1.Central obesity and metabolic syndrome in community residents

ObjectiveTo assess the relationship between central obesity and metabolic syndrome (MS) in community population.MethodsOne thousand and fifty-three individuals aged≥18 (mean,45.6),including 542 men and 511 women,were recruited for questionnaire survey and medical exarmnation from 4 communities of Hangzhou during March and November 2007.Their blood pressure,waist circumference,serum glucose,lipid profile,and other biochemical markers were measured.All data were used for variance analysis.ResultsThe prevalence of MS,central obesity,hypertension,and diabetes mellitus (DM) was 11.2%,23.6%,30.5%,and 8.5%,respectlvely.The prevalence of MS,DM,and hypertension was increased with waist circumference(P<0.05).Waist circumference was positively correlated to body mass index (BMI).systolic blood pressure(SBP),diastolic blood pressure(DSP),fasting blood sugar,total cholesterol,and triglyceride (r=0.812,0.649,0.712,0.657,0.598,and 0.676,respectively),although negativelv with high-density lipoprotein cholesterol(r=-0.578).Waist circumference also showed positive rclationship with the number of lipid metabolism abnormity(r=0.658,P<0.01).CondusionFor Hangzhou community population.central obesity might be one of the risk factors of MS.

3.Expression of nm23 Gene Protein in Epithelial Ovarian Cancer

Objective: Our purpose was to study the expression of protein of nm23 gene in patients with epithelial ovarian carcinoma.Methods:Protein of nm23 gene was detected by immunohistochemical technique in 45 patients with epithelial ovarian cancer, 12 with borderline, and 10 with benign ovarian tumor.Results:The positive expression rate in ovarian cancer (73.3%) was significantly higher than that of the benign and borderline tumors. There was no significant correlation between the expression of nm23 gene and the histological type, grade, the existence of ascites or the size of residul after operation. However, the expression of nm23 gene protein was negatively related with the FIGO stage. The overall survival rate of the patients with positive expression of nm23 gene was much higher than that of the patients with negative expression. Conclusion:The findings suggest that nm23 gene protein might serve as a valuable and independent marker for the prognosis of ovarian cancer.
4.Ultrasound study in the diagnosis and treatment of the female stress urinary incontinence

Objective To evaluate the significance and role of ultrasound parameters in the female stress urinary incontinence (SUI). Methods The changes of the distance of bladder neck mobility,posterior urethra-vesical angle,urethral angle and residual urine volume before and after the operation of transobturator tension-free vaginal tape surgery (TVT-O) for 46 cases of female SUI (experimental group ) by ultrasound were studied,and compared with 43 normal women (control group). Results The experimental group before the operation had the greater distance of bladder neck mobility and posterior urethra-vesical angle [(14.46 ± 1.28) mm, (124.87 ±2.95)°] than the control group [(7.47 ±0.55) mm, (107.83 ±3.24)°] (P < 0.01 ), but the urethral angle [( 23.61 ± 2.28 )°] was smaller than the control group [(36.24 ±2.23 )°] (P < 0.01 ). There was no significant difference in the distance of bladder neck mobility, posterior urethra-vesical angle, urethral angle and residual urine volume between the experimental group after the operation and the control group (P > 0.05 ). The experimental group before the operation had the greater distance of bladder neck mobility and posterior urethra-vesieal angle than those after the operation [(7.84±0.76) mm, (108.74±3.63)°] (P <0.01), the urethral angle was smaller than that after the operation [(34.39 ± 3.46)°] (P < 0.01 ), but the residual urine volume had no significant difference (P > 0.05 ).Conclusions After the operation, there are some changes in the distance of bladder neck mobility, posterior urethra-vesical angle and urethral angle for the patients with female SUI,and these parameters can be restored the normal state,and have the relatively stable effect. The residual urine volume dosen't increase after the operation. Ultrasound has the advantage in objectively assessing the severity and recovery status after the operation exclusion of mental and psychological factors.
5.ATP-tumor chemosensitivity assay directed chemotherapy in patients with cervical cancer

