1.Effects of atorvastatin on treatment of patients with non-alcoholic fatty liver disease

Objective To evaluate the efficacy and safety of atorvastatin in treatment of patients with non-alcoholic fatty liver disease(NAFLD).Methods Of 72 patients with NAFLD6,8 patients with elevated alanine transaminase(ALT)and cholesterol levels at baseline had completed the study.The patients were randomly divided into treatment trial group(n=38)and control group(n=30,ithout any treatment).The patients in trial group were orally received 10 or 20 mg of atorvastatin daily according to basal serum cholesterol levels being≤or>6.5 mmol/L,respectively.The parameters including body mass index,serum cholesterol level and ALT level were tested and liver density was assessed by echography at baseline and 6 months after treatment.Results There was a significant difference in syndrome scores between baseline and 6 months after treatment[(11.05±1.29)vs.(6.08±0.87)]in trial group(t=1.96,P<0.05).Six months after treatment,11 patients in trial group were presented with normal ALT levels.whereas only 2 patients in control group had normal ALT levels (P<0.05).There was a significant differenee in cholesterol level between baseline and 6 months after treatment[(6.6±0.54)ramol/L vs.(5.4±0.5)mmol/L]in trial group(t=1.72,P<0.05).In addition,there was no differenee in body mass index between baseline and 6 months after treatment both in rial group[(26.8±2.9)kg/m~2 vs.(26.4±2.8)kg/m~2]and control group[(26.5±2.3)kg/m~2 vs.(26.4±2.2)kg/m2~](P>0.05).There was no significant difference in liver density of two groups before and after treatment(P>0.05).No side effect was reported.Conclusions Serum ALT and cholesterol levels are reduced significantly in all patients treated with atorvastatin.It is effective and safe for atorvastatin in treating NAFLD patients with hypercholesterolemia.
2.Genotoxicity of Liquid Formaldehyde on Brain Cells of Goldfishs

Objective To explore theDNA damage of brain cells of goldfishs through formaldehyde exposure.Methods The goldfishs were treated with liquid formaldehyde at different concentrations(0.01 mmol/L,0.1 mmol/L和1mmol/L)for three days.Single cell gelelectrophoresis technique(comet assay)was used to test the DNA damage of the brain cells.Results Formaldehyde caused significant DNA strand break at the concentration of 0.01mmol/L,0.1mmol/L and 1.0mmol/L.Conclusions Formaldehyde was both a DNA strand breaker when the concentration is higher,it may cause crosslinkagent to the brain in vivo.
3.A preliminary study of three-dimensional ultrasound images automatic registration based on hepatic vessel

Objective To evaluate the feasibility and accuracy of three-dimensional ultrasound (3DUS) automatic image fusion based on hepatic vessel in clinical application.Methods Forty pairs of 3D ultrasound volumetric images from 10 healthy volunteers were acquired and enrolled in the study,including 10 pairs of each following lobe of the liver:left lateral lobe,left medial lobe,right anterior lobe and right posterior lobe.3DUS automatic registrations were performed based on hepatic vessel.The technical successful rate and accuracy of the image fusion using the 3DUS data from four hepatic lobes were observed and compared.Results Thirty-six pairs of volumetric data were successfully fused together by automatic registration.The technical successful rate was 90% (36/40) and the registration error distance was 1.48 mm (0.98-2.76 mm).There were no statistical differences in the successful rate for the four different hepatic lobes (P =0.891),but the registration error of right anterior lobe was less than left lateral lobe of liver (P =0.014).Conclusions The new technology of 3DUS automatic registration based on hepatic vessel was feasible and reliable,and it has a wide prospect in future clinical application.
4.Planning of the hepatocellular carcinoma's ablation by three-dimensional ultrasound:a preliminary study

