1.Grosmomoside Ⅰ, a new cucurbitane triterpenoid glycoside from fruits of Momordica grosvenori

Objective To carry out a systematic study on the chemical constituents in the fruits of Momordica grosvenori. Methods To isolate pure compounds by using repeated column chromatography, while the structure of a new compound was determined by detailed spectral analysis. Results Four cucurbitane triterpenoid glycosides, mogroside Ⅱ_E(Ⅰ), mogroside Ⅲ(Ⅱ), grosmomoside Ⅰ (Ⅲ), and mogroside Ⅴ (Ⅳ) were isolated from the 50% ethanolic extract of the fruits of M. grosvenori. Conclusion Grosmomoside Ⅰ is a new compound identified as mogrol-3-O-?-D-glucopyranoside-24-O-{[?-D-glucopyranosyl(2-1)]-[?-D-glucopyranosyl (6-1)]-?-D-galactopyranoside} and the other three compounds are known compounds.
2.Analysis on monitoring results of antibody levels after 4-vaccine inoculation among healthy children in Guangshui City during 2013

Objective To understand the antibody levels after the inoculation of measles ,poliomyelitis ,epidemic encephalitis B and hepatitis B vaccines among healthy children aged 7 - < 13 years old in Guangshui City of Hubei province to provide the scientif ‐ic basis for formulating the immune prevention and control strategy and risk assessment in whole city .Methods 4 616 healthy chil‐dren aged 7 - < 13 years old were sampled from the whole city .The enzyme‐linked immunosorbent assays (ELISA) was used to de‐tect the 4‐antibody levels .Results The antibody positive rates of measles ,poliomyelitis ,epidemic encephalitis B and hepatitis B a‐mong healthy children aged 7 - < 13 years old in Guangshui City were 94 .41% ,93 .07% ,93 .78% and 68 .72% respectively .The protection levels of first three kinds of antibody reached more than 85% ;the positive rates had statistical difference among 4 kinds of antibody(χ2 = 1 987 .08 ,P = 0 .000) .The antibody positive rates of epidemic encephalitis B had no statistical difference among different age periods (χ2 = 10 .141 ,P= 0 .071) ;the antibody positive rates of measles ,poliomyelitis and hepatitis B had statistical difference among different age periods(χ2 = 40 .471 ,P = 0 .000 ;χ2 = 25 .174 ,P = 0 .000 ;χ2 = 283 .641 ,P = 0 .000) .The positive rates of 4 kinds of antibody had no statistical difference between different genders (χ2 = 0 .019 ,P= 0 .889 ;χ2 = 1 .017 ,P= 0 .313 ;χ2 = 0 .018 ,P= 0 .892 ;P= 0 .639 ,P= 0 .424) ;the antibody positive rates of measles ,poliomyelitis and epidemic encephalitis B had no statistical differences among 17 villages and towns in the whole city (χ2 = 0 .099 ,P= 1 .000 ;χ2 = 0 .117 ,P= 1 .000 ;χ2 = 0 .134 , P= 1 .000) ,while the antibody positive rate of hepatitis B had statistical difference among these villages and towns (χ2 = 186 .179 , P= 0 .001) .Conclusion The antibody levels of measles ,poliomyelitis and epidemic encephalitis B reach the protection rate ,but the antibody level of hepatitis B needs to be increased .The monitoring work should be continuously strengthened and the seeking missed inoculation and re‐inoculation work should be reinforced .
3.A road to new healthy countryside:Policy design and initial effects of China's healthy rural re-construction

