1.Relationship between serum concentrations of interleukin-6 (IL-6), soluble IL-6 receptor and the activity of Graves′ ophthalmopathy

Serum concentrations of interleukin-6 (IL-6), soluble IL-6 receptor (sIL-6R) were determined in patients with Graves′ ophthalmopathy (GO), Graves′disease without GO (NGO) and controls. Patients with GO treated by corciosteroids were assessed by the clinical activity score (CAS). The results suggested that serum levels of both IL-6, sIL-6R and CAS could reflect the activity of GO and predict the outcome of corcicosteroid therapy.
2.The relationship between hyperhomocyseinemia and atherosclerosis in hemodialysis patients

Objective To determine the relationship between hyperhomocyseinemia and atherosclerosis in hemodialysis patients.Methods Plasma total homocysteine (tHcy)concentrations were measured by using fluorescence polarization immunoassay.Carotid initial-medial thickness(IMT)was measured by echocardiography.Cardiovascular parameters including carotid Young elastic index and the prevalence of atherosclerosis plaques were calculated by established formulas.Results The prevalence of hyperhomocyseinemia was 68.3% in the hemodialysis patients.The folate and vitamin B_(12) concentrations were higher in patients than those in controls.Cartid IMT,carotid Young elastic index and the prevalence of atherosclerosis plaques were positively and independently correlated with tHcy (r=0.34,0.31,0.23).Multivariate stepwise regression indicated that the correlation also existed independently of other conventional risk factors such as age,sex,blood pressure,Scr,glucose.Conclusion The results indicate that hyperhomocyseinemia may be an independent risk factor for atherosclerosis found in hemodialysis patients.
3.Characteristics and establishment of ischemic tolerance rat models with hypoxic preconditioning

BACKGROUND: We can investigate mechanism of endogenous neuroprotection in rat cerebral hypoxic tolerance trial. OBJECTIVE: To observe the characteristics of cerebral hypoxic tolerance in rat models with cerebral hypoxic preconditioning. DESIGN: Randomized controlled observation. SETTING: Department of Neurology, China-Japanese Friendship Hospital, Jilin University. MATERIALS: The experiment was performed in the Basic Animal Experimental Center, China-Japan Friendship Hospital, Jilin University from April 2003 to April 2004. Inbred line healthy Wistar rats, of either sex, with the body mass of 200-300 g, were randomly assigned into normal control group (n=6), sham operation group (n=6), ischemic control group (n=20), hypoxic preconditioning (3 hours, 8% O2 and 92% N2) plus ischemic group (n=60) (according to different hypoxic phases, there were 5 time phases: 30 minutes, 1, 3, 5 and 6 hours with 12 rats in each time phase), hypoxic preconditioning group (n=18) [according to different hypoxic phases, there were 3 time phases: 1, 3 and 5 hours with 6 rats in each time phase, 3 rats received TTC staining and 3 rats received hematoxylin and eosin (HE) staining]. METHODS: ①Hypoxic preconditioning: Firstly, natrica calx was put into closed glass container to absorb CO2 and O2, secondly, mixed gas of 8% O2 and 92% N2 was input, and then animals were put into the container, 3 rats each time. Temperature and humidity were kept steadily. ②Permanent ischemic middle cerebral artery rat models were established. ③The models were determined with a series in procedures: neurological score, infarcted volume evaluation, pathological sample preparation, immunohistochemical staining, imaging analysis and so on. ④The data were compared in groups with variance analysis.MAIN OUTCOME MEASURES: Changes in cerebral infarcted volume, neurological score and pathological morphology in rats of experimental group and control group. RESULTS: Neurological score in the hypoxic preconditioning (8% O2, at hours 1, 3 and 5) plus ischemic group was lower than in the ischemic control group(P<0.01). Neurological score at minute 30 and hour 6 after hypoxia (8% O2) had insignificant difference in the ischemic control group. Mean cerebral infarcted volume ratio in the hypoxic preconditioning (8% O2, at hours 1, 3 and 5) plus ischemic group was lower than in the ischemic control group(P<0.01). Mean cerebral infarcted volume ratio after hypoxia (8% O2, at minute 30 and hour 6) had insignificant difference with ischemic control group (P>0.05). CONCLUSION: Hypoxic preconditioning in rats can effectively release nerve injury induced by focal cerebral ischemia, suggesting that it has protective effect on brain. The procedure of establishing cerebral ischemic tolerance models with hypoxic preconditioning, which is simple and stable, with little injury on experimental animals, is a useful tool for studying cerebral ischemic tolerance.
4.The experimental study of the microbial contamination after the puncture needle downward and ex-posed in the air of infusion bottle stopper