Objective To discusse the feasibility and application value of the computer-assisted liver cancer ablation planning based on the three dimensional ultrasound.Methods Forty three-dimensional ultrasound images of 39 patients with 40 tumors'maximum diameter between 21 to 70 mm were collected and then acquired image segmentation and visualzation.The computer-assisted liver cancer ablation planning based on three dimensional ultrasound was comparied with the artificial ablation planning based on two dimensional ultrasound to find out the differences in the success rate,damage rate,time-consuming and the number of insertions between these two methods.Results Compared with the artificial ablation planning based on two dimensional ultrasound,the computer-assisted liver cancer ablation planning had a higher success rate(92.31% vs 53.85%,P =0.000),lower damage rate(7.50% vs 25.00%,P =0.034),shorter time-consuming(44.0 s vs 263.0 s,P =0.000)and less insersion times(3 vs 4,P =0.009).Conclusions The computer-assisted liver cancer ablation planning based on three dimensional ultrasound is more efficient and safety than the traditional way.
5.Fingerprints of Different Ethanol Extracts of Tripterygium Wilfordii Hook.f.and Spectrum-activity Relationship in Rat Mesangial Cells

Objective:To investigate the proliferation of rat glomerular mesangial cells (GMC) influenced by different ethanol extracts of Tripterygium wilfordii HooK F.(TWHF).Methods:An HPLC method was used to establish the fingerprints of 5 different ethanol extracts of TWHF,and GMC was chosen to study the effects of different ethanol extracts of TWHF on cell proliferation.After statistical analysis,the spectrum-activity relationship was analyzed by using partial least squares regression(PLSR).Results:The HPLC fingerprints of the 5 different ethanol extracts of TWHF were established,and 32 characteristic peaks were characterized by the HPLC fingerprints.60%,70% and 95% ethanol extracts and glycosides tablets showed dose-effect relationship,and with the increase of dose,the more significant inhibition of cell proliferation was exhibited.The absorbance values of the 60% ethanol extracts at medium and high doses were lower than those of the other extracts at the same dose.The proliferation inhibition rate of GMC was used as the potency index and analyzed by PLSR,and 20 peaks were potency peaks at high dose(40 μg·L-1),17 ones were potency peaks at medium dose(20 μg·L-1) and 15 ones were potency peaks at low dose(10 μg·L-1).Conclusion:Part of the potency peaks has regular dose-effect relationship with the changes of dose.
6.Study on the Purification Technology of 4 Active Components from Tripterygium wilfordii by Macroporous Resin

OBJECTIVE:To study the purification technology of 4 active components from Tripterygium wilfordii by macropo-rous resin. METHODS:The purification abilities of nine macroporous resins(ADS-5,ADS-8,HPD100,HPD300,HPD400, HPD450,HPD700,HPD722 and HPD750) were studied with the adsorption and desorption rates of triptolide,wilforlide,trip-tonide and tripterine as the index by static adsorption and desorption experiments for 4 active components from T. wilfordii,so as to screen optimal macroporous resins. Using transfer rate of active component as index,single factor test was used to investigate the effects of different sampling method,ratio of mixing sample to total resin quantity,ratio of resin to medicinal material,cleaning so-lution(type,amount and cleaning flow rate),diameter- height ratio of resins,eluant(volume,flow rate)on adsorption,so as to determine the optimal elution technology;validation test was also conducted. RESULTS:HPD722 macroporous resin was chosen as adsorption resin;ratio of diameter to height was 1∶10,resin-medicinal material ratio was 1∶2,and ratio of mixing sample to to-tal resin quantity was 1∶10;wet column installing and mixing resin for sample loading were adopted. The macroporous resin was washed with 12 BV 20%ethanol at the rate of 12 BV/h,and then eluted with 12 BV 95%ethanol at the rate of 6 BV/h. The verifi-cation test results showed that the total transfer rate of 4 active components from T. wilfordii was more than 90%(RSD=0.99%, n=3). CONCLUSIONS:The optimized technology is stable and feasible,and suitable for the purification of 4 active components from T. wilfordii.
7.The clinical significance of the injury and functional change of hypothalamic-pituitary-adrenal axis after acute severely traumatic brain injury in the rats

