1.Effect Mechanism and Law of Sterilization by 60Co-γ Ray Irradiation on Chemical Composition of Chinese Materia Medica: A Review

60Co-γ ray irradiation has the unique advantages of high efficiency, strong penetration, operation at room temperature and no residue, which has been widely used in the sterilization of Chinese medicinal materials, decoction pieces, Chinese patent medicine. However, the irradiation effect may cause changes in the content of chemical components in Chinese materia medica or the emergence of new radiolysis products, leading to reduced efficacy and uncontrollable safety risks. This paper reviewed the relevant literature at home and abroad, summarized the effect of irradiation sterilization on various types of chemical compositions of Chinese medicinal materials and their preparations, and analyzed and explored the rule of change. The results showed that the content changes of various chemical components in Chinese materia medica after 60Co-γ ray irradiation sterilization varied. The contents of most flavonoids, terpenoids, phenylpropanoids and quinones decreased after irradiation, and the degree of decrease increased with the elevated irradiation dose. The contents of lignans, alkaloids, isoflavones and some terpenoids did not change significantly before and after irradiation, while the content changes of triterpenoid saponins, dihydroflavonols, chalcones, sugars and glycosides after irradiation were not yet uniform. Therefore, it is recommended to pay attention to the compositional changes of irradiated Chinese medicines, strengthen the research on the standards of irradiated Chinese medicines, and standardize the irradiation and sterilization of Chinese medicines in order to promote the healthy and rational application of irradiated Chinese medicines.
2.Prevotella nigrescens exacerbates periodontal inflammation and impairs cognitive function in mice.

OBJECTIVES:
To investigate the effects of periodontitis induced by Prevotella nigrescens (Pn) combined with ligation on cognitive functions in mice.
METHODS:
Twenty-four C57BL/6J mice were randomly divided into control group, ligation group, and ligation + Pn treatment (P+Pn) group. Experimental periodontitis was induced by silk ligation of the first molars followed by topical application of Pn for 6 weeks. After modeling, alveolar bone resorption was assessed using micro-CT and histological analysis. Learning and memory abilities of the mice were evaluated using open field test (OFT), novel object recognition test (NORT), and Morris water maze test (MWM). Seven weeks after the start of modeling, the mice were sacrificed for examining histopathological changes in the hippocampus using HE and Nissl staining.
RESULTS:
After 6 weeks of molar ligation, micro-CT revealed horizontal alveolar bone resorption and furcation exposure in the mice, and histological analysis showed apical migration of the junctional epithelium, epithelial ridge hyperplasia, and lymphocyte infiltration, and these changes were obviously worsened in P+Pn group. Alveolar bone height decreased significantly in both ligation groups compared to the control group. Cognitive tests showed that the mice in both of the ligation groups traveled shorter distances in OFT, showed reduced novel object preference in NORT, and exhibited longer escape latencies in MWM, and the mice in P+Pn group had significantly poorer performances in the tests. Histologically, obvious neuronal cytoplasmic degeneration, necrosis, nuclear pyknosis, vacuolation, and reduced Nissl bodies and viable neurons were observed in the hippocampal regions of the mice in the two ligation groups.
CONCLUSIONS
Pn infection aggravates alveolar bone destruction, accelerates necrosis and causes morphological abnormalities of neuronal cells in the hippocampus to reduce cognitive functions of mice with periodontitis.

3.The prevalence and related risk factors of hyperuricemia in Bozidum Kinghiz township of Xinjiang Aksu region

Objective:To investigate the prevalence of hyperuricemia (HUA) in Bozidun Kirghiz township of Xinjiang Aksu region, and to explore the risk factors for the occurrence of HUA in the local area.Methods:A cross-sectional survey study was conducted by randomly selecting 9 villages in Bozidun Kirgiz Township by the whole-group sampling method and questionnaire were distributed to the households. The questionnaire included: demographic information, history of past illness, personal history, and all subjects were measured for height, weight, blood pressure, abdominal circumference, etc. The diagnostic of HUA if the serum uric acid (SUA) level >420 μmol/L in men or >360 μmol/L in women. The incidences of HUA in different age, sex, food type and life style behavior were analyzed. T test, non-parametric test and Chi-square test were used to analyze the differences among the groups, and logistic regression was used to analyze the risk factors. Results:①A total of 2 138 subjects were surveyed, among which 68 patients were with HUA, the prevalence of HUA in Bozidun Kirghiz township, Aksu region in the general population was 3.18%(68/2 138); the prevalence rate in men was 4.60%(45/978), 45 patients were identified; and the prevalence rate in women was 1.98%(23/1 160), 23 patients were identified. The peak age of HUA in male and female patients was 51~60 years old. ②The prevalence of HUA was lower in those who consumed dairy products ( χ2=6.91, P=0.017), nuts ( χ2=8.43, P=0.038) and eggs ( χ2=7.38, P=0.023), and lower in those who consumed more. Different intake of cereals ( χ2=0.87, P=0.647), meat( χ2=0.82, P=0.662), vegetables and fruits( χ2=5.22, P=0.073) had no effect on the prevalence of HUA.③In terms of different life behaviors, the prevalence of HUA in men who had been smoking was higher than those who had never smoked (57.78%, 28.89%, 13.33%, χ2=8.16, P=0.017). In the relationship between drinking and HUA, the prevalence rates of male always drinking, ever drinking and never drinking were 80.00%, 11.11% and 3.89%, respectively, the difference was statistically significant ( χ2=6.67, P=0.038). ④Multi-factor logistic regression analysis showed that high BMI, old age, high TG, increased Cr and increased WBC were risk factors for the occurrence of HUA [ OR(95% CI)=1.13(1.04, 1.23), 1.03(1.00,1.05),1.39(1.00, 1.93), 1.03(1.02, 1.05), 1.27(1.07, 1.49), all P<0.05]. Conclusion:The prevalence of HUA in Bozidun Kirgiz township in Aksu prefecture of Xinjiang is lower than that in other areas with continental climate. High BMI, old age, high TG, increased Cr and increased WBC count are risk factors for the development of HUA .
4.Progress on the researches of acupuncture preconditioning before surgery.

