatocellular carcinoma in nude mice model has a synergistic effect.Probably, it can be a new type of treatment in curing cancers.
3.Mechanical test and two variable regression analysis of cavity design

Objective: To study the relationship between the depth and width of cavity design and to analyze the tendency of breaking load in preparations. Methods:Plaster models of mandibular first molar were prepared and magnified by 150%. Cavity on occlusal surface was designed and prepared in a cylinder shape with the depth(mm) of 3.0,4.5 and 6.0,and diameter(mm) of 3.0,4.5 and 6.0,respectively. 8 cavity samples were prepared for each design. The breaking load of the tooth models before and after restoration with self-curing resin was measured by Instron tester.Mechanical model static load test and two variable regression analysis were applied to study the mechanical property of the cavity design. Results:Before restoration, with the increase of depth and diameter, the incidence rate of tooth fracture increased. The width was the main factor. After restoration, the load was undertaken by restoration directly, and could be transfered to tooth and periodontal tissue through restoration uniformly. When the diameter of the cavity increased, the diameter of restoration also increased, and thus decreased the load on local structure of the tooth. With two-regression analysis, the regression equation and regression curve were obtained. Conclusion: The fracture of tooth before and after restoration is related to the diameter and depth of cavity design in cylinder shape. The regression equations of changing tendency of breaking load (Y) in preparation caused by the change of depth(X 1) and diameter(X 2) of cavity obtained by two-regression analysis are presented as follows: Before restoration: Y=1593.317-51.178 X 1-79.489X 2.After restoration:Y=1802.928 -192.461X 1+225.128X 2.
4.Knowledge management and technological innovation in hospitals

Knowledge management, as a new managerial concept, is the specific embodiment of knowledge based economy in the field of management while technological innovation, constituting the core of technological progress, is the effective extension and application of scientific and technological knowledge. Hospitals, which are knowledge based institutions, possess good foundations and advantages of implementing knowledge management and technological innovation. There is a close link between knowledge management and technological innovation in hospitals, the former being the basis and primary guarantee of the latter and the latter being the result and manifestation of the former. Hospital managers should take measures to promote the sound interaction between the two so as to enhance the core competency of their hospitals.
5.Practice Research on Hygiene Status of Anchorage Molar Wearing Band and Buccal.

Inspect the anchorage molars with band and the anchorage molars with buccal tube with dental plaque index and gum in- dex and then statistically process the objective data obtained.The results show that both wearing band and wearing buccal tube lead to dental plaque.The influence is less worse on the the anchorage molars with buccal tube.
6.Effects of propofol on myocardial NT-κB and INOS during myocardial ischemia-reperfusion in isolated rat hearts

Objective To investigate the effects of propofol on myocardial nuclear factor kappaB(NF-κB)and inducible nitric oxide synthase (iNOS) during myocardial ischemia-reperfusion (I/R) in isolated rat hearts.Methods Twenty-four adult SD rats of both sexes weighing 200-300 g were randomly divided into 3 groups (n=8 each):group Ⅰ control (C) ; group Ⅱ I/R and group Ⅲ propofol + I/R (P).The animals were anesthetized with intrxperitoneal urethane 1.0 g/kg and their hearts were excised.The aorta was cannulated and the hearts were retrogradely pedused with oxygenated K-H solution in a Langandorff apparatus for 20 min.In group C the hearts were continuously perfused with K-H solution for 110 min.In I/R group after 20 min stabilization the hearts were made globally ischemic for 30 min followed by 60 rain reperfusion.In group P the hearts were peffused with K-H solution containing 50 μmol/L propofol for 20 min,the hearts were then made globally ischemic for 30 min followed by 60 min repedusinn with K-H solution containing 50 μmol/L propofol.Cardiac troponinI (cTnI) concentration in coronary effluent was measured at the end of 20 min stabilization before myocardial isobemia (baseline) and at 10 min(T1) and 60 min (T2) of reperfusion.Myocardial specimen was obtained frem left ventricle at 60 min of repeffusion for determination of MDA content,SOD and iNOS activity,the expression of NF-κB and IκB (by immuno-histochemistry).Results The cTnI concentration in coronary effluent was significantly higher at T1 and T2 in group I/R and at T2 in group P than in group C.The NF-κB expression was significantly higher while IκB expression was lower in I/R and P group than in group C.iNOS activity was higher in I/R group than in control group but there was no significant difference in iNOS between group P and C.Propofol administered before and after myocardial ischemia significantly attenuated I/R-induced changes listed above.Conclusion Propofol can attenuate myocardial ischemia-reperfusien injury through reducing the NF-κB activation and suppressing the iNOS activity.
7.Etiological detective analysis in female urinary tract infection

Objective To discuss the disposition feature of pathogenic bacterium in female urinary tract infection(UTI) ,so as to elevate the level of clinical diagnosis and therapy. Methods To cultivate medistream urine,assess vaginal secretions in the aspects of bacteriology, mycoplasma, chlamydia, mycetes and parasite. All specimen collected from 129 female patients who chiefly complained irritation of urinary tract ,from January 2003 to December 2006. Results Pathogenic microorganisms that found in the 129 female patients with UTI are gram-negative bacteria( 53.49 % ), gram- positive bacteria ( 19.38 % ), mycoplasma ( 14.73 % ), mycetes (9.30 % ), chlamydia (4.65 % ),parasite(1.55% ). Among them, escherichia coli, klebsiella pneumoniae bacilli, enterococcus faecalis, staphylococcus aureus are common species. Sexually transmitted disease(27.91% ) include the infection of diplococcus gonorrhoeae,mycoplasma and ehoamydiae. Among them,non-gonococcal urethritis is common. Combined infection(17.38 % ). Infection combined with mycoplasma urealytium and other pathogen is the most, these patients are most young or middle aged. Conclusion Pathogen in female UTI is mostly gram-positive bacteria. STD and combined infection should be paid attention to by clinicians. We suggest female UTI patients take etiological test regularly,so as to elevate the level of clinical diagnosis and therapy.
8.Advances in epidemiological studies of vitamin D deficiency

Factors that may influence vitamin D nutritional status include latitude,season,age,sex,race,skin color,clothing,cultural practices,dietary nutrition,and living conditions.This article reviews the effects of these factors on vitamin D status and recommends that any high-risk people of vitamin D deficiency should take vitamin D supplementation.
10.Clinical analysis of 72 cases with lower respiratory tract acinetobacterial infection

Objective To explore the clinical features,risk factors and the sensitivity to antibiotics of lower respiratory tract acinetobacterial infection.Methods The clinical data,results of sputum bacterial culture and drug sensitivity of the 72 cases with lower respiratory tract acinetobacterial infection were netrospectively reviewed.Results 53 cases(73.6%)were elder than 60 years old,69 cases(95.8%)had basic diseases,and 59 cases(81.9%)were nosocomial infection.Risk factors included basic diseases,lower immune function,all kinds of invasive operations,aerosol inhalation,unreasonable usage of antibiotics.Drug sensitivity test showed that the drugs which sensitive to acinetobacter was upper than 50% included Imipenen,Levofloxacin,Cefooerazone/sulbactum and Amikacin.Conclusion The patients who sufferred from lower respiratory tract acinetobacterial infection usually are elder than 60 years old,and have basic diseases.Most of this kind of infection is nosocomial infection.Because of severe clinical symptom and high drug resistant rate,the prognosis is poor.Reasonable selection of sensitive antibiotics is important.
