1.Effects of endodontic treatment on heart rate, blood pressure and electrocardiogram of elderly patients with cardiovascular diseases

Objective To study the safety of opening pulp chamber therapy for elderly patients with cardiovascular diseases. Methods Blood pressure, heart rate and electrocardiography were examined during the treatment by opening of pulp chamber for patients with acute and chronic pulpititis and cardiovascular diseases. Patients without cardiovascular disease were used as controls. Results Both blood pressure and heart rates were increased during local anesthetic injection(14/6 mm Hg, 14 4/6 6 mmHg; 4 8, 4 1 times/min) and during the opening of pulp chamber(16/9 mm Hg, 19/12 mm Hg; 6 4, 5 9 times/min) for both patients and control groups with statistically significant differences compared with that before operation. But there was no statistically significant differences between the patients with cardiovascular diseases and the controls during the local anesthetic injection and the opening of pulp chamber. Electrocardiogram showed myocardial ischemia deteriorated in some patients with cardiovascular diseases. Conclusions Endodontic treatment under monitoring by electrocardiography is safe and can be recommended to the elderly patients with cardiovascular diseases

Objective To study the changes of serum soluble tumor necrosis factor receptor Ⅰ (sTNFR Ⅰ) and immunosuppressive acidic protein (IAP) of patients with pancreatic cancer before and after intra arterial chemotherapy and evaluate their significance. 3.Effects of head rotation on the anatomic relationship of the internal jugular vein and the common carotid artery-an ultrasonographic study

Objective To observe the relationship of right internal jugular vein (RIJV)and common carotid artery (CAA)by scanning strictly from the anterior to posterior using ultrasound at different head rotation.Methods Using ultrasonic scanning,the anatomic features of RIJV and CAA both at thyroid cartilage level (prominentia laryngea level)and at the apex of the angle formed by the division of the sternocleidomastoid muscle (triangle level)with 0°,1 5°,30° and 45° right rotation were observed in 131 patients with ASA physical status Ⅰ or Ⅱ (male 55 cases,female 76 cases, aged 18~74 years).Based on the ultrasound images,the safe puncture range,the overlapping ratio, the angle between the horizontal axis and the line from the midpoint of RIJV to that of CAA (αangle) were measured.In addition,the relationship between the RIJV and CAA was defined as anterior-lat-eral, lateral, posterior-lateral or extremely-posterior-lateral position according to α angle. Results The safe puncture range of RIJV augmented as head rotated from 0° to 30° position(P <0.05);The safe puncture range of RIJV at triangle level was significantly higher than at prominentia laryngea level at all the four head positions(P <0.05).The overlapping degree decreased as head rota-ted from 0°to 30°head position at prominentia laryngea level(P <0.05).No siginificant differences of the overlapping degree were found between head positons at triangle level;The overlapping degree at triangle level was less than at prominentia laryngea level when at 0°and 1 5°head positon(P <0.05). At both prominentia laryngea and triangle levels,RIJV located mainly at lateral and posterior-lateral positions.In addition,the part of lateral position increased while the part of posterior-lateral position decreased as the head rotated from 0°to 45°position(P <0.05).Conclusion The puncture conditions for RIJV catheterization were more optimal at 30°to 45°head rotation for a safer puncture range and less overlapping between RIJV and CAA.RIJV located mainly at lateral and posterior-lateral positions at different rotations and RIJV gradually shifted to lateral position while head rotation increasing.It would be much better to select triangle level for central venous catheterization than prominentia laryn-gea level.
4.Efficacy of moxifloxacin, esomeprazole plus furazolidone triple therapy to eradicate Helicobacter pylori