This paper reviews the researches on acupuncture preconditioning before surgery in recent years and explores its application value from three aspects, i.e. relieving preoperative anxiety, preventing from postoperative cognitive dysfunction, and preventing from postoperative gastrointestinal dysfunction. As a relatively safe non-drug treatment, acupuncture has the underlying advantages in participating into multidisciplinary coordination in the enhanced recovery after surgery (ERAS). By building up higher-quality medical evidences and revealing the effect mechanism of acupuncture from multi-dimenisonal aspects, it is expected that acupuncture technology can be coordinated with ERAS to optimize the clinical path in the perioperative period, and boost the development of the perioperative medicine ultimately.

5.Treatment of patent ductus arteriosus in very preterm infants in China.

Objective: To describe the current status and trends in the treatment of patent ductus arteriosus (PDA) among very preterm infants (VPI) admitted to the neonatal intensive care units (NICU) of the Chinese Neonatal Network (CHNN) from 2019 to 2021, and to compare the differences in PDA treatment among these units. Methods: This was a cross-sectional study based on the CHNN VPI cohort, all of 22 525 VPI (gestational age<32 weeks) admitted to 79 tertiary NICU within 3 days of age from 2019 to 2021 were included. The overall PDA treatment rates were calculated, as well as the rates of infants with different gestational ages (≤26, 27-28, 29-31 weeks), and pharmacological and surgical treatments were described. PDA was defined as those diagnosed by echocardiography during hospitalization. The PDA treatment rate was defined as the number of VPI who had received medication treatment and (or) surgical ligation of PDA divided by the number of all VPI. Logistic regression was used to investigate the changes in PDA treatment rates over the 3 years and the differences between gestational age groups. A multivariate Logistic regression model was constructed to compute the standardized ratio (SR) of PDA treatment across different units, to compare the rates after adjusting for population characteristics. Results: A total of 22 525 VPI were included in the study, with a gestational age of 30.0 (28.6, 31.0) weeks and birth weight of 1 310 (1 100, 1 540) g; 56.0% (12 615) of them were male. PDA was diagnosed by echocardiography in 49.7% (11 186/22 525) of all VPI, and the overall PDA treatment rate was 16.8% (3 795/22 525). Of 3 762 VPI who received medication treatment, the main first-line medication used was ibuprofen (93.4% (3 515/3 762)) and the postnatal day of first medication treatment was 6 (4, 10) days of age; 59.3% (2 231/3 762) of the VPI had been weaned from invasive respiratory support during the first medication treatment, and 82.2% (3 092/3 762) of the infants received only one course of medication treatment. A total of 143 VPI underwent surgery, which was conducted on 32 (22, 46) days of age. Over the 3 years from 2019 to 2021, there was no significant change in the PDA treatment rate in these VPI (P=0.650). The PDA treatment rate decreased with increasing gestational age (P<0.001). The PDA treatment rates for VPI with gestational age ≤26, 27-28, and 29-31 weeks were 39.6% (688/1 737), 25.9% (1 319/5 098), and 11.4% (1 788/15 690), respectively. There were 61 units having a total number of VPI≥100 cases, and their rates of PDA treatment were 0 (0/116)-47.4% (376/793). After adjusting for population characteristics, the range of standardized ratios for PDA treatment in the 61 units was 0 (95%CI 0-0.3) to 3.4 (95%CI 3.1-3.8). Conclusions: From 2019 to 2021, compared to the peers in developed countries, VPI in CHNN NICU had a different PDA treatment rate; specifically, the VPI with small birth gestational age had a lower treatment rate, while the VPI with large birth gestational age had a higher rate. There are significant differences in PDA treatment rates among different units.

The main contents include: (1) clinical evaluation of GGN, (2) procedures, indications, contraindications, outcomes evaluation and related complications of thermal ablation for GGN and (3) future development directions.