Objective:To select suitable chemotherapy for cervical cancer patients by ATP-tumor chemosensitivity assay.
Methods:Seventy-two hospitalized patients with cervical cancer between July 2007 and October 2009 were enrolled. The patients were randomly divided into a trial group (n=35) and a control group(n=37). ATP-TCA was used to detect the sensitivity of 35 samples of cervical cancer in the trial group to 6 combined chemotherapy regimens. The chemotherapy regimen in the trial group was confirmed by the results of susceptibility testing and that in the control group was confirmed by clinical experience. One-year recurrence rate and 3-year survival rate of two groups were compared after 3 year follow-up.
Results:ATP-TCA was measured in 32 of the 35 patients in the trial group. The sensitive patients for paclitaxel+carboplatin, paclitaxel+oxaliplatin, bleomycin+ifosfamide+cisplatin, bleomycin+vincristine+cisplatin, fluorouracil+cisplatin, and gemcitabine+cisplatin were 20, 18, 17, 18, 17, and 21, respectively. There was no significant difference in the 1-year recurrence between the two groups (P>0.05), while the 3-year survivors in the trial group were more than those in the control group (P<0.05).
Conclusion:ATP-TCA method is good for patients with cervical cancer because it is sensitive, effective, and individualized.
6.Correlation among risk factors of hypertension and waist-to-height ratio in officers of regiment and a-bove in troops stationed in Zhejiang

Objective:To explore the correlation among risk factors of hypertension and waist-to-height ratio (WHtR) in officers of regiment and above in troops stationed in Zhejiang .Methods :A total of 2641 officers of regi-ment and above from troops stationed in Zhejiang ,who participated in annual physical examination in our hospital from 2012 to 2013 ,were selected .Correlation among body mass index (BMI) ,waist circumference (WC) ,WHtR , waist/hip ratio (WHR) and blood pressure were analyzed ;receiver operator characteristic curve (ROC) was used to compare and analyze the predictive value of above four indexes on hypertension and judge the optimal cut-off point . Results :(1) Pearson correlation analysis indicated that WHtR was positively correlated with WC ,BMI ,WHR ,sys-tolic and diastolic blood pressure (r=0.25~0.94 ,P<0.005~ <0.001);(2) Among above four indexes ,WHtR had the largest area under the ROC curve 0.697 (95% CI 0.676~0.718) and the optimal cut-off point was 0.51 ;and it's 0.651 (95% CI 0.639~0.662) for BMI ,0.689 (95% CI 0.664~0.751) for WC and 0.685 (95% CI 0.672~0.698) for WHR ;for morbidity risk of hypertension ,compared with BMI ,WC and WHR ,WHtR possessed the most balanced sensitivity (60.68% ) and specificity (70.74% ) ,and the largest OR 4.12 (3.71~4.59) .Conclusion:(1) Among officers of regiment and above in troops stationed in Zhejiang ,anthropometric indexes are closely relat-ed to blood pressure ,which can be regarded as simple indexes for predicting and assessing morbidity risk of the popu-lation ;(2) Waist-to-height ratio may be the best anthropometric index evaluating risk of hypertension in these popu-lation and its optimal cuf-off point is 0.51 for predicting hypertension .
7.Depressive symptoms in first episode schizophrenic patients.