Objective To evaluate the efficacy and safety of 10-day moxifloxacin,esomeprazole plus furazolidone triple therapy as first-line treatment to eradicate Helicobacter pylori (Hp)in comparison with the 14-day quadruple therapy containing esomeprazole,amoxicillin, clarithromycin and colloidal bismuth pectin. Methods A total of 1 26 cases of endoscopically confirmed Hp-positive chronic active gastritis or peptic ulcer were randomly assigned to the treatment group to receive esomeprazole,moxifloxacin plus furazolidone triple therapy for 1 0 days;or to the control group to receive esomeprazole, amoxicillin,clarithromycin,and colloidal bismuth pectin quadruple therapy for 14 days.Clinical efficacy and safety were evaluated after 4-week treatment.Results At the end of treatment,the Hp eradication rate was 89.4%in the treatment group, and 88.3% in the control group(P>0.05).The incidence of adverse reactions in the treatment group (16.7%)was significantly lower than that in the control group (36.7%)(P<0.05).Conclusions The 10-day moxifloxacin,esomeprazole plus furazolidone triple therapy is effective and well-tolerated as first-line treatment to eradicate Hp with samilar efficacy and fewer adverse reactions compared to the 14-day bismuth-based quadruple therapy.
5.Joint Manipulation under Brachial Plexus Anesthesia Combined with Physical Therapy for Treatment of Periarthritis of Shoulder

Objective To observe the treatment effect of joint manipulation under brachial plexus anesthesia combined with comprehensive physical therapy in treatment of periarthritis of shoulder. Mehods From 2010.3 to 2010.9, 60 patients with periarthritis of shoulder in the Department of Pain, The First People's Hospital of Jingzhou,were divided into 3 groups:group A,B and C.Twenty patients in Group A were given Joint manipulation under brachial plexus anesthesia for treatment of the periarthritis of shoulder. Twenty patients in Group B were given comprehensive physical therapy fro treatment of periarthritis of shoulder. Twenty patients in Group C were given Joint manipulation under brachial plexus anesthesia combined with comprehensive physical therapy for treatment of periarthritis of shoulder.Results All 60 patients were followed up for 1 months. At the 1st month after surgery, vas score was evaluated for patients. Postoperative and preoperative vas score difference was compared between 3 groups by t test. There was statistically significant difference in the postoperative and preoperative vas score between Group C and Group A,and between Group B and Group A (P<0.05) . Conclusion The treatment effect of Joint manipulation under brachial plexus anesthesia combined with physical therapy in treatment of periarthritis of shoulder is good, but has no obvious difference compared with comprehensive physical therapy.
6.Treatment of Cervical Intervertebral Disc Herniation by Radio Frequency Target Combined with Collagenase

Objective To observe the treatment effect of radio frequency target combined with collagenase in treatment of cervical intervertebral disc herniation. Methods From March 2011 to September 2011, 40 patients with cervical intervertebral disc herniation were divided into 2 groups: Group A and Group B. Patients in Group A were given radio frequency target radiofrequency for treatment of the cervical intervertebral disc herniation and patients in Group B of 20 were given radio frequency target combined with collagenase for treatment of cervical intervertebral disc herniation. Results 40 patients were followed up for 6 months. At the 1st week and 6 months after surgery, the VAS score difference between before and after surgery in patients were compared between two groups by t test. There was no statistically significant difference at the 1st week after surgery between two groups (P>0.05), while 6 months after surgery, there was statistically significant difference between two groups (P<0.05) .Conclusion Radio frequency target combined with collagenase for treatment of cervical intervertebral disc herniation can alleviate the pain symptoms of patients,and the short-term and long-term curative effect is very good. The long-term curative effect has statistically significant difference with pure target radiofrequency ablation treatment. Radio frequency target combined with collagenase for treatment of cervical intervertebral disc herniation has good safety, few side effects, low risks and good short-term and long-term curative effect, so it deserves to be generalized.
7.A study on safety of opening pulp chamber therapy for patients with a history of remote myocardial infarction.

OBJECTIVETo study the indication and contraindication of opening pulp chamber therapy for patients with a history of remote myocardial infarction.
METHODSBlood pressure (BP), heart beat rate and electrocardiographic examination were determined during opening pulp chamber therapy in patients with acute and chronic pulpititis and a history of remote myocardial infarction.
RESULTSHigh blood pressure and quick heart beat were found during local anaesthetic injection and opening of pulp chamber (during local anaesthetic injection BP increased by 12.52/8.04 mmHg, heart rate by 4.02 times/min;during opening of pulp chamber BP and heart rate increased by 15.43/8.0 mmHg, 6.37 times/min respectively). Electrocardiogram showed myocardial ischemic pattern deteriorated in some patients with the history of coronary insufficiency.
CONCLUSIONSMost patients can bear it. Comprehensive physical examinations, evaluations of the patients' cardial status, mental state and indispensible equipment are necessary. The therapy is recommended to be operated under the monitor electrocardiography for the safety of the patients.