Objective To study the relationship between the exposure time of puncture needle of infusion bottle stopper and microbial contamination during clinical intravenous transfusion. Methods A total of 600 cases from November 1, 2014 to January 31, 2015 who have received the clinical intravenous transfusion for investigation were selected.When replacing the infusion bottle (bag), inserting the puncture needle slowly across the bottle stopper and making the needle tip be canted to the transfusion bottle mouth (bag) of the rubber plug, gently squeezing the Murphy's tube until solution was not dripping, recording down the exposure time in the air of the needle tip from medicine droplet to the end. To dip the lower part with sterile swabs and culture the swabs in nutrient broth medium. Meanwhile, to replace the next bottle of medicine and get the remaining 2 ml of liquids into the culture broth medium, after 48 h, both of which medium were switched to blood plate culture cultivation for observing the general situation of the bacteria growth. Results Among the 600 cases of clinical transfusion, 24 cases were positive for sterile swabs microorganisms culture, positive rate was 4.0%, among which microorganisms, 15 cases were gram-positive coccus, 3 cases were gram-negative bacillus, 3 cases were gram-positive bacillus and 3 cases were fungi. Correspondingly, 3 cases were positive for liquid broth culture, positive rate was 0.5%as the gram-positive coccus. The exposure time and broth microbial culture result was statistically significant, while the exposure time and medicinal broth microorganisms culture result possesses had no statistical significance. Conclusions Inserting the puncture needle across the bottle stopper could successfully reduce the liquid drug residues in the infusion bottle (bag), however, which might also cause time-dependent microbial contamination during the exposure process in the air.
5.The expression and its clinical significance of microRNA-301 in pancreatic carcinoma

