1.Observation on the effect of azithromycin in the treatment of acute exacerbation of chronic obstructive pulmonary disease and its influence on the serum inflammatory cytokines level of patients
Chinese Journal of Primary Medicine and Pharmacy 2013;20(18):2791-2793
Objective To observe the effect of azithromycin in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD) and its influence on the serum interleukin-10(IL-10),intedeukin-8 (IL-8)and tumor necrosis factor-αt (TNF-a) level of patients. Methods According to the digital table,92 patients with AECOPD were divided into the control group and the observation group, each group 46 cases. The control group was given conventional therapy, and the observation group was given azithromycin on the basis of the treatment of the control group, and the treatment lasted one week. The effect and the change of the serum IL-I0,IL-8 and TNF-α level were compared between the two groups. Results The total effective rate of the observation group was 91.3%,which was significantly higher than that of the control group(χ2 =8.165,P < 0.05).After treatment, there was no statistically significant difference in the serum IL-8 and TNF-α level between the two groups (t =0.655,0.447,all P > 0.05),but the serum IL-10 level of the observation group was (15.71 ± 2.88)pg/L,which was significantly higher than that of the control group[(12.59 ± 1.12) pg/L] (t =6.848,P < 0.01).Conclusion Azithromycin in the treatment of AECOPD has definite effect, and can adjust the immune system of patients to release the inflammatory inhibitors and inhibit airway inflammation.

The pathogenic effects of crops from Keshan disease area were studied by rat-feeding. As double controls, another two groups were fed with crops from non-endemic area and synthetic diet. Those from disease area caused necrosis of heart muscle vascular changes of myocardial mitochondria and fatty degeneration of liver. Compared with the controls, the free radical and lipid peroxide content in heart and liver were significantly higher, while the ATP and AN contents in these organs were apparently lower. The multielement contents and its constituent patterns in the feeds, whole blood, heart, liver, kidney and brown adipose tissue of rats in experimetal group were apparently different from those in the control groups. The statistic analysis of multiple factors showed that eight elements Se, Mo, Cu, Mn, Cr, Fe, Zn and Pr might be the perilous factors, that is, under the condition of low selenium, the constituent pattern of selenium and the eight elements might be the complicated factors of endemic crops.

Soluble epoxide hydrolase (sEH) is related to arachidonic acid cascade and is over-expressed in a variety of diseases, making sEH an attractive target for the treatment of pain as well as inflammatory-related diseases. A new series of memantyl urea derivatives as potent sEH inhibitors was obtained using our previous reported compound 4 as lead compound. A preferential modification of piperidinyl to 3-carbamoyl piperidinyl was identified for this series via structure-based rational drug design. Compound A20 exhibited moderate percentage plasma protein binding (88.6%) and better metabolic stability in vitro. After oral administration, the bioavailability of A20 was 28.6%. Acute toxicity test showed that A20 was well tolerated and there was no adverse event encountered at dose of 6.0 g/kg. Inhibitor A20 also displayed robust analgesic effect in vivo and dose-dependently attenuated neuropathic pain in rat model induced by spared nerve injury, which was better than gabapentin and sEH inhibitor (±)-EC-5026. In one word, the oral administration of A20 significantly alleviated pain and improved the health status of the rats, demonstrating that A20 was a promising candidate to be further evaluated for the treatment of neuropathic pain.
