1.Clinical characteristics and surgical repair of ethmo-maxillary bone buttress in orbital blowout fractures
Chinese Journal of Experimental Ophthalmology 2021;39(5):469-472
Ethmo-maxillary bone buttress (EMBB) is a special complex bone structure in the interface between the internal wall and inferior wall of the orbit, which plays a vital role in maintaining the stability of the orbit, ethmoidal sinus and maxillary sinus.Although the incidence of compound fractures in orbital blowout fractures is lower than that of single orbital wall fractures, they have more obvious clinical symptoms such as enophthalmia and binocular diplopia. The mechanism of EMBB may be more supportive of the orbital floor compression theory, that is, the external force will lead to torn periostum and bone seam separation at the ethmoidomaxillary suture, and the orbital contents will form a funnel-like hernia due to extrusion and gravity.Or the extrusion results in the fracture and displacement of the orbital lamina of ethmoid bone, the upper wall of the maxilla and the ethmoidomaxillary plate, causing a large area of orbital wall collapse and obvious expansion of the orbital cavity.Recently, the study of orbital internal wall and inferior wall joint fracture has become a new hotspot, but research on the orbital blowout fracture involving ethmo-maxillary bone buttress is still in its infancy. The current orbital fracture repair technique can only restore the connection between the two orbital walls, and there is no surgical plan can overall recover EMBB's scaffolds role. Therefore, how to restore the scaffolds role of EMBB, and to regain the part of ostiomeatal complex in normal sinus drainage is the challenge we need to conquer. In this article, the anatomy and clinical characteristics of the supporting structure, fracture mechanism, and the research progress and prospect of surgical repair technique were reviewed.

Objective To analyze the clinical results of modified N-butyl 2-cyanoacrylate skin adhesive in the application of facial trauma. Methods A total of 687 patients had been analyzed retrospectively from August 2009 to July 2014,and all the wounds of these patients had not obtained the anatomical repositioning after pure manual reduction with tissue glue or pure stitches.Improved tissue adhesive method was as follows:point coating, decomposition and glue with stitches. Results Anatomical repositioning was obtained in 685 wounds of 687 cases with improved tissue adhesive method.Infection occurred in the wounds of 2 cases (0.29%),but no other complications occurred in all patients.During postoperative 3 months of follow-up,clinical results were satisfactory for all patients. Conclusions The improved method of tissue glue has fewer postoperative complications, beautiful and more satisfatory appearance, economic, safer and reliable in the application of facial trauma.

Objective To evaluate the clinical effect of ultrasound - guided sclerosing agent lauromacrogol injection in treating lymph leakage. Methods A total of 31 patients with postoperative lymph leakage were selected for this study. Of the 31 patients, successful conservative oppression treatment was accomplished in 16, and lauromacrogol injection had to be carried out in 15 as conservative oppression treatment failed. The patients were followed up and the results were analyzed. Results In 15 patients receiving lauromacrogol injection treatment, complete cure of lymph leak was obtained in 14 with a success rate of 93.33%. Among the 14 cases, the second lauromacrogol injection was employed in 3 at one week after the first injection. Infection occurred in another case one day after the injection , which was cured after dressing change for 15 days. Conclusion For the treatment of lymph leakage, ultrasound-guided sclerosing agent lauromacrogol injection is effective and safe.

Objective To explore the correlation between up-regulation of cyclooxygenase-2(COX-2) (expression) and hepatocellular carcinoma(HCC) angiogenesis. Methods The expression of COX-2,vascular endothelial growth factor(VEGF)、basic fibroblast growth factor(bFGF) and angiopoientin-2(Ang-2) were examined in eighty matched sets of HCC specimens using immunohistochemistry and reverse (transcription-polymerase) chain reaction(RT-PCR).Results In HCC,the expression rate of COX-2,VEGF,bFGF and Ang-2 was 75.0%,62.5%,60.0% and 61.25%,respectively.(Immunohistochemical) staining scores of VEGF、bFGF and Ang-2 were 5.98?1.16,4.57?0.26 and(5.87)?0.12,respectively in strongly postive group of COX-2;and were 3.30?0.22,2.61?0.16 and 2.63?0.13,respectively in moderately weak postive group of COX-2.The expression rates of VEGF,(bFGF) and Ang-2 were 100.0%(95/95),94.29%(33/35) and 97.14%(34/35),respectively in strongly postive group of COX-2;and were 60.0%(15/25),60.0%(15/25) and 60.0%(15/25),respectively in moderately weak postive group of COX-2.There was significant difference in HCC(angiogenesis) between the two groups(P

Objective:To observe the clinical efficiency, duration of action and adverse reactions of lip contour after local injection of type A botulinum toxin into the orbicularis oculi muscle. Methods:From March 2019 to June 2019, a total of 11 patients (6 males and 5 females, mean age 42.2 years) received superficial injection of botulinum toxin A in the orbicular muscle in our hospital. The change of lip thickness was assessed by Medicis lip fullness scale (MLFS) and the 3D structured light camera was used to collect and analyze the facial data. Pre- and post-treatment effects, duration of action and complications were evaluated. Results:Improvement of upper lip thickness was noted in 11 cases (100%). The average onset time after injection was 14.6 days and average effect maintenance time was 124.1 days. The height of the red lips ( Z=-2.940, P<0.05), the distance from the upper lip to the Ricketts line ( Z=-2.137, P<0.05) and nasolabial angle ( Z=-2.137, P<0.05) were significantly changed one month after the injection. No allergic reaction was observed, and the adverse reactions were mild and reversible. Conclusion:Local injection of botulinum toxin type A can achieve mild lip augmentation.