Objective To study the expression of microRNA-301 in pancreatic carcinoma andvalidate the significance of miR-301 in invasion and metastasis of pancreatic carcinoma.Methods miR-301 expression were detected by FQ-PCR in 5 pancreatic cancer eell lines(PANC-1,PaCa-2,AsPC-1,Hs766T.BxPC-3).Further immunohistochemistry in pancreatic cancer tissue microarrays was detected miR-301 expression,which contained 60 pancreatic cancer specimens along with 10 normal adjacent tissues and 10 normal pancreas tissues.After high expression of miR-301 in pancreatic carcinoma being confirmed.the clinical significance of high expression of miR-301 in invasion and metastasis of pancreatic carcinoma were studed.Pancreatic cancer cell lines(PANC-1.PaCa-2)were transfected by 100 nmoml/L miR-301 inhibitor(anti-miR-301)or negative eontrol(Anti-miR~(TM) Negative Control#1).COX-2 and MMP-2 protein expression in pancreatic cancer cell lines were detected by WB.and cell migration assays were performed using transwell technology.Results FQ-PCR resuhs indicated that miR-301 expression was higher in pancreatic cancer cell lines than normal pancreatic cells.The relative level of miR-301 in 5 pancreatic cancer cell lines(PANC-1,PaCa-2,AsPC-1,Hs-766T,BxPC-3)and normal pancreatic cell were 33.09± 4.21,30.76±3.18,47.57±3.56,20.20 ±1.21,76.75±13.51 and 1.00±0.08 respectively.The miR-301 level in all 5 pancreatic cancer cells were significantly higher than those of normal pancreatic cell(t=8.86,9.53,6.39,6.77,11.18,P<0.01).Immunohistochemistry results also showed miR-301 expression was higher in pancreatic carcinoma tissues than those in the cancer adjacent tissues and normal pancreatic tissues.The relative levels of miR-301 in pancreatic carcinoma tissues.normal adjacent tissues and normal pancreas tissues were 0.88±0.09,0.22±0.04 and 0.14±0.05 respectively.The miR-301 levels in pancreatic carcinoma tissues were significantly higher than those of normal adjacent tissues and normal pancreatic tissues(t=15.1,10.6,P<0.01).There was no significant difference between normal adjacent tissues and normal pancreas tissues(t=1.32,P=0.22).After miR-301 inhibitor was introduced into pancreatic cancer cells PANC-1 and PaCa-2.miR-301 levels were reduced while the protein levels of COX-2 and MMP-2.which were invasion and metastasis related factors,were down-regulated.The cell migration assay indicated the numbers of PANC-1 and PaCa-2 cells,which migrated to lower chamber.were 587±27 and 363±13 respectively after miR-301 inhibitor was applied.The numbers of migrated cells were 1091 4-15.737±44 when the netative control was applied.The cell invasion ability was decreased significantly in the inhibitor group compared with the negative group(t=7.89,7.56,P<0.01).Conclusions miR-301 is highly expressed in pancreatic cancer cell lines and pancreatic cancer tissues.Inhibition of miR-301 expression can effectively supress the invasion of pancreatic cancer cells.miR-301 may serve as a new biomarker for early detection of pancreatic cancer and molecular target for early treatment of pancreatic cancer.
6.Clinical application of neotype controllability artificial nephrohydrosis in percutaneous nephrolithotomy

Objective To explore clinical application of neotype controllability artificial nephrohydrosis in percutaneous nephrolithoto-my. Methods 400 patients with renal lithiasis or superior segment ureter lithiasis in our hospital were divided into group A and group B. 200 cases of group A received neotype controllability artifiicial nephrohydrosis;200 cases of group B received conventional artificial nephro-hydrosis. The puncture duration, amount of bleeding and times of puncture of the two groups were compared. Results 195 cases in Group A were developed renal tract for PCNL effectively,157 cases were developed renal tract for PCNL effectively in Group B. The puncture duration, amount of bleeding and times of puncture of group A and group B were respectively (1. 8 ± 0. 7) vs. (2. 5 ± 1. 2) min,(112. 7 ± 51. 0) vs. (270. 2 ± 89. 3) mL,(1. 1 ± 0. 2) vs. (1. 8 ± 0. 7) times. The differences were significant. Conclusion Neotype controllability artificial nephrohydrosis can develop renal tract for PCNL effectively.
7.Impact of positive end-expiratory pressure on systemic hemodynamics in patients with central respiratory failure