Objective To investigate the prevalence, characteristics and correlates of depressive symptoms in first episode schizophrenic patients. Methods To examine 164 first episode schizophrenic patients at the time of admission and at 3,6,9, and 12 months after starting treatment using the HAMD, BPRS, the Chinese version of SANS, CGI and GAF. Results 71% of the patients had depressive symptoms (mild or more) at damission, but the prevalence of depressive symptoms dropped to a mean of 12% during the recovery period. The most prominent depressive symptoms during the acute phase of schizophrenia were ‘cognitive disturbance’ and ‘retardation’ (the respective subscales constituted 35% and 29% of the total HAMD score on admission). Depressive symptoms improved in parallel with the schizophrenic illness. The severity of depressive symptoms was not related to gender, age of onset, educational level, duration of prodromal period or duration of illness. At admission the severity of depressive symptoms was only related to the BPRS anxiety and depression subscale score, but during the recovery period the HAMD total score was significantly correlated with all of the other clinical scales. The level of depressive symptoms at admission and at three months after starting treatment was not related to the subsequent course of positive or negative symptoms. Conclusions Depressive symptoms appeared to be a separate symptom cluster during the acute phase of first episode schizophrenia. The severity of depressive symptoms did not predict the clinical outcome of first episode schizophrenic patients.
8.Imaging Diagnosis of Intracranial Tuberculoma

Objective:To analyse the imaging featrues of intractranial tuberculoma and improve the diagnostic accuracy.Methods:31 patients with clinical characteritics and pathological proved intracranial tuberculomas were studied retrospectively.Results:"egg-shell"calcification were the feature of giant calcified and ossified tuberculoma.CT scaning were single and multiple nodular lesion.In the contrast enhancing CT scaning,plate shaped or ring form shadows were shown.MRI were provided hypointense on T1WI and hyperintense on T2WI.The rim homogeneous enhancement were showd in the Gd-DTPA.Conclusion:The diagnosis of typical intracranial tuberculomas can be made.After antituberculosis chemotherapy,CT and MRI can help made differsntial diagnosis.Operative indications should be select strictly.
9.Study on the Expression of Suppressor of Cytokine Signaling -1 ( SOCS1 ) in the Liver of Septic Mice

Objective To investigate the change of the content of suppressor of cytokine signaling (SOCS-1) in the liver of septic mice and its working mechanism.Methods Adopted Cecalligation and puncture (CLP) to create models of sepsis and divided randomly adult male BALB/c mice into 8 groups,including normal controlled group,sham-operated group,and the killed groups 2 hours,6 hours,12 hours,24 hours and 48 hours after operation.After extracting the RNA and protein from the liver tissue of the mouse groups,reverse transcription polymerase chain reaction (RT-PCR) was adopted to determine the relative content of SOCS-1 mRNA in the tissue,Western blot was adopted to determine the relative content of protein and the SPSS statistics software was adopted to calculate the correlation.Then observed the pathological change of liver tissues,and detected SOCS-1 protein expression by immunohistochemistry.Results After CLP suergery,the expression of SOCS-1 on gene degree in the liver and the expression of SOCS1 on protein degree in the liver increased rapidly at the 6th hour ( P < 0.05 ),with the former reaching peak ( P < 0.05 ) at the 24th hour and the latter remaining high all the time.There were pathological changes such as fatty degeneration and necrosis in the septic liver tissue,hepatic SOCS-1 protein expression could be detected by immunohistochemistry.Conclusions CLP induced sepsis could lead to the increase of the expression of SOCS1 in the liver.
10.TriVex based transilluminated powered phlebectomy staged management for lower limb vein ulcers

Objective To evaluate TriVex based transilluminated powered phlebectomy (TIPP)three-stage management for lower limb venous ulcers.Methods This retrospective study included 86 patients with 103 diseased limbs treated in our hospital from October 2005 to July 2013.All received TriVex TIPP three-stage management.Results After therapy there was no acute cellulitis,skin necrosis nor lower limb deep venous thrombosis.There were 2 cases of subcutaneous ecchymosis,1 case of subcutaneous hematoma,1 case of skin abnormal sensation and 1 case of subcutaneous induration.Ulcers healed within one month postoperation in all cases,while skin abnomalities were alleviated with no ulcer recurrence during the follow-up ranging from 4 to 48 months.Conclusions TriVex TIPP centered threestage management is a good option for lower limb venous ulcer,with shortened healing time,low recurrence rate.
