Objective To establish the rabbit brain death model, and observe the change of kidney function and morphology in the brain death rabbits, fully evaluate the possibility of transplantation surgery carried out with brain death donor kidney.Method 40 male healthy New Zealand rabbits were randomly divided into sham group (n =20) and brain death group (n =20),and each group further was divided into four groups according to 2,4,6 and 8 h after brain death groups. At the end of the experiment, the blood samples and kidney tissues were collected. The level of blood urea nitrogen(BUN),serum creatinine (Cr) and the apoptosis of kidney cells were determined by automatic biochemistry analyzer and TUNEL method, respectively. The morphology changes of liver were detected by HE staining.Result Although BUN values were with no obviously alteration (P>0.05) within 8 h after brain death, the renal Cr levels in the states of 2,4 and 8 h (P<0.05) brain death were found significantly increased compared with shamed groups. The denaturalization in the epithelial cells of renal tubule, glomerulus atrophygradually, swelling and vacuolation of epithelial cells, edema in the interstitium and inflammatory cell infiltration and partly occlusion in proximal convoluted tubule gradually emerged in the brain dead groups. The inflammatory factor intercellular adhesion molecule (ICMA) and the apoptosis index in brain dead groups increased in a time dependent manner but the expressions of protective factor HSP70 conducted a contrary tendency.Conclusin Brain death could lead to the damage of kidney function and morphology and this injury aggravated in a time dependent manner. The alterations might be relevant to the production of inflammatory factors.Within 8 h after brain death there were obvious changes in the kidney function and morphology, which provide experimental basis for the rational use of brain death donor kidney transplantation.

Sixty-one newly-diagnosed diabetic patients from high altitude area were treated individually plus metformin 1.5 g/d for 14 days. The difference of oxygen partial pressure was 3.5 mm Hg (1 mm Hg=0. 133kPa) between altitude 2 260 m and 2 780 m districts. In these patients, their serum lactic acid was increased before treatment. At altitude 2 780 m district, serum lactic acid in middle-aged and elder groups were even higher [(3.90±0.85 vs 3.65±0.70), (4. 67±0.80 vs 3.69±0.78) mmol/L, both P<0. 05=. After 14-day treatment, serum lactic acid in middle-aged diabetic patients remained statistically unchanged [(4.50±0.50 and 3.79±0.62 ) mmol/L, P>0.05], while that in elder patients was increased significantly [(5.59±0.55 and 5.27±0.43 ) mmol/L,P<0. 05]. The results suggested that metformin should not be used or be used carefully in elder diabetic patients at high altitude area.

OBJECTIVETo evaluate the efficacy and safety of etanercept plus Tripterygium wilfordii polyglycoside (TWP) in elderly patients with active rheumatoid arthritis (RA).
METHODSTotally 46 elderly patients with active RA were randomly assigned to the treatment group (22 cases) and the control group (24 cases). All patients received subcutaneous injection of etanercept, 25 mg each time, twice per week. The dosage was reduced to once per week 3 months later. Patients in the treatment group took TWP Tablet (10 mg each time, three times per day), while those in the control group took methotrexate (MTX), 10 mg each time, once per week. The whole course lasted for 24 weeks. Patients' rest pain, tender joint number, swollen joint number, health assessment questionnaire (HAQ), patients' global assessment, physicians' global assessment, erythrocyte sediment rate (ESR), C reactive protein (CRP), rheumatic factor were assessed at week 0, 4, 8, 12, and 24. The curative effect was statistically evaluated by the United States Institute of Rheumatology ACR20, ACR50, and ACR70 improvement criteria. Meanwhile, any adverse event was recorded and evaluated.
RESULTSTotally 41 completed the trial, and 5 dropped off (3 in the treatment group and 2 in the control group). Compared with the control group, there was no statistical difference in ACR20, ACR50, or ACR70 in the treatment group (P > 0.05). Compared with before treatment in the same group, there was some improvement in tender joint number, swollen joint number, visual analogue scale (VAS) for patients' global assessment, VAS for physicians' global assessment, ESR, CRP, and HAQ between the two groups, showing statistical difference (P < 0.05). Compared with the control group in the same phase, there was no statistical difference in the treatment group (P > 0.05). There was no statistical difference in the occurrence of adverse events between the two groups.
CONCLUSIONSEtanercept plus TWP could achieve equivalent therapeutic effect to that of Etanercept plus MTX. The two regimens could improve clinical signs, symptoms, and QOL related to RA. They were well tolerated in the treatment of elderly patients with active RA.