Objective To evaluate the impact of positive end-expiratory pressure (PEEP) on hemodynamic variables including central venous pressure (CVP),mean arterial pressure (MAP) and heart rate (HR) in patients with central respiratory failure treated by mechanical ventilation.Methods Thirty two neurosurgical patients with central respiratory failure,male 19,female 13,mean age (58.8 ± 13.9)years,GCS ≤ 8 points,were enrolled in this prospective,self-control study between June 1,2009,and May 31,2011.Patients were excluded in cases of severe cardiopulmonary disorders,pericardial diseases,person machine confrontation,administration of vasoactive drugs,and hypervolemia or hypovolemia.On admission to neurosurgical intensive care unit,all patients were mechanically ventilated in the mode of synchronized intermittent mandatory ventilation.Hemodynamic effects of six randomly set levels of end-expiratory pressures (0,3,6,9,12,15 cm H2O,every 10 min,1cm H2O =0.098 kPa) were studied in all patients.CVP,MAP and HR were recorded at each of the six end-expiratory pressure levels.One-way analysis of variance and simple linear regression model were used for data analysis.Results The levels of central venous pressure were elevated with increase in end-expiratory pressures.CVP levels were positively correlated with the levels of PEEP (R =0.468,P =0.000),with a simple linear regression equation expressed as:CVP (cm H2O) =7.870 +0.344 ×PEEP (cm H2O),The levels of MAP showed no statistically significant changes at different PEEP levels (F =1.390,P =0.227).No linear correlation between MAP,HR and PEEP levels was found (R =0.042 and 0.160,P =0.413 and 0.002).Conclusions CVP values would be overestimated during mechanical ventilation at different PEEP levels in mechanically ventilated patients due to central respiratory failure,positive correlation existed between CVP values and PEEP levels,whereas MAP was unaffected by different PEEP levels.This study could probably offer a quantitative reference for correct assessment of such a hemodynamic variable as CVP for mechanically ventilated patients without discontinuance of PEEP.Further studies are needed to determine whether these findings could be confirmed in a prospective manner.

Yet during the long histroy of development, the management system of production, quality standard, and registration have been gradually establised in Japan. Research and development pattern and quality control management system for Chinese prescription medicine are formed and are different from those in China. Many of them are different with our current situation, especially in the aspect of quality management. Thus their experience is worth learning and using for reference.
9.Feasibility of induction with sevoflurane-midazolam-remifentanil for tracheal intubation without muscle relaxants in neck brake patients

Objective To evaluate the feasibility of induction with sevoflurane-midazolam-remifentail for tracheal intubation without muscle relaxants in neck brake patients.Methods Forty ASA Ⅰ or Ⅱ patients with cervical spine fracture with dislocation,aged 13-68 yr,scheduled for surgery under general anesthesia,were enrolled in this study.Anesthesia was induced with iv injection of midazolam 0.03 mg/kg and inhalation of 5%sevoflurane through a mask.Sevoflurane was inhaled at the initial concentration of 5%,followed by decrement of 1% every 30 s until 3%.When the eyelash reflex disappeared,remifentanil 2 μg/kg was injected slowly over 45s and 30 s later sevoflurane inhalation was stopped.The patients were mechanically ventilated after tracheal intubation.The time of disappearance of eyelash reflex was recorded.The intubation condition was evaluated using VibyMogensen score.Results All patients were successfully intubated at the first attempt.The time period from sevoflurane inhalation to disappearance of eyslash reflex was(69 ± 4)s.Coughing occurred in 3 cases during intubation.The satisfactory intubation conditions were found in 100% of cases.SpO2 > 95% in all patients.BlS was maintained at 45-55 during the period(before intubation until 3 min after intubation).Conclusion Induction with sevoflurane-midazolam-remifentail is rapid and smooth,provides good conditions for intubation and can be applied to tracheal intubation without muscle relaxants in neck brake patients.
10.Studies on chemical constituents in the seeds of Nigella glandulifera.

OBJECTIVETo investigate the chemical constituents in the seeds of Nigella glandulifera.
METHODThe chemical constituents were isolated and repeatedly purified on silica gel column. They were identified and structurally elucidated by means of physio-chemical constants and spectral analysis.
RESULTNine compounds were identified as nigellamose (I), alpha-hederin (II), 3-O-[beta-D-xylopyranosyl-(1-->3)-alpha-L-rhamnopyranosyl-(1-->2)-alpha-L-arabinpyranosyl]-hederagenin (III), sucrose (IV), stearic acid (V), 1-O-hexadecanolenin (VI), beta-sitosterol (VII), daucosterol (VIII) and p-hydroxybenzoic acid (IX).
CONCLUSIONI, II, III, VI, VIII and IX were obtained from the plant for the first time, and I is a new compound.