Objective To study the clinical significance of the injury and functional change of hypothalamic-pituitary-adrenal (HPA) axis after acute severe traumatic brain injury (TBI) in the rats.Methods A total of 60 adult healthy male Spraque-Dawley rats were randomly (random number) divided into 3 groups (n =20 in each group):sham operation group,model group and treatment group.The TBI models of rats were established by Feeney' s method.A low dose of dexamethasone (0.6 mg/kg) was injected into the abdominal cavity 20 minutes,24 hours and 48 hours after injury in treatment group,while rats of sham operation group and model group received equal volume of normal saline instead.All the rats were injected 1 μg adrenocorticotropic hormone (ACTH) into the abdominal cavity.The related parameters were detected at four time points,3 hours,12 hours,24 hours and 72 hours after cerebral contusion.The plasma corticosterone (CORT) and ACTH levels were measured by chemiluminescence.The hypothalamic,pituitary and adrenal of the rats were taken out for observing interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) expression detecting by immunohistochemical techniques at 72nd hour after TBI.One-way ANOVA and SNK-q test were used to analyze the results with SPSS 17.0 software package.Results The levels of ACTH and CORT on 3rd hour of model group raised remarkably compared with that of sham operation group,then they reduced gradually.The levels of CORT were lower than that of sham operation group at every time points after ACTH stimulation test (P <0.05 or P <0.01).The levels of CORT at all time points of treatment group were changed remarkably compared with that of model group.However,the ACTH levels of treatment group on 24 h increased slightly than that of model group.And the tendency of them was similar to model group (P < 0.05 or P < 0.01).The number of the hypothalamus and pituitary cells which express IL-6 and TNF-α in model group was more significantly increased when compared with that in sham operation group (P < 0.01),while the number of this kind of cell in treatment group was significantly decreased than that in model group (P < 0.01).The number of the adrenal cortex cells which express IL-6 in treatment group was more significantly decreased when compared with that in model group (P< 0.01),while the number of this kind of cell in model group was significantly increased than that in sham operation group (P < 0.01).However,there was no significant difference of the TNF-α between all the groups (P > 0.05).Conclusions Functional change of adrenal occurs early in the severe acute traumatic brain injury rats,and the response of adrenal to ACTH decreased as time goes by.Low-dose,short-course dexamethasone can delay the pathological changes,reduce the inflammatory response of HPA axis and increase the sensitivity of adrenal response to ACTH.
8.Screening of early prediction ultrasonic index for prognosis of severe hepatitis and its value evaluation

Objective To select early prediction indexes related to the prognosis of severe hepatitis in ultrasonography,and evaluate its clinical value.Methods Clinical data of 269 cases of severe hepatitis patients was analyzed retrospectively.Cox proportional hazards regression model was adopted to analyze 13 possible prediction ultrasonic parameters for severe hepatitis,including thickness of the right hepatic lobe,portal vein (PV) diameter,volume of ascites,etc.After selected from these parameters,the clinical value of early prediction factor was evaluated with ROC curve.Results Multivariate analysis showed that thickness of the right hepatic lobe,portal vein (PV) diameter,opening of paraumbilical vein and bile viscosity were related to the death of severe hepatitis patients (P < 0.05).The thickness of the right hepatic lobe contributed most (Wald =40.479),its sensitivity and specificity for prediction were 93.4%,82.8% respectively(P < 0.01).Conclusions Thickness of the right hepatic lobe,PV diameter,opening paraumbilical vein and bile viscosity are early prediction factors for severe hepatitis,need to be monitored in treatment,especially for thickness of the right hepatic lobe.
9.Expression of fas and P53 protein in resected non-small cell lung cancer and its prognostic significance.