8.Study on pharmacokinetics of HCPT nanosuspensions with ability of inhibiting P-gp in rats after oral administration.

OBJECTIVETo study on pharmacokinetics of hydroxycamptothecine (HCPT) nanosuspensions in rats after oral administration.
METHODThe plasma concentrations of HCPT were determined by HPLC-FD. The analysis was performed on a diamonsil C18 column (4.6 mm x 200 mm, 5 microm) with 0.3% acetic acid-triethylamine buffer (pH 5.0) and methanol (57: 43) as mobile phase. The flow rate was 1.0 mL x min(-1); the excitation wave was set at 363 nm, and emission wave was set at 550 nm; the temperature was 35 degrees C. All data of concentration-time of HCPT were treated with pharmacokinetics program DAS 2.0.
RESULTThe concentration-peak area of this assay had a good linear relation in the range from 1 to 50 microg x L(-1), and the minimum limit of quantitation was 1 microg x L(-1). The inter- and intra-day precisions of HCPT were smaller than 4.3%, and the accuracy were between -5.59% and 5.59%. The recoveries of HCPT in three plasma concentrations including high, medial, low concentration were 98.94%, 95.88% and 102.69%, respectively, which was in line with the request of biopharmaceutical analysis. The plasma concentration time profiles of HCPT fitted in two-compartment models well, and the main pharmacokinetic parameters found for HCPT after oral administration were as follows: Cmax 13.10 microg x L(-1), Tmax 0.75 h, t(1/2alpha) 8.242 h, t(1/2beta) 136.122 h, AUC(0-t) 116.77 microg x h x L(-1), AUC(0-infinity) 161.93 microg x h x L(-1).
CONCLUSIONThe HPLC-FD method was simple, with good specificity, reproducibility, and could be used to investigate the pharmacokinetics and determinate the concentration of hydroxycamptothecin. The nanosuspension in this study could accelerate the oral absorption rate of HCPT, and make improving bioavailability of HCPT possible.

9.The clinical application of the open supraclavicular approach in thyroidectomy

Objective: To analyze the clinical application of the open supraclavicular approach in thyroidectomy. Methods: The clinical practicability of open supraclavicular thyroidectomy was explored by comparing the traditional anterior low arc incision thyroidectomy procedure with open supraclavicular thyroidectomy in terms of patients'aesthetic satisfaction, effectiveness of the operation, operation time, and so on. Result: Twenty-two cases of open supraclavicular thyroidectomy (group B) had better aesthetic satisfaction than 29 cases of traditional incision thyroidectomy (group A)(P<0.05), and had the same operative effect with traditional incision. Open supraclavicular thyroidectomy is associated with good aesthetic satisfaction, and has the same effect as the traditional incision does. Conclusions: Open supraclavicular thyroidectomy has good clinical value for benign thyroid tumors and some malignant tumors that require unilateral lobectomy, and even for tumors larger than the incision diameter. It has good aesthetic value while ensuring the curative effect of surgery.
10.Emerging role of natural products in cancer immunotherapy.

Cancer immunotherapy has become a new generation of anti-tumor treatment, but its indications still focus on several types of tumors that are sensitive to the immune system. Therefore, effective strategies that can expand its indications and enhance its efficiency become the key element for the further development of cancer immunotherapy. Natural products are reported to have this effect on cancer immunotherapy, including cancer vaccines, immune-check points inhibitors, and adoptive immune-cells therapy. And the mechanism of that is mainly attributed to the remodeling of the tumor-immunosuppressive microenvironment, which is the key factor that assists tumor to avoid the recognition and attack from immune system and cancer immunotherapy. Therefore, this review summarizes and concludes the natural products that reportedly improve cancer immunotherapy and investigates the mechanism. And we found that saponins, polysaccharides, and flavonoids are mainly three categories of natural products, which reflected significant effects combined with cancer immunotherapy through reversing the tumor-immunosuppressive microenvironment. Besides, this review also collected the studies about nano-technology used to improve the disadvantages of natural products. All of these studies showed the great potential of natural products in cancer immunotherapy.