8.Tracer Effect of Carbon Nanoparticles in Thyroid Cancer with Different Thyroid Diseases

【Objective】 To investigate the tracer effect of carbon nanoparticle suspension in central neck lymph node dissection of differentiated thyroid cancer with other thyroid diseases. 【Methods】 A total of 500 patients with differentiated thyroid cancer hospitalized in Meizhou People's Hospital from July 2015 to July 2019 were retrospectively analyzed. Patients were divided into carbon nanoparticles group(group A, 314 cases) and non-carbon nanoparticles group(group B, 186 cases). Group A and B were further divided into 4 subgroups according to the classifications of no other disease (subgroup 0), with nodular goiter(subgroup 1), with Hashimoto's thyroiditis(subgroup 2) and large tumor(the diameter > 30 mm, subgroup 3), and the number of lymph nodes dissected in each group and the incidence of complications were analyzed. 【Results】 The number of lymph nodes harvested and metastasis rate in group A was higher than that in group B [6.0(3.0 ~ 9.0) vs. 3.0(1.0 ~ 5.3), P < 0.001; 24.4% vs. 20.7%, P = 0.041) ] . The number of lymph nodes harvested in group A0, A1 and A2 was higher than that in group B0 [8.0(6.0 ~ 11.0) vs. 3.0(2.0 ~ 6.0), P < 0.001) ] , B1 [4.0(3.0 ~ 6.8) vs. 3.0(0.0 ~ 5.0), P < 0.001) ] and B2 [8.0(5.0 ~ 12.0) vs. 4.0(0.0 ~ 6.5), P = 0.002] . The number of lymph nodes harvested in group A1 and A3 was significantly lower than that in group A0 [4.0(3.0 ~ 6.8) vs. 8.0(6.0 ~ 11.0), P < 0.001; 3.0(2.0 ~ 6.0) vs. 8.0(6.0 ~ 11.0), P < 0.001) ] . The incidence of hoarseness in group A1, A2 and A3 was comparable with that in group A0(P > 0.05), and the incidence of postoperative parathyroid dysfunction in group A2 and A3 was significantly higher than that in group A0(51.8% vs. 34.1%, P < 0.05; 54.2% vs. 34.1%, P < 0.05) . 【Conclusions】 The application of carbon nanoparticles in the operation of differentiated thyroid cancer increases the number of lymph nodes harvested, and the best effect is without complicated diseases. The tracking effect of carbon nanoparticles is affected by the combination of nodular goiter. When the maximum diameter of the tumor is greater than 30 mm, it has no obvious effect on the dissection of lymph nodes.
9.The mechanism of Huanglian-Ejiao Decoction in the treatment of insomnia based on network pharmacology

Objective:By taking network pharmacology approach to search for active components and the rapeutic targets of Huanglian-Ejiao Decoction. Methods:According to the pharmacokinetic parameters, this research screened components of this decoction, and diged the drugs and disease targets with network pharmacology and constructed the drug-target-pathway network by Cytoscape system to investigate the mechanism of Huanglian-Ejiao Decoction. Results:It was found that 19 components such as hydroberberine, β-sitosterol, curcumin, kaempferol, berberine and Ramie flavonoids in Huanglian-Ejiao Decoction could take effecton insomnia by regulating 25 targets such as 5-hydroxytryptamine, dopamine, gamma aminobutyric acid, opioid receptor and acetylcholine. Conclusion:Huanglian-Ejiao Decoction may play an important role in treating insomnia by regulating multiple-targets.
10.Molecular mechanism of Andrographis herba for breast cancer based on network pharmacology

Objective:Based on the methodology of network pharmacology, this study preliminarily predicted the major targets and pathways of Andrographis herba in the treatment of breast cancer. Methods:The active components of Andrographis herba were screened by Traditional Chinese Medicine Systems Pharmacology Database (TCMSP); breast cancer targets were predicted and screened by Genecard database platform and OMIM database platform; active ingredient-targets networks were built by Cytoscape (3.7.2) software, and protein interaction networks were built by String database platform. Finally, Gene Ontology (GO) analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis were performed by using Bioconductor platform and R language. Results:A total of 24 active ingredients of Andrographis herba and 13 234 targets for breast cancer were obtained, involving 71 targets related to Andrographis herba. There were 51 common targets for Andrographis herba-breast cancer. The top five targets are TP53, IL6, JUN, CCND1, and ERS1. Andrographis herba-breast cancer KEGG-enriched pathways are estrogen signaling pathway, P53 signaling pathway, Notch signaling pathway, and Wnt signaling pathway. Conclusions:The TP53, ERS1, and CCND1 and other targets may be the key targets for Andrographis herba paniculata to treat breast cancer. It plays an anti-breast cancer effect by regulating the signal pathways such as the estrogen receptor pathway and the P53 signaling pathway. This paper provides a theoretical basis for further exploration of the mechanism research of Andrographis herba in the treatment of breast cancer.
