1.Clinical study about the effect of modified mi-type elastic bandage on hemostasis of puncture point of radial artery

Objective To compare the effect of hemostasis of puncture point of radial artery be-tween using modified mi- type elastic bandage and TR- Band tourniquet after the operation of coronary in-tervention. Methods 142 patients who were to undergo transradial coronary intervention were divided in-to the observation group (73 cases)and the control group(69 cases). The observation group applied modi-fied mi- type elastic bandage and the control group adopted TR- Band tourniquet. The effect of hemostasis, discomfort degree and incidence rate of local complication were observed in the two groups. Results The success rate of hemostasis of the observation group (98.63%) was higher than that of the control group (94.20%), but the difference between two groups was of no statistical significance. For the discomfort de-gree: the feeling of pain was higher in the control group, incidence of swelling was higher in the observa-tion group,the difference between the two groups had statistical significance,for the incidence of numb-hess,the test was of no statistical significance,for the local skin complication,the difference between two groups was of no statistical significance. Conclusions Putting modified mi- type elastic bandage into the application after the operation of coronary intervention,we find that choosing material is convenient,it is easy to observe and operate,the effect of hemostasis is safe and reliable,the rate of complication is low. So it is one safe, effective and ecnomical hemostasia method.

OBJECTIVE:To study the preparation of compound levofloxacin eye drops and its quality control,then observe its clinical effect.METHODS:Levofloxacin was used as basic remedy mixed with dexamethasone sodium phosphate and phos?phate was adoped as buffer,while EDTA-2Na as stabilizer.Finally the content of levofloxacin was determined by UV-spec?trophotometric method.RESULTS:The preparations was stable and within the range of2~6?g/ml,the concentration of lev?ofloxacin had a linear relationship with absorbance(C=26.1780A—1.0838,r=0.9997).The average rate of recovery was100.34%and RSD was0.44%(n=5).CONCLUSION:This preparation has a good effect with simple preparing technique and reliable quality control.
6.The rural hypertension and diabetes patients'perception and evaluation on the health manage-ment services

Objective:To analyze the perception and evaluation of rural patients with hypertension and diabetes on the health management services , thereby identifying the problems and proposing countermeasures .Methods:570 rural patients with hypertension and diabetes were investigated by the stratified random sampling method .Descriptive statistic method and logistic regression analysis were applied to analyze the rural patients'perception and evaluation on the health management services .Results: The majority of rural patients with chronic diseases such as hypertension and diabetes were alerted of the health management service .The satisfaction rate on the disease control effect was 82.28%, and that among patients below the age of 60 years old was lower than that of the others ( P <0.05 ) . 77 .24%of patients deemed that some services such as on-site visit , physical examination and prevention of complica-tions were deficient (the selection rates among patients were 11.90%, 10.41% and 9.29% respectively).The health management services problems mainly included deficiency of on-site visits, incomplete medical equipments and service items ( the selection rates among patients were 16.36%, 9.29%and 8.92%respectively ) .Conclusions:In order to improve the quality and positive effect of health management for patients with hypertension and diabetes , the collaborative mechanism between township institutions should be developed , the government should increase invest-ment in rural public health , strengthen the rural heath personnel brainstorming , and on-site visits and debates on health should be reasonably arranged according to the needs of rural patients , and the quality and effectiveness of the management of hypertension and diabetes together with other chronic diseases should be improved .
7.Analysis of influencing factors of outpatient pre-registration preference for large hospitals in Nanjing city

ObjectiveTo understand the urban preference for outpatient pre-registration in large hospitals,and probe into the influencing factors,for recommendations to improve outpatient preregistration at the hospitals.Methods With Nanjing City as an example,the authors used tailoreddesigned questionnaire and qualitative interviews to investigate the preference of 1200 households of local residents regarding outpatient clinical pre-registration service at the tertiary hospitals.Results Age,education and economic factors are found to be the most important influential ones for urban preference in their outpatient pre-registration at large hospitals.Other factors include traditional approaches to outpatient clinic,inconvenience for outpatient pre-registration,and concerns for appointment breaking.Conclusion in order to increase their preference for outpatient pre-registration at large hospitals,hospitals should work to explore and improve approaches for outpatient pre-registration,and better their service quality,which will involve greater participation of both the hospital staff and residents.
8.Genetic Construction and Characterization of Murine Interleukin-18 Immunotoxin with a Truncated Pseudomonas Exotoxin