BACKGROUNDTo study the expressions and its clinical significances of fas and P53 protein in human non-small cell lung cancer (NSCLC) after complete surgical resection.
METHODSImmunohistochemical stain of fas and P53 was performed on paraffin-embedded sections from 269 NSCLC patients who underwent surgery and were followed up for 1.1 to 122.2 (median, 48.4) months postoperatively. Differences in survival rates and clinical characteristics were evaluated by SPSS10.0 statistical software packet.
RESULTSThe rate of fas and P53 expression in NSCLC cancer tissue was 43.1% and 49.4% respectively. Fas expressions were seen more frequently in female patients (59.3% vs 39.1%, P < 0.01). Univariate analysis showed that fas expression was a good factor for predicting prognosis. The 5-year survival rate of the patients whose tumors had positive fas expression was significantly better than those individuals whose tumors had negative fas expression (51.4% vs 42.4%, P=0.02), especially in patients in stage I and IIIA . The expression of P53 had no significant influence on the prognosis of these 269 NSCLC patients. Combined analysis of fas and P53 expression in NSCLC cancer tissues showed significant prognostic influence (P=0.01). The 5-year survival were 40.1% (fas+ and P53-), 45.4% (both positive or both negative) and 57.2% (fas- and P53+), respectively. COX multivariate analysis showed that reduced fas expression in 269 NSCLC is an independent risk factor, especially in stage IIIA NSCLC.
CONCLUSIONSFas and P53 protein are indicators of NSCLC biological behavior. Reduced fas expression in NSCLC is an independent risk factor for early stage patients. Analysis of apoptosis related proteins expression in tumors might be helpful to predict the prognosis of patients with NSCLC.
10.Relationships between islet β-cell function and infection,inflammation and major organ function in multiple organ dysfunction syndrome patients with severe traumatic hemorrhage

Objective To analyse the relationships between islet β-cell function and infection,inflammation and major organ function in multiple organ dysfunction syndrome (MODS) patients with severe traumatic hemorrhage.Methods A total of 187 cases of MODS patients hospitalized in the 94th Hospital of PLA from January 2013 to January 2016 were selected,and were divided into the MODS survival group (MODS-S group,104 cases) and MODS dead group (MODS-D group).Other 100 healthy subjects were selected as the control group.The fasting blood glucose (GLU0) and insulin (INS0) levels,blood glucose (GLU30) and insulin (INS30) levels after 30 min of glucose loading,and levels of soluble triggering receptor expressedon myeloid cells-1 (sTREM-1),tumor necrosis factor-α (TNF-a),interleukin-6 (IL-6),alanine aminotransferase (ALT),creatinine (Cre) and creatine kinase isoenzyme (CK-MB) in different groups were determined.The insulin-β-cell function was evaluated by homeostasis model assessment of β-cell function (HOMA-β) index and ratio of insulin increment and blood glucose increment after 30 min of glucose loading (ΔINS30/ΔGLU30),and their relationships to other indexes,including sTREM-1,TNF-α,IL-6,GLU0,ALT,Cre and CK-MB,in MODS patients with severe traumatic hemorrhage were analysed.Results The HOMA-β and AINS30/AGLU30 ratio in the MODS-D group were lower than those in the MODS-S group,and levels of sTREM-1,TNF-α,IL-6,ALT,Cre and CK-MB in the MODS-D group were higher than those in the MODS-S group,there were statistically significant differences (P<0.01).In MODS patients with severe traumatic hemorrhage,HOMA-β and ΔINS30/AGLU30 was both negatively correlated with sTREM-1,TNF-α,IL-6,GLU0,ALT,CreandCK-MB (r=-0.356 4,-0.532 1,-0.345 8,-0.772 1,-0.762 5,-0.684 8,-0.606 4;r=-0.428 5,-0.567 8,-0.487 0,-0.743 6,-0.781 7,-0.717 6,-0.640 1,P<0.01).Conclusion MODS patients with severe traumatic hemorrhage have islet β-cell dysfunction which may be used as a prognostic and diagnostic indicator.