Healthy rural reconstruction is a widely explored intervention implemented by China Rural Health Devel-opment Project with the support of the World Bank and UK Department for International Development. By using initial pro-ject baseline data survey, mid-term evaluation and supervision, terminal evaluation of acquired qualitative and quantitative data,this paper analyzes the intervention policy design and initial effects of China's healthy rural reconstruction. With transforming the living environment in rural areas as the starting point, the project has relatively achieved good results, by using different policy measures such as improving the environment, creating a social environment conducive to villagers' health, expanding and providing high-quality health services, foster healthy behaviors and lifestyles, etc. The main results include the expansion of the project to cover 745 administrative villages, the environment of healthy villa-ges is much better than other neighboring villages, villagers' health knowledge has been greatly increased, lifestyle be-haviors of villagers have improved, the attitudes and participation rate of residents in cultural and sports activities have improved, etc. This paper suggests implementing the healthy rural reconstruction nationwide. It is very important to in-tegrate the concept of healthy village throughout the connotation of all kinds of new rural reconstruction, so the concept of health is deeply rooted in rural areas and imperceptibly change people's living behaviors and habits.
4.Influence of Dexamethasone on IL-25 and IFN-γin Bronchoalveolar Lavage Fluid of Asthmatic Mice

Objective To investigate the mechanism of therapeutic action of dexamethasone on asthmatic mice by detecting the levels of IL-25 and IFN-γ in bronchoalveolar lavage fluid (BALF). Methods Balb/c mice with SPF grade were randomly divided into normal control group, asthma group and dexamethasone group. Asthma group and dexamethasone group were sensitized and challenged with ovalbumin ( OVA) . Dexamethasone group was intraperitoneally injected with dexamethasone one hour before challenging. The mice were executed 24 hours after the last challenge, and the HE stained pathological sections of the right lung were made. Pathological sections of lung were observed. BALF in the left lung was also collected. The total white blood cell count and absolute eosinophile ( EOS) count were observed, and the percentage of EOS was calculated. The levels of IL-25 and IFN-γwere measured with ELISA, and correlation analyses were made. Results The counts of total white blood cell and EOS, and the percentage of EOS were significantly higher in the asthma group than in the normal control group and dexamethasone group (P<0. 05). No differences were found between the normal control group and dexamethasone group. The IL-25 level was higher in the asthma group than in the normal control group and dexamethasone group (P<0. 05), and its level in the dexamethasone group was also higher than that in the normal control group. The IFN-γlevel was lower in the asthma group than in the normal control group and dexamethasone group (P<0. 05), while there was no significant difference between the normal control group and dexamethasone group. IL-25 was negatively correlated with IFN-γin each group. Conclusion Part of the mechanisms of dexamethasone acting on asthma are related to its inhibition on the pulmonary inflammation and promotion on the expression of IFN-γ, and possible inhibition of IL-25 expression.
5.Gradient biocomposite with hydroxyapatite/zirconia for repair of cyno-bone defects

Objective To evaluate the bonding condition of hydroxyapatite (HA)/zirconia ( ZrO2 ) composite and bone interface and the ability of HA/ZrO2 in repair of bone defects.Methods Bone defect models were established in the lumbar vertebral body of 24 Beagle dogs and were implanted with HA/ZrO2 gradient composite (Group A ),HA/ZrO2 unilayer composite (Group B ),pure ZrO2 (Group C) and pure HA (Group D) successively.Dogs were sacrificed and lumbar vertebral specimens were harvested 6,12,16 weeks postoperatively and before the sacrifice at postoperative 6 and 12 weeks,the dogs were intramuscularly administered of quadracycline for fluorescence labeling.The interface bonding and repair of bone defects were observed through X-ray films,histomorphology and biomechanical test.Results The X-ray films displayed that the Group A achieved more formation of osteotylus and better repair of bone defects with the extension of the implantation period,followed by the Groups B and D and that the Group C had relatively worse results.Histomorphology study showed that the fluorescence labeling was enhanced gradually from 6 to 12 weeks in the Group A,with its growth from the edge of the implanted material to the inner part and its tight adhesion to the material,indicating active osteogenesis and massive bone formation.While the fluorescence labeling of the Groups B,C and D centered in the edge of implanted materials without presence in the material inner part.The mineralization rate of the four materials at 6 and 12 weeks had significant differences ( P < 0.05).Synostosis rates at 6,12 and 16 weeks were the highest in the Group A,with the rate of up to (90.26 ±3.82) % at 16 weeks (P <0.05 ).Biomechanical test showed the maximum shear strengths at 6,12,16 weeks in the Group A were (2.64±0.16) MPa,(2.95 ±0.19) MPa and (3.45 ±0.23) MPa respectively (P<0.05).Conclusion HA/ZrO2gradient biocomposite bonds well with the bone and possesses good repair ability for bone defects and hence is an ideal novel material for bone defect reconstruction.
6.Applied value of muitislice CT in selecting living donor kidneys and excision methods