Objective To construct a new recombinant immunotoxin expression vector fused with a murine interleukin18(IL18) gene and a truncated pseudomonas exotoxin (PE38) gene, and examine the expression of IL-18-PE38 fusion protein in Escherichia coli (E. coli). Method Murine IL-18 (mIL-18) cDNA was cloned from murine liver tissue through reverse transcription-polymerase chain reaction (RT-PCR). The mIL-18 cDNA was ligased with a PE38 gene carried by PRKL expression vector through T4 DNA ligase and constructed into fusion protein expression plasmid PRKL-IL18-PE38. The recombinant vector was identified by restriction endonucleases digestion, PCR and DNA sequencing. After transformed into E.coli BL21 and induced by IPTG, the expressed product was obtained and the molecular weight and specificity were determined by SDS-PAGE and Western-blotting. Result The new recombinant immunotoxin expression vector was constructed successfully. DNA sequencing revealed that the mIL-18 and PE38 gene were consistent with NCBI Gene Bank. The IL-18-PE38 fusion protein was expressed in E.coli BL21, and Western-blotting analysis indicated that the molecular weight of the expression product is about 56 kDa, and could react with the specific antibody against mIL-18. Conclusion IL-18-PE38 recombinant immunotoxin expression vector will provide the basis for study on the targeted cytotoxic activity to Th1 cells and may have some potential value in the treatment of Th1 cell-mediated autoimmune diseases.
9.Clinical pathologic features and prognosis of triple negative breast cancer

Objective To analyze the clinical pathologic features and prognosis of triple negative breast cancer. Method A total of 570 cases of breast cancer between January 2005 and June 2007 were analyzed,113 cases were negative for estrogen receptor(ER), progesterone receptor(PR) and HER-2 as the triple negative group, 48 cases were negative for ER, PR and positive for HER-2 as the control group. The clinical pathologic features, recurrence and survival of the patients were compared. Results The median age was 46 years old, 15 cases (13.3%) had a breast cancer family history or oophoroma family history,86.7%(98/113) of pathological types were invasive ductal carcinoma,lymph node metastases were detected in 31.0%(35/113 ) in the triple negative group. No significant difference was found for p53 staining between the triple negative group[58.4%(66/113)positive] and the control group [41.7%(20/48) positive] (P>0.05).The median time of follow-up was 24 months,the loss was 5 cases, 10 cases (9.3%, 10/108) had local recurrence and metastasis,and 8 cases (7.4%,8/108) died from breast cancer metastases,2-year survival rate was 96.2% in the triple negative group.There was no significant difference in local recurrence and survival time between the triple negative group and the control group (P>0.05). Conclusions The incidence of triple negative breast cancer is higher than other data reported. Triple negative breast cancer exhibits the same clinical pathological features compared with ER-/PR-/HER-2 + subtypes,and the prognosis is similar to the controls.
10.Expression of glucose transporter-1 mRNA in cerebral ischemia/reperfusion penumbra in rats

AIM: To investigate the volume percentage of infarct and expression of glucose transporter-1 (GLUT1) transcription and protein levels at different ischemic time point and different reperfusion time point in rat focal cerebral ischemic penumbra. METHODS: Focal ischemic models of middle cerebral artery occlusion (MCAO) in rats were made by inserting nylon thread. Brain samples were harvested from ischemic penumbra. Infarct volume were analyzed quantitively by Kontron IBAS 2.5 image auto-analyses system. The expressien of GLUT1 mRNA was assessed by RT-PCR, and the expression of GLUT1 protein was assessed by immunohistochemistry. RESULTS: The infarction volume in MCAO 1 h/reperfusion (R) group was obviously smaller than that in MCAO 3 h/R group. GLUT1 increased at (1 h) MCAO 1 h/R group, climbed to climax at 24 h and remained higher than normal at 1 week. In contrast, in the MCAO 3 h/R group, the corresponding index was at 3 h, 24 h and 1 week, but the increasing degree of GLUT1 was slighter than MCAO 1 h/R. GLUT1 protein began to ascend at 1 h, reached climax at 24 h and was higher than normal at 1 week in MCAO 1 h/R group, while in MCAO 3 h/R group, the corresponding index was at 3 h, 24 h and 1 week. CONCLUSION: GLUT1 expression is notably up-regulated in the penumbra region after focal cerebral ischemia, it may be a protective reaction against ischemic injury. [