Objective To evaluate the applied value of multislice CT (MSCT) in the selection of living donor kidneys and excision methods.Methods Ninety living renal donors underwent MSCT assessment.The nonenhanced,arterial,venous and excretory phase examinations were performed.Using maximum intensity projection and volume rendering techniques for vascular imaging,two blinded radiologists independently analyzed and evaluated all MSCT images.According to the CT reconstructive images,radiologists and physicians selected the left renal or the right renal donors,and chose laparoscopic or open live donor nephrectomy.Results On the 90 cases of donors,78 donors underwent nephrectomy in the left kidney.Seventy-one left kidney donors having no significant variation received the routine laparoscopic live donor nephrectomy.Seven left kidney donors on both sides had relatively obvious anatomical variations such as accessory renal artery,multi-branch renal vein and renal vein in the back of the abdominal aorta,and they were subjected to the left kidney open donor nephre.ctomy.Other 12 donors having significant variation in the left kidney were given nephrectomy in the right kidney,and all of them received hand-assisted laparoscopic live donor nephrectomy.All intraoperative records of urine collection system and renal vascular anatomy were consistent with the preoperative evaluation of MSCT,and the accuracy was 100 %.Two imaging experts in the evaluation of renal artery,renal vein and urine collection system showed good consistency.Nephrectomy was successfully performed on 90 cases of donors,and.postoperative recipients had no renal vein thrombosis and other vascular complications.Conclusion MSCT can provide accurate and valuable information for the selection of living donor kidneys and excision methods as a “one-stop” technique for the preoperative evaluation of living renal donors.
7.Analysis on the status and influencing factors of glycemic control in type 2 diabetic patients with hypertension in urban areas of Huai'an

Objective To explore the clinical features of type 2 diabetic patients with hypertension , and to analyze the influencing factors of glycemic control. Methods Using stratified cluster random sampling method, 5 communities were selected from urban areas of Huai'an in 2014. Type 2 diabetic patients managed by the communities were surveyed with questionnaire, physical and biochemical examinations. The related information and clinical features were compared between diabetic patients with and without hypertension. Logistic regression analysis was used to analyze the influencing factors of glycemic control. Results The number of well-controlled diabetic patients (HbA1c<7.0%) with and without hypertension (HbA1c<7.0%) were 419 (39.3%) and 480 (52.1%), respectively. Mean values of body mass index (BMI), diabetic duration and serum creatinine in diabetic patients with hypertension were significantly higher than those in diabetic patients without hypertension (P<0.05). The proportions of macrovascular complications and dyslipidemia in diabetic patients with hypertension were significantly higher than diabetic patients without hypertension (P<0.05). Multiple logistic regression analysis showed that high degree of education, high annual family income and high frequency of glucose monitoring were beneficial factors for glycemic control in diabetic patients with hypertension. Older age, hypertension, higher waist to hip ratio (WHR), the elevated triglyceride and serum creatinine were harmful factors for glycemic control. Conclusions The situation of glycemic control in diabetic patients with hypertension in urban areas of Huai'an is not optimistic. Therefore, community managements of risk factors such as central obesity and increased triglyceride in elder diabetic patients should be strengthened.
8.Pathway analysis of community doctors' motivation mechanism against the essential medicine system

Objective To identify the impact factors for community doctors with the essential medicine system in place,in support of mobilizing them and successful implementation of the essential medicine system in the community.Methods Based on the theory of ERG hierarchy of needs,the pathway analysis method was called into play to identify the three factors affecting community doctors,namely personal survival,interpersonal relations and self-development.Results Major factor of which is the interpersonal factor,with a score of 0.549,while the latter two factors do not directly affect the survival and development.The two however work on each other,and work on the interpersonal factor as well,with the impact coefficients of 0.320 and 0.402 respectively.Conclusion With the essential medicine system in place,the largest impact factor for community doctors'enthusiasm is the interpersonal one,involving the doctor-patient communications among others.This serving as the breakthrough point may improve the enthusiasm of the community doctors,paving the way for the system in the communities.
9.MRI manifestations of bone marrow changes after recombinant human granulocyte colony stimulating factor was subcutaneous injected for healthy adults

Objective To investigate MRI manifestations of lumbar and proximal femoral bone marrow changes before and after recombinant human granulocyte colony stimulating factor (rhG-CSF) was subcutaneous injected for healthy adults.Methods Twenty healthy blood stem cell donors without hematologic disease were enrolled in this study. All of them underwent lumbar sagittal and proximal femur coronal MRI examination with spin echo T1 WI and fat-suppressed T2WI.The first examination were performed before subcutaneous injection of rhG-CSF for comparison. In 4-7 days and 30-60 days after injection, the other two examinations were performed. The signal changes of lumbar and proximal femoral bone marrow were investigated by reading pictures and calculating the contrasted noise ratio (CNR).ResultsBefore rhG-CSF injection, all patients presented normal signal intensity of hone marrow. In 4-7 days after injection, all the 20 cases presented homogeneous signal decrease in lumbar vertebral bodys on T1 WI, accompanied by reduced fatty signal. In proximal femur, patchy or stripped hypointensity areas were found in intertrochanteric and subtrochanteric areas on T1 WI. On fat-suppressed T2 WI images, the signal of lumbar and proximal femoral bone marrow changed to equal or slightly-high signal intensity. In all cases,abnormal signal areas presented in lumbar and proximal femoral bone marrow occurred simultaneously in the same case.In the 10 cases received the third MRI during 30-60 days after rhG-CSF injection, signal intensity of lumbar bone marrow turned to normal in all sequence, but abnormal signal intensity areas were still existed and extended to distal part in femoral bone marrow, which appeared as symmetric stripped or patchy equal or slightly-low signal intensity on T1 WI and equal or slightly-high signal intensity on T2 WI. The CNR of lumbar bone marrow to subcutaneous fat before rhG-CSF injection, in 4-7 days and 30-60 days after rhG-CSF injection were 114. 11 ± 15. 11,71.04 ± 12. 25 and 91.64 ± 1 I. 68, respectively. Significant difference was found between before rhG-CSF injection and 4-7 days after injection ( P < 0. 05 ) , but no significant difference between the others( P > 0. 05 ). Conclusion After injection of rhG-CSF, the short-term changes of hematopoietic cells and fat content in bone marrow can be displayed on MRI, which provided non-invasive information for bone marrow transplantation.
10.Study of rural doctors′social mentality based on relative deprivation theory

Objective To analyze rural doctors′ social mentality with the theory of relative deprivation.Methods 642 rural doctors from 225 villages of 1 5 counties in Shandong were selected for a questionnaire survey.The factor analysis method was used to reduce the scale dimension and simplify the scale.Analysis of rural doctors′vertical and horizontal relative deprivation was made based on different reference groups.Results Rural doctors tend to identify themselves with rural teachers and doctors in township hospitals,and 60.0% identify themselves as the low income group.39.9% of them found a higher income,while 33.5% of them found a higher social status.76.3% of them found their social contributions are higher.91.4% of them hold that villagers benefit from the new healthcare reform policies,while 65.9% of them hold themselves as benefiting from such policies.This indicates a low sense of achievement.Conclusions Compared with the reference groups,rural doctors feel an obvious sense of relative deprivation comparing both horizontally and vertically.They hold themselves as underpaid and have little sense of achievement for the policies.In this consideration,the government should increase subsidies to lift pay for rural doctors,and to identify their legitimacy,in order to stabilize these medical workers.
